Characterisation of young stroke presentations, pathways of care, and support for 'invisible' difficulties: a retrospective clinical audit study.
ABSTRACT Background: Young stroke survivors are likely to be discharged home from acute hospital care without rehabilitation more quickly than older survivors, but it is not clear why. File-audit studies capturing real-world clinical practice are lacking for this cohort. We aimed to compare characteristics and care pathways of young and older survivors and describe stroke presentations and predictors of pathways of care in young survivors (≤45years), including a focus on care received for 'invisible' (cognitive, psychological) difficulties. Methods: A retrospective audit of 847 medical records (67 young stroke survivors, mean age=36years; 780 older patients, mean age=70years) was completed for stroke survivors admitted to an Australian tertiary hospital. Stroke characteristics and presence of cognitive difficulties (identified through clinician opinion or cognitive screening) were used to predict length of stay and discharge destination in young stroke survivors. Results: There were no differences in length of stay between young and older survivors, however, young stroke survivors were more likely to be discharged home without rehabilitation (though this may be due to milder strokes observed in young stroke survivors). For young stroke survivors, stroke severity and age predicted discharge destination, while cognitive difficulties predicted longer length of stay. While almost all young survivors were offered occupational therapy and physiotherapy, none received psychological input (clinical, health or neuropsychology). Conclusions: Cognitive and psychological needs of young stroke survivors may remain largely unmet by a service model designed for older people. Findings can inform service development or models of care, such as the new Australian Young Stroke Service designed to better meet the needs of young survivors.</p

Genetic Gain and Inbreeding in Different Simulated Genomic Selection Schemes for Grain Yield and Oil Content in Safflower
Safflower (Carthamus tinctorius L.) is a multipurpose minor crop consumed by developed and developing nations around the world with limited research funding and genetic resources. Genomic selection (GS) is an effective modern breeding tool that can help to fast-track the genetic diversity preserved in genebank collections to facilitate rapid and efficient germplasm improvement and variety development. In the present study, we simulated four GS strategies to compare genetic gains and inbreeding during breeding cycles in a safflower recurrent selection breeding program targeting grain yield (GY) and seed oil content (OL). We observed positive genetic gains over cycles in all four GS strategies, where the first cycle delivered the largest genetic gain. Single-trait GS strategies had the greatest gain for the target trait but had very limited genetic improvement for the other trait. Simultaneous selection for GY and OL via indices indicated higher gains for both traits than crossing between the two single-trait independent culling strategies. The multi-trait GS strategy with mating relationship control (GS_GY + OL + Rel) resulted in a lower inbreeding coefficeint but a similar gain compared to that of the GS_GY + OL (without inbreeding control) strategy after a few cycles. Our findings lay the foundation for future safflower GS breeding

Electrostatic complexation of growth factors in thiol-armed hydrogel enhances in situ cartilage tissue engineering
Tissue engineering is a promising approach to addressing the poor natural healing capacity of articular cartilage. However, the challenge is to sustain stem cells' chondrogenic differentiation post-implantation in a long-term manner. Herein, we propose the electrostatic complexation of growth factors in thiol-armed hydrogels. Transforming growth factor beta-3 (TGFB3) and bone morphogenic protein-6 (BMP6) are electrostatically complexed to poly-lysine and thiol-heparin which in turn is bound via thiol-acrylate to a gelatin methacryloyl (GelMA) hydrogel. Growth factor and dose-dependency are evaluated in human adipose-derived stem cells (hADSCs) highlighting the minimal dose and growth factor selection for triggering chondrogenic response. The efficiency of growth factor retention through the functionalized hydrogels reveals slower release compared to non-functionalised hydrogels highlighting the ability for long-term stimulation. Long-term (56 days) ex vivo evaluation of the in situ tissue engineered implant shows that our novel functionalisation strategy exhibits enhanced hyaline cartilage matrix deposition when compared to non-functionalised hydrogels. This flexible functionalisation design strategy clearly advances in situ tissue engineering for the treatment of articular cartilage injuries

Development of the essential learning outcomes for the midwifery student continuity of care learning model: A Delphi study
Background: In Australia, midwifery students are required to undertake at least ten Continuity of Care Experiences (CoCE) during their education. The learning outcomes of this experience have never been explicit or standardised resulting in inconsistent assessment. Aim: To develop and identify standardised learning outcomes for the CoCE. Methods: A modified Delphi survey was conducted with an expert panel. Intended learning outcome statements were developed, reflecting the learning objectives identified in a previous study. Bloom's taxonomy levels of thinking complexities guided the wording of the outcomes. Participants were asked to rank and rate their level of agreement with each statement over two survey rounds. Findings: Round one was completed by 32 participants, with 92.5% of the 40 statements reaching consensus. The second round was completed by 23 participants, with 70.7% of the 33 statements reaching consensus. Content analysis of participant comments from each round identified duplicates that were removed and informed refining the wording of some statements. A final set of 15 learning outcomes were agreed upon. The outcomes were broadly grouped within the themes of accountability, advocacy, and autonomy. Discussion: This study has identified agreed learning outcomes for midwifery students undertaking CoCE. The consensus agreement of experts reinforced the learning model enables the development of woman-centred practice that is underpinned by accountability, advocacy, and autonomy. Conclusion: Purposeful learning outcomes for the CoCE have been developed, informing how the model can be embedded in curricula, guide student learning and assessment to standardise the pedagogy of the model to prepare future midwives
