    Biofilm formation by the global outbreak strain of Mycobacterium chimaera results in significantly reduced efficacy of standard disinfectants

    Background: In 2013, a global outbreak of Mycobacterium chimaera infections due to contaminated heater-cooler units emerged. This ongoing problem has highlighted the question of whether disinfection recommendations for medical devices containing water circuits are adequate for preventing contamination and possible recontamination by nontuberculous mycobacteria. The formation of biofilms in such devices exacerbates the problem. This study aimed to assess the efficacy of disinfectants on biofilms and suspensions of the M. chimaera strain ZUERICH-1, and to compare it with two unrelated M. chimaera strains obtained from different sources. Methods: Disinfection efficacy testing for biofilm was performed using a Bead Assay for Biofilms and for bacteria in suspension according to the European Standard EN 14348. Three different disinfectants, glutaraldehyde, sodium hypochlorite and peracetic acid, were assessed. M. chimaera ZUERICH-1, two genetically unrelated M. chimaera isolates and M. avium subsp. avium ATCC 15769, which is included in European standards for disinfectant testing on mycobacteria, were analyzed. The biofilms’ structure and composition were analyzed by chemical and molecular techniques and advanced imaging methods. Results: We found that peracetic acid and glutaraldehyde in standard concentrations were able to effectively inactivate (≥ 4 log10 reduction) suspended bacteria of all three strains, but chlorine failed in all cases. Formation of biofilm generally enhanced the tolerance of M. chimaera to disinfectants. Peracetic acid in standard concentration could not effectively inactivate biofilms of M. chimaera ZUERICH-1, but was effective against biofilms of the other M. chimaera strains tested. Similarly, glutaraldehyde in standard concentration could not inactivate biofilm of ZUERICH-1. Biomass analysis showed higher amounts of extracellular matrix of ZUERICH-1 when compared to the other two strains. Conclusions: The data suggest that current standard disinfection recommendations do not ensure sustained inhibition of M. chimaera when embedded in biofilm. Additional measures are needed to prevent nosocomial transmission of M. chimaera through contaminated heater-cooler units

    Maligne Speicheldrüsenkarzinome sind eine seltene Tumorerkrankung im Kopf-Hals-Bereich und insbesondere mit dem Hintergrund der neuen S3-Leitlinie "Diagnostik und Therapie von Speicheldrüsentumoren des Kopfes" kommt der adjuvanten Strahlentherapie im chirurgischen Primärkonzept ein relevanter Stellenwert zu. Offen ist jedoch, inwiefern der Zeitraum zwischen Operation und Beginn oder Abschluss der Strahlentherapie für die onkologische Prognose eine Rolle spielt. Für orale Plattenepithelkarzinome wird beispielsweise ein Zeitraum von 6 Wochen empfohlen, wobei Studien gezeigt haben dass es eine hohe Abweichung zwischen den einzelnen Kopf-Hals-Lokalisationen und Entitäten gibt. Ziel dieser Studie ist es daher den Einfluss auf maligne Speicheldrüsenkarzinome zu untersuchen.     Wastewater-based Surveillance on SARS-CoV-2

    In the AMELAG project, SARS-CoV-2, influenza viruses and respiratory syncytial virus (RSV) are monitored in wastewater. Nationwide, 168 wastewater treatment plants were participating by the end of 2024; around 70 sites participate in 2025. The data collected will be integrated into this report in the coming weeks. For the calendar week 22 data from 57 WWTPs for SARS-CoV-2, from 60 WWTPs for influenza A virus and influenza B, from 40 WWTPs for RSV A and B and from 21 WWTPs of RSV A/B (joint measurements of RSV A and B) were available. Data from other WWTP are provided continuously. The aggregated SARS-CoV-2 viral load is currently on a low level. The values of the aggregated influenza A and B viruses are currently on a low level. The RSV load values in wastewater are on a low level for RSV A and further declining for RSV B. The aggregated viral load of the combined measurements of RSV A and B is also on a low level. The development observed over the last few weeks may still change due to late notifications

