The change in the clinical presentation of Graves disease : a 30 years retrospective

É uma condição auto-imune classicamente marcada pela Tríade de Merserburg (bócio, tireotoxicose e orbitopatia), mas a mudança na apresentação da DG ao longo do tempo raramente foi estudada. Objetivo: Determinar alterações na apresentação clínica de pacientes com DG nos últimos 30 anos. Métodos: O estudo avaliou 475 pacientes diagnosticados com DG entre 1986 e 2016 em um único centro. Os pacientes foram avaliados quanto a aspectos epidemiológicos, função tireoidiana, atividade inflamatória dos olhos avaliada pelo Clinical Activity Score; CAS, gravidade avaliada pela classificação NOSPECS e volume tireoidiano estimado por ultrassonografia. Resultados: A avaliação dos pacientes identificou um aumento na idade média do diagnóstico de DG (p <0,02), uma redução no volume da tireóide (p <0,001) e um envolvimento orbital menos intenso entre 2007-2016 em comparação com 1986-2006 (p = 0,04). O número de pacientes fumantes foi menor de 2007 a 2016 (28,7%) do que de 1986 a 2006 (42,8% p = 0,001). Os valores de TSH e TRAb não apresentaram alterações significativas. Conclusão: A apresentação GD parece ter sido alterada nos últimos anos em comparação com a apresentação inicial típica. Há um envolvimento inflamatório menos frequente do tecido orbital, bócios menores, menor número de fumantes e diagnóstico em idade mais avançadaAbstract: Introduction: Graves¿ disease (GD) is the main cause of hyperthyroidism among adults. It is an autoimmune condition classically marked by the Merserburg Triad (goiter, thyrotoxicosis, and orbitopathy), but the change in presentation of GD over time has rarely been studied. Objective: To determine changes in the clinical presentation of patients with GD in the last 30 years. Subjects and Methods: The study evaluated 475 patients diagnosed with GD between 1986 and 2016 in a single center. Patients were evaluated regarding epidemiological aspects, thyroid function, inflammatory activity of the eyes evaluated by the Clinical Activity Score; CAS, severity evaluated by NOSPECS classification and thyroid volume estimated by ultrasonography. Results: Patients assessment identified an increase in the mean age of diagnosis of GD (p < 0.02), a reduction in thyroid volume (p < 0.001) and less intense orbital involvement from 2007-2016 compared to 1986-2006 (p = 0.04). The number of smoking patients was smaller from 2007 to 2016 (28.7%) than 1986 to 2006 (42.8% p = 0.001). The TSH and TRAb values did not had significant changes. Conclusion: GD presentation appears to be changed in the last years compared to the typical initial presentation. The enactment of schoolar managers of the Municipal Network Education of Campinas in the production of gender curriculas
Orientador: Antonio Carlos Dias JúniorDissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de EducaçãoResumo: Esta pesquisa investigou a atuação de gestoras/es escolares na Rede Municipal de Ensino de Campinas para compreender como as políticas curriculares do município são implementadas criativamente para os contextos escolares na criação de currículos de gênero. Destacamos nesses currículos o que denominamos educação inclusiva de gênero, uma cultura profissional de educadoras/es capaz de incentivar práticas pedagógicas para o combate às violências de gênero provocadas por currículos ocultos e mobilizadas nas práticas pedagógicas de educadoras/es, apresentada em revisão de literatura. A metodologia consistiu na abordagem do ciclo de políticas articulada a uma epistemologia feminista pós-estruturalista, o que possibilitou compreender como atores nos contextos de influência, de produção do texto da política e da prática participam da produção e tradução de uma agenda de gênero pela adoção de diferentes estratégias, discursos e práticas políticas. Para o contexto da prática realizamos oito entrevistas semiestruturadas com gestoras/es escolares da rede acompanhada do contexto de produção do texto da política, o qual se dedica à análise documental dos Projetos Pedagógicos das quatro escolas participantes. Estes resultados são precedidos pela exposição de estratégias e práticas de movimentos neoconservadores e neoliberais em nível nacional e municipal na redefinição do papel de Bem-Estar Social e na resistência oferecida por educadoras/es nos contextos de influência e de produção do texto da política. Concluímos que a gestão das escolas