    Samartphone sebagai Media Edukasi pada Pasien Diabetes Mellitus: A Systematic Review

    Background: Type 2 diabetes (T2D) is associated with various health complications and health service visits, resulting in high costs for patients and the community. As a result, worldwide exponential growth of T2D has become a major issue. One strategy is to provide electronic (e-) health interventions. This intervention can reach many individuals in cost-effective and effective way to change behavior.Objective: To find out the development of smartphones as an educational medium in patients with type 2 diabetes mellitus. Method: This Systematic Review was based on Preferred Reporting Items for Systematic Reviews and Meta-Analyzes (PRISMA). The database used in this study was Scopus, Proquest and Pubmed were limited to the last 5 years of publication from 2016 to 2020, full text article and in English. The keywords used were "diabetes mellitus type 2" AND "education" AND "smartphone". This systematic review used 10 articles that fit the inclusion criteria. Results: Increased used of technology in the treatment of diabetes facilitates increased communication between nurses and patients. Social networking technology was developing at an impressive pace. Recent advances in mobile health (mHealth) have created new opportunities to improve DMT2 self-management through tools to facilitate healthy eating, exercise, and access to health services. One such innovative model involves an integrated system that connects patients via smartphone phones with their support network.     Motivational Interviewing as a Problem Solving Intervention to Improve Adherence: Review of the Related Literature

    Patient behavior changing through adherence therapy is very difficult. Identifying the most effective approach is very important regarding adherence. Motivational Interviewing (MI) is a clinical approach involving communication, collaboration, evocation, autonomy and increasing motivation alongside the final results of behavior change. The principles underlying MI include empathy, developing differences, avoiding debate and supporting self-efficacy. MI interventions allow for the changing of the relevant cognitions and self-regulation processes that leads to adherence to the treatment. Method: A literature review was conducted focused on journals published between 2013 and 2018. Fifty relevant journals were obtained and data was extracted from 25 relevant selected journals. The search was carried out by entering the keywords ‘adherence’, ‘motivation’ and ‘MI’ into the SCOPUS, ScienceDirect, Sage, ProQuest, Elsevier, SpringerLink and Google Scholar databases. Conclusion: MI studies show that MI-based interventions are effective in promoting health behavior changes. It is also associated with positive health outcomes such as low blood pressure, diet, decreased smoking, lower cholesterol, and better blood sugar control. Keywords: adherence, motivation, M

    An Effect of Breath Dhikr on the Stress Level of Patients with Pulmonary Tuberculosis

    Introduction: Pulmonary tuberculosis is an infectious chronic disease that can cause a stress condition on patients, such as the symptoms of tuberculosis disease, the long treatment process with a large amount of drugs, daily activity disruption, job loss and death threats. Unresolved stress can lead to irritability, anxiety, negative thinking, despair and helplessness. Some even blame God. Objective: This study aimed to determine the effect of dhikr breath on the stress levels of patients with pulmonary tuberculosis in the public health centres of Tanah Kalikedinding Surabaya. Method: A quasi-experimental design method with pre- and post-control groups was used. There was a treatment group and a control group; each group consisted of 16 respondents. The data analysis was performed using the Wilcoxon Signed Rank test and the Mann Whitney U test with a significance level of p <0.05. Results: The result of the Wilcoxon Signed Rank test showed that the stress level of the treatment group had significance with p=0.000. The control group had no significance; p=0.317. The Mann Whitney U test showed the differences in the stress levels post-intervention with p=0,000. Discussion: Dzikir breath was a positive stimulus that affected the cognator process according to Roy’s adaptation theory in the form of merging dhikr and breath, which is aimed at realizing God. Further research is expected to use cortisol measurements to obtain more accurate results. This should be performed at stress levels in other chronic disease patients. Conclusion: Breath Dzikir is a spiritual therapy that can change stress into eustress (positive stress) and decrease the stress level of pulmonary tuberculosis patients overall.     Analysis of the Health Tasks of Families with Diabetes Mellitus (DM)

