Anhedonia symptoms: The assessment of brain functional mechanism following music stimuli using functional magnetic resonance imaging
Purpose: This study aimed to investigate the effect of music stimulation on the brain functional mechanism of depressed patients with anhedonia symptoms using functional magnetic resonance imaging (fMRI). Methods: Participants in this study included 20 healthy subjects as the control group, 25 subjects with depression and no anhedonia as the intervention group A, and 24 subjects with depression and anhedonia as the inter-vention group B. The safely emotional stimulation was done by Iranian music. To investigate the effect of music therapy on the brain, a task including 50 tracks of 12 s Iranian music (traditional and pop) was used. Finally, the data were analyzed using SPM Toolbox in MATLAB software. Results: The results showed that brain patterns in depressed patients with and without anhedonia could be distinguished based on positive and negative musical stimuli (p < 0.05). Important fMRI biomarker such as effective connectivity strength related to the fronto-limbic network, including the supragenual ACC, subgenual ACC, AMYG, and FFG were evaluated in depressed patients with anhedonia. Conclusion: This was the first study to investigate the neural circuits involved in music-related emotional pro-cessing in patients with anhedonia symptoms. These findings could help advance neurological understandings of anhedonia and suggest new treatments
Prevalence of renal scaring caused by urinary tract infections in children: a systematic review and meta-analysis
PURPOSE. SOURCES OF INFORMATION: Urinary tract infection (UTI) is the second most common infection in children, the most important complication of which is renal scarring. The aim of present study was to evaluate the prevalence of renal scarring after UTI in children through systematic review and meta-analysis. METHODS: The international databases of Science Direct, PubMed, Scopus, and Web of Science and the Google Scholar search engine were searched using standard keywords. The sources found were from 2010 to 2020 and the search stage was updated until 2021.02.16. Data were analyzed using STATA-14 software and the significance level was considered at P<0.05. RESULTS: In 29 studies with a sample size of 9,986 children, the prevalence of renal scarring in children was estimated at 35 (95 CI: 29-41). Also, the prevalence of renal scarring was in girls 61 (95 CI: 40-81) and in boys 34 (95 CI: 11-57). The prevalence of unilateral renal scarring in children was 56 (95 CI: 48-65) and bilateral renal scarring was 31 (95 CI: 14-48). In addition, the prevalence of scar was 54 in children with reflux and 12 in children without vesicoureteral reflux. CONCLUSIONS: More than one third of people under the age of 18 after UTI have renal scarring. The prevalence of this complication in girls is about 2 times higher than that in boys and in people with reflux, it is about 4 times higher than people who do not have urinary reflux. Also, about half of people under the age of 18 suffer from unilateral renal scarring and about one third of them suffer from bilateral renal scarring
Molecular epidemiology and recycling of Staphylococcus aureus resistant to methicillin among the staff, patients, and surfaces in university Hospital in West Iran, Ilam
Staphylococcus aureus is a human pathogen causing nosocomial infections and increased hospitalization and mortality among human communities. Methicillin-resistant S. aureus strains are considered a severe threat in nosocomial infections and cause complications in the remedy process of bacterial infections. In this study, 137 samples were collected from different departments, staff, and patients in Ilam hospital. Eighty-eight samples of these strains were examined to test antibiotic resistance and diffusion. MIC (minimum inhibitory concentration) and PCR (polymerase chain reaction) were performed on the samples resistant to oxacillin. 36 (40.9) strains were MRSA, and 52 (59.1) isolates were MSSA. 44.4 of MRSA strains with IV SCCmec type. Fourteen different spa types were found using spa typing, of which the most abundant types were t037, t030, and t701, and three new types, including t15471, t15474, and t17470, were identified among the strains. The molecular analysis by MLST showed that the strains are classified into 11 different sequence types. Sequence type 239 and clonal complexes of 329 and 22 were dominant. ST239-spat037-SCCmec III was also identified as the most frequent clone of MRSA. The most identified clones were MRSA ST239-spa t037-SCCmec III. The results show the spa-type distribution between samples of patients, personnel, and surfaces, demonstrating MRSA circulation between patients and the environment. Economic evaluation of laboratory diagnostic test types in Covid-19 epidemic: A systematic review
