The effect of altered pH growth conditions on the production, composition, and proteomes of Helicobacter pylori outer membrane vesicles
Gram-negative bacteria release outer membrane vesicles (OMVs) that contain cargo derived from their parent bacteria. Helicobacter pylori is a Gram-negative human pathogen that produces urease to increase the pH of the surrounding environment to facilitate colonization of the gastric mucosa. However, the effect of acidic growth conditions on the production and composition of H. pylori OMVs is unknown. In this study, we examined the production, composition, and proteome of H. pylori OMVs produced during acidic and neutral pH growth conditions. H. pylori growth in acidic conditions reduced the quantity and size of OMVs produced. Additionally, OMVs produced during acidic growth conditions had increased protein, DNA, and RNA cargo compared to OMVs produced during neutral conditions. Proteomic analysis comparing the proteomes of OMVs to their parent bacteria demonstrated significant differences in the enrichment of beta-lactamases and outer membrane proteins between bacteria and OMVs, supporting that differing growth conditions impacts OMV composition. We also identified differences in the enrichment of proteins between OMVs produced during different pH growth conditions. Overall, our findings reveal that growth of H. pylori at different pH levels is a factor that alters OMV proteomes, which may affect their subsequent functions
Patient and clinician perspectives of online-delivered exercise programmes for chronic musculoskeletal conditions: a mixed-methods systematic review
Purpose: To synthesize common or differing perceptions of patients’ and clinicians’ that influence uptake of online-delivered exercise programmes (ODEPs) for chronic musculoskeletal (MSK) conditions. Methods: Eight databases were searched from inception to April 2023 for studies including (1) patients with and/or clinicians delivering ODEPs for chronic MSK conditions, and (2) synchronous ODEPs, where information is exchanged simultaneously (mode A); asynchronous ODEPs, with at least one synchronous feature (mode B); or no ODEPs, documenting past experiences and/or likelihood of participating in an ODEP (mode C). Critical Appraisal Skills Programme checklists were used to assess study quality. Perceptions of patients’ and clinicians’ influencing uptake of ODEPs were extracted. Quantitative and qualitative data were synthesised and integrated. Results: Twenty-one studies were included (twelve quantitative, seven qualitative, and two mixed-methods) investigating the perceptions of 1275 patients and 534 clinicians on ODEP mode A (n = 7), mode B (n = 8), and mode C (n = 6). Sixteen of the 23 identified perceptions related to satisfaction, acceptability, usability, and effectiveness were common, with 70% of perceptions facilitating uptake and 30% hindering uptake. Conclusions: Findings highlight the need to promote targeted education for patients and clinicians addressing interconnected perceptions, and to develop evidence-based perception-centred strategies encouraging integrated care and guideline-based management of chronic MSK conditions
Optimizing Speech Emotion Recognition with Machine Learning Based Advanced Audio Cue Analysis
In today’s fast-paced and interconnected world, where human–computer interaction is an integral component of daily life, the ability to recognize and understand human emotions has emerged as a crucial facet of technological advancement. However, human emotion, a complex interplay of physiological, psychological, and social factors, poses a formidable challenge even for other humans to comprehend accurately. With the emergence of voice assistants and other speech-based applications, it has become essential to improve audio-based emotion expression. However, there is a lack of specificity and agreement in current emotion annotation practice, as evidenced by conflicting labels in many human-annotated emotional datasets for the same speech segments. Previous studies have had to filter out these conflicts and, therefore, a large portion of the collected data has been considered unusable. In this study, we aimed to improve the accuracy of computational prediction of uncertain emotion labels by utilizing high-confidence emotion labelled speech segments from the IEMOCAP emotion dataset. We implemented an audio-based emotion recognition model using bag of audio word encoding (BoAW) to obtain a representation of audio aspects of emotion in speech with state-of-the-art recurrent neural network models. Our approach improved the state-of-the-art audio-based emotion recognition with a 61.09% accuracy rate, an improvement of 1.02% over the BiDialogueRNN model and 1.72% over the EmoCaps multi-modal emotion recognition models. In comparison to human annotation, our approach achieved similar results in identifying positive and negative emotions. Furthermore, it has proven effective in accurately recognizing the sentiment of uncertain emotion segments that were previously considered unusable in other studies. Improvements in audio emotion recognition could have implications in voice-based assistants, healthcare, and other industrial applications that benefit from automated communication.</p
