    Vergleichende Analyse von Inzidenz und Letalität von Sarkomen des Kopf-und Hals-Bereichs anhand vorliegender klinisch-pathologischer Parameter

    Sarkome sind seltene Tumore. Insbesondere sind nur 9% der Fälle im Kopf Hals Bereich lokalisiert. Zudem existieren über 100 verschiedene Subentitäten. Um die Qualität in Diagnostik und Therapie zu bessern, werden zunehmend Patienten an überregionalen Zentren zusammengeführt. Trotz dessen liegen kaum genauere Daten zu Inzidenz und Letalität für den Kopf Hals Bereich vor. Daher soll dieses Projekt die Inzidenz und Letalität vorliegender Subentitäten unter Berücksichtigung klinischer Faktoren (TNM, Grading) untersuchen, um so die vorliegende Evidenz bei der Beratung und Therapie von Sarkompatienten des Kopf und hals Bereiches zu verbessern. Es sollen die Inzidenz in Lokalisation und TNM für einzelne Subentitäten des Kopf-Hals Bereichs untersucht werden, um so einen Überblick für die vorherrschenden Tumorsubentitäten zu erlangen und ggfs. weitere Forschung auf diese zu konzentrieren. Zudem sollen die Letalität bzw. das 5-Jahres Gesamtüberleben für einzelne UICC Stadien, TNM Kategorien und Subentitäten erhoben werden. Anhand der BL-kennziffern sollen Inzidenzen und Letalität nach groben Geographischen Kennziffern untersucht werden.     Towards a European health information system: The BRIDGE Health, InfAct and PHIRI projects

    Background: National health systems in Europe are facing similar challenges – demographic change, a rising burden of disease due to chronic non-communicable diseases, and health inequalities. Comparable health data and knowledge sharing between countries are therefore an important basis for policy decision-making. However, health information in the European Union (EU) is fragmented and approaches to establishing a comprehensive system are largely project-based. Methods: This contribution describes the European projects BRIDGE Health (2015 – 2017), InfAct (Information for Action, 2018 – 2021) and PHIRI (Population Health Information Research Infrastructure, 2020 – 2023), which aimed at developing a sustainable health information infrastructure. Results: The projects, which build on one another, laid the groundwork for an EU-wide health information system. For example, a health information portal was established, a federated research infrastructure was implemented, handouts were created and training on capacity building was provided. Conclusions: An integrated EU-wide health information system is an important basis for policy decision-making and a prerequisite for a rapid and coordinated response to health crises. A sustainable structure or institution with a mandate for non-communicable diseases (NCD) and their determinants at the EU level would be desirable.Peer Reviewe

    Die Aktivität akuter Atemwegserkrankungen (ARE-Inzidenz) in der Bevölkerung blieb in den letzten drei Wochen relativ stabil auf einem vergleichsweise hohen Niveau. In der 40. KW 2024 lag die ARE-Inzidenz bei rund 8.700 ARE pro 100.000 Einw. (Vorwoche: 8.600). Die Inzidenz der grippeähnlichen Erkrankungen (ILI) ist in den letzten zwei Wochen gesunken und lag bei rund 1.700 ILI pro 100.000 Einw. gesunken (Vorwoche: 2.100). Auch die ILI-Inzidenz befindet sich trotz des Rückgangs auf einem vergleichsweise hohen Niveau. Die geschätzte COVID-19-Inzidenz in der Bevölkerung basierend auf Angaben der GrippeWeb-Teilnehmenden war bis zur 38. KW deutlich angestiegen und ist in den letzten zwei Wochen auf rund 1.100 COVID-19-Erkrankungen pro 100.000 Einw. gesunken. In der virologischen Überwachung in der Bevölkerung (GrippeWeb-Plus) wurden seit der 38. KW 2024 hauptsächlich Rhino-/Enteroviren nachgewiesen, mit Abstand gefolgt von SARS-CoV-2 sowie vereinzelt anderen Viren oder Bakterien des Erregerpanels

    Wastewater-based Surveillance on SARS-CoV-2

    The SARS-CoV-2 viral load in wastewater is being tested at selected wastewater treatment plants (WWTP) in Germany as part of AMELAG (“Abwassermonitoring für die epidemiologische Lagebewertung”, German for wastewater monitoring for epidemiological situation assessment). Data from 169 WWTPs are currently included in the analyses for SARS-CoV-2 and 99 for Influenza A Virus and Influenza B Virus. For the calendar week 46 data from 104 WWTP were available for SARS-CoV-2 and from 72 WWTPs for Influenza A Virus and Influenza B Virus. Data from other WWTP are provided continuously. A slight decrease in the aggregated viral load can be observed since calendar week 42. The viral loads of both the influenza A and B virus are currently at low levels. The development observed over the last few weeks may still change due to late notifications, especially from WWTP with large population coverage

