    Ehf controls mammary alveolar lineage differentiation and is a putative suppressor of breast tumorigenesis

    The transcription factor EHF is highly expressed in the lactating mammary gland, but its role in mammary development and tumorigenesis is not fully understood. Utilizing a mouse model of Ehf deletion, herein, we demonstrate that loss of Ehf impairs mammary lobuloalveolar differentiation at late pregnancy, indicated by significantly reduced levels of milk genes and milk lipids, fewer differentiated alveolar cells, and an accumulation of alveolar progenitor cells. Further, deletion of Ehf increased proliferative capacity and attenuated prolactin-induced alveolar differentiation in mammary organoids. Ehf deletion also increased tumor incidence in the MMTV-PyMT mammary tumor model and increased the proliferative capacity of mammary tumor organoids, while low EHF expression was associated with higher tumor grade and poorer outcome in luminal A and basal human breast cancers. Collectively, these findings establish EHF as a non-redundant regulator of mammary alveolar differentiation and a putative suppressor of mammary tumorigenesis

    What are participant beliefs regarding physical therapy led treatment? A qualitative study of people living with femoroacetabular impingement syndrome

    Background: Physical therapist-led treatment programs are recommended for patients with femoroacetabular impingement (FAI) syndrome. Views of people with FAI syndrome regarding such interventions are currently unknown, including perceptions of potential barriers and facilitators to participation and adherence to exercise programs. Objectives: To explore participant perceptions of physical therapist-led programs for FAI syndrome, including barriers and facilitators for accessing physical therapy, and adhering to a rehabilitation program. Methods: Our qualitative study used semi-structured interviews to explore the perceptions of patients with FAI syndrome undertaking physical therapy-led treatment, where treatment was ceased due to Coronavirus Disease 2019 (COVID-19). The interview topic guide was informed by the Theoretical Domain Framework. Interviews were transcribed verbatim and data categories were developed using inductive thematic analysis. Themes were discussed between researchers until consensus was reached. Results: Fourteen participants (mean age: 30 years) with a range of physical activity backgrounds undertook interviews. We identified four key themes, 1) Patients believed their hip pain was caused by structural damage worsened through exercise; 2) Barriers and facilitators on the feasibility of physical therapist-led programs; 3) Participants held beliefs regarding the importance of adjunct treatments to exercise; and 4) Impact of FAI syndrome on physical activity participation. Conclusion: People with FAI syndrome believe they have structural damage which leads to their hip pain and are often afraid to exercise due to fear of causing more damage to their hip. Our findings suggest that people with FAI syndrome want clear education about exercise, imaging, and expectations of cost and duration of treatment

    Validation of a sleep-disordered breathing screening questionnaire during pregnancy and comparison between mothers and bedpartners prediction of risk

    Background: Sleep Disorder Breathing (SDB) in pregnant patients ranges from 3 to 27% and varies depending on gestational age and method used to diagnose. SDB increases the risk of advanced pregnancy complications such as gestational diabetes mellitus, pregnancy-induced hypertension, and preeclampsia. Screening and diagnosis of SDB during pregnancy remains a challenge, with existing screening tools underperforming during pregnancy. This study aimed to validate a previously developed model for predicting SDB during late pregnancy and compare the predictive value of bedpartner responses. Methods: Ninety-six women in the third trimester of pregnancy underwent polysomnography and completed the Berlin Questionnaire (BQ), with 81 bedpartners completing the BQ about their pregnant partner. A subset of BQ items (snoring volume and tiredness upon awakening) along with BMI > 32 kg/m2 was utilised to calculate the Wilson Optimized Model (WOM), which demonstrated strong predictive properties in development. Results: SDB (RDI/hr ≥ 5) was detected in 43.8% of women. BQ identified 72% of pregnant mothers as high risk for SDB (Sensitivity = 83%, Specificity = 37%), compared to 29% of mothers identified by the WOM (Sensitivity = 45%, Specificity = 83%). At RDI of ≥ 15, the WOM correctly classified more women according to SDB risk than the BQ (76.0% vs. 41.7% cases correct, X2(1) = 23.42, p <.001), with no difference at RDI ≥ 5. Bedpartners were more likely to report high risk for SDB on the WOM than pregnant women themselves (38.3% vs. 28.4%), however predictive ability was not improved by bedpartner input (RDI ≥ 5 bedpartner AUC = 0.69 v mother AUC = 0.73). Conclusion: BQ largely overestimates the prevalence of SDB in pregnancy compared to the WOM which underestimates. Utilising bedpartner responses didn’t improve screening for SDB in late pregnancy. More work is needed to develop a pregnancy-specific tool for quick and accurate screening for SDB