Advancing perioperative nursing education and surgical skills acquisition: A comprehensive approach
Postgraduate nursing education serves as a crucial connection between theoretical knowledge and clinical practice, playing an important role in enhancing the competencies of nurses in specialised fields, such as perioperative nursing. The delivery of higher education has undergone a transformation, focussing on online and blended learning, making higher education more accessible and more flexible to adult learners than traditional face-to-face education programs. However, acquiring hands-on surgical skills poses a potential challenge for online education.The expanding need for surgical service delivery from Perioperative Nurse Surgical Assistants (PNSAs) is increasing the demand for novice practitioners to acquire surgical skills. Simulation-based learning is an effective way to enhance skills in learners, and cost-effective solutions using innovative methodologies, that are explored in this discussion, pave the way for immersive learning experiences that enhance proficiency in surgical skills and ensure patient safety.</p

Internal migration and settlement patterns in Australia's regional urban centres
The distribution of population between metropolitan regions and their rural counterparts is a topic of interest, due to the growth pressures in large cities and areas of decline in some, typically remote regions. In Australia, fostering population growth in regional urban centres (RUCs) has been a long-term goal of government policy. Understanding the different profiles, structures, and population dynamics of Australia's RUCs is critical in informing Australian regional development, as well as factors in regional population dynamics in general. This research is focused on patterns of population change through internal migration across Australian RUCs. In addition to a detailed spatial analysis of patterns of population migration flows, this research offers a comprehensive analysis of a range of regional social, economic and infrastructure development factors and their effects on regional migration and population changes. The outputs of the method are new evidence to inform the future regional plan to increase economic and population growth in RUCs. This research contributes to population research and regional planning and provides a baseline for the prepandemic status of regional population dynamics in Australia
Experiences of participation in a longitudinal cohort study of people who inject drugs in Victoria, Australia
Aim: Longitudinal cohort studies are important for understanding the social and health trajectories of people who inject drugs. Little research exists, however, about the motivations, challenges, and experiences of participation in these studies. Methods: To address this gap, we conducted in-depth interviews with participants of an open-ended natural history longitudinal cohort study (SuperMIX) and with fieldworkers who collect the data for this study. Interviews were audio-recorded, and data analyzed thematically. Results: Research findings revealed several motivating factors that encouraged participants to get involved in and stay in the study including financial reimbursement and the opportunity to contribute to a study aiming to improve the lives of peers. Many participants discussed their appreciation of the kindness and respect they were afforded by fieldworkers, and the advocacy role fieldworkers had undertaken with them. While some participants described challenges responding to some questions, there were also expressions of finding the experience therapeutic. Conclusion: Findings provide learnings for future studies and highlight how participation in longitudinal cohort research can provide an opportunity for on-going engagement that affords participants not only a chance to share their experiences, but to contribute to knowledge production and achieve clarity about their own experience.</p
Prevention of foot ulcers in persons with diabetes at risk of ulceration: A systematic review and meta-analysis
