    Knowledge and Risk Perception Regarding Keratinocyte Carcinoma in Lay People: A Systematic Review and Meta-Analysis

    Background/Objectives: The increasing incidence rates of keratinocyte carcinoma (KC), particularly in fair-skinned populations, call for efforts to intensify health education of the general population in addressing this prevalent skin cancer type. As a preparatory step, this systematic review summarizes the published research on the knowledge and risk perception regarding KC among individuals without medical training. Methods: The review was registered in PROSPERO (CRD42024618851) and adheres to PRISMA guidelines. The databases PubMed, Scopus, Web of Science, PsycArticles, and PsycINFO were searched on 30 July 2024. Studies were eligible if knowledge and/or risk perception was assessed in lay people. Risk of bias (ROB) was assessed with the Joanna Briggs Institute checklist for prevalence studies. Comparable outcomes (e.g., awareness of terms for KC) were meta-analyzed. Results: Included reports ( n = 17) were published between 1991 and 2024 with 16,728 individuals assessed. Awareness for the most common type of KC, basal cell carcinoma (BCC), was low (20.75% of respondents (95% confidence interval (CI): 15.24–27.61)), while more respondents were familiar with colloquial terms (60.9–72.8%). Meta-analysis indicated an underestimation of the frequency of KC, with only 7.21% (CI: 4.03–12.58) identifying BCC as the most common type of skin cancer. Furthermore, concern about developing KC as assessed in only two overlapping studies was reported by only 25–30% of respondents, indicating a significant gap in risk awareness and a lack of research on risk perception regarding KC. Conclusions: This review highlights the need for targeted health education interventions to improve knowledge and preventive behaviors regarding KC. Given the limitations of the included studies, characterized by high ROB, heterogeneity of results, and a lack of standardized assessment tools, further research is essential to enhance the understanding and awareness of KC in diverse populations.This research received no external funding

    Diese Dissertation beschäftigt sich mit der modifizierten Clifford-Analysis auf der Kugel. Ausgangspunkt ist das von H. Leutwiler eingef¨uhrte H-System, das auf dem oberen Halbraum R^n+1_+ definiert ist und eine Erweiterung der klassischen Cauchy-Riemann- Gleichungen in höheren Dimensionen darstellt. Im ersten Kapitel der Arbeit wird dieses System vorgestellt und es werden ein paar Konsequenzen dieses Systems erwähnt. Im zweiten Kapitel wird das H-System vom oberen Halbraum auf die Einheitskugel übertragen. Dies geschieht mithilfe der Cayley-Abbildung, einer mehrdimensionalen Möbius Transformation. Da das System der H-Lösungen aus der Poincar´e Metrik hervorgeht, genügt es diese vom oberen Halbraum auf die Einheitskugel zu übertragen. Das daraus resultierende Differentialgleichungssystem auf B(0, 1) wird dann W-System gennant. Es wird sodann gezeigt wie man unter Verwendung der Clifford Algebra die H-L¨osungen bequem in W-Lösungen überf¨uhren kann und umgekehrt. Da die W-Lösungen aus der diesbezüglichen Poincar´e Metrik hervorgehen, genügt es die Laplace-Beltrami Gleichung dieser Metrik zu studieren. Man beginnt mit der Bestimmung der Fundamentall¨osung dieser Gleichung. Mit dem Ansatz u = g(r^2)H_k, wobei H_k ein harmonisches, homogenes Polynom vom Grad k ist, finden wir polynomiale L¨osungen der Laplace-Beltrami Gleichung, die im nächsten Kapitel zu Entwicklungssätzen führen. Dies allerdings nur in geraden Dimensionen. Ferner wird gezeigt wie man mit Hilfe einer geeignetenMöbius Transformation W-Lösungen auf sich abbilden kann. Es wird auch untersucht wie der auf H-Lösungen anwendbare Differential Operator ∂/∂xi (i = 1, . . . , n − 1) auf W-Lösungen wirkt. Mit der Übertragung der für H-Lösungen bekannten Charakteristik auf W-Lösungen erhält man ein Werkzeug mit dem man feststellen kann wann eine W-Lösung die auf der Einheitssphäre, dem Rand von B(0, 1), verschwindet identisch Null ist. Im dritten Kapitel werden im Falle der Dimensionen n = 4, 6 und 8 Entwicklungssätze für die Lösungen der oben erw¨ahnten Laplace-Beltrami Gleichung hergeleitet. Diese Sätze beruhen auf dem für alle geraden Dimensionen n = 2m gültigen Resultat, wonach für jede dieser Lösungen u die Gleichung Δ^m u = 0 gilt. Darüber hinaus werden im vierten Kapitel hypermonogene Funktionen betrachtet, eine natürliche Verallgemeinerung der H-Lösungen. Da sich diese Funktionen als endliche Summen von H-Lösungen darstellen lassen, kann man im Prinzip auch diese Funktionen vom oberen Halbraum auf die Einheitskugel übertragen.     Tumor-assoziierte neutrophile Granulozyten im frühen Mammakarzinom