participantes disputa sua atuação política principalmente com famílias e governantes de Campinas em torno dos currículos de gênero. Além disso, os currículos de gênero nas escolas são organizados partindo de percepções diversas e ambíguas sobre gênero no cotidiano escolar, influenciadas, sobretudo, pelas concepções de professoras/es sobre o tema e experiências não formais das/os participantes com o temaAbstract: This research investigated the enactment of school managers in the Municipal Education Network of Campinas to understand how curricular politics in the city are creatively implemented to the schoolar contexts in the gender curricula creation. We highlight in these curriculas what we denominate inclusive gender education, a professional culture of educators capable of encourage pedagogical practices for the combat to the gender violences provoqued by hidden curriculas and mobilized in the pedaggical practices of educators, presented in literature review. The metodology consists in the policy cicle approach entwined to a poststructuralist feminist epistemology, what enable to comprehend how actor in the contexts of influencie, of the production of policy text and of practices participate in the production and translation of a gender agenda by the adoption of different strategies, discourses and political practices. For the context of practices we accomplish eight semistructured interviews with school managers of the network followed by the context of the production of policy text, which is dedicated to the documental analisys of the four Pedagogical Projects of participant schools. This results are preceded by the exposition of the strategies and practices of neoconservatives and neoliberals movements in national and municipal level in the redefinition of the Welfare role and in the resistance offered by educators in the contexts of influence and the production of the policy text. We conclude that management in the participants schools dispute their political enactment mainly with the families and governors of Campinas around the gender curriculas. Besides that, the gender curriculas in schools are organized starting from diverse and ambiguous perceptions about gender in the school daily life, influenced specially by the teachers conceptions about the theme and no formal experiences of the participants with the themeMestradoEducaçãoMestre em EducaçãoCAPE

Heart fossilization is possible and informs the evolution of cardiac outflow tract in vertebrates
Elucidating cardiac evolution has been frustrated by lack of fossils. One celebrated enigma in cardiac evolution involves the transition from a cardiac outflow tract dominated by a multi-valved conus arteriosus in basal actinopterygians, to an outflow tract commanded by the non valved, elastic, bulbus arteriosus in higher actinopterygians. We demonstrate that cardiac preservation is possible in the extinct fish Rhacolepis buccalis from the Brazilian Cretaceous. Using X-ray synchrotron microtomography, we show that Rhacolepis fossils display hearts with a conus arteriosus containing at least five valve rows. This represents a transitional morphology between the primitive, multivalvar, conal condition and the derived, monovalvar, bulbar state of the outflow tract in modern actinopterygians. Our data rescue a long-lost cardiac phenotype (119-113 Ma) and suggest that outflow tract simplification in actinopterygians is compatible with a gradual, rather than a drastic saltation event. Dental adhesives and strategies for displacement of water/solvents from collagen fibrils
To evaluate the influence of temperature of evaporation in adhesive systems with different solvents on the apparent modulus of elasticity and mass change of macro-hybrid layers modified by proanthocyanidins (PACs). Methods Adhesive resin beams (A) from Single Bond Plus (SB), Excite (EX) and One Step Plus (OS) were prepared after solvent evaporation at 23 °C or 40 °C (n = 12). Macro-hybrid layers (M) (n = 12) were prepared using demineralized dentin beams sectioned from extracted human third molars. The demineralized dentin specimens were infiltrated with each one of the three adhesive systems at 23 °C or 40 °C; with or without prior dentin treatment with PACs for 10 min. The apparent modulus of elasticity (E) and mass change (Wmc, %) of adhesives beams and resin-infiltrated specimens were assessed in dry and wet conditions after immersion in water (24 h, 1, 3 and 6 months). The E was statistically analyzed by Tukey–Kramer test and the Wmc, % by Kruskal Wallis, and Dunn (α = 0.05). Results Solvent evaporation at 40 °C resulted in higher E values for adhesive resin beams at all storage conditions, regardless of the adhesive system (p 0.05). In vitro antimicrobial activity of probiotics in carbapenemase producing Klebsiella pneumoniae, Escherichia coli, Salmonella enteritidis and Shigella flexneri