    Parameter priority of circular concrete-filled double-skin tubular slender beam-columns under eccentric loading

    This study aimed to determine the parameter priority for the design optimization of circular concrete-filled double skin tubular (CFDST) slender beam-columns under eccentric loading. Experimental investigations were conducted on four geometrically distinct CFDST beam-columns subjected to eccentric loading to analyze their behavioral responses. A finite element (FE) model was developed and calibrated using the experimental data. The FE simulations were performed on seven parameters, including load eccentricity, concrete strength, steel strength, outer tube thickness, inner tube thickness, lateral curvature, and end fixity to determine their respective impacts. By varying each parameter by 10%, the ultimate capacity of the columns was affected by different percentages. The load eccentricity was found to have the highest contribution of 58.7%, followed by fixity (16.0%), steel strength (9.3%), outer tube thickness (8.8%), inner tube thickness (3.5%), concrete strength (2.5%), and lateral curvature (1.2%). These findings establish the priority of the parameters, highlighting the significance of load eccentricity in the optimization of circular CFDST slender beam-columns under eccentric loading

    An examination of nursing students’ performance in and Satisfaction with a Patient Safety E-Learning Module

    Background: The knowledge nursing students acquire during their undergraduate degree influences the quality of patient care they provide for many years to come. However, previous studies indicate that students may have a limited understanding of core patient safety concepts. Objectives: The objectives of this study were to (i) examine nursing students’ performance in an interactive patient safety e-learning module titled ‘One shift, four patients … a day in the life of a new graduate nurse’; and (ii) explore students’ level of satisfaction with the module using the Satisfaction with the Patient Safety E-Learning Module scale. Design: A cross-sectional design was used with students’ knowledge and levels of satisfaction examined using descriptive statistics. Setting and participants: In total, 1038 third-year undergraduate nursing students from 22 Australian universities attempted the module. Results: The mean correct score was 74%, but there were significant differences in knowledge levels evident across the four activities that comprised the module. Participants achieved the highest mean score in Activity 2 (Predicting, monitoring, and responding to adverse events [79%]) and the lowest in Activity 3 (Clinical reasoning [66%]). The mean score for Activity 1 (Infection control and medication safety) was 74%, and for Activity 4 (Cultural competence), the score was 77%. The level of student satisfaction with the module was high with responses to each survey item exceeding 4.0 out of 5.0. The Cronbach's alpha for the satisfaction scale was 0.99, and the Content Validity Index was >0.9. Conclusions: Universities are responsible for preparing nursing students to become safe clinicians. The results from this study indicate that participants’ overall level of knowledge of key patient safety concepts was adequate. However, as knowledge is the foundation for safe practice, these results suggest that further attention to imbedding patient safety in nursing curricula is required.</p

    An Alternative Method of Obtaining the Particle Size Distribution of Soils by Electrical Conductivity

    This study proposes a new method to determine the particle size distribution (PSD) of soils by considering the electrical conductivity (EC) technique. A new EC probe was designed with a transparent thermoplastic, commonly known as acrylic, and brass electrodes. At first, the EC of a soil–water homogeneous suspension was calculated at different densities to obtain a calibration curve of each tested soil sample. During the PSD analysis, as the particles started to settle down in the basement due to gravity, the EC was measured at different time intervals, and the corresponding EC values were then matched with the calibrated EC values at different densities. The proposed method considered the conventional Stokes’ law to determine the diameter of soil particles and the general percentage of passing mathematical expression to obtain the final PSD curve of each soil. The PSD analysis by the EC approach was later validated with that of hydrometer and laser diffraction methods, and in general, good agreements were obtained for identical soil samples at different classifications of soil particles such as clay, silt, and sand. Finally, reproducibility tests were also conducted and the new EC probe overperformed the hydrometer method in terms of both accuracy and precision. The finding from this study aimed to propose an alternative to determine the PSD of soils by using the EC technique with a high level of accuracy and efficiency

    Pragmatism as a Compatible Theoretical Lens for Mixed Methods Research in Prehospital Care

    This study extends on work investigating the increased application of mixed methods research in prehospital care. The complexity and diversity of the prehospital environment warrants flexibility and applicability to guide the research process, yet little attention has focused on the theoretical lens used in prehospital research when using mixed methods research. Pragmatism’s characteristics of human inquiry, problem solving, and action align with a clinical reasoning approach, supporting the prehospital researchers epistemological understanding of the world. Through further exploration of this alignment, this article proposes pragmatism as a compatible theoretical lens for mixed methods in prehospital research

    Implementing menstrual cycle tracking: A pilot concept mapping study investigating considerations of coaches, support staff, and female athletes