    1. Hintergrund und Ziele: Die onkologische Forschung thematisiert zunehmend die immunologische Tumormikroumgebung und ihre Auswirkung auf die Entstehung und Progression von Karzinomen und folglich die prognostische Bedeutung für die Erkrankten. So wird auch die Interaktion zwischen Brustkrebszellen und Tumor-assoziierten neutrophilen Granulozyten (TANs), die sowohl pro- als auch antitumoral agieren können, intensiver untersucht. Dabei sollten die unterschiedlichen Subtypen des Mammakarzinoms differenziert betrachtet werden. Obwohl der prognostisch günstigere Hormonrezeptor-positive Brustkrebs (hormone-receptor positive breast cancer, HR+BC) den Großteil aller diagnostizierten Mammakarzinome darstellt, ist dieser im Vergleich zu aggressiveren Subtypen weniger gut erforscht. Auch wenn der HR+BC als weniger immunogen beschrieben wird, darf diese Untergruppe nicht vernachlässigt werden. Daher beschäftigte sich die Forschungsarbeit „Tumor-assoziierte Neutrophile sind ein negativer prognostischer Faktor beim frühen luminalen Brustkrebs bei einer ausbleibenden immunsuppressiven Makrophagenrekrutierung“ mit der prognostischen Bedeutung von Immunzellen der Tumormikroumgebung, insbesondere TANs, im frühen Mammakarzinom. Dafür wurden die Zelldichten von neutrophilen Granulozyten nicht nur im Tumorgewebe, sondern auch im Normalgewebe und in Wächterlymphknoten von Patientinnen mit HR+BC analysiert und ins Verhältnis zur Langzeitprognose gesetzt. 2. Methoden: Im Zeitraum von 2000 bis 2005 wurden 144 Patientinnen mit einem Hormonrezeptor-positiven Brustkrebs im Frühstadium im Rahmen der deutsch-österreichischen Phase-II-Studie zur beschleunigten Teilbrustbestrahlung am Universitätsklinikum Erlangen therapiert. Dabei wurden aus den Operationsresektaten, die zuvor in Formalin fixiert und in Paraffin eingebettet worden waren, Stanzen aus verschiedenen Lokalisationen des Brustgewebes mit einem Durchmesser von 2 mm entnommen und jeweils 60 dieser zu Tissue Microarrays (TMAs) angeordnet. Aus den TMAs wurden im Anschluss 4 μm dicke Schnitte hergestellt und mittels Immunhistochemie angefärbt. Die daraus resultierende CD66b-Färbung ermöglichte die Sichtbarkeit neutrophiler Granulozyten in den Gewebeschnitten als bräunlich angefärbte Zellen. Nach der Herstellung digitaler Scans konnten die Gewebeschnitte mit einer Bildverarbeitungssoftware analysiert werden. Hierbei waren Größe, Morphologie und Farbe der Zellen Auswertungs-kriterien. Die intraepithelialen und stromalen Flächen und die Anzahl der darin enthaltenen Immunzellen wurden semiautomatisch erfasst und im Anschluss aus den gewonnenen Daten die Zelldichten berechnet. Daraufhin wurden die erhobenen Zelldichten zusammen mit den klinischen Patientendaten zur Analyse der Überlebenszeit genutzt. 3. Ergebnisse und Beobachtungen: Eine hohe Zelldichte von intraepithelialen und stromalen CD66b+-neutrophilen Granulozyten in zentralen Tumorproben stellte einen negativen Prognosefaktor dar. Auch im angrenzenden Normalgewebe und in Proben aus den Wächterlymphknoten korrelierte die Neutrophilendichte mit einem reduzierten krankheitsfreien Überleben. Interessanterweise zeigte sich ein Zusammenhang zwischen der TAN-Dichte und der Dichte von Tumor-assoziierten Makrophagen (TAMs), die wir in einer früheren Studie untersucht haben. So zeigten sich TANs als ein negativer Prognosefaktor nur bei einem erhöhten M1/M2-TAM-Verhältnis, während dieser Einfluss bei einem niedrigen M1/M2-Verhältnis verloren ging. 4. Schlussfolgerungen: Beim luminalen Mammakarzinom im Frühstadium sind CD66b+-Neutrophile in der Einzelmarkeranalyse ein negativer Prognosefaktor. Der Zusammenhang mit dem M1/M2-Verhältnis der TAMs sollte in zukünftigen Studien ausführlicher analysiert werden. Gleichwohl könnte die Rekrutierung von TANs eine Strategie zur Steigerung der Aggressivität von Mammakarzinomen sein, die nicht fähig sind, ausreichende Zelldichten von TAMs anzulocken und in den M2-Phänotyp umzuwandeln