Orientador: Carlos Emilio LevyDissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciências MédicasResumo: O uso de probióticos tem sido crescente para o tratamento e prevenção de diarreia causada por enteropatógenos, e os gêneros Lactobacillus e Bifidobacterium têm sido os mais utilizados. O objetivo deste estudo foi avaliar a atividade antimicrobiana de alguns probióticos contra enterobactérias patogênicas e seu potencial uso para descolonização de pacientes. Probióticos comercialmente disponíveis foram semeados e incubados para contagem de colônias (UFC) e identificação por espectrometria de massas (MALDI-TOF/MS). Para teste de sensibilidade a drogas, os probióticos foram submetidos ao E-test®. A atividade antimicrobiana dos probióticos contra Klebsiella pneumoniae produtora de carbapenemase (KPC), Escherichia coli, Salmonella enteritidis e Shigella flexneri foi avaliada pelo método de co-cultura, na concentração de 106 UFC/mL, tanto das enterobactérias como de cada microrganismo isolado dos produtos. Amostras foram semeadas nos tempos T0, 24hs, 48hs e 72hs para contagem de UFC remanescentes. Para Lactobacillus reuteri foi adicionado 1% de glicerol. Encontramos 40 produtos probióticos disponíveis no comércio, entre medicamentos e alimentos contendo probióticos, e um total de 23 espécies de diferentes microrganismos. Foram selecionadas e submetidas à identificação 6 medicamentos, 5 produtos alimentícios lácteos e uma cepa padrão ATCC® para controle. Para o teste de atividade antimicrobiana foram testados: Lactobacillus reuteri DSM 17938 Colidis®, Lactobacillus rhamnosus SD5217 AtillusMulti®, Lactobacillus acidophilus LA14 Prolive®, Lactobacillus casei Shirota Yakult®, Lactococcus lactis subsp. lactis ATCC 11454, Lactococcus lactis Activia® Shot, Bacillus clausii Enterogermina® e Saccharomyces boulardii 17 Floratil AT®. O MALDI-TOF/MS permitiu a identificação de todos os gêneros e a maioria das espécies dos probióticos analisados. A contagem de UFC dos produtos analisados se mostrou igual ou muito próxima as contagens que constam nas bulas e rótulos. As CIMs dos probióticos mostraram que todos os Lactobacillus foram resistentes à Vancomicina e com exceção do L. reuteri, resistentes ao Meropenem, sendo os demais resultados variáveis, mas sempre sendo sensíveis a dois ou mais antibióticos. Todos Lactobacillus e Lactococcus apresentaram atividade inibitória contra todas enterobactérias testadas, sendo >99% de redução em 48hs contra KPC, enquanto que B. clausii e S. boulardii não mostraram atividade inibitória. Em conclusão, todos os Lactobacillus spp e Lactococcus spp analisados mostraram potencial uso para inibição da colonização intestinal por enterobactérias patogênicas, incluindo KPC, em pacientes hospitalizados com risco de translocação bacterianaAbstract: Probiotics have been progressively used for treatment and prevention of diarrhea caused by enteropathogens with genera Lactobacillus and Bifidobacterium most commonly used. This study aimed to evaluate antimicrobial activity of some probiotics against pathogenic enterobacteria and its potential use for patient decolonization. Commercially available probiotics were incubated for colonies counting (CFU) and identification by mass spectrometry (MALDI-TOF/MS). Probiotics were submitted to E-test® for drug sensitivity. Co-culture, at 106 CFU/mL, was used to evaluate probiotic antimicrobial activity against carbapenemase-producing Klebsiella pneumoniae (KPC), Escherichia coli, Salmonella enteritidis and Shigella flexneri. Samples were seeded at times T0, 24, 48 and 72 hours for remaining CFU counting. For Lactobacillus reuteri 1% glycerol was added. We found 40 probiotic products commercially available among medicines and food, and a total of 23 species of different microorganisms. Selection and identification covered 6 medicines, 5 fermented dairy products and a standard strain ATCC® for control. For co-culture tests the following probiotics were checked: Lactobacillus reuteri DSM 17938 Colidis®, Lactobacillus