    Die Aktivität akuter Atemwegserkrankungen (ARE-Inzidenz) in der Bevölkerung war rund um Ostern erneut deutlich zurückgegangen und blieb in der 18. KW im Vergleich zur Vorwoche relativ stabil auf einem niedrigen Niveau bei rund 3.900 ARE pro 100.000 Einw. (Vorwoche: 3.700 pro 100.000 Einw.). Die aktuelle ARE-Inzidenz entspricht rund 3,3 Millionen akuten Atemwegserkrankungen. Auch die Inzidenz der grippeähnlichen Erkrankungen (ILI) befindet sich durch den deutlichen Rückgang in den letzten Wochen auf einem niedrigen Niveau. In der 18. KW lag der Wert bei rund 700 ILI pro 100.000 Einw. In der virologisch-mikrobiologischen Surveillance in der Bevölkerung (GrippeWeb-Plus) wurden im April hauptsächlich Rhino-/Enteroviren nachgewiesen, gefolgt von humanen saisonalen Coronaviren. Die für die Bevölkerung in Deutschland geschätzten Inzidenzen beruhen auf den Selbstauskünften von Personen, die sich bei GrippeWeb registriert haben. Für die 18. KW 2025 haben bisher 8.822 GrippeWeb- Teilnehmende eine Meldung abgegeben, von diesen hatten 349 eine ARE und 61 eine ILI (Datenstand: 6.5.2025, 0:00 Uhr). Durch Nachmeldungen, die bis zu vier Wochen lang möglich sind, können sich noch Änderungen ergeben. Aktuell können durch die Nachmeldungen rund 13.000 Meldungen pro Woche für die Auswertungen berücksichtigt werden. Neue Teilnehmende sind herzlich willkommen.     Der hier dargestellte Indikator (Kennzahl) ist Teil des Indikatorensets der Diabetes-Surveillance (Forschungsprojekt am Robert Koch-Institut 2015-2024) mit insgesamt 40 definierten Indikatoren beziehungsweise Indikatorgruppen. Zusätzlich zu den Indikator-Ergebnissen werden das methodische Vorgehen und die Datengrundlage beschrieben sowie die Daten in aggregierter Form bereitgestellt. Alle Indikatoren sind in vier Handlungsfelder eingeordnet: (1) Diabetesrisiko reduzieren, (2) Diabetesfrüherkennung und behandlung verbessern, (3) Diabeteskomplikationen reduzieren bzw. (4) Krankheitslast und Krankheitskosten senken. Zudem wird zwischen Kindern und Jugendlichen sowie Erwachsenen unterschieden. Der Ergebnisstand der Diabetes-Surveillance zum Projektende 2024 ist auf dem Publikationsserver des Robert Koch-Institutes (https://edoc.rki.de/handle/176904/12467) veröffentlicht. Zukünftig werden einige Diabetes-Indikatoren als Teil einer erweiterten Surveillance nichtübertragbarer Erkrankungen unter http://www.gbe.rki.de regelmäßig mit neuen Daten aktualisiert werden

In addition to the indicator results, the methodological approach and the data source are described and the data are provided in aggregated form. All indicators are categorised into four fields of action: (1) Reducing the risk of diabetes, (2) Improving the early detection and treatment of diabetes, (3) Reducing the complications of diabetes, and (4) Reducing the burden and costs of disease. There are indicators for children and adolescents as well as for adults. The results of the Diabetes Surveillance at the end of the project in 2024 are published on the publication server of the Robert Koch Institute (https://edoc.rki.de/handle/176904/12467). In future, some diabetes indicators will be regularly updated with new data as part of the extended surveillance of non-communicable diseases at http://www.gbe.rki.de

    Anaplastische Schilddrüsenkarzinome (ATC) sind äußerst selten. Das mediane Überleben nach Diagnose liegt bislang bei nur etwa 3-6 Monaten, da die Biologie des Tumors in der Regel sehr aggressiv ist. Daher wird bei dieser Erkrankung auch nur das Tumorstadium IV (nach UICC) vergeben. Die Tumore weisen häufig genetische Variationen auf, die den Krebs so schwer behandelbar machen. Andererseits ergeben sich dadurch Ansätze für immun-onkologische Therapien, die seit einigen Jahren erprobt werden und aufgrund positiver Effekte Eingang in die europäischen und amerikanischen Leitlinien gefunden haben. Epidemiologische Fragestellungen: . Wie hat sich die rohe und die altersstandardisierte Inzidenz von ATC in Deutschland zwischen 2012 und 2022 entwickelt (jahresgenau, geschlechts- und altersgruppenstratifiziert)? 2. Wie hat sich das relative Überleben von ATC entwickelt? (gleiche Bedingungen)? 3. Wie hoch ist der Anteil unter den Patienten mit follikulären, papillären und schlecht differenzierten Schilddrüsentumoren, bei denen sich ein ATC entwickelt? (aggreggiert über die Jahre 2012 bis 2022)? 4. Wie viele Menschen leben derzeit mit einem ATC in Deutschland? (geschätzte Prävalenz für den letztmöglichen Zeitpunkt) Klinische Fragestellungen: 1. Wie viel Prozent der Patienten mit ATC erhielten in den Jahren 2020, 2021, 2022 neoadjuvante, adjuvante und palliative systemische Therapie? 2. Wie hoch ist der Prozentsatz der operierten und der bestrahlten Patienten in den Jahren 2020, 2021, 2022? 3. Pro Tumorstadium: Wie ist das mediane Überleben bei Patienten je nach Behandlungsstrategie?     Die ARE-Aktivität bleibt weiterhin auf hohem Niveau. Wie in den Vorwochen dominiert die starke Zirkulation der Influenzaviren das ARE-Geschehen. In der 9. KW wurden überwiegend Influenza B-Viren im ambulanten Bereich nachgewiesen. Influenzaerkrankungen werden in allen Altersgruppen verzeich-net. Bei schwer verlaufenden Erkrankungen unter älteren Patientinnen und Patienten wurden weiterhin am häufigsten Influenza A(H1N1)pdm09-Viren nachgewiesen.