    Extracorporeal photopheresis reduces the T cell stimulatory capacity of human primary blood conventional dendritic cells type 1

    Introduction Extracorporeal photopheresis (ECP) is an immunomodulatory treatment option for different T cell-mediated diseases such as cutaneous T cell lymphoma (CTCL) and chronic graft-versus-host disease (GvHD). While in CTCL the polarization of T cells is shifted towards T helper cells type 1 (TH1) and an immune response against the lymphoma is induced, ECP in GvHD rather leads to the expansion of regulatory T cells (Treg). How ECP regulates the immune response dependent on the underlying disease is still not exactly known. As dendritic cells (DCs) are crucial regulators of the immune response, it is supposed that they are key players in the immunomodulatory effects of ECP. However, due to the scarcity of primary DCs in blood, research has focused on in vitro -generated monocyte-derived DCs so far. Methods Here, we present for the first time how the primary human blood DC subpopulations, i.e., conventional DCs type 1 (cDC1), cDC2, DC3, and plasmacytoid DCs (pDC), directly isolated from blood of healthy donors, respond to in vitro ECP treatment. Results We demonstrate that the exposure to 8-methoxypsoralen and UV-A light irradiation induces apoptosis in Toll-like receptor ligand-activated cDC1 and pDC as well as - to a minor extent - in steady state cDC1, cDC2, and DC3. However, the selective effect of ECP on viability of DC subpopulations was dependent on culture duration (18h vs. 42h) as well as condition (steady state vs. TLR ligand activated). Further, ECP modulates the expression of the co-stimulatory and co-regulatory molecules CD40, CD86, and PD-L1 on DC subpopulations. While ECP did not affect the T cell stimulatory capacity of cDC2 and DC3, ECP-treated cDC1 and - to a minor extent - pDC showed reduced activation of memory T cells and diminished secretion of TH1- and TH17-associated cytokines. Conclusion Thus, especially blood cDC1 are direct targets of ECP and the reduction of their T cell stimulatory capacity might contribute to the clinical efficacy observed in chronic GvHD patients

    Beyond the Scale: The Hidden Burden of Underweight and Cachexia in Adults with Congenital Heart Defects and Heart Failure—Results from the Pathfinder CHD-Registry

