Analyse der schmerzlindernden und immunologischen Effekte von seriellen Radonbädern bei Patienten mit chronisch degenerativen Gelenkerkrankungen im Rahmen der RAD-ON02 Studie
1 Abstract
1.1 Background
Chronic degenerative diseases play a major role in today’s health care system due to their high preva-
lence. These diseases are characterized by destructive processes in the affected joints. The cartilage
perishes, the bony joint surfaces change and the joint it self is finally affected by destruction. Pa-
tients, who suffer from this disease, endure chronic pain, restricted mobility and a reduced quality
of life. Common treatment strategies are primarily symptom-based, often involve a long-term intake
of non-steroidal anti-inflammatory drugs (NSAID) or opiods, and have only a limited effect on the
progression of the disease. Therefore it is important to improve treatment options and find alternative
approaches. The treatment of diseased joints with low-dose radiation either by local irradiation with
X-Rays or by spa treatments with low doses of radon gas are some of these alternatives. In contrast to
the high doses of radiation, which are used for the treatment of malignant diseases, it is assumed that
low-dose radiation has anti-inflammatory effects. In previous studies, a long-lasting analgesic effect
has been proven for radon bath therapy. The RAD-ON01 study provided evidence for radon bath-
induced immunomodulation, which in turn was correlated with pain reduction. Nevertheless, high
quality placebo-controlled trials remained scarce. The RAD-ON02 study was therefore conducted to
provide a more sophisticated evidence for this type of treatment and also to differentiate it from pos-
sible placebo or bath effects.
In the placebo-controlled, double-blind RAD-ON02 study it was assumed, that radon baths are su-
perior to warm water baths in terms of pain relief and that radon baths lead to anti-inflammatory
immunomodulation. In the following, data of 40 patients of the total cohort are presented to get an
impression about the modulations
Attention-guided erasing for enhanced transfer learning in breast abnormality classification
Purpose Breast cancer remains one of the most prevalent cancers globally, necessitating effective early screening and diagnosis. This study investigates the effectiveness and generalizability of our recently proposed data augmentation technique, attention-guided erasing (AGE), across various transfer learning classification tasks for breast abnormality classification in mammography. Methods AGE utilizes attention head visualizations from DINO self-supervised pretraining to weakly localize regions of interest (ROI) in images. These localizations are then used to stochastically erase non-essential background information from training images during transfer learning. Our research evaluates AGE across two image-level and three patch-level classification tasks. The image-level tasks involve breast density categorization in digital mammography (DM) and malignancy classification in contrast-enhanced mammography (CEM). Patch-level tasks include classifying calcifications and masses in scanned film mammography (SFM), as well as malignancy classification of ROIs in CEM. Results AGE significantly boosts classification performance with statistically significant improvements in mean F1-scores across four tasks compared to baselines. Specifically, for image-level classification of breast density in DM and malignancy in CEM, we achieve gains of 2% and 1.5%, respectively. Additionally, for patch-level classification of calcifications in SFM and CEM ROIs, gains of 0.4% and 0.6% are observed, respectively. However, marginal improvement is noted in the mass classification task, indicating the necessity for further optimization in tasks where critical features may be obscured by erasing techniques. Association of HLA-A*02:01 type with efficacy and toxicity of immune checkpoint inhibitor therapy in melanoma patients: a retrospective cohort study