    Die Aktivität akuter Atemwegserkrankungen (ARE-Inzidenz) in der Bevölkerung ist in der 50. KW im Vergleich zur Vorwoche stabil geblieben und lag insgesamt bei rund 8.200 ARE pro 100.000 Einw. (Vorwoche: 8.100). Dabei sind die Werte bei den Kindern gestiegen, insbesondere bei den 0- bis 4-Jährigen, und bei den jungen Erwachsenen gesunken. Die Inzidenz der grippeähnlichen Erkrankungen (ILI) ist in der 50. KW im Vergleich zur Vorwoche bei rund 1.400 ILI pro 100.000 Einw. ebenfalls stabil geblieben. Die aktuelle AREInzidenz befindet sich weiterhin auf einem vergleichsweise hohen Niveau, die ILI-Inzidenz liegt im Bereich der vorpandemischen Jahre. Die geschätzte COVID-19-Inzidenz in der Bevölkerung basierend auf Angaben der GrippeWeb-Teilnehmenden war seit der 39. KW gesunken und lag in der 50. KW bei rund 400 COVID-19- Erkrankungen pro 100.000 Einw. In der virologischen Überwachung in der Bevölkerung (GrippeWeb-Plus) wurde im November/Dezember eine Vielzahl an unterschiedlichen Atemwegserregern nachgewiesen, wobei Rhino-/Enteroviren hauptsächlich detektiert wurden. Die für die Bevölkerung in Deutschland geschätzten Inzidenzen beruhen auf den Selbstauskünften von Personen, die sich bei GrippeWeb registriert haben. Für die 50. KW 2024 haben bisher 8.821 GrippeWeb- Teilnehmende eine Meldung abgegeben, von diesen hatten 712 eine ARE und 131 eine ILI (Datenstand: 17.12.2024, 0:00 Uhr). Durch Nachmeldungen, die bis zu vier Wochen lang möglich sind, können sich noch Änderungen ergeben. Aktuell können durch die Nachmeldungen rund 11.000 Meldungen pro Woche für die Auswertungen berücksichtigt werden. Neue Teilnehmende sind herzlich willkommen.     The Role of High-Level Isolation Units in Response to High-Consequence and Emerging Infectious Diseases: Experience of the German STAKOB Network

    This case study describes the experience of the German Permanent Working Group of Competence and Treatment Centers for High Consequence Infectious Diseases, known as STAKOB (Ständiger Arbeitskreis der Kompetenz- und Behandlungszentren für Krankheiten durch hochpathogene Erreger). STAKOB brings together public health authorities (competence centers) and high-level isolation units (treatment centers) to collaborate on the clinical management of high-consequence infectious diseases (HCIDs) and emerging infectious diseases. The network is coordinated by the Robert Koch Institute, Germany’s federal public health institute. The main tasks of STAKOB are to strengthen HCID clinical and public health management and increase expert knowledge on HCID and non-HCID emerging infectious diseases in Germany. STAKOB enables the exchange of knowledge and experiences; development of guidelines on infection prevention and control measures, clinical management, and therapy; and support for the World Health Organization and other outbreak responses internationally. The past years have shown how important the STAKOB network is for Germany—not only in providing critical care for HCID cases but also increasing capacity to support public health and clinical management of emerging infectious disease cases. However, maintaining several high-level isolation units in Germany requires a high commitment of financial, material, and human resources. Due to the rarity of HCID and emerging infectious disease events, maintaining the appropriate level of preparedness and ensuring sufficient investments is an ongoing struggle. Nevertheless, it is essential to have a network ready to react to HCID and non-HCID emerging infectious diseases in times of a changing biosecurity and infectious landscape.Peer Reviewe

    Aspergillus fumigatus—a systematic review to inform the World Health Organization priority list of fungal pathogens

    Recognizing the growing global burden of fungal infections, the World Health Organization established a process to develop a priority list of fungal pathogens (FPPL). In this systematic review, we aimed to evaluate the epidemiology and impact of invasive infections caused by Aspergillus fumigatus to inform the first FPPL. The pre-specified criteria of mortality, inpatient care, complications and sequelae, antifungal susceptibility, risk factors, preventability, annual incidence, global distribution, and emergence were used to search for relevant articles between 1 January 2016 and 10 June 2021. Overall, 49 studies were eligible for inclusion. Azole antifungal susceptibility varied according to geographical regions. Voriconazole susceptibility rates of 22.2% were reported from the Netherlands, whereas in Brazil, Korea, India, China, and the UK, voriconazole susceptibility rates were 76%, 94.7%, 96.9%, 98.6%, and 99.7%, respectively. Cross-resistance was common with 85%, 92.8%, and 100% of voriconazole-resistant A. fumigatus isolates also resistant to itraconazole, posaconazole, and isavuconazole, respectively. The incidence of invasive aspergillosis (IA) in patients with acute leukemia was estimated at 5.84/100 patients. Six-week mortality rates in IA cases ranged from 31% to 36%. Azole resistance and hematological malignancy were poor prognostic factors. Twelve-week mortality rates were significantly higher in voriconazole-resistant than in voriconazole-susceptible IA cases (12/22 [54.5%] vs. 27/88 [30.7%]; P = .035), and hematology patients with IA had significantly higher mortality rates compared with solid-malignancy cases who had IA (65/217 [30%] vs. 14/78 [18%]; P = .04). Carefully designed surveillance studies linking laboratory and clinical data are required to better inform future FPPL.Peer Reviewe
