Ureterovaginalfistel, a distal portion complication of the ureter injury - Case report
In the female complication of the distal portion of the ureter injury is the ureterovaginal fistula, seen most often after the Wertheim hysterectomy. The true incidence of the disease is difficult to ascertain in the literature. Incontinence isthe first symptom of the ureterovaginal fistula patient. An IVP, RPG, cystography, cystoscopy and double dye test investigations are to be advocated for diagnosis of the disease.Ureterovaginal fistula is repaired by standard ureteroneocystostomy, incorporating an antirefluxing submucosal tunnel.Key Words: Wertheim hysterectomy - ureterovaginal fistula - incontinence - ureteroneocystostomy - ureter injur
Hiperglikemia dan Komplikasi Akut Diabetes Mellitus
Diabetes mellitus is a chronic metabolic disorder due to relative or absolute deficiency of insulin. The disease may be characterized by acute symptoms and signs of insulin deficiency, which are called acute complications in this paper. Diabetes is also associated with chronic complications such as atherosclerosis, neuropathy, nephropathy, retinopathy, and' cataract. The paper presented a brief review of the role of hyperglycemia in the development of complic ations in the diabetic patient. There is little controversy as to the necessity of insulin for therapy and for prevention of diabetic complications. The impact of the development of science and technology on the fade of mankind
The article discusses the participation of women in science and technology, and more extensive ly in medicine. It reiterates the nature of science and technology, and explains why science cannot be absolutely objective. Furthermore, it stresses on the increasing speed in the advancement of science and its influence, both positive and negative, on man at different levels of the living systems. In addition, the article probes the menace of the cult of technology and the dangers of the dynamic autonomy of science, especially the application of basic sciences in war efforts, the information revolution and the new biology which would affect life itself, the dignity of man. and even his very existence. Several ways out are suggested in confronting the negative impact of science and technology, such as adopting a new paradigm, the strengthening of scientific ethics, the protection of the consumers, and the rehumanization of science and technology. Supravengricular tachycardia in children: The role of infectious diseases and its relationship to serum enzyme
In twenty four children an episode of supraventricular tachycardia occurred before 15 years (median age 12 months). Fourteen male and 10 female children were reviewed, of which one had a congenital heart disease. Lown-Ganong-Levine syndrome was present on surface ECG in 1/24 (4 per cent). Forty six per cent of children were undernourished as determined by the weight to height ratio according to the WHO criteria (1983), and 58 per cent of them were anemic, according to the WHO criteria (1972). All children have had fever, mostly above 38.5° C. In ten children with an SVT episode the serum enzyme level, was also studied, i. e. the serum glutamine oxalo-transaminase (SCOT), lactate dehydrogenase (LDH) and creatinine phosphokinase (CPK). Five out of 11 children had a high serum enzyme level, ranging 25-68 U/dI (mean: 40.88 ± 16.30) for GOT, 315 889 U/dI (mean: 482 ± 232.53) for LDH, and 57-581 U/d1 (mean: 251.33 ± 287.02) for CPK, instead of a normal level range of 1-19 U/c11, 80-240 U/dl and 0-50 U/dl respectively. It is concluded that the episode of SVT often occurs in children with infection, especially gastrointestinal intention, meningitis, encephalitis. bronchopneumonia and septicemia. Fever, undernutrition and anemic status are considered as precipitating factors. Preexitation syndromes, such as WPW and LGL syndromes, were infrequently found. Digitalis treatment has to be changed with other preparations which are often used such as calcium antagonist or ATP, especially for patients who do not respond to digitalis. Physicians who work in the surgery department, should be aware of an SVT episode, especially if the children have fever, undernutrition and anemia. Penulis menganggap aspek holisma sebagai hal yang.ter• penting pada pengobatan tradisional yang harus diambil oleh kedokteran modern, .diantara berbagai perbedaan yang terdapat diantara kedua system itu. Aspek ini menjadi lebih penting, karena adanya hyperspesialisasi dan modemisasi dalam kedokteran, massifikasi pelayanan kesehatan serta industrialisasi, dan terurama terasa dalam kedokteran populasi dan kedokteran pedesaan. Dengan pengharripiran holistis, pasien baik berupa individu maupun populasi dilihat sekurang.kurangnya pada tiga peringkat system hayat. Holisma juga akan melihat manusia sebagai makhluk biokultural yang berevolusi dan memperlakukan tiap-tiap pasien secara unik dan with. Holisma tak dapat tidak akan memperhatikan ethika kedokteran dan keadilan distributif dalam pelayanan kesehatan. The relationship between blood pressure and anthropometric measurements of junior high school students in the Municipality of Yogyakarta
The relationship between blood pressure and body weight, body height, and other anthropometric measurements, i, e,, upper arm circumference and fat thickness, was studied in 280 junior high school students, aged 12-17 years. Blood pressure was measured by using mercury sphygmomanometer; measurements were conducted once in the sitting position, and the subjects were considered to be normal based on the physical diagnostic examination. The results found showed that there are correlations between either systolic or diastolic blood pressure with anthropometric meas• ;urements. Since upper arm circumference indicates the muscle, bone and fat components of the body mass, further study is needed to determine which component has the highest correlation with blood pressure.Key Words: blood pressure - adolescence - anthropometry - nutrition - body mas
Gene and antibody molecule
Five major structural types of immunoglobulin in man can be distinguished. i. e. immunoglobulin G (abbreviated to IgG), IgM, IgA, IgD and IgE. Each type of immunoglobulin can synthesize specific antibody according to the inducing antigen. The specificity of immunoglobulin molecule is found in the antigen-binding fragment.Actually the specificity and the diversity of the immunoglobulin is controlled by the gene in the chromosome. Kedokteran Sebagai Ilmu Pandangan Kedokteran Sekarang Teerhadap dirinya
The author defines medicine as efforts to understand, maintain and manage health condition of human beings, as individuals as well as members of a society, in a biosocial matrix. Thus, three sets of disciplines are involved, i. e. the basic medical sciences, the clinical medical sciences and the social medical sciences.Besides, four aspects of medicine should be taken into consideration, namely medicine as science, technology, art and craft. Also, attention should be paid to the fact that diseases and medical interventions could occur at any level of the I :ing system, each having its own charac• teristics, and with emergents appearing in cross:level surveys.The unavoidable subspecialization in modern medicine should be balanced by transdisciplinary communication and interdisciplinary efforts, and by holistic approach to health problems utilizing the biosociomedical concept of health. Four tracts of study are suggested in medical education, i. e. population medicine, surgical sciences, medical sciences, and basic medical sciences.In conjunction with these steps early compartmentalization in the senior high school curriculum should be avoided.Key Word:: medical education - scientific medicine - living systems - concepts of disease - medical specializatio
The prevalence of intestinal parasites in orphanages in Yogyakarta
Stool specimens for microscopic examination collected from orphanages in Yogyakarta indicated the species and the overall percentages of prevalence of human intestinal parasites. These samples were examined in our laboratory by direct smear and concentration methods.
