Sistem distribusi dan cakupan suplementasi tablet besi ibu hamil pascabencana tsunami 2004 di Kabupaten Aceh Besar Provinsi Nanggroe Aceh Darussalam
Background: The prevalence of anemia is one of indicators applied to determine pregnant woman nutrition status. The coverage of iron supplementation is still low due to poor iron distribution. In Aceh Besar regency, the prevalence of anemia in 2003 was 18.71% and become 45.5% in 2005 (report of Rapid Nutrition Assessment in Tsunami Affected Districts in NAD, February-March 2005).Objective: The study was meant to explore the distribution system and iron supplementation coverage post tsunami in Aceh Besar regency, NAD Province.Method: This was a qualitative naturalistic study; the design was investigative exploration study, and the data was collected by indepth interview. The analysis unit was health department, primary health care and villages in Aceh Besar regency. Subjects were stakeholders and pregnant women taken by purposive sampling and showed descriptively.Result: Poor health service and unreadiness of health staff affected the stagnation of iron distribution program. The un-available of guidance book and lack of nutrition staff and midwife development become so crucial. Indeed, they could not understand their jobs description in expanding iron distribution network. Furthermore, the distribution of iron tablet was done passively of pregnant woman visiting health service place. The policy of iron distribution by health department of Aceh Besar regency post tsunami 2004 were collecting data and pregnant woman ANC service held by mid-wife using iron program. The achievement of iron supple-mentation coverage was still low, though the attitude of pregnant woman has changed.Conclusion: The stagnation of basic health service and lack of health staff empowerment affected poor iron distribution for pregnant woman
Perbedaan PaCO2 darah pada pasien kritis yang mendapat diet makanan enteral komersial dengan diet makanan enteral kombinasi di rumah sakit Dr. Sardjito Yogyakarta
Background: Malnutrition often occurs in a critical patient caused by various things such as decreasing function of gastrointestinal tract, high process of catabolism and homeostatie destruction. Other problem that may occur when nutrition is given with high total calories could be dangerous because it could cause hiperglikemia, the production improvement of CO blood and worsen the homeostasis destruction.Objective: To find out the difference of PaCO blood in critical patients who obtain commercial enteral food diet or combined enteral food diet.Methods: This was an experimental study using randomized controlled trial with pretest-posttest control group design. The subjects were critical patients who were given care in the intensive installation Dr. Sardjito hospital Yogyakarta who met the criteria: adult patient, using ventilator and receiving enteral food diet.Results: There was no difference between PaCO blood in critical patient who obtained commercial enteral diet and com- bined enteral food diet. There was a change difference of PaCO with 55% compared with <55% from energy total (p=0.03).Conclusion: The improvement of PaCO was not caused by the type of enteral formula given, cause of the high intake of energy and carbohydrate

Background : Numerous research have been made to find out contributive factors of the exclusive breastfeeding.Objective: The objective of this study was to find out whether mothers’ nutritional status had any influence on the exclusivity of breastfeeding and on the growth of their babies up to 4 months.Methods: This observational study was carried out with a prospective cohort design and lasted from November 2004 to May 2005. The subjects were 80 pairs of mother and baby treated at the Obstetric and Gynecology Unit (UPF) of Local Public Hospital (RSUD) Prof. Dr. W.Z. Johannes at Kupang – NTT. The subjects were divided into two groups namely Undernutrition status group marked by 23,5 cm. Observed babies were those of normal or spontaneous delivery, non twin with >2500 g birth weight. Follow up observations were made at family home and the data on breastfeeding characteristics were collected by interview, babies weight were measured by Secca baby scale with 0,01 kg level of accuracy mid arm-circumference was measured by means of LILA tape, and body length was measured using length-board. Tools measure with 0,1 cm level of accuracy. Some statistical tests were used namely Chi-Square; student’s t-test; and while Kaplan Meier log-rank test as needed.Results: More than 50% of under-nutrition mothers were under 25 years old. Chi-Square test shows that significant difference between mother’s age was (p=0,04). More than 50% of the KEK group gave birth for the first time or ‘primipara’ (p=0,25). Kaplan Meier analysis showed that there was no difference between survival median of breastfeeding be- tween the both group (p=0,53). t-test showed that there were significant differences of the babies body weight of the both KEK and non KEK mothers, namely pd”0,01, p=0.01, and p=0.03 in the first, second, third, and the fourth month respectively. In terms of the average babies body length, the t-test also showed significant differences between both groups with p=0.02 and p=0.01 in the third and the fourth month respectively.Conclusion: I can be concluded that mother’ nutrition status did not have any influence on the exclusivity of breastfeeding, however, it did significantly influence babies growth from 0- 4 month
Analisis potensi pengembangan instalasi gizi Rumah Sakit Umum Banyumas menjadi profit center
Background: A hospital is supposed to be self-financed. Nutrition installation as one of units in the hospital is categorized as a cost center unit. It may be changed into a profit center unit if it is developed by providing foods for staff, students and patients’ families.Objectives: To know feasibility of nutrition department development potential of Banyumas Hospital through provision of food service for staff, students and patients’ families in order to contribute for hospital income.Methods: Primary data were collected through questionnaires to know: willingness to pay for from staff, students and patients’ families and commitment of nutrition installation staff; and through focus group discussion to know the commitment of hospital management. Secondary data were obtained from non-experiment observation at nutrition installation and hospital secretariat. Nutrition service development feasibility was viewed from market, commitment of staff and hospital management, and technical aspects.Results: Result of the study showed than potential of nutrition department development to become a profit center by serving foods for staff, students and patients’ families was feasible to implement viewed from: market aspect, there was definite potential market, willingness and capacity to pay, high interest to become customers and marketing mix control; Investment criteria, Net Present value was as much as Rp166,333,504.04, Internal Rate of Return Value was 81.9%, Pay Back Period was 9 months, and Break Even Point was 2 years 10 months and 24 days; Nutrition Installation staff and hospital management commitment was high; Technical aspect, location, raw material resources, manpower, production capacity and facilities were justified.Conclusion: Nutrition department development was feasibly potential to be implemented
Hubungan kadar serum albumin dengan kejadian morbiditas dan mortalitas pada pasien penyakit ginjal kronik dengan hemodialisis di RS Dr. Sardjito Yogyakarta
Background: Chronic kidney disease is a characterized by decreasing kidney function with creatine clearance test <15 mL/minute. At such condition toxic in the body accumulates so that dialysis is needed to eliminate it. Blood albumin serum level may indicate nutrition status and is closely related to morbidity and mortality of hemodialysis patients.Objective: To find out association between albumin serum level and occurrence of morbidity and mortality among patients of chronic kidney disease with hemodialysis.Method: The study was observational with prospective cohort design. It was conducted at Dialysis Installation of Dr. Sardjito Hospital from August 2004 to December 2005. Population were all patients of hemodialysis. Samples were purposively taken using inclusion criteria with as many as 44 minimal samples. Dependent variables were morbidity and mortality. Independent variables were albumin serum level, age and long time hemodialysis. Data analysis was done in phases, univariable, and then bivariable.Results: There was association between albumin serum level and morbidity at observation I (p=0.021), observation II (p=0.036), and observation III (p=0.012) which indicated that p<0.05. There was also association between albumin serum level and mortality at observation I (p=0.011), observation II, III and IV (p=0.000) which indicated that p<0.05.Conclusion: There was association between albumin serum level and morbidity and mortality of chronic kidney disease patients with hemodialysis
Pengaruh suplementasi Fe dan Zn terhadap kadar hemoglobin dan kesegaran jasmani pada lansia anemia di Kabupaten Bantul