    Background/Objectives: Heart failure (HF) poses a major challenge in managing adults with congenital heart defects (ACHD). Emerging evidence suggests that HF in ACHD increases the risk of underweight due to heightened metabolic demands, gastrointestinal complications, and psychological factors such as anxiety and depression. Despite its critical implications, few studies have examined this association. This study evaluates the relationship between HF and underweight—defined as a body mass index (BMI) < 18.5—in ACHD. Methods: The Pathfinder-CHD Registry is a prospective, observational, web-based HF registry including ACHD with manifest HF, history of HF, or significant risk for HF. It documents congenital diagnoses, HF type, comorbidities, and treatments. Patients were categorized by BMI into mild (17.00–18.49), moderate (16.00–16.99), and severe (<16.00) underweight. Results: As of September 2024, the registry enrolled 1420 adults (mean age 31.8±11.3 years; 49.2% female). Underweight was present in 59 patients (4.2%): 62.7% mild, 18.6% moderate, and 18.6% severe. Among the remaining 1361 patients, 52.8% had normal weight, 32.8% were overweight, and 14.2% were obese. Women had significantly lower metabolic body weight than men ( p = 0.002). Underweight correlated with younger age ( p < 0.001) and CHD type ( p = 0.02). Notably, 42.9% of underweight patients had cyanotic CHD. Conclusions: Underweight is an underrecognized problem in ACHD with HF. Adults with complex CHD or connective tissue disorders are disproportionately affected.     4-Octyl Itaconate ameliorates diesel exhaust particle-induced oxidative stress in nasal epithelial cells

    Background and objective Particulate matters such as diesel exhaust particles induce oxidative stress in cells and thereby have a negative impact on health. The aim of this study was to test whether the membrane-permeable, anti-inflammatory metabolite 4-Octyl Itaconate can counteract the oxidative stress induced by diesel exhaust particles and to analyze the downstream-regulated pathways both in human nasal epithelial cells and PBMCs. Methods Human nasal epithelial cells were cultured from nasal swabs, and the response of the cells to diesel exhaust particles either alone or in combination with 4-Octyl Itaconatee was investigated using RNA sequencing, qPCR, and cytokine measurement. The presence of reactive oxygen species in the cells was analyzed using CellROX staining and flow cytometric DCFDA assay. Results Diesel exhaust particles caused an upregulation of CYP1A1 in nasal epithelial cells. The administration of 4-Octyl Itaconate reduced the reactive oxygen species and increased the expression of antioxidant genes regulated by the transcription factor NRF2, which was also confirmed in PBMCs. IL-6 secretion from NEC was elevated by diesel exhaust particles and reduced when 4-Octyl Itaconate was administered. Conclusion 4-Octyl Itaconate can reduce the diesel-exhaust-particle-induced oxidative damage by the activation of NRF2-regulated antioxidative pathways

    Control of Buckling of Colloidal Supraparticles

    The properties of clusters of colloidal particles, often termed supraparticles, are determined by the arrangement of the primary particles. Therefore, controlling the structure formation process is of key importance. While buckled morphologies can result from fast drying kinetics as found in spray drying, controlling the morphology under slow drying conditions remains a challenge. The final morphology of a supraparticle formed from an emulsion droplet can be controlled by manipulating particle–surfactant interactions. Water/oil emulsions are used to template supraparticle formation. The interactions of negatively charged colloidal particles with the surfactants stabilizing the water/oil‐interface are tailored via the local pH within the aqueous droplet. At low pH, protonation of the anionic headgroup of the surfactant decreases electrostatic repulsion of the particles, facilitates interfacial adsorption, and subsequently causes buckling. The local pH of the aqueous droplet phase continuously changes during the assembly process. The supraparticle formation pathway can therefore be controlled by determining the point in time at which interfacial adsorption is enabled by adjusting the initial pH. Consequently, the final supraparticle morphology can be tailored at will, from fully buckled structures, via undulated surface morphologies to spherically rough and spherically smooth supraparticles and crystalline colloidal clusters.During the confined colloidal self‐assembly in emulsion droplets, the morphology of the resulting supraparticles is determined by particle–surfactant interactions. These interactions can be manipulated via the pH of the aqueous emulsion droplets. The change of pH in the course of the assembly process allows controlling these interactions in time and provides a simple means to tailor the final supraparticle morphology, from fully buckled structures to spherical supraparticles. imageDeutsche Forschungsgemeinschaft 10.13039/50110000165

