    The importance of support starting from the child's wishes ― Based on the experience of facing and accepting the distress of having roots in a foreign country ―

    application/pdf本研究の目的は,外国にルーツを持つ子どもが日本で生活を送る中で,どのような経験をし,それが本人の人 生にどのような影響を与えるのかを明らかにして,現状調査の結果とを併せて考察することにより,クライエン トが求める支援の在り方を提言することである。今回,複線径路等至性アプローチ(TEA)を用いて外国にルー ツを持つ当事者 1 名にインタビューを実施し,得られた語りを分析後,現状調査を踏まえ作業療法士の視点で考 察した。その結果,本人が取り組みたいことに取り組むという経験をすることで,日本社会においてルーツを個 性のひとつとして受け入れ,人との関わりの中で自分らしく未来を描くことができるようになることが分かった。 外国にルーツを持つ子どもが陥りやすい自己否定から脱し,自分らしい生活を送るために,本人が取り組みたい 活動に取り組むことができるよう支援する視点の必要性が示唆される。In recent years, the number of children with foreign roots has been increasing in Japan. Although the support provided for such children is increasing accordingly, it is still far from suffi cient. In this study, we interviewed a subject with foreign roots who grew up in Japan, collected information about his experiences, and discussed what kind of support is necessary. It was found that the experience of being able to work on what he wanted to do was important for him to accept his roots as a part of his personality in Japanese society and envision his future in his own way. For people with foreign roots to break free from the self-denial that they tend to fall into, and to lead a life that is true to themselves, our study suggested that it is necessary to have a perspective that supports them in engaging in the activities that they want to engage in.事例研究departmental bulletin pape

    Recognition of dysphagia rehabilitation among university students

    application/pdf医療・介護・福祉現場では,種々の職種が摂食嚥下障害当事者に,見守りも含めた食事介助を行う。しかし,我々 が医療・介護・福祉職と一般市民を対象に以前行なった調査では,言語聴覚士 (ST) と他職種との間で,食事介 助や嚥下リハに対する意識や情報量に有意な差があり,それが医療・介護関連肺炎の反復や窒息事故の一因では ないかと危惧する。  本研究は,情報量の差が生じる時期とその内容を明らかにする目的で,広島県内の医療・福祉職養成課程とそ れ以外の分野で学ぶ大学生計 530 名に対して,嚥下リハの基礎的情報に関するアンケートを行い,基本属性(学 部・学科,学年等),食事介助の見学・経験,家族・親戚の摂食嚥下障害の経験,嚥下造影・内視鏡の見学経験 との関連を検討した。情報量の差は保健福祉学部内の学生間にもみられた。当事者の安全確保には,学修内容の 工夫等の方策検討が必要と考える。Every patient with dysphagia is supervised or fed by multidisciplinary staff in various ways at hospitals and facilities. However, there are significant differences of quantity and quality of experiences as well as information and attitudes in regards to appropriate feeding techniques between Speech-Language-Hearing Therapists (ST) and other medical and care staff according to our previous research. Inappropriate feeding environments and techniques could jeopardize dysphagic patients and contribute to recurrent nursing and healthcare-associated pneumonia (NHCAP). The aim of this study was to clarify when and how the differences occur. We investigated 530 university students in Hiroshima prefecture using a true- false quiz of basic knowledge on dysphagia rehabilitation, their specialty and grade, experiences of feeding, family members’ dysphagia, and observation for videofl uorography and videoendoscopy. We found differences in experiences and information even among students studying in the same department. As STs, we should share information as to dysphagia rehabilitation not only with multidisciplinary staff, but also our fellow students more initiatively. It might be necessary for curriculum changes to be made in order to ensure that all staff are able to feed all patients and residents safely.資料departmental bulletin pape

    Effi cacy of Vocal Function Exercise-short version for Presbyphonia

    application/pdf加齢により声帯は硬く薄くなり,気息性で弱い声になる。今回,加齢による音声障害に対し,Vocal Function Exercise (VFE) の短縮版を施行し,その効果について検討を行った。「VFE 短縮版」は,VFE の手技の中の発声 持続時間の延長と特定の高さでの発声持続練習のみを訓練内容とした。対象は加齢性声帯萎縮症例 18 例であっ た。性別は男性 10 例,女性 8 例,年齢は 68 歳から 82 歳で平均年齢は 73.5 歳であった。結果は 18 例中 17 例で 治療後の聴覚印象,最長発声持続時間の改善を認めた。また,訓練回数は 6 回以内が 72.2%,訓練期間は 3 カ月 以内が 55.6%と従来の VFE に比して治療期間が短縮された。「VFE 短縮版」は手技が簡便な上,治療期間の短縮 が期待でき,加齢による音声障害に対して治療効果があると考えられた。Vocal folds become hard and thin with aging, resulting in a weakened voice. In this study, we conducted and examined the effi cacy of the short version of Vocal Function Exercise (VFE) program in cases of Presbyphonia. The VFE-short version focuses only on the following exercise contents in the terms of VFE techniques: 1) extension of phonation time, and 4) VFE with specifi c voice pitch. The study targeted 18 cases (10 males, 8 females; average age, 73.5 years) of Presbyphonia. As a result, improvements in auditory impression and maximum phonation time were observed for 17 of the 18 cases after treatment. In addition, the treatment period was shorted compared with the conventional VFE program through the number of training sessions (less than six times: 72.2%) and the period of the training session (less than 3 months: 55.6%). These fi ndings suggest that the VFE-short version provides simpler techniques and reduced treatment periods, and is therefore effective for the treatment of Presbyphonia.報告departmental bulletin pape