Aims: Prevention of foot ulcers in persons with diabetes is important to help reduce the substantial burden on both individual and health resources. A comprehensive analysis of reported interventions is needed to better inform healthcare professionals about effective prevention. The aim of this systematic review and meta-analysis is to assess the effectiveness of interventions to prevent foot ulcers in persons with diabetes who are at risk thereof. Materials and Methods: We searched the available scientific literature in PubMed, EMBASE, CINAHL, Cochrane databases and trial registries for original research studies on preventative interventions. Both controlled and non-controlled studies were eligible for selection. Two independent reviewers assessed risk of bias of controlled studies and extracted data. A meta-analysis (using Mantel-Haenszel's statistical method and random effect models) was done when >1 RCT was available that met our criteria. Evidence statements, including the certainty of evidence, were formulated according to GRADE. Results: From the 19,349 records screened, 40 controlled studies (of which 33 were Randomised Controlled Trials [RCTs]) and 103 non-controlled studies were included. We found moderate certainty evidence that temperature monitoring (5 RCTs; risk ratio [RR]: 0.51; 95% CI: 0.31–0.84) and pressure-optimised therapeutic footwear or insoles (2 RCTs; RR: 0.62; 95% CI: 0.26–1.47) likely reduce the risk of plantar foot ulcer recurrence in people with diabetes at high risk. Further, we found low certainty evidence that structured education (5 RCTs; RR: 0.66; 95% CI: 0.37–1.19), therapeutic footwear (3 RCTs; RR: 0.53; 95% CI: 0.24–1.17), flexor tenotomy (1 RCT, 7 non-controlled studies, no meta-analysis), and integrated care (3 RCTs; RR: 0.78; 95% CI: 0.58–1.06) may reduce the risk of foot ulceration in people with diabetes at risk for foot ulceration. Conclusions: Various interventions for persons with diabetes at risk for foot ulceration with evidence of effectiveness are available, including temperature monitoring (pressure-optimised) therapeutic footwear, structured education, flexor tenotomy, and integrated foot care. With hardly any new intervention studies published in recent years, more effort to produce high-quality RCTs is urgently needed to further improve the evidence base. This is especially relevant for educational and psychological interventions, for integrated care approaches for persons at high risk of ulceration, and for interventions specifically targeting persons at low-to-moderate risk of ulceration
Understanding factors influencing inclusion and participation in community gym settings for adults with cerebral palsy
This Seminar was recorded in February of 2024.Georgia McKenzie will discuss the findings of her PhD research, which explored inclusion and participation in community gym settings for adults with cerebral palsy. Georgia will present the findings from a series of studies that have identified and examined key relationships between young adults with cerebral palsy and the environments around them that are influencing their participation in community gyms. Georgia will conclude the seminar with a discussion on the implications of the findings, and recommendations for change across the rehabilitation and recreation sectors.</p
‘Quitlink’: Outcomes of a randomised controlled trial of peer researcher facilitated referral to a tailored quitline tobacco treatment for people receiving mental health services
Objective: The aim of this study was to test the effectiveness of a tailored quitline tobacco treatment (‘Quitlink’) among people receiving support for mental health conditions. Methods: We employed a prospective, cluster-randomised, open, blinded endpoint design to compare a control condition to our ‘Quitlink’ intervention. Both conditions received a brief intervention delivered by a peer researcher. Control participants received no further intervention. Quitlink participants were referred to a tailored 8-week quitline intervention delivered by dedicated Quitline counsellors plus combination nicotine replacement therapy. The primary outcome was self-reported 6 months continuous abstinence from end of treatment (8 months from baseline). Secondary outcomes included additional smoking outcomes, mental health symptoms, substance use and quality of life. A within-trial economic evaluation was conducted. Results: In total, 110 participants were recruited over 26 months and 91 had confirmed outcomes at 8 months post baseline. There was a difference in self-reported prolonged abstinence at 8-month follow-up between Quitlink (16%, n = 6) and control (2%, n = 1) conditions, which was not statistically significant (OR = 8.33 [0.52, 132.09] p = 0.131 available case). There was a significant difference in favour of the Quitlink condition on 7-day point prevalence at 2 months (OR = 8.06 [1.27, 51.00] p = 0.027 available case). Quitlink costs AU$9231 per additional quit achieved. Conclusion: The Quitlink intervention did not result in significantly higher rates of prolonged abstinence at 8 months post baseline. However, engagement rates and satisfaction with the ‘Quitlink’ intervention were high. While underpowered, the Quitlink intervention shows promise. A powered trial to determine its effectiveness for improving long-term cessation is warranted.</p