Background Immune checkpoint inhibitors (ICI) are highly effective but may induce severe or even fatal and unpredictable immune-related adverse events (irAEs). It is unclear whether human leukocyte antigen (HLA) genes contribute to the susceptibility of developing irAEs during ICI therapy. Methods This multicentre retrospective study investigated the association of irAE and outcome with HLA-A*02:01 status in a cohort of 97 patients with metastatic melanoma undergoing ICI therapy. Organ-specific irAEs and therapy outcome as assessed by response rate, progression-free survival (PFS) and overall survival (OS) were analysed depending on HLA type HLA–A*02:01. For the outcome only patients with cutaneous melanoma were analysed. Chi square test, exact fisher test, Kruskal Wallis test and log rank test were employed for statistical analysis ( p ≤ 0.05). Results The cohort included 38 HLA-A*02:01 positive (39.2%) and 59 HLA-A*02:01 negative (60.8%) patients. Data showed no evidence of an association of HLA-A*02:01 with organ-specific irAEs except for a numerical difference in immune-related colitis. Furthermore, response rates of the subgroup of patients with metastatic cutaneous melanoma did not differ between the two cohorts. The median PFS was 5 months and 8 months in HLA-A*02:01 positive and negative patients with cutaneous melanoma, respectively. Conclusion HLA-A*02:01 was not associated with specific checkpoint inhibitor-induced organ toxicity in this cohort of HLA-A-typed melanoma patients. Interestingly, in the relatively small subgroup of patients with cutaneous melanoma an earlier progression in HLA-A*02:01 positive patients was observed, however not in the long term. Interprofessionelle Zusammenarbeit aus Sicht der Hausärzte in Bayern - eine qualitative Studie
Hintergrund und Ziele Durch den demographischen Wandel und die dadurch bedingte Überalterung der Gesellschaft wird der medizinische Bedarf der Bevölkerung in den nächsten Jahren deutlich steigen, während gleichzeitig immer weniger Arbeitskräfte zur Verfügung stehen werden. Die zusätzlich bestehende Unattraktivität des Faches Allgemeinmedizin und des Landlebens führen dazu, dass gerade die ländliche hausärztliche Versorgung gefährdet sein wird. Daher müssen alternative Versorgungsmöglichkeiten ausgearbeitet werden. Eine Möglichkeit hierfür bieten interprofessionelle Fallbesprechungen. Hierbei treffen sich verschiedene ärztliche und nichtärztliche medizinische Berufsgruppen, um sich über die Behandlung gemeinsamer Patienten auszutauschen. Durch interprofessionelle Fallbesprechungen kann die Arbeitsbelastung der Hausärzte verringert werden. In anderen Ländern zeigte sich bereits, dass dadurch die Versorgung der Patienten verbessert wird und Patienten und Behandler insgesamt zufriedener sind. In Deutschland werden bisher nur in wenigen Bereichen Fallbesprechungen durchgeführt. Ziel dieser Studie ist, herauszufinden, wie Hausärzte momentan interprofessionell zusammenarbeiten, welche Möglichkeiten und Hürden bestehen und welche Einstellung sie bezüglich interprofessionellen Fallbesprechungen vertreten. Diese Arbeit ist ein Teilprojekt der Studie „Intensiv – Interprofessionelles Team zur nachhaltigen Sicherung der ambulanten Versorgung“, welche die Sichtweisen von Hausärzten, Angehörigen anderer medizinischer Berufsgruppen und Patienten zu interprofessioneller Zusammenarbeit untersucht.
Methoden Es wurde ein qualitatives Studiendesign gewählt, um möglichst explorativ zu arbeiten. Mithilfe von leitfadengestützten Fokusgruppen und Einzelinterviews wurden insgesamt zehn in Bayern tätige Hausärzte befragt. Die Gespräche fanden online statt. Die Auswertung der Daten erfolgte mit der inhaltlich strukturierenden qualitativen Inhaltsanalyse nach Kuckartz. Zusätzlich erhielten die Studienteilnehmer einen soziodemographischen Fragebogen.
Ergebnisse und Beobachtungen Interprofessionelle Zusammenarbeit bedeutet für die befragten Hausärzte Zusammenarbeit sowohl mit ärztlichen als auch mit nichtärztlichen Berufsgruppen. Es gibt mehrere Faktoren, die dazu führen, ob sie eine Zusammenarbeit als positiv oder negativ bewerten, eine wichtige Rolle hierfür spielt Kommunikation. Sie sehen durch interprofessionelle Zusammenarbeit den Vorteil der besseren Versorgung von Patienten, stellen aber auch einen erhöhten Zeitaufwand fest. Ein weiterer Kritikpunkt ist die Wahrnehmung der Befragten, dass ihnen von anderen Berufsgruppen Kompetenz abgesprochen wird. Um eine bessere Zusammenarbeit zu ermöglichen, müssen laut den Hausärzten niederschwelligere Kommunikationsmöglichkeiten ausgearbeitet werden, wobei die zunehmende Digitalisierung hier zur Verbesserung beitragen kann. In ländlichen Gebieten funktioniert aus ihrer Sicht die Zusammenarbeit besser als in der Stadt. Für die zukünftige Zusammenarbeit wünschen sie sich eine stärkere Nutzung der Digitalisierung sowie räumliche Nähe zu anderen Behandlern. Interprofessionelle Fallbesprechungen sehen sie aktuell als nicht in den Alltag integrierbar. Der angenommene Aufwand rechtfertigt nach Ansicht der meisten befragten Hausärzte nicht den erwarteten Nutzen. Am ehesten können laut ihnen interprofessionelle Fallbesprechungen in Pflegeheimen durchgeführt werden.
