Digital empowerment on hold: DiGA adoption gaps−a German national cross-sectional patient survey study
Digital health applications (DiGAs), prescribable and reimbursed in Germany since 2020, have the potential to enhance patient self-management. This study aimed to assess rheumatology patients’ awareness, willingness to use, suitability, and actual adoption of DiGAs. Between February 17 and April 8, 2025, adult patients attending seven German rheumatology outpatient clinics completed an electronic survey. A total of 246 patients participated (mean age 50.4 years; 71.1% female), with most treated at university hospitals (59.8%). The predominant diagnoses were rheumatoid arthritis (41.1%), psoriatic arthritis (18.3%), and axial spondyloarthritis (10.2%). While only 19.5% reported prior use of medical apps, 39.8% were aware of DiGAs, and 12.6% had used one. Notably, 84.6% reported at least one comorbidity matching an approved DiGA indication, most commonly back pain (54.8%), chronic pain (52.0%), and sleep disorders (35.8%). A majority expressed willingness to regularly use a DiGA (72.4%) and were open to recommendations from their rheumatologists or health insurers (72.8%). Additionally, 76.0% showed interest in a rheumatology-specific DiGA. Despite high interest and relevant comorbidities among patients, current DiGA use was limited. These findings underscore the need for targeted implementation strategies to increase uptake and realize the full potential of digital health applications in rheumatology care. The results also emphasize the need for rheumatologists to actively educate and guide their patients regarding the availability and potential benefits of DiGAs
Pedagogical content knowledge for simulations and mathematical modelling with digital tools: a quasi-experimental study with pre-service mathematics teachers
This study investigates the promotion of pedagogical content knowledge for simulations and mathematical modelling with digital tools among pre-service teachers. This knowledge can be described as modelling-specific TPaCK in the context of digital tools. In a quasi-experimental design, three groups of pre-service teachers (N = 230) were analysed and compared over four semesters. Experimental Group 1 attended a course on simulations and mathematical modelling with digital tools, Experimental Group 2 attended a course on mathematical modelling without the use of digital tools and the control group attended mathematics education courses without specific reference to modelling or digital tools. The results show that TPaCK was promoted significantly more in the group that worked with digital tools than in the other two groups in its four theoretically and empirically derived dimensions (theory knowledge, task knowledge, process knowledge and intervention knowledge). This study confirms that courses specifically designed for digital tools can effectively promote the modelling-specific TPaCK of pre-service mathematics teachers in all four dimensions. The results underscore the importance of integrating digital tools into teacher training and show that general pedagogical knowledge for modelling is not sufficient without a focus on digital tools to build pedagogical knowledge for simulations and mathematical modelling with digital tools
Effects of LASP1-Depletion on Cholesterol Homeostasis in an Atherosclerotic Mouse Model
Atherosklerose beschreibt chronisch-inflammatorische Prozesse, die zum fortschreitenden Verschluss des betroffenen Gefäßes führen. Dadurch ausgelöst, entstehen multiple Krankheitsbilder, welche zusammengefasst als kardiovaskuläre Erkrankungen, innerhalb Deutschlands noch immer an der Spitze der Todesursachen stehen (Statista, zuletzt abgerufen 17.06.2024). Für die Atherogenese spielen insbesondere Zellmigration, -proliferation und -invasionsfähigkeit eine entscheidende Rolle. Als Keyplayer sind hier besonders Makrophagen zu nennen. Das LIM- und SH3-Protein 1 (LASP1), das unter anderem in Makrophagen exprimiert wird, ist ein Aktin-bindendes Gerüstprotein, das an der Zelladhäsion, -migration und -proliferation beteiligt ist und folglich die Pathogenese der Atherosklerose beeinflussen kann. Seine Rolle in der Atherogenese blieb bisher jedoch unerforscht. Dieser Frage wurde nachgegangen, indem Lasp1 im atherosklerotischen Ldlr-/--Mausmodell ausschaltet wurde, um Ldlr/Lasp1-/--Doppelknockout Mäuse zu erzeugen. Unter einer fettreichen Diät (HFD) wiesen Ldlr/Lasp1-/--Mäuse im Vergleich zu den Kontrollmäusen zwar niedrigere Serumcholesterolwerte auf, zeigten jedoch unerwartet mehr atherosklerotische Läsionen in der Aorta. Aufgrund dieser konträren Ergebnisse ist davon auszugehen, dass LASP1 die Cholesterolhomöostase durch zwei unabhängige Mechanismen beeinflusst. Diese darzustellen, war Ziel dieser Arbeit:
Einfluss von LASP1 auf die Entwicklung atherosklerotischer Läsionen:
qRT-PCR- und FACS-Analysen ergaben, dass der Verlust von Lasp1 zu einer erhöhten Expression des LDL-Scavenger-Rezeptors Cd36 auf Knochenmarksmakrophagen führt. Dies konnte nach 12-wöchiger HFD in Schnitten der Aortenwurzeln Lasp1-defizienter Tiere ebenfalls in vivo mittels immunhistochemischer Färbung gegen CD36 bestätigt werden. Kongruent dazu zeigten Vorversuche der Arbeitsgruppe eine erhöhte oxLDL Aufnahme und Schaumzellbildung LASP1-defizienter Makrophagen. Die vergrößerten Läsionen der Doppelknockout Maus scheinen so durch eine erhöhte oxLDL Aufnahme zu entstehen, die durch die erhöhte Cd36 Expression vermittelt wird.