    Objectives: To identify what coaches, support staff, and athletes believe should be considered when implementing menstrual cycle tracking in sport. Design: Concept mapping (mixed-methods participatory approach). Methods: Participants brainstormed statements in response to the focus prompt “What should be considered when tracking menstrual cycles in sport?” Participants then sorted the statements into groups according to meaning and rated each statement on its importance and feasibility to address. Results: Twenty-six participants (12 athletes and 14 coaches/support staff) generated 53 ideas that the research team synthesised to 57 unique statements. Sixteen participants sorted the statements into an average of 8 (±4) groups. Six clusters were identified as the most appropriate representation of the sorting data following multidimensional scaling and hierarchical cluster analysis: 1) Ethics, 2) Tracking Tools and Methods, 3) Performance and Health, 4) Education, 5) Interpretation and Framing and 6) Communication. Participants rated each statement based on their importance (n = 12) and feasibility (n = 10), respectively. The most important (mean = 3.88 out of 5) and feasible (mean = 3.85 out of 5) cluster was Ethics, whilst the least important (mean = 3.17) and feasible (mean = 3.04) was Communication. Conclusions: Ethical considerations pertaining to privacy, consent, and scope of practice should be prioritised when tracking menstrual cycles in sport. A low burden method of tracking, which allows for some individualisation based on athletes' preferences or needs, and menstrual health education should also be undertaken to improve the uptake and impact of menstrual cycle tracking

    Identifying health and healthcare priorities in rural areas: A concept mapping study informed by consumers, health professionals and researchers

    Background: It is vital that health service delivery and health interventions address patients’ needs or preferences, are relevant for practice and can be implemented. Involving those who will use or deliver healthcare in priority-setting can lead to health service delivery and research that is more meaningful and impactful. This is particularly crucial in rural communities, where limited resources and disparities in healthcare and health outcomes are often more pronounced. The aim of this study was to determine the health and healthcare priorities in rural communities using a region-wide community engagement approach. Methods: This multi-methods study was conducted in five rural communities in the Grampians region, Western Victoria, Australia. It involved six concept mapping steps: (1) preparation, (2) generation (brainstorming statements and identifying rating criteria), (3) structuring statements (sorting and rating statements), (4) representation of statements, (5) interpretation of the concept map and (6) utilization. Community forums, surveys and stakeholder consultations with community members and health professionals were used in Step 2. An innovative online group concept mapping platform, involving consumers, health professionals and researchers was used in Step 3. Results: Overall, 117 community members and 70 health professionals identified 400 health and healthcare issues. Six stakeholder consultation sessions (with 16 community members and 16 health professionals) identified three key values for prioritizing health issues: equal access for equal need, effectiveness and impact (number of people affected). Actionable priorities for healthcare delivery were largely related to access issues, such as the challenges navigating the healthcare system, particularly for people with mental health issues; the lack of sufficient general practitioners and other health providers; the high travel costs; and poor internet coverage often impacting technology-based interventions for people in rural areas. Conclusions: This study identified actionable health and healthcare priorities from the perspective of healthcare service users and providers in rural communities in Western Victoria. Issues related to access, such as the inequities in healthcare costs, the perceived lack of quality and availability of services, particularly in mental health and disability, were identified as priorities. These insights can guide future research, policy-making and resource allocation efforts to improve healthcare access, quality and equity in rural communities

    New evidence for the sensorimotor mismatch theory of weight perception and the size-weight illusion

    Abstract: The size-weight illusion is a phenomenon where a smaller object is perceived heavier than an equally weighted larger object. The sensorimotor mismatch theory proposed that this illusion occurs because of a mismatch between efferent motor commands and afferent sensory feedback received when lifting large and small objects (i.e., the application of too little and too much lifting force, respectively). This explanation has been undermined by studies demonstrating a separation between the perceived weight of objects and the lifting forces that are applied on them. However, this research suffers from inconsistencies in the choice of lifting force measures reported. Therefore, we examined the contribution of sensorimotor mismatch in the perception of weight in the size-weight illusion and in non-size-weight illusion stimuli and evaluated the use of a lifting force aggregate measure comprising the four most common lifting force measures used in previous research. In doing so, the sensorimotor mismatch theory was mostly supported. In a size-weight illusion experiment, the lifting forces correlated with weight perception and, contrary to some earlier research, did not adapt over time. In a non-size-weight illusion experiment, switches between lifting light and heavy objects resulted in perceiving the weight of these objects differently compared to no switch trials, which mirrored differences in the manner participants applied forces on the objects. Additionally, we reveal that our force aggregate measure can allow for a more sensitive and objective examination of the effects of lifting forces on objects.</p