Schlussfolgerungen und Diskussion Kommunikation ist ein wesentlicher Faktor, der zu einer gelingenden interprofessionellen Zusammenarbeit beiträgt. In der Literatur wird die Wichtigkeit effizienter Kommunikation ebenfalls hervorgehoben. Hausärzten fällt meist die Aufgabe zu, die Zusammenarbeit zu initiieren und koordinieren. Dies wird von den befragten Hausärzten teilweise als Last empfunden, kann aber durch die unterschiedlichen Aufgaben von Haus- und Gebietsärzten erklärt werden. Daher ist es problematisch, wenn Hausärzte interprofessionellen Fallbesprechungen kritisch gegenüberstehen, da sie gerade zu Beginn eine wesentliche Rolle einnehmen. Die bestehenden Hürden, wie Vergütung und eine zu hohe Arbeitslast, müssen also minimiert werden, um interprofessionelle Fallbesprechungen auch für Hausärzte ansprechender zu gestalten
Measuring Personalized Learning in the Smart Classroom Learning Environment: Development and Validation of an Instrument
Smart classrooms leverage intelligent and mobile technologies to create highly interactive, student-centered environments conducive to personalized learning. However, measuring students’ personalized learning experiences in these technologically advanced spaces remains a challenge. This study addresses the gap by developing and validating a Smart Classroom Environment–Personalized Learning Scale (SCE-PL). Drawing on a comprehensive literature review, content-expert feedback, and iterative item refinement, an initial pool of 48 items was reduced to 39 and subsequently to 34 following item-level analyses. Two datasets were collected from Chinese middle-school students across three provinces, capturing diverse socio-economic contexts and grade levels (7th, 8th, and 9th). EFA on the first dataset ( n = 424) revealed a nine-factor structure collectively explaining 78.12% of the total variance. Confirmatory factor analysis (CFA) on the second dataset ( n = 584) verified an excellent model fit. Internal consistency indices (Cronbach’s α > 0.87, composite reliability > 0.75) and strong convergent and discriminant validity evidence (based on AVE and inter-factor correlations) further support the scale’s psychometric soundness. The SCE-PL thus offers researchers, policymakers, and practitioners a robust, theory-driven instrument for assessing personalized learning experiences in smart classroom environments, paving the way for data-informed pedagogy, optimized learning spaces, and enhanced technological integration.This research was funded by the China Scholarship Council’s 2023 Program for Study Abroad for Graduate Students from Key Universities (Grant No. 202306870075). High-Valent Intermediate Observed in a Cu-Based OER Electrocatalyst by Operando X‑ray Absorption Spectroscopy
Understanding the chemical transformations governing the oxygen evolution reaction (OER) in Cu-based electrocatalysts is critical for advancing cost-efficient alkaline water-splitting technologies. In this study, we employ synchrotron-based operando Cu L3-edge X-ray absorption spectroscopy (XAS) and potentiodynamic techniques to probe the key intermediate species involved in alkaline OER. Our findings reveal that this metastable species exhibits an electronic structure resembling high-valent Cu complexes, particularly those associated with the CuO2– ion. Potentiodynamic measurements indicate that the high-valent intermediate emerges at potentials as low as 1.62 VRHE, coinciding with the oxidative process traditionally attributed to the Cu2+ ↔ Cu3+ redox transition, suggesting that the formation of the high-valent intermediate is directly linked to this redox process. Porosity‐Zoned Porous‐Transport Layer for Proton‐Exchange Membrane Water Electrolysis by High‐Velocity Flame Spraying
The porous‐transport layer (PTL) is a crucial component in proton‐exchange membrane water electrolyzers (PEMWE) enabling water and gas transport as well as electrically contacting the catalyst layer (CL). To reduce the overall costs of PTLs, a fabrication method by high‐velocity oxy‐fuel (HVOF) spraying is introduced. Free‐standing PTLs are obtained via the application of a titanium coating onto a substrate and its subsequent separation and thermal treatment. The obtained PTLs feature two sides of different roughness and porosity as analyzed and visualized by X‐ray microscopy. This way, the side with decreased porosity (21%) is intended to function as a microporous layer, improving the contact with the CL. The presented fabrication process promises decreased costs compared to vacuum plasma spraying, a simplified, chemical‐free mechanical separation of the PTL from the substrate, and a high scale‐up suitability. In the results, it is demonstrated that HVOF can produce titanium PTLs with low oxygen content. Additionally, PEMWE single‐cell tests demonstrate that the sprayed PTLs perform on par with a commercially available PTL material.High‐velocity oxy‐fuel spraying is introduced to fabricate cost‐effective porous‐transport layers (PTLs) for proton‐exchange membrane water electrolysis (PEMWE). Vergleich der roboterassistierten minimalinvasiven dorsalen Instrumentierung mit der klassischen Navigation an der Wirbelsäule
Hintergrund und Fragestellung
Ziel der vorliegenden Studie war es, die Durchführung der minimalinvasiven
perkutanen Pedikelschraubenplatzierung an der Wirbelsäule mit der bildge-
steuerten Navigation (Curve, Brainlab AG, München) und einer roboterge-
stützten Lösung (Cirq, Brainlab AG, München) in Bezug auf Präzision und Zeit-
aufwand zu vergleichen.
