    Introduction: Based on the concept of third-party disability family members of people with aphasia experience limitations in many areas of life and need support. Specific support is rare in Germany. Self-help groups offer a suitable intervention as they support coping. This is the connection point of this dissertation, which is part of the shalk-project (self-help groups in aphasia to enhance quality of life and competence). Aim: The presented study pursues the description of the relatives’ individual situation and their quality of life. The design of a specific measure for relatives, which is based on the needs of the target group, is presented. At least, its impact on the quality of life and the individual situation of the relatives is shown. Material and Methods: According to the mixed-methods-approach the relatives’ individual situation and their quality of life was assessed at three points in time via questionnaires and semi-structured qualitative interviews. In addition, non-recurring focus groups were conducted to evaluate the measure. Data was analyzed using descriptive statistics and qualitative content analysis. Results: A reduced quality of life exists in 50% of the examined family members in comparison to their age cohort of the WHOQOL-100. They report various restrictions due to their relative’s aphasia. The designed measure provides opportunities and challenges in its realization. Relatives have a positive attitude towards the measure. Significant changes in the quality of life cannot be determined. Discussion: The data collected on the individual situation supports the concept of third-party disability and expands it with the aspect of individual resilience. Although the measure does not lead to any significant changes in quality of life, the relatives describe it as positive for dealing with the situation, which can also be attributed to resilience. There are opportunities and challenges in the implementation, and these should be investigated and addressed in more detail. The study carried out is of high methodological quality. Conclusion: Because of their third-party disability family members need support, that addresses the empowerment of resilience to handle the situation. Shalk offers such a support. A modified shalk-intervention, which considers the reported opportunities and challenges, can be implemented by multiplicators in the health care sector