Einfluss von LASP1 auf das Serumcholesterollevel:
Als einer der Mediatoren für die veränderten Serumcholesterolgehalt konnte der intestinale Cholesteroltransporter NPC1L1 identifiziert werden. Dieser wandert, vermittelt über den PI3K-AKT-Signalweg, je nach Cholesterol Gehalt der Zelle vom Zytosol an die apikale Membran, um die intestinale Cholesterolabsorption zu katalysieren. Lasp1-defiziente Mäuse zeigten eine verringerte AKT- Phosphorylierung im Duodenum und schieden vermehrt Cholesterol über die Fäzes aus. Auch im humanen zellulären CaCo2 Zellmodell konnte nach LASP1 Knockdown eine geringere AKT-Phosphorylierung und zudem ein geringerer transzellulärer Cholesteroltransport nachgewiesen werden. Somit kann angenommen werden, dass der LASP1 Knockout den Shuttle des NPC1L1 über AKT beeinträchtigt und so zu einer geringeren intestinalen Cholesterolaufnahme führt, was den gesenkten Serumcholesterollevel in Kombination mit der vermehrten Cholesterolausscheidung der Doppelknockout Mäuse erklärt.Atherosclerosis describes chronic inflammatory processes that lead to progressive occlusion of the affected vessel. Triggered by this, multiple clinical pictures develop, which, summarized as cardiovascular diseases, are still at the top of the causes of death in Germany (Statista, last accessed 17.06.2024). For atherogenesis, cell migration, proliferation and invasion capacity in particular play a decisive role. Macrophages especially are key players here. LIM and SH3 protein 1 (LASP1), expressed among others in macrophages, is an actin-binding scaffold protein involved in cell adhesion, migration, and proliferation and consequently may influence the pathogenesis of atherosclerosis. However, its role in atherogenesis has remained unexplored. This question was addressed by knocking out Lasp1 in the atherosclerotic Ldlr-/- mouse model to generate Ldlr/Lasp1-/- double knockout mice. On a high-fat diet (HFD), Ldlr/Lasp1-/- mice had lower serum cholesterol levels compared with control mice but unexpectedly showed more atherosclerotic lesions in the aorta. Based on these contrasting results, LASP1 is thought to influence cholesterol homeostasis through two independent mechanisms. To delineate these was the aim of this work:
Influence of LASP1 on the development of atherosclerotic lesions:
qRT-PCR and FACS analyses revealed, that loss of Lasp1 leads to increased expression of the LDL scavenger receptor Cd36 on bone marrow macrophages. This was also confirmed in vivo by immunohistochemical staining against Cd36 after 12 weeks of HFD in sections of the aortic roots of Lasp1-deficient animals. Congruent with this, preliminary experiments by the study group showed increased oxLDL uptake and foam cell formation of LASP1-deficient macrophages. Thus, the enlarged lesions of the double knockout mouse appear to result from increased oxLDL uptake mediated by increased CD36 expression.
Influence of LASP1 on serum cholesterol levels:
The intestinal cholesterol transporter NPC1L1 was identified as one of the mediators for the altered serum cholesterol levels. This migrates from the cytosol to the apical membrane, mediated by the PI3K-AKT pathway, depending on the cholesterol content of the cell, to catalyze intestinal cholesterol absorption. Lasp1-deficient mice showed decreased Akt phosphorylation in the duodenum and excreted increased cholesterol in the feces. In addition, in the cellular model, human Caco2 cells showed decreased AKT phosphorylation after LASP1 knockdown as well as decreased transcellular cholesterol transport. Thus, it can be hypothesized that LASP1 knockout impairs the shuttling of NPC1L1 via AKT, leading to lower intestinal cholesterol uptake, which explains the lowered serum cholesterol level and increased cholesterol excretion in the double knockout mice
A Strategic Roadmap to Support Communication on and Acceptance of Surrogate Endpoints: The REnal Surrogacy accEpTance in Chronic Kidney Disease (RESET CKD) Collaboration
Introduction
Developing effective treatments in chronic, progressive diseases like chronic kidney disease (CKD) is challenging because patients may only experience relevant outcomes such as kidney failure after long periods of disease progression. Surrogate endpoints provide a valuable alternative to definitive final patient-relevant outcomes, which may accelerate clinical development processes. However, optimal utilization of surrogate endpoints for reimbursement decisions requires alignment across multiple stakeholders, including health technology assessment (HTA) bodies and reimbursement agencies, who are generally more cautious than regulatory bodies in their acceptance of surrogate endpoint evidence. The aim of this paper is to propose a strategic roadmap to facilitate cross-stakeholder collaboration and support the consideration of surrogate endpoints in regulatory and reimbursement decisions.
Methods
An international group of experts in surrogate endpoints, reimbