What are the predominant parameters for Down's syndrome risk estimation in first- trimester screening: a data mining study
Objectives: This study aimed to evaluate the effect size of each parameter used in the first trimester Down Syndrome (DS) risk analyses by using multiple regression analysis techniques. Methods: This data mining study included data of 44,260 pregnant women screened at the Acibadem Labmed laboratories from 2010 to 2019. In this study, risk was calculated using the PRISCA software on the basis of nuchal translucency (NT), crown-rump length measurement, in vitro fertilization application, diabetes mellitus, Down Syndrome story, smoking, maternal age, and the level of maternal serum biochemistry markers including pregnancy-associated plasma protein-A (PAPP-A) and free beta-human chorionic gonadotropin (hCG beta). Results: Forty-four thousand two hundred sixty risk analysis patients result data were re-investigate, and 851 (1.93\%) risk analysis results were found as positive. PAPP-A 747 (CI\%, 476-1,170) times, NT value 512 (CI\%, 343-764) times, DS story 21 times (CI\%, 6.7-63.2) and hCG beta value 7.01 (CI\%, 6.31-7.79) times affect the combined first-trimester risk analysis results. Conclusions: We have suggested that those accurate PAPP-A levels and NT levels evaluation are the most critical point of combined risk analysis and that the risk of free hCG beta levels after PAPP-A is essential as a biochemical test.6DEC 28704-7094
Dupilumab combined with a short-term narrowband-ultraviolet B phototherapy in a pediatric case of severe atopic dermatitis
Recently, dupilumab treatment in atopic dermatitis (AD) has been an effective and safe option in terms of adverse effects. However, combination therapies can sometimes be better at increasing efficacy or reducing adverse effects or cumulative doses. Early combination therapies may be more rational, especially considering that dupilumab treatment alone is less effective and has a late-onset efficacy. Oxamate targeting aggressive cancers with special emphasis to brain tumors
Cancer is one of the main causes of human mortality and brain tumors, including invasive pituitary adenomas, medulloblastomas and glioblastomas are common brain malignancies with poor prognosis. Therefore, the development of innovative management strategies for refractory cancers and brain tumors is important. In states of mitochondrial dysfunction - commonly encountered in malignant cells - cells mostly shift to anaerobic glycolysis by increasing the expression of LDHA (Lactate Dehydrogenase-A) gene. Oxamate, an isosteric form of pyruvate, blocks LDHA activity by competing with pyruvate. By blocking LDHA, it inhibits protumorigenic cascades and also induces ROS (reactive oxygen species)-induced mitochondrial apoptosis of cancer cells. In preclinical studies, oxamate blocked the growth of invasive pituitary adenomas, medulloblastomas and glioblastomas. Oxamate also increases temozolomide and radiotherapy sensitivity of glioblastomas. Oxamate is highly polar, which may preclude its clinical utilization due to low penetrance through cell membranes. However, this obstacle could be overcome with nanoliposomes. Moreover, different oxamate analogs were developed which inhibit LDHC4, an enzyme also involved in cancer progression and germ cell physiology. Lastly, phenformin, an antidiabetic agent, exerts anticancer effects via complex I inhibition in the mitochondria and leading the overproduction of ROS. Oxamate combination with phenformin reduces the lactic acidosis-causing side effect of phenformin while inducing synergistic anticancer efficacy. In sum, oxamate as a single agent and more efficiently with phenformin has high potential to slow the progression of aggressive cancers with special emphasis to brain tumors.MAR14
Defining mitochondrial protein functions through deep multiomic profiling
Mitochondria are epicentres of eukaryotic metabolism and bioenergetics. Pioneering efforts in recent decades have established the core protein componentry of these organelles(1) and have linked their dysfunction to more than 150 distinct disorders(2,3). Still, hundreds of mitochondrial proteins lack clear functions(4), and the underlying genetic basis for approximately 40\% of mitochondrial disorders remains unresolved(5). Here, to establish a more complete functional compendium of human mitochondrial proteins, we profiled more than 200 CRISPR-mediated HAP1 cell knockout lines using mass spectrometry-based multiomics analyses. This effort generated approximately 8.3 million distinct biomolecule measurements, providing a deep survey of the cellular responses to mitochondrial perturbations and laying a foundation for mechanistic investigations into protein function. Guided by these data, we discovered that PIGY upstream open reading frame (PYURF) is an S-adenosylmethionine-dependent methyltransferase chaperone that supports both complex I assembly and coenzyme Q biosynthesis and is disrupted in a previously unresolved multisystemic mitochondrial disorder. We further linked the putative zinc transporter SLC30A9 to mitochondrial ribosomes and OxPhos integrity and established RAB5IF as the second gene harbouring pathogenic variants that cause cerebrofaciothoracic dysplasia. Our data, which can be explored through the interactive online MITOMICS.app resource, suggest biological roles for many other orphan mitochondrial proteins that still lack robust functional characterization and define a rich cell signature of mitochondrial dysfunction that can support the genetic diagnosis of mitochondrial diseases.7913JUN 9382+60

Comparison of Laparascopic and Open Adrenalectomy
ABSTRACT
Purpose: Laparoscopic adrenalectomy, which was performed successfully for the first time in 1992, has become the preferred method
in adrenal surgery especially in adrenal diseases. In this study, our aim is to compare the demographic and operative data of laparoscopic
and open adrenalectomies performed in the general surgery clinic of our hospital.