Background: Province of Yogyakarta Special Territory is a province of “elderly” structure population, as viewed from the number of elderly which is 15% of total population. Anemia is one of nutrition problems of elderly which reaches as high as 50%. This is caused by low intake of iron and other substances related to iron metabolism process in forming hemoglobin. Low hemoglobin concentration can be one of causes of elderly declining physical fitness. Efforts to overcome anemic problems can be done through the supply of iron and zinc for the elderly twice a week.Objective: To know effect of iron and zinc supply to hemoglobin concentration and physical fitness for anemic elderly at Bantul District, Province of Yogyakarta Special Territory.Methods: The study was a randomized double blind controlled trial design. Subject of the study were anemic elderly that fulfilled inclusion criteria. The supply of iron, combination of iron and zinc to trial group was given twice a week on different days, supply of iron was given on Monday and Thursday, while supply of zinc was given on Tuesday and Saturday. Laboratory and physical fitness examination to both group were done prior and after treatment. Nutrition intake data were achieved through food record. Next, to know effect of iron, iron and zinc supply to elderly hemoglobin concentration and physical fitness, covariant analysis was done.Results: The result of the study showed that after 3 months’ treatment, there was significant difference of hemoglobin concentration caused by different treatment of group with iron supply, iron and zinc combination supply and control group (p=0.012) although iron and combined iron and zinc supply had equal effectiveness in increasing hemoglobin concentration (p=0.404). There was no significant relationship between elderly hemoglobin concentration and physical fitness (p=0.448). This showed that treatment given affected increase of hemoglobin concentration but the supply and increase of hemoglobin concentration did not affect increase of elderly physical fitness.Conclusion: The supply of iron and combined iron and zinc increase hemoglobin concentration, but the supply and in- crease of hemoglobin concentration did not increase the elderly physical fitness
Pengaruh pijat bayi terhadap kenaikan berat badan bayi umur 4 bulan
Background: Optimum growth and development reflect the interaction among genetics, environments, attitude factors, and positive stimulations. The positive effect of massage on infant growth has long been recognized, but research on this phenomenon has not much been performed. The growth faltering in children begins at 1-6 months of age in Indonesia.Objective: To identify the effect of massage toward weight gain in infants of 4 months old.Method: The study was quasi-experimental with 2 groups of posttest design. The subjects were infants of 4 months old that were born in two hospitals. Those who were born in one hospital were selected as the control group and the rest were selected as the treatment group. The massage was performed twice a week for four weeks. The data of weight, food intake, illness were collected once a week, whereas the data of mothers’ knowledge and attitude toward breast milk were gathered once during the research period.Results: There was a significant effect of massage on weight gain. The median weight gain of the control group and treatment group were 0.40 kg (Q1=0.35; Q3=0.40) and 0.60 kg (Q1=0.50; Q3=0.70) respectively. The massage also affected the waz (weight for age Z-scores) where the median difference of waz at the beginning and the end of the study of the control group was –0.27 (Q1=-0.34; Q3=-0.23), whereas in the treatment group was -0.03 (Q1=-0.13; Q3=0.09). It meant that control group faced the growth faltering of –0.27 SD waz in a month and treatment group did it for –0.03 SD waz in a month.Conclusion: Infants who got massage treatment had higher weight gain than those who did not

Background: KADARZI could be realized by optimizing the role of family, namely by increasing nutrition knowledge, changing attitude and behavior and developing independent family. The poor knowledge, attitude and behavior of society, especially house wives, to-ward nutrition and health are important factors which influence the realization of KADARZI programme. The evaluation of KADARZI programme in 2002 in Indragiri Hilir resulted 22.41% from the target 80%. One way to increase society’s knowledge, attitude and behavior toward nutrition is by giving nutrition education during early life.Objective: The study was purposed to investigate the influence of nutrition education among elementary school students to KADARZI house wives knowledge, attitude and behavior.Method: This was quasi experimental with non-randomized control group pretest-posttest study. Subjects were elementary students in the fourth, fifth, and sixth class along with their mothers. The place was in Indragiri Hilir district. Subjects were gathered by purposive. The quantity of subjects were determined with sample formulation, knowledge, attitude and behavior data were gathered by interview