Healthcare Systems-Based Exercise Oncology Programs: Emphasizing and Speaking the Language of the Clinic and Patient
Introduction Exercise oncology services are evidence based and aim to reduce symptom burden and potentially improve outcomes in adults living with and beyond cancer. Healthcare system-based exercise oncology programs include exercise prior to, during, and after treatment, but the implementation and maintenance of these programs are not well documented. We aimed to describe five healthcare system-based exercise oncology programs in the United States, including service information and barriers to and facilitators of program success.Methods This was a qualitative case study of five healthcare-based exercise oncology programs in the United States informed by the Exploration, Preparation, Implementation, Sustainment framework. We conducted semistructured online interviews with the founders and other key representatives of each program assessing program structure, adoption, and integration into the healthcare system. Data were evaluated using qualitative descriptive methodology.Results In each healthcare-based exercise oncology program, trained exercise professionals delivered in-person, virtual, or hybrid exercise programs to adults with cancer. Buy-in from healthcare system leadership, clinicians, and administration was key to success. All programs were designed to meet the needs of the individual healthcare systems. Institutionalization of the programs into existing healthcare systems facilitated sustainment. Length and exercise prescription for each program varied, but individually tailored exercise programs with physical or functional assessments (often pre/post) were reported by most programs. Funding was a common barrier.Conclusions Institutional priorities and buy-in from both leadership and clinicians were used to establish and maintain five healthcare-based exercise oncology programs from across the United States. These programs followed evidence-based recommendations provided by exercise professionals within the healthcare system to serve patients from the healthcare system’s catchment area. Healthcare systems exploring the creation of exercise oncology programs should consider institutional structures, provider priorities, resources (e.g., staff, finances), leadership buy-in, and financial support
Comment les usages de la vidéo 360° facilitent l’apprentissage d’habiletés en Éducation Physique et en Sport
L’analyse SWOT que nous avons réalisée porte sur les études scientifiques de ces dix dernières années dont l’objet d’étude concerne l’usage de vidéos 360°. Ces dernières constituent un nouvel outil technologique au service d’objectifs d’optimisation de la performance sportive. Elles rejoignent ainsi la vidéo 2D, le chronomètre ou le capteur de fréquence cardiaque traditionnellement utilisé dans l’entraînement. La plupart des études révèlent qu’elles peuvent constituer des outils bénéfiques d’accompagnement des processus d’apprentissage des habiletés et de soutien de la motivation. Cette potentialité dépend des circonstances particulières de leurs usages (niveau d’expertise des athlètes, conditions de visionnage notamment). La littérature révèle que l’usage de vidéos 360° ne peut se substituer aux processus adaptatifs générés par l’exécution effective des tâches à réaliser. De plus amples recherches permettront une compréhension plus fine de l’efficacité des usages de la vidéo 360°. Les faiblesses et les menaces dévoilées dans cette SWOT constituent des éléments à considérer pour améliorer l’usage de la vidéo 360° dans une perspective de soutien aux processus d’apprentissages en éducation physique et en sport.This SWOT analysis investigates scientific studies conducted over the last ten years on the use of 360° videos in physical education and sport. 360° videos represent a generally new technological tool for optimizing sports performance. It offers usefulness to accompany other traditional tools such as 2D video, stopwatch, or heart rate sensor to enhance training. Most studies show that 360° video can be a beneficial tool for supporting the process of learning motor skills and increasing motivation. These potential benefits depend on the particular circumstances in which such videos are used (e.g., athletes’ level of expertise, viewing conditions, etc.). The literature shows, if proof was needed, that the use of 360° videos cannot replace the adaptive processes generated by the actual execution of the physical or sport task performed. Further research is needed to provide a more detailed understanding of the effectiveness of 360° video use. The weaknesses and threats revealed in this SWOT analysis are elements to be considered to improve the use of this original representation in a perspective of support for learning processes in education and sport
Early mobilization in patients on venoarterial extracorporeal membrane oxygenation: A scoping review
BACKGROUNDExtracorporeal membrane oxygenation (ECMO) is mainly applied to patients with significant cardiorespiratory failure who do not respond to existing conventional treatments. Patients that are supported with veno-arterial ECMO (VA-ECMO) are considered very-high risk patients to participate in any type of physical therapy (PT) or mobilization. However, cumulative evidence suggests that early mobilization of critically ill patients is feasible, safe, and efficient under certain circumstances.AIMTo summarize the existing evidence on the impact of early mobilization and physiotherapy on VA-ECMO patients.METHODSThis is a scoping review that used systematic electronic literature searches (from inception until January 2025) on MEDLINE (PubMed), PEDro, DynaMed, CINAHL, Scopus, Science direct and Hellenic Academic Libraries. Snowball searching method was also applied. Eligible studies included those reporting patients on VA-ECMO who participated in early mobilization or PT, published in English and utilized any primary evidence study design. Studies on children, animals and patients placed on any other ECMO, secondary evidence, and ‘grey’ literature were excluded.RESULTSA total of 316 articles were retrieved and 13 were included in the study. Of those, 1 study was a randomized control trial, 4 retrospective studies, 4 retrospective cohort studies, 1 case series and 3 case reports. The sample size of the included studies ranged from 1 to 104 VA-ECMO patients, who were ambulated or received PT interventions, and mobilization frequency ranged from 2 per day to 4 per week. Mobilization of VA-ECMO patients seems to be safe regardless the cannula’s position. PT and early mobilization were associated with better weaning from mechanical ventilation, gradual reduction of inotropes and functional capacity improvement after ECMO discharge.CONCLUSIONEarly mobilization in VA-ECMO seems to be safe and can potentially help reduce vasoconstrictors and speed up rehabilitation times. High quality research on early mobilization in VA-ECMO patients is warranted

