    Innovation readiness in public sector service delivery: An exploration

    We set out in this study to examine the mechanisms (specific knowledge management practices) required to operationalize service innovation readiness within public sector service delivery. Data is obtained from 150 service delivery managers drawn from public sector service organizations within the emirate of Sharjah. Sampling is undertaking utilizing a 38 factor knowledge management-focused service innovation readiness questionnaire developed from the literature. Analysis of the data is via variable ranking and multidimensional scaling (MDS). The findings of the study suggest the existence of four knowledge management-based imperatives which are construed as managerial mechanisms for public sector service innovation readiness. These are (i) ‘Knowledge core competence’ (knowledge as a key strategic asset) (ii) ‘Organizational structure’ (internal patterns of organizational communication, authority and relationships) (iii) ‘Responsiveness to change’ (propensity to engage in both purposeful and timely behavioral change in response to modulating stimuli), and (iv) ‘Innovativeness’ (relative earliness in adopting innovation). The findings lead to a service innovation readiness typology which is oriented towards ensuring that public sector entities are able to effectively and efficiently deliver innovative services that meet both operational mandates and national visions of service expectations

    CSP2023: 307 Rehabilitation of brachial plexus injury in contact sport: Where are the data that underpin clinical management? A systematic review

    Purpose: There is no specific framework for the clinical management of sports related brachial plexus injuries. Necessarily, rehabilitation is based on injury presentation and clinical diagnostics but it is unclear what the underlying evidence base to inform rehabilitative management is.Methods: A systematic review of the literature was undertaken in line with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. We applied the PEO criteria to inform our search strategy to find articles that reported the rehabilitative management of brachial plexus injuries sustained while playing contact sports. An electronic search of Medline, CINAHL, SPORTDiscus and Web of Science from inception to 3rd November 2022 was conducted. MESH terms and Boolean operators were employed. We applied an English language restriction but no other filters. Manual searches of Google Scholar and citation searching of included manuscripts were also completed. All study types were considered for inclusion provided they were published as peer-reviewed primary research articles and contained relevant information. Two investigators independently carried out the searches, screened by title, abstract and full text. Two researchers independently extracted the data from included articles. Data was cross-checked by a third researcher to ensure consistency. To assess internal validity and risk of bias, the Joanna Briggs Institute (JBI) critical appraisal tools were utilised.Results: The search generated 88 articles. Following removal of duplicates, 43 papers were evaluated against the eligibility criteria. Nine were eligible for full text review, with the majority of exclusions being expert opinion articles. Eight case reports were included. One article reported three individuals, resulting in data for ten athletes. The mean age was 19.8 years (±4.09). Injuries occurred in five American football players, two wrestlers, two rugby players, and a basketball player. No two studies applied the same diagnostic terminology and the severity of injury varied widely. Burning pain and altered sensation was the most commonly reported symptom, alongside motor weakness in the upper limb. Clinical presentation and management differed by injury pattern. Traction injuries caused biceps motor weakness and atrophy of the deltoid region, whereas compression injuries led to rotator cuff weakness. In all cases treatment was separated into acute and rehabilitative management phases, however the time frames related to these differed. Acute interventions varied but essentially entailed soft tissue inflammation management. Rehabilitation approaches variously included strengthening of shoulder complex and cervical musculature. Return-to-play criteria was opaque. The methodological quality of the case reports was acceptable. Four met all nine of the JBI evaluation criteria, and a further three met at least 75% of items.Conclusion(s): There is a distinct lack of evidence supporting rehabilitation management of sports related brachial plexus injury. Through systematic review we found only eight reports, representing ten individual case studies. No trials, cohort studies, or even retrospective registry-based studies are available to inform clinical management, which, necessarily, is driven by expert opinion and application of basic rehabilitation principles.Impact: Primary data as to rehabilitation following sports brachial plexus injury remains unpublished or unavailable to the scientific community and practising therapists, further research and data is urgently needed

    Implementing digital respiratory technologies for people with respiratory conditions: A protocol for a scoping review