Material und Methoden
In der Studie wurden 90 Pedikel an der Wirbelsäule zweier Fresh-Frozen-Torsi
instrumentiert. Die Hälfte der Pedikel wurde navigationsgestützt instrumen-
tiert, während die andere Hälfte roboterassistiert eingebracht wurden. Es
wurde eine umfassende Analyse hinsichtlich der Präzision, des Zeitaufwands
und des subjektiven Eindrucks der Operateure durchgeführt. Zeit und Präzi-
sion beider Verfahren wurden auf signifikante Unterschiede untersucht. Die
Studie war als Nicht-Unterlegenheitsstudie konzipiert.
Ergebnisse
Die Präzisionsanalyse zwischen den beiden OP-Techniken erbrachte keinen
signifikanten Unterschied bezüglich der Drahtlage. Die durchschnittliche Dis-
tanzabweichung des Roboters betrug 1,0 mm (min. 0 mm; max. 7 mm; SD
±1,3 mm). Die Winkelabweichung betrug 1,0° (min. 0°; max. 7°; SD ±1,4°). Die
Abweichung bei der konventionellen Navigation betrug 1,0 mm (min. 0 mm;
max. 6 mm; SD ±1,3 mm), die Winkelabweichung 1,5° (min. 0°; max. 7°; SD
±1,6°). Die Distanzabweichung war im Durchschnitt bei beiden Methoden
gleich, es gab keinen signifikanten Unterschied (p=0,44). Der Unterschied bei
der Winkelabweichung war signifikant (p=0,01).
Hinsichtlich des subjektiven Eindrucks der Operateure konnte kein signifikan-
ter Unterschied gezeigt werden.
Die Zeitmessung ergab eine signifikant längere Dauer pro Draht am ersten
Torso von durchschnittlich 120 Sekunden (s) für den Roboterarm (236s (min.
86s; max. 536s; SD ±131s) vs. 116s (min. 62s; max. 242s; SD ±47s), p<0,001).

Der zweite Torso wies eine nicht signifikante, durchschnittlich 16 Sekunden
längere Zeit pro Draht im robotergestützten Verfahren auf (154s (min. 89s;
max. 355s; SD ±68s) vs. 138s (min. 68s; max. 322s; SD ±60s), p=0,2). Der
Zeitunterschied pro Draht bei der Navigation war bei beiden Torsi nicht signi-
fikant (p=0,09). Bei den mit dem Roboterarm instrumentierten Wirbelkörpern
verringerte sich die mittlere Dauer der Positionierung pro Draht im Vergleich
zwischen Torso 1 und Torso 2 signifikant um 81 Sekunden (p=0,007).
Diskussion und Schlussfolgerung
Die robotergestützte Wirbelsäulenchirurgie zeigt vielversprechende Ergeb-
nisse und bietet großes Potenzial für weitere Untersuchungen. Sie scheint
eine sichere und vorteilhafte Methode für die präzise Platzierung von Pedikel-
schrauben zu sein, was die Relevanz dieser Technologie in der modernen
Wirbelsäulenchirurgie unterstreicht.
Ein Zeitvorteil durch den Einsatz des Roboterarms konnte in der vorliegenden
Studie nicht nachgewiesen werden. Es zeigte sich allerdings eine steile Lern-
kurve, so dass nach häufigeren Einsätzen eine Verringerung der OP-Zeit mit
einem Roboter in der Wirbelsäulenchirurgie erwartet werden kann.
Zusammenfassend zeigt sich das robotergestützte Verfahren dem Goldstan-
dard Navigation nicht unterlegen und ist für einen klinischen Einsatz geeignet.
Weitere klinische Studien mit z.B. höheren Fallzahlen sind nötig, um die Vor-
teile dieses Verfahrens weiter zu evaluieren und validieren