    High-Resolution Nanoscale AC Quantum Sensing in CMOS Compatible SiC

    High-resolution nanoscale nuclear magnetic resonance (NMR) allows measurement of chemical structure at the single-molecule level for determining molecular dynamics. Until now, nitrogen vacancy centers in diamond have been the only platform to demonstrate single-defect NMR sensing at sub-Hz spectral resolution. Using a single silicon vacancy defect prepared under CMOS-compatible conditions in commercial 4H-silicon carbide at room temperature, we use the Synchronized Readout technique to measure a test signal. We achieve a spectral resolution of 0.33 Hz, necessary for understanding molecular structure, and estimate a magnetic sensitivity of 358 μT/ Hzfor our system. We also explore the necessary improvements for achieving single-proton spin sensitivity. Combining these results with future integrated photonics shows a promising path toward scalable nanoscale sensing for low-cost NMR spectrometers based on an industry-mature silicon carbide material.Bundesministerium f?r Bildung und Forschung 10.13039/501100002347Universit?t Ulm 10.13039/50110000897

    Essays in Empirical Public Economics

    This dissertation is of a cumulative nature and it collects chapters covering distinct topics within the realm of empirical public economics. Each chapter is self-contained, but the topics covered can be divided in two main areas. The first area and the corresponding chapter relate to the literature on charitable giving, focusing on the supply of voluntary work. Specifically, chapter 1 asks whether an increase in community needs, as measured by the presence of refugees at the local level, affects the decision of a person to volunteer. One of the aims of chapter 1 is to empirically investigate the role of “pure altruism” (Andreoni 1989, 1990) as a determinant of the decision to volunteer. The second area includes two chapters that relate to the growing literature on the value-added tax (VAT). In particular, the two chapters investigate the effects of the introduction of reverse charge on VAT reporting. In a nutshell, reverse charge is a tax reform that affects VAT remittance by shifting the VAT remittance liability from the seller to the buyer in business-to-business transactions. While reverse charge is widely applied in the European Union and beyond, evidence on the effects of this reform on VAT systems is scant. Chapter 2 (co-authored with Thiess Büttner.) exploits the introduction of reverse charge in Germany to estimate the volume of VAT fraud that was ongoing prior to the reform. Chapter 3 investigates the effect of the introduction of reverse charge on VAT compliance at the business-to-consumer stage. The following paragraphs provide an abstract of each chapter, while more extensive summaries are provided in the Preface. Chapter 1 studies the relationship between changes in community needs and the supply of voluntary work. I present basic theoretical considerations, which suggest that the relationship between voluntary work and an increase in community needs is ambiguous. Then, I test the relationship empirically by proxying community needs with the number of needy people, i.e., refugees, in a county, and by exploiting the quasi-experiment of refugees’ allocation within Germany. I find that doubling the number of refugees increases the probability of volunteering by about 2 percentage points. These estimates imply that 1.45 million people additionally volunteered during the refugee crisis, i.e., more than one person per refugee. Chapter 2 focuses on the introduction of the so-called reverse-charge (RC) mechanism. In order to stop Value-Added Tax (VAT) fraud, EU member states use RC, which effectively removes VAT withholding and refunding in business-to-business transactions. Using the German VAT return data, we examine the effects of the introduction of RC and find that requests of input tax refunding decline sharply in the affected industries, supporting the presence of fraud prior to the introduction of RC. Based on our estimates, we quantify the revenue losses from VAT fraud prior to RC implementation in these industries to be around 5% of VAT revenues. Chapter 3 explores the effects of the implementation of reverse charge on VAT remittances at the retail stage, using data on individual firms’ tax files from the universe of German VAT returns. The key feature of reverse charge is that VAT remittance liability is shifted from the seller to the buyer, in business-to-business transactions. The shift in remittance liability implies the elimination of multi-stage VAT collection and the concentration of VAT remittance at the retail stage. While an effective tool in the fight against cross-border VAT fraud, reverse charge may thus lead to greater VAT evasion at the retail stage. After identifying retailers affected by the reform, I examine if the introduction of reverse charge reduces their VAT compliance. The findings do not indicate that reverse charge leads to greater evasion at the retail stage
