    Three-Dimensional Speckle-Tracking Echocardiography-Derived Left Ventricular Global Longitudinal Strain and Mitral Annular Plane Systolic Excursion Are Associated in Healthy Adults—Insights from the MAGYAR-Healthy Study

    Introduction: Mitral annular plane systolic excursion (MAPSE) is a quantitative feature of left ventricular (LV) function that can be easily measured by M-mode echocardiography. The more recent LV strains represent LV contractility, and they can be determined for the three main directions in the radial (RS), longitudinal (LS), and circumferential (CS) directions. Three-dimensional (3D) speckle-tracking echocardiography (3DSTE) seems to be ideal for the simultaneous assessment of all LV strains from the same acquired 3D dataset. Their associations, however, have not been fully analyzed in clinical settings in healthy volunteers yet. Therefore, the present study aimed to find associations between MAPSE and 3DSTE-derived LV strains in healthy circumstances. Methods: The present study consisted of 106 healthy adult volunteers (mean age 28.1 ± 6.3 years, 48 men). Complete two-dimensional Doppler echocardiography with MAPSE assessment and 3DSTE-derived LV strain measurements were performed in all cases. Results: Tendentiously reduced LV volumes with preserved LV ejection fraction were present in the case of mean MAPSE as compared to decreased/increased MAPSE. Global and regional LV-RS and LV-CS did not differ if MAPSE was increased, mean or decreased. Global LV-LS was reduced in the case of decreased MAPSE, mainly due to tendentiously higher apical LV-LS. MAPSE did not differ if global LV-RS and LV-CS were increased, mean or decreased. MAPSE was tendentiously increased in the presence of increased global LV-LS. Conclusions: Only 3DSTE-derived global LV-LS and MAPSE are associated in healthy adults; LV global strains in other directions show no relationship with MAPSE

    The Role of Natural Deep Eutectic Solvents in a Hydrogel Formulation Containing Lidocaine

    Background/Objectives: This study investigates the use of natural deep eutectic solvents (NADESs) in enhancing the solubility and skin permeation of a lidocaine base, a lipophilic form, in hydrogel systems. The aim was to develop an environmentally sustainable and biocompatible alternative to conventional lidocaine formulations, improving the dermal permeation and therapeutic efficacy. Methods: The lidocaine base was dissolved in a hydrophilic NADES system composed of choline chloride and citric acid, facilitating enhanced solubility, likely through new molecular interactions. Then, pH-adjusted hydrogels were formulated and optimized by employing a 32 full factorial design. Raman and nuclear magnetic resonance (NMR) spectroscopy were applied to evaluate the stability of lidocaine in the optimal formulation. The biopharmaceutical properties were investigated using in vitro drug release and skin permeation studies. In vivo tests assessed physiological skin parameters such as the hydration and transepidermal water loss. Results: The developed NADES-containing hydrogel significantly improved the solubility and stability of lidocaine. Skin permeation studies demonstrated enhanced dermal permeation compared with conventional hydrogel and ointment. These improvements, namely the enhanced solubility of lidocaine in the formulation and its increased permeation, were attributed to the dual effect of the NADES. Conclusions: NADES-containing hydrogels represent a promising green technology for formulating lidocaine-containing dermal preparations. This approach offers a biocompatible, natural-based alternative that can enhance the bioavailability and efficacy of topical anesthetics

    PICASO Set Operator for Computational Nephropathology

    BACKGROUND: The advent of digital nephropathology offers the potential to integrate deep learning algorithms into the diagnostic workflow. We introduce PICASO, a novel permutation-invariant set operator to dynamically aggregate histopathologic features from instances. We applied PICASO to two nephropathology scenarios: detecting active crescent lesions in sets of glomerular crops with IgA nephropathy and case-level classification for antibody-mediated rejection (AMR) in kidney transplant.; METHODS: PICASO is a Transformer-based set operator that aggregates features from sets of instances to make predictions. It utilizes initial Histopathologic Vectors as a static memory component and continuously updates them based on input embeddings. For active crescent detection in IgA nephropathy cases, we obtained 6206 Periodic acid-Schiff-stained (PAS) glomerular crops (5792 no Active Crescent, 414 Active Crescent) from three different health institutes. For the AMR classification, we have 1655 PAS glomerular crops (769 AMR and 886 Non-AMR images) from 89 biopsies. The performance of PICASO as a set operator was compared with other set operators such as DeepSet, Set Transformer, DeepSet++, and Set Transformer++ using metrics including area under the receiver operating characteristic curves (AUROC), area under the precision-recall curves (AUPR), recall, and accuracy.; RESULTS: PICASO achieved superior performance in detecting active crescent in IgA nephropathy cases, with an AUROC of 0.99 (95% confidence interval, 0.98 to 0.99) on internal validation and 0.96 (95% confidence interval, 0.95 to 0.98) on external validation, significantly outperforming other set operators (P<0.001). It also attained the highest AUROC of 0.97 (95% confidence interval, 0.90 to 1.0, P=0.02) for case-level AMR classification. The AUPR, recall, and accuracy scores were also higher when using PICASO, and it significantly outperformed baselines (P<0.001).; CONCLUSIONS: PICASO can potentially advance nephropathology by improving performance through dynamic feature aggregation.