BACKGROUND: Corona 2 virus (SARS-CoV-2) is known as the causative agent of COVID-19 disease; the World Health Organization (WHO) declared it an epidemic on March 11, 2020. The Joint Guidelines of the Centers for Disease Control and Prevention (CDC) and the WHO including social distancing, the use of face masks, emphasis on hand washing, quarantine, and using diagnosis tests have been used widely, but the value of diagnostic interventions to prevent the transmission of SARS-CoV-2 is unclear. We compared the economic evaluation of different laboratory diagnostic interventions with each other and also with implementing the conservative CDC & WHO guidelines. MATERIAL AND METHODS: Electronic searches were conducted on PubMed, Embase, Science Direct, Scopus, Cochrane Library, Web of Knowledge, NHSEED, NHS Health Technology assessment (CRD), and Cost-Effectiveness Analysis Registry databases. Related articles were reviewed from January 2020 to the end of November 2021. RESULTS: Out of 1791 initial studies, 13 articles had the inclusion criteria. According to the Consolidated Health Economic Evaluation Reporting Standards (CHEERS) checklist, ten studies were of excellent quality, and the remaining two studies were of very good quality. Most studies were cost-effectiveness analysis studies. The entered studies had different time horizons. Diagnostic tests reviewed in the studies included real-time polymerase chain reaction (RT-PCR) test, immunoglobulin G (IgG) & Antigen, point of care tests. Although polymerase chain reaction (PCR) testing improves the quality of life and survival for patients with infected Covid-19 based on its greater effectiveness compared to standard protection protocols, due to the high cost of this intervention, it has been considered a cost-effective method in some countries. CONCLUSION: Since most studies have been conducted in developed countries, it unquestionably does not make sense to extend these results to low-income and developing countries. Therefore further studies are required in low-income and developing countries to evaluate the cost-effectiveness of laboratory-based diagnostic methods (RT-PCR) of covid-19 in variable prevalence of infectious cases
The prevalence of gonococcal and non-gonococcal infections in women referred to obstetrics and gynecology clinics
Bacterial vaginosis is a condition caused by changes in the vaginal microbial ecosystem and increases the risk of preterm delivery, premature rupture of membranes, endometritis, and weight loss of the baby. This study aimed to evaluate the frequency of gonococcal and non-gonococcal genital infections in women referred to clinics in Ilam, Iran. Two swab samples were taken from each patient using a sterile swab, one swab was placed in a THB medium for the culture of Streptococcus agalactiae and the other in PBS buffer for PCR. PCR method was conducted for the identification of the other bacterial agents such as Neisseria gonorrhoeae, Chlamydia trachomatis, Mycoplasma genitalium, Mycoplasma hominis, Ureaplasma urealyticum, and also S. agalactiae. Sampling was performed on 169 women with symptomatic vaginosis. The frequency of S. agalactiae by culture and PCR methods was 4.7 (8 samples) and 13.6 (23 samples) respectively. Also, 6.5 (11 samples), 3.5 (6 samples), 4.1 (7 samples), 1.2 (2 samples), and 0 of the samples were positive for N. gonorrhoeae, M. genitalium, M. hominis, U. urealyticum and C. trachomatis by PCR method. Except for a significant association between S. agalactiae colonization and abortion, there was no significant correlation between the prevalence of these bacteria and the patient's age, age of marriage, number of deliveries, and number of abortions. Overall, the prevalence of gonococcal and non-gonococcal infection in women referred to clinics in Ilam is similar to the other parts of Iran

The global burden of cancer attributable to risk factors, 2010-19: a systematic analysis for the Global Burden of Disease Study 2019
Background Understanding the magnitude of cancer burden attributable to potentially modifiable risk factors is crucial for development of effective prevention and mitigation strategies. We analysed results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019 to inform cancer control planning efforts globally. Methods The GBD 2019 comparative risk assessment framework was used to estimate cancer burden attributable to behavioural, environmental and occupational, and metabolic risk factors. A total of 82 risk-outcome pairs were included on the basis of the World Cancer Research Fund criteria. Estimated cancer deaths and disability-adjusted life-years (DALYs) in 2019 and change in these measures between 2010 and 2019 are presented. Findings Globally, in 2019, the risk factors included in this analysis accounted for 4.45 million (95 uncertainty interval 4.01-4.94) deaths and 105 million (95.0-116) DALYs for both sexes combined, representing 44.4 (41.3-48.4) of all cancer deaths and 42.0 (39.1-45.6) of all DALYs. There were 2.88 million (2.60-3.18) risk-attributable cancer deaths in males (50.6 47.8-54.1 of all male cancer deaths) and 1.58 million (1.36-1.84) risk-attributable cancer deaths in females (36.3% 32.5-41.3 of all female cancer deaths). The leading risk factors at the most detailed level globally for risk-attributable cancer deaths and DALYs in 2019 for both sexes combined were smoking, followed by alcohol use and high BMI. Risk-attributable cancer burden varied by world region and Socio-demographic Index (SDI), with smoking, unsafe sex, and alcohol use being the three leading risk factors for risk-attributable cancer DALYs in low SDI locations in 2019, whereas DALYs in high SDI locations mirrored the top three global risk factor rankings. From 2010 to 2019, global risk-attributable cancer deaths increased by 20.4% (12.6-28.4) and DALYs by 16.8% (8.8-25.0), with the greatest percentage increase in metabolic risks (34.7% 27.9-42.8 and 33.3% 25.8-42.0). Interpretation The leading risk factors contributing to global cancer burden in 2019 were behavioural, whereas metabolic risk factors saw the largest increases between 2010 and 2019. Reducing exposure to these modifiable risk factors would decrease cancer mortality and DALY rates worldwide, and policies should be tailored appropriately to local cancer risk factor burden. COVID-19-associated opportunistic infections: a snapshot on the current reports