    The family is the entry point in providing nursing services in the community, the involvement of all family members is needed in carrying out health tasks as an effort to prevent and treat DM. This aim of the study was to analyse personal and family factors towards family health tasks that have family members with DM patients. The study design used an analytic observational design with a cross sectional approach, a population of 184 DM families, a sample of 67 respondents using cluster random sampling. Analysis was conducted using the Spearman rho test. The results of the research of the patient factors influencing family tasks are age (p = 0.003), gender (p = 0.837), education (p = 0.013), work (p = 0.936), duration of suffering (p = 0.538). Family factors that affect family health tasks are family type (p = 0.137), family development (p = 0.097), income (p = 0.001), knowledge (p = 0.001). The average health assignment is 56.7% less. DM patients and families need to increase their knowledge about DM, complications, prevention, treatment therapy and be able to maximize health facilities in providing continuous care.     Pendahuluan: Perawat yang betugas didaerah terpencil dan daerah bencana akanmengalami beberapa kendala diantaranya jarak ketempat rujukan, kejenuhan saat bekerja, tekanan psikologis, maupun sulitnya mengontrol tingkat personal-profesional perawat selama bertugas. Penelitian ini bertujuan untuk mendeskripsikan self-efficacy perawat instrumen dalam respon gempa bumi di Nusa Tenggara Barat. Metode: Penelitian menggunakan desain kualitatif dengan pendekatan fenomenologi deskriptif. Populasi penelitian adalah perawat instrumen yang bertugas di dalam respon gempa bumi di Nusa Tenggara Barat (NTB), sampel penelitian diperoleh dengan snowball sampling menggunakan pesan whatsapp. Partisipan adalah 16 perawat instrumen. Metode pengumpulan data menggunakan wawancara mendalam. Alat pengumpulan data yang digunakan adalah voice recorder, pedoman wawancara, dan catatan lapangan. Data diolah dengan analisis tematik menurut Colaizzi. Hasil: Self-efficacy perawat dapat dilihat dari penilaian positif terhadap kapabilitas dirinya dibuktikan dengan mengungkapkan pengertian, terbentuknya tim, dan tujuan umum dibentuknya tim tanggap darurat. Selain itu, perawat memiliki motivasi melakukan penugasan. Partisipan menunjukkan sikap positif saat ditunjuk untuk ditugaskan di Nusa Tenggara Barat (NTB) Kesimpulan: Self-efficacy dapat dipengaruhi oleh beberapa aspek, diantaranya adalah pemahaman tentang suatu hal (pengetahuan), sikap atau perilaku, dan motivasi.     Pendahuluan: Klien TB paru yang menjalani pengobatan fase intensif sering kali merasa sedih, bosan, menolak eadaan, tidak berguna dan tidak berdaya, mengeluh dengan perubahan kondisi yang dialami. Studi ini bertujuan untuk mengetahui gambaran pengalaman klien TB paru yang menjalani pengobatan fase intensif di wilayah kerja Puskesmas Taji Kabupaten Magetan Metode: Penelitian ini menggunakan desain penelitian kualitatif pendekatan fenomenologi dengan metode in-depth interview terhadap 15 partisipan dengan kriteria inklusi klien TB paru berusia ≥16 tahun, klien baru terdiagnosa TB paru, klien sudah menjalani pengobatan TB paru fase intensif selama 1 bulan. Sedangkan kriteria eksklusi klien TB paru yang sedang hamil, TB-HIV, TB-MDR, TB ekstra paru, klien dengan penyakit penyerta seperti diabetes mellitus, hipertensi, skizofrenia, dan penyakit kronis lainnya. Analisis data dalam penelitian ini menggunakan teknik sembilan langkah Colaizzi.     Introduction: Choking is a condition of emergency breathing that must be dealt immediately. Babies and children are the age group with the most potential to experience choking caused by not chewing food properly, eating too much at one time and inserting small solid objects into the mouth. Objective: The aim of this study was to empower the community in this case the teacher, care giver / mother of the play group and parents of the Khadijah KB-TK Surabaya students in handling choking cases in children and infants. Method: The method used in community empowement is lecture, discussion, simulation/demonstration by the facilitator which is then followed by a demonstration by the participants/audience as an evaluation. Results: After this community empowerment there was an increase in participants' abilities which was indicated by increased knowledge and self-efficacy before and after community service activities. Teachers and parents can also demonstrate properly after this community service. Discussion: Knowledge occurs after people have sensed a certain object. Simulation/demonstration methods make it easier for teachers and parents to know how to handle choking through the eyes and ears, so that it is easy to understand. Cognitive knowledge is a very important domain in shaping one's actions.     Many Muslims fast during Ramadan. The fasting period typically lasts 12–13 hours a day for 29–30 days. In general, there are two major meal times – before dawn and at dusk – often resulting in increased sugary drink and carbohydrate intake (Benaji et al, 2006). The EPIDIAR (Epidemiology of Diabetes and Ramadan) study reported that 43% of people with type 1 and 79% people with type 2 diabetes fast during Ramadan in Muslim countries, increasing their risk of severe hypo- or hyperglycaemia (Salti et al, 2004). Research suggests there is often inadequate consultation with healthcare professionals regarding medication adjustment and dietary modifications during Ramadan (Hassanein et al, 2017; Lee et al, 2017a).     Perceived barriers on mental health services by the family of patients with mental illness

    Background: Various efforts have been made by the Indonesian government to improve mental health services. In 2014, the government established Law no 18, which is about mental health and the treatment of people with mental illness covered by the universal health coverage. However, many people still experience difficulty in accessing mental health services. In Indonesia, family plays the role of a caregiver to people with mental illness. Objective: This study aims to identify the perceived barriers on mental health services by families whose members suffers from mental illness. Methods: This study is a qualitative research study with a phenomenological approach. Sampling was conducted by purposive sampling with a sample size of 12 participants. Data were collected using semistructured in-depth interviews. Thematic analysis was performed using Colaizzi steps. Results: The obtained results presented three themes. Theme 1, mental health service affordability; theme 2, mental health service availability; and theme 3, negative attitudes (stigma).Conclusion: Families whose members suffered from mental illness still experienced barriers in relation to mental health services even with universal health coverage. Improved mental health services are related to the health insurance coverage, affordability, availability of mental health services and stigma reduction in the health professionals and wide community