Methods: The records of the patients who were operated in hospital due to a surrenal mass between August 2006 and December 2012
have been retrospectively assessed. Demographic characteristics of the patients, such as age, sex, the size and location of tumors were
included. Surgical indications were classified in terms of their radiologic and biochemical investigations. Type of the surgery (lateral
transperitoneal laparoscopic or open approach), the incision used in these surgeries (midline, subcoastal and paramedian), the length of
operation, the length of hospital stay, postoperative complications and the final pathology results were all examined.
Results: During the afformentioned period, adrenalectomy was performed in 106 patients. Eighty of these patients (%75,4) were
operated by open and 26 of them (%24,6) by lateral transperitoneal laparoscopic surgery. Subcostal incision was used 61 of 80 patients
(% 82,4), whereas 17 patients (% 22,9) were operated by midline incision and 2(% 2,7) were by paramedian incision. No mortality
was encountered. Operation time was statistically significantly shorter in the open group (p<0,0166). Duration of hospital stay was
statistically significantly longer in open group (p<0,0083). Hospital stay time was statistically significantly longer in midline incision
group (p<0,0083). Except for the weight of masses, statistically significant difference was not found between two groups (laparascopic
and open surgery) in demographic and operative features (age,gender,side,pathological diagnose,size of mass) releated to postoperative
complications.
Conclusion: In carefully selected group of patients laparoscopic adrenalectomy offered lesser postoperative hospital stay than the open
adrenalectomy. The longer operative time can be decreased when more experience is gained in laparascopic adrenalectomy. Laparoscopic
adrenalectomy should be preferred in adrenal disease as the gold standard in centers with completed learning curve

Easily accessible, up-to-date and standardised training model in Urology: E-Learning Residency training programme (ERTP)
Objectives There is no standardised and up-to-date education model for urology residents in our country. We aimed to describe our National E-learning education model for urology residents. Methodology The ERTP working groupconsisting of urologists was established by the Society of Urological Surgery to create E-learning model and curriculum in April 2018. Learning objectives were set up in order to determine and standardise the contents of the presentations. In accordance with the Bloom Taxonomy, 834 learning objectives were created for a total of 90 lectures (18 lectures for each PGY year). Totally 90 videos were shot by specialised instructors and webcasts were prepared. Webcasts were posted at uropedia.com.tr, which is the web library of the Society of Urological Surgery. The satisfaction of residents and instructors was evaluated with feedbacks. An assessment of knowledge was measured with the multiple-choice exam. Results A total of 43 centres and 250 urology residents were included in ERTP during the academic year 2018/2019. There were 93/55/43/34/25 urology residents at 1st/2nd/3rd/4th and 5th year of residency, respectively. Majority of the residents (99.1\%) completed the ERTP. The overall satisfaction rate of residents and instructors were 4.29 and 4.67 (min: 1 so bad, max: 5 so good). An assessment exam was performed to urology residents at the end of the ERTP and the mean score was calculated as 57.99 points (min: 20, max: 82). Conclusion As a result of the COVID-19 pandemic, most of the educational programmes had to move online platforms. We used this reliable and easily accessible e-learning platform for the standardisation of training in urology on national basis. We aim to share this model with international residency training programmes.3MAR7