rhamnosus SD5217 AtillusMulti®, Lactobacillus acidophilus LA14 Prolive®, Lactobacillus casei Shirota Yakult®, Lactococcus lactis subsp. lactis ATCC 11454, Lactococcus lactis Activia® Shot, Bacillus Clausii Enterogermina® e Saccharomyces boulardii 17 Floratil AT®. MALDI-TOF/MS allowed the identification of all genera and most probiotics species analyzed. All dairy products demonstrated CFU counting close to that declared in the lable. MICs showed that all Lactobacillus were resistant to Vancomycin and with the exception of L. reuteri, resistant to Meropenem. The other results were variable, but always sensitive to two or more antibiotics. All Lactobacillus and Lactococcus showed inhibitory activity against tested enterobacteria, with >99% reduction in 48 hours against KPC, while B. clausii and S. boulardii showed no inhibitory activity. Cashew gum and inulin: new alternative for ginger essential oil microencapsulation
This study aimed to evaluate the effect of partial replacement of cashew gum by inulin used as wall materials, on the characteristics of ginger essential oil microencapsulated by spray drying with ultrasound assisted emulsions. The characterization of particles was evaluated as encapsulation efficiency and particle size. In addition, the properties of the microcapsules were studied through FTIR analysis, adsorption isotherms, thermal gravimetric analysis, X-ray and scanning electron microscopy. It was found that the solubility of the treatments was affected by the composition of the wall material and reached higher values (89.80%) when higher inulin concentrations were applied. The encapsulation efficiency (15.8%) was lower at the highest inulin concentration. The particles presented amorphous characteristics and treatment with cashew gum as encapsulant exhibited the highest water absorption at high water activity. High seroprevalence for Rickettsia rickettsii in equines suggests risk of human infection in silent areas for the Brazilian spotted fever
Equines play a role in the epidemiology of Brazilian spotted fever (BSF) since they are a primary host for the tick Amblyomma sculptum. We studied the seroprevalence for three species of Rickettsia in equines in four endemic (with human cases) and in four non-endemic areas (no human cases) in the Piracicaba River Basin, Sao Paulo, Brazil. A serological survey of 504 equines was performed: around 63 animals were sampled in each area and tested through indirect immunofluorescence assay for R. rickettsii, R. parkeri, and R. bellii in 2012-2013. Blood samples were seropositive for 183 equines (36.3%) in which 73 (39.9%) were from non-endemic areas. In the studied sites equines were highly exposed to Rickettsia infection ranging from 6.1% to 54.7%, with Geometric Mean Titers greater in endemic area (p = 0.012). Results suggest that Rickettsia may be more widespread than the surveillance of BSF has detected. Insights into temperature modulation of the eucalyptus globulus and eucalyptus grandis antioxidant and lignification subproteomes
Eucalyptus grandis and Eucalyptus globulus are among the most widely cultivated trees, differing in lignin composition and plantation areas, as E. grandis is mostly cultivated in tropical regions while E. globulus is preferred in temperate areas. As temperature is a key modulator in plant metabolism, a large-scale proteome analysis was carried out to investigate changes in the antioxidant system and the lignificadon metabolism in plantlets grown at different temperatures. Our strategy allowed the identification of 3111 stem proteins. A total of 103 antioxidant proteins were detected in the stems of both species. Hierarchical clustering revealed that alterations in the antioxidant proteins are more prominent when Eucalyptus seedlings were exposed to high temperature and that the superoxide isoforms coded by the gene Eucgr.B03930 are the most abundant antioxidant enzymes induced by thermal stimulus. Regarding the lignin biosynthesis, our proteomics approach resulted in the identification of 13 of the 17 core proteins involved in this metabolism, corroborating with gene predictions and the proposed lignin toolbox. Quantitative analyses revealed significant differences in 8 protein isoforms, including the ferulate 5-hydroxylase isoform F5H1, a key enzyme in catalyzing the synthesis of sinapyl alcohol, and the cinnamyl alcohol dehydrogenase isoform CAD2, the last enzyme in monolignol biosynthesis.