    The value of ‘data-enabled’, digital healthcare is evolving rapidly, as demonstrated in the COVID-19 pandemic, and its successful implementation remains complex and challenging. Harmonisation (within/between healthcare systems) of infrastructure and implementation strategies has the potential to promote safe, equitable and accessible digital healthcare, but guidance for implementation is lacking. Using respiratory technologies as an example, our scoping review process will capture and review the published research between 12th December 2013 to 12th December 2023. Following standard methodology (Arksey and O’Malley), we will search for studies published in ten databases: MEDLINE, EMBASE, CINAHL, PsycINFO, Cochrane Library, Web of Science, Scopus, IEEE Xplore, CABI Global Health, and WHO Medicus. Our search strategy will use the terms: digital health, respiratory conditions, and implementation. Using Covidence, screening of abstracts and full texts will be undertaken by two independent reviewers, with conflicts resolved by a third reviewer. Data will be extracted into a pilot-tested data extraction table for charting, summarising and reporting the results. We will conduct stakeholder meetings throughout to discuss the themes emerging from implementation studies and support interpretation of findings in the light of their experience within their own networks and organisations. The findings will inform the future work within the ERS CONNECT clinical research collaboration and contribute to policy statements to promote a harmonised framework for digital transformation of respiratory healthcare

    A bidirectional Siamese recurrent neural network for accurate gait recognition using body landmarks

    Gait recognition is a significant biometric technique for person identification, particularly in scenarios where other physiological biometrics are impractical or ineffective. In this paper, we address the challenges associated with gait recognition and present a novel approach to improve its accuracy and reliability. The proposed method leverages advanced techniques, including sequential gait landmarks obtained through the Mediapipe pose estimation model, Procrustes analysis for alignment, and a Siamese biGRU-dualStack Neural Network architecture for capturing temporal dependencies. Extensive experiments were conducted on large-scale cross-view datasets to demonstrate the effectiveness of the approach, achieving high recognition accuracy compared to other models. The model demonstrated accuracies of 95.7%, 94.44%, 87.71%, and 86.6% on CASIA-B, SZU RGB-D, OU-MVLP, and Gait3Ddatasets respectively. The results highlight the potential applications of the proposed method in various practical domains, indicating its significant contribution to the field of gait recognition

    Exploring the role of the clinical decision support system in delivering multimodal interventions for delirium care in ICU adults: a scoping review protocol

    Delirium is a frequently reported significant complication in acute and critically ill patients. Multimodal interventions can potentially decrease delirium incidence and severity by reducing predisposing factors. Previous research has documented the efficacy of delirium assessment and management interventions. However, the heterogeneity and complexity of these multimodal interventions make them challenging to disseminate and integrate into clinical practice. To support nurses in decision-making for delirium care, a clinical decision support system (CDSS) can increase situational awareness by reducing the cognitive load and facilitating relevant clinical information gathering. It can have a positive impact on nursing efficacy as well as on patient outcomes. This scoping review aims to map the existing literature on the use of CDSS to deliver multimodal interventions in delirium care for ICU adults, explore how it has been applied in this context, and identify gaps in the existing literature

    Financial Fair Play and the Court of Arbitration for Sport

    This article examines four major Financial Fair Play (FFP) cases that have come before the Court of Arbitration for Sport (CAS). Although significant previous work has addressed FFP in European football, there is a major gap where its treatment at the CAS is concerned. In addition to creating substantial holes in the effectiveness of FFP regulations, the progression of the four cases discussed have let some of FFP’s most egregious offenders avoid sanctions they would consider damaging. In considering two especially important cases, this article argues that Paris Saint-Germain and Manchester City have provided ample guidance for transforming mediocre clubs through disguised equity infusions to circumvent FFP until a sustainable revenue structure is created. For the CAS, the case line resulting in major FFP offending clubs escaping meaningful sanctions builds upon a body of anti-doping jurisprudence that has had the effect of shifting the cost-benefit analysis of rule breaking. This failure in both the FFP and CAS realms leads into a larger emergent policy reform conversation

    The neonatal gut microbiota: A role in the encephalopathy of prematurity

    Preterm birth correlates with brain dysmaturation and neurocognitive impairment. The gut microbiome associates with behavioral outcomes in typical development, but its relationship with neurodevelopment in preterm infants is unknown. We characterize fecal microbiome in a cohort of 147 neonates enriched for very preterm birth using 16S-based and shotgun metagenomic sequencing. Delivery mode strongly correlates with the preterm microbiome shortly after birth. Low birth gestational age, infant sex assigned at birth, and antibiotics associate with microbiome composition at neonatal intensive care unit discharge. We integrate these data with term-equivalent structural and diffusion brain MRI. Bacterial community composition associates with MRI features of encephalopathy of prematurity. Particularly, abundances of Escherichia coli and Klebsiella spp. correlate with microstructural parameters in deep and cortical gray matter. Metagenome functional capacity analyses suggest that these bacteria may interact with brain microstructure via tryptophan and propionate metabolism. This study indicates that the gut microbiome associates with brain development following preterm birth
