    Bedload transport rates for coarse-bed streams in an Atlantic region (Narcea River, NW Iberian Peninsula)

    Rivers from northern Cantabrian Mountain Range (NW Spain) are emplaced in an Atlantic region, draining to the Bay of Biscay and saving 2000 m in a short path. Thus, high gradient streams normally develop with coarse-bed sediment and a strong importance of bedload transport processes in their morphosedimentary dynamics. In the present work, tagged clasts (painted and with inserted magnets) were used as tracers in order to estimate bedload transport rates in two of those coarse bed streams (River Pigüeña and River Coto) belonging to River Narcea drainage basin. Four methodological steps were followed: (1) grain size analysis of bed sediment; (2) preparing tracer clasts in the laboratory; (3) seeding the studied reaches with the tracers, measuring their displacements after flood events; and (4) analysis of data. Ten flood episodes with the ability to disturb tracer position were registered along two hydrological years (2009–2010 and 2010–2011); bedload transport rates were estimated for six of these episodes. Values ranging from 1.1 to 4.1 kg/s were obtained for Pigüeña, and values ranging from 0.20 to 0.28 kg/s were obtained for Coto.     From a literature review, contribute to update the knowledge of therapeutic communication of nurses and to put it into practice when performing care in the Primary Care consulting room. Development: The therapeutic communication is an inherent competence of the nurse´s work and has as its aim to improve the patient’s quality of life and to increase efficiency and professional satisfaction. Their basis are established on receiving and asking efficiently, integrating the information, negotiating, motivating, listening and showing empathy, being these two characteristics together with to the proper utilization and interpretation of the nonverbal language the most highlighted ones in the studies that had been carried out. Conclusions: Given the significance of making an effective therapeutic communication and insisting that it is not an inborn competence, nurses must educate themselves and be up-to-date in this aspect of their profession.Objetivos: A partir de una revisión bibliográfica contribuir a actualizar los conocimientos sobre comunicación terapéutica de las enfermeras y a su puesta en práctica a la hora de realizar los cuidados en las consultas de Atención Primaria. Desarrollo: La comunicación terapéutica es una competencia inherente al trabajo enfermero y tiene como objetivo mejorar la calidad de vida del paciente y aumentar la eficacia y satisfacción del profesional. Sus bases se asientan en recibir y preguntar eficazmente, integrar la información, negociar, motivar, escuchar y mostrar empatía, siendo estas dos características junto con la correcta utilización e interpretación del lenguaje no verbal las mas destacadas en los estudios realizados. Conclusiones: Dada la trascendencia de realizar una comunicación terapéutica eficaz y de que no se trata de una competencia con la que se nace, las enfermeras deben de formarse y mantenerse al día en este aspecto de su profesión

    Efectividad de una intervención educativa breve en pacientes con insomnio en atención primaria

    Abstract Objetive To evaluate the effectiveness of providing an educational intervention in primary care (PC) alongside a pharmacological treatment for insomnia. Design Single blinded non randomised clinical trial. Location Two urban primary health centers in Gijón (Asturias, Spain). Participants Patients who consulted for insomnia between July 2012-January 2013 and met the inclusion criteria (n = 50) were assigned systematically to the control group (CG) or intervention group (IG). All patients initiated treatment with lorazepam 1 mg in the evenings and had four weekly 15 min visits plus a follow-up visit after another month. Interventions The IG received training for control of stimuli, sleep hygiene and respiration and relaxation techniques in the four visits. The CG had only non invasive measures taken. Measurements Considering as cured those who reached a Pittsburgh Sleep Quality Index PSQI < 6 or a 50% reduction from baseline level. It was also analyzed the change in the PSQI from baseline to final visit and to follow-up visit, and voluntary interruption of lorazepam. Analysis by Bayesian inference. Results Twelve out of recoveries after intervention against one out of 24 among control group. Mean change in PSQI to final visit and follow-up visit was: –4.7 (95%CrI:–5.9 to –3.5) and –6,3 (95%ICred: –7.5 to –5.1) in IG; –1.8 (95%ICred: –3 to –0.5) and –1.7 (95%ICred: –2.9 to –0.4) in CG. Interruption of lorazepan: in 4 controls (16,7%) and 9 (34,6%) in IG. Twenty nine patients in GI and 17 in GC completed the trial. Per protocol analysis showed similar results. Conclusions The educational intervention in PC improves sleep quality and reduces the need of treatment with benzodiacepines

    Phenotypic plasticity in seed germination relates differentially to overwintering and flowering temperatures

    Phenotypic plasticity in seed dormancy may allow plant species to cope with fast environmental changes such as climate warming. In controlled experimental settings, plasticity in dormancy has been found to relate to temperature during seed maturation, but this relationship has not been tested in field conditions. Here we analyse for the first time the variation in dormancy during five successive years in wild populations of the study species Centaurium somedanum, to determine how this variation is related to average temperatures during specific seasons of plant activity. We performed laboratory germination experiments to measure (1) the degree of dormancy at dispersal, and (2) the sensitivity to dormancy breaking factors. We calculated average temperatures during four seasons of plant activity (overwintering, vegetative growth, flowering and seed maturation) for each year, and tested the relationship between these temperatures and patterns of dormancy variation using Generalized Linear Models. Dormancy varied among sites and years, seeds being more dormant in colder years. For the degree of dormancy at dispersal, we found a positive relationship with flowering temperature and a significant effect of collection site. For the sensitivity to dormancy breaking factors, we found no significant differences among sites, a positive relationship with flowering temperature and a negative relationship with overwintering temperature. Phenotypic plasticity in dormancy in C. somedanum is thus especially marked in the sensitivity to dormancy breaking factors. Temperatures during overwintering and flowering have differential effects on this plasticity, allowing the plant to detect the gradient of seasonality, a main ecological feature of its distribution. These results highlight the importance of taking into account more than average temperatures when assessing the response of plant phenotypic plasticity to climate change

    The reorientation of health care systems towards chronicity from a comprehensive model in which to ensure the continuity of care and seek excellence and efficiency seems essential for health services. The Health District V working group, composed of professionals from different disciplines, both Primary Care and various services of Hospital Care, was created in order to develop a proposal for improving the care of patients with pluripathology. A summary of the proposal, developed by primary care nurses, will de subsequently presented.La reorientación de los sistemas sanitarios hacia la atención a la cronicidad desde un modelo integral en el que se garantice la continuidad de la atención y se busque la excelencia y la eficiencia parece una tarea ineludible para los diferentes servicios de salud. En el Área Sanitaria V se puso en marcha un grupo de trabajo compuesto por profesionales de distintas disciplinas, tanto de Atención Primaria (AP) como de diversos servicios de Atención Hospitalaria (AH), para elaborar una propuesta de mejora de la atención a los pacientes con pluripatología.     La calidad de un ensilado resulta de la interacción entre la naturaleza del material de origen, su composición química y el proceso de conservación, y viene dada por su contenido en principios nutritivos, la digestibilidad de los mismos y por su fermentación. Su caracterización por métodos tradicionales exige un proceso analítico largo, tedioso y contaminante, por ello, se necesita un cambio radical en los procesos de análisis y control. La búsqueda de nuevas estrategias debería contemplar un alto potencial de muestreo, estimaciones rápidas, fiables e idealmente a pie de campo a fin de tomar las determinaciones de manejo en el momento de ensilar y a bajo coste. Estas características se encuentran entre los atributos de la Espectroscopia de Infrarrojo Cercano (NIRS)