Adult ADHD in Scotland: a human rights issue
Thousands of people across Scotland are currently being denied access to ADHD assessment, treatment and care. In some parts of Scotland, waiting lists for adult ADHD assessment now exceed 17 years. Several NHS health boards have paused new referrals altogether or only provide a pathway for assessment when there is evidence of significant co-occurring mental health issues, leaving 30% of the population without access to support. There is no consistent national pathway for assessment or treatment, creating a postcode lottery in access to services. Access to medication for those diagnosed privately is inconsistent, with unclear guidance and local policies that contradict national advice. Communication from and between local health boards, NHS Scotland and the Scottish Government has been poor, leaving many with ADHD unclear on how - or even if - they can access help.This report explores why this matters and what needs to change. It outlines how current legislative, policy and practice decisions are having a profound impact on the rights and wellbeing of people with ADHD in Scotland

The Impact on Redeployed Nurses Working in Critical Care During the COVID‐19 Pandemic: A Cross‐Sectional Study
BackgroundMany nurses with little critical care experience were redeployed to critical care units during the COVID-19 pandemic to assist with the increased numbers of critically ill patients. The impact of this redeployment on nurses and their employing organization merits detailed assessment.AimsTo (a) measure the impact on redeployed nurses of working in critical care during the COVID-19 pandemic and identify the predictors of that impact, (b) identify any differences between redeployed and critical care nurses and (c) measure the organizational impact.Study DesignA cross-sectional study of redeployed (n = 200) and critical care nurses (n = 461) within the United Kingdom's National Health Service between January 2021 and March 2022. A survey measured components of the Job Demand-Resources Model of occupational stress. Free text questions enabled nurses to describe their experiences of being redeployed to critical care during the pandemic.ResultsSurvey data indicated high levels of health impairment; 70% of redeployed nurses met the threshold for psychological distress, 52% for burnout and 35% had clinically significant symptoms of posttraumatic stress. When job demands (emotional load, mental load, pace and amount of work and role conflict) were high, health impairment was worse and when job resources (staffing, focus on well-being and learning opportunities) were low, work engagement was reduced. Free text comments illustrated both the stress and distress experienced by redeployed nurses.ConclusionMany redeployed nurses experienced significant negative consequences and potentially enduring sequelae of working in critical care during the pandemic. These may continue to affect individual and organizational outcomes.Relevance to Clinical PracticeNurses' well-being should be monitored, and appropriate services provided. Improvements in ongoing and meaningful communications with senior management alongside prioritization of ongoing professional development are required
Blending the ‘craft’ and ‘science’ of policing: challenges and opportunities for police as professional learning organizations
Police organizations in many liberal democracies are in crisis, prompting calls for cultural and organizational reforms within policing. Central to some of these debates is the ongoing push for police professionalization through academization. This paper offers a critical analysis of the structures underpinning efforts to professionalize policing. We draw on data from two PhD studies, which investigated police learning in Scotland, Sweden, and Finland between 2018 and 2022. The foundational elements needed to foster learning innovations and the ongoing process of professionalization are discussed. Senge’s theory of the learning organization is used to better understand this stabilizing support structure. We argue that a balance between the ‘craft’ and ‘science’ within policing is essential for meaningful professionalization to occur. Without this, officers may become alienated from the organization and fail to develop the practical wisdom that may help to address police crises
Beyond the Pandemic: A Cross-Sectional Study of Hematology Cancer Patients’ Unmet Needs and Experiences of Cancer Care in Ireland and the UK
ObjectivesThe rising incidence of hematological cancers, novel treatments, and increased survivorship highlight the need to understand patient needs postpandemic. This study explored the experiences of individuals affected by hematological cancers in Ireland and the UK as COVID-19 restrictions lifted.MethodsA cross-sectional descriptive survey was conducted from March 2023 to March 2024 using validated measures of distress, resilience, and unmet needs. Adults with hematological cancers were recruited online through cancer organizations and networks across Ireland and the UK. Quantitative data were analyzed using descriptive and inferential statistics, and qualitative data through thematic analysis.ResultsNinety-two individuals completed the survey. The top unmet needs were related to tiredness, concerns about appearance, and managing others’ expectations to return to “normal.” Many continued to feel the pandemic’s impact years later. Minimal differences in unmet needs were observed between countries, with participants reporting low distress and moderately high resilience. Thematic analysis of text responses identified three key themes: “psychological impact and self-care methods,” “navigating changing cancer care delivery,” and “sourcing social support,” reflecting the complexities of living with and beyond hematological cancers.ConclusionsPeople with hematological cancers in Ireland and the UK continue to experience distress despite moderate resilience, indicating the lasting impact of the pandemic.Implications for Nursing PracticePersistent unmet needs around fatigue, appearance, and societal expectations remain. The identified themes underscore the need for supportive interventions focusing on psychological coping strategies, continuity in cancer care delivery, and social support to enhance the well-being of individuals living with and beyond hematological cancers