Treatment of the novel Coronavirus Disease 2019 (COVID-19) remains a complicated challenge, especially among patients with severe disease. In recent studies, immunosuppressive therapy has shown promising results for control of the cytokine storm syndrome (CSS) in severe cases of COVID-19. However, it is well documented that immunosuppressive agents (e.g., corticosteroids and cytokine blockers) increase the risk of opportunistic infections. On the other hand, several opportunistic infections were reported in COVID-19 patients, including Aspergillus spp., Candida spp., Cryptococcus neoformans, Pneumocystis jiroveci (carinii), mucormycosis, Cytomegalovirus (CMV), Herpes simplex virus (HSV), Strongyloides stercoralis, Mycobacterium tuberculosis, and Toxoplasma gondii. This review is a snapshot about the main opportunistic infections that reported among COVID-19 patients. As such, we summarized information about the main immunosuppressive agents that were used in recent clinical trials for COVID-19 patients and the risk of opportunistic infections following these treatments. We also discussed about the main challenges regarding diagnosis and treatment of COVID-19-associated opportunistic infections (CAOIs)
The Iranian blood pressure measurement campaign, 2019: study protocol and preliminary results
Purpose Hypertension is one of the most important risk factors for premature mortality and morbidity in Iran. The objective of the Iranian blood pressure (BP) measurement campaign was to identify individuals with raised blood pressure and providing appropriate care and increase the awareness among the public and policymakers of the importance of tackling hypertension. Methods The campaign was conducted in two phases. The first (communication) phase started on May 17th (International Hypertension Day). The second phase started on June 8th, 2019, and lasted up to July 7th during which, blood pressures were measured. The target population was Iranians aged >= 30 years. Participants voluntarily referred to health houses in rural and health posts and comprehensive health centers in urban areas in the setting of the Primary Health Care network. Additionally, over 13,700 temporary stations were set up in highly visited places in urban areas. Volunteer healthcare staff interviewed the participants, measured their BP, and provided them with lifestyle advice and knowledge of the risks and consequences of high blood pressure. They referred participants to physicians in case their BP was high. Participants immediately received a text message containing the relevant advice based on their measured BP and their past history. Results Blood pressure was measured for a total of 26,678,394 participants in the campaign. A total of 13,722,148 participants (51.4) were female. The mean age was 46 +/- 14.1 years. Among total participants, 15,012,693 adults (56.3) with no past history of hypertension had normal BP, 7,959,288 participants had BP in the prehypertension range (29.8), and finally, 3,706,413 participants (13.9) had either past medical history of hypertension, used medications, or had high BP measured in the campaign. Conclusion The campaign was feasible with the objective to increase the awareness among the public and policymakers of the importance of tackling hypertension in Iran
Bioassay-guided isolation of phenolic apoptosis inducer anticancer agents from Berberis vulgaris fruits
Background: Cancer development is a multi-stage process in which apoptosis plays an important role. Apoptotic agents from natural products like phenolic compounds can be used effectively in the treatment of cancer. Berberis vulgaris L. fruits are a rich source of natural phenolic compounds and have been evaluated for their apoptotic effects. Objective: this study was carried out to evaluate the apoptotic effect of B. vulgaris fruits in normal and cancerous cells by investigating p53, Bax, and Bcl-2 genes expression and identifying an active compound. Methods: B. vulgaris fruits were extracted with ethanol using maceration method. To yield an active compound, the extract was divided into fractions and subfractions that could be evaluated using chromatography techniques, and the purified compound structure was determined by nuclear magnetic resonance (NMR). The cytotoxic activity of the selected fraction against 3T3 and MCF-7 cell lines along with its effect on the mRNA apoptosis regulating genes like p53, Bax, and Bcl-2 were investigated subsequently. Results: Results showed that B. vulgaris extract increased the mRNA level of p53 and Bax and decreased Bcl-2 mRNA level in cancer cell lines. The structure of the purified compound was determined as the 3-caffeoylquinic acid (chlorogenic acid). Conclusion: B. vulgaris fruit has in vitro antineoplastic activity by stimulating cellular apoptosis via increasing the levels of p53 and Bax proteins and decreasing Bcl-2 and comprises chlorogenic acid as one of its phenolic phytochemicals. © 2022 Institute of Medicinal Plants.