and observation adjusted in questionnaire list. Characteristic was analyzed with chi square, statistic test with t-test, with significance level of p=0.05.Result: The analysis of age, education, occupation, the number of household members and family income of both group were similar. Pretest analysis of knowledge, attitude and behavior among students and mothers in both groups were also similar in both groups before intervention. Posttest analysis of knowledge, attitude and behavior of treatment group were significantly increased (p0.05). Analysis among mothers whose children got no intervention showed there were no significant differences (p>0.05). While mothers whose children got intervention showed insignificant differences in knowledge and attitude but behavior.Conclusion: Nutrition education about KADARZI on elementary student couldn’t increase mother’s knowledge and attitude but behavior
Pola konsumsi makanan di luar rumah sebagai faktor risiko terjadinya obesitas pada remaja SMP di Kota Palembang
Background: Teenagers nutriture are complicated because teenagers often ignore meal time, have snacks and do not eat regularly so that eating out such as having fast foods becomes a favorite choice. Prevalence of obesity among teenagers in Yogyakarta is 7.8%, Tangerang 5.01% and Surabaya 6.5%. Changes of eating pattern in the family and habit of eating out at restaurants or food courts are presumed as factors that increase prevalence of obesity at Palembang Municipality.Objectives: To explore prevalence of obesity and identify relationship of eating out pattern among obese and non obese junior high school teenagers at Palembang Municipality.Method: First, a survey was done to 1,300 junior high school teenagers at Palembang Municipality to find out prevalence of obesity using cross sectional design with cluster random sampling technique. Next, to find out relationship between eating out consumption pattern and obesity, case control study with simple random sampling technique was used. Total number of samples in case control design was as many as 174 people, matched with age, sex, and grade. The study was conducted in July–December 2005. Data were analyzed using univariable, bivariable, and multivariable techniques.Results: Prevalence of obesity among junior high school teenagers at Palembang Municipality was 6.62%. There was no significant relationship (p>0.05) between eating out fre- quency, consumption of energy, protein, fat and carbohydrate, and occurence of obesity. There was significant relationship (p<0.05) between total consumption of energy, fat, carbohydrate, frequency of eating in, obese status of parents, social economic status of the family, education of fathers and mothers, and occurence of obesity. Conclusions: Factors related to occurence of obesity were frequency of eating in, total consumption of energy, obese status of parents and education of fathers. Pola makanan pendamping air susu ibu dan status gizi bayi 0-12 bulan di Kecamatan Lhoknga Kabupaten Aceh Besar
Background: The prevalence of malnutrition in Indonesia is still high. One of potential factor contributing to the high prevalence of malnutrition is improper complementary feeding pattern and breast feeding practices. In the community there are three complementary feeding patterns; those are traditional feeding, industries feeding and combination.Objective: This study was conducted to analyze the association between complementary feeding, energy and protein intake, and breast feeding status with nutritional status of infants 0-12 month old.Method: This study used a cross sectional design. Subjects were 151 infants 0-12 month old. Complementary feeding pattern and breast feeding status were collected using interview methods. Energy and protein intakes were collected using food recall methods. Nutritional status was measured using weight for length (WHZ). The chi square test was used to analyze the data.Result: There wasn’t any association between complementary feeding pattern and nutritional status of infants 0-6 m.o. old (p=0.04) and 6-12 month (p=0.62). There wasn’t any association between energy intake and nutritional status of infants 0-6 month old (p=0.40), but there was any association in infants 6-12 month old (p=0.01). Protein intake associated with nutritional status of infants 0-6 m.o. old and 6-12 month old (p=0.033 and p=0.04). Breast feeding status didn’t associated with nutritional status of infants 0-6 month old and 6-12 month old (p=0.689 and p=0.10).Conclusion: Complementary pattern and breast feeding status were not associated with nutritional status. Energy intake was associated with nutritional status of infants 6-12 month old. Protein intake associated with nutritional status of infants 0-6 month old and 6-12 month old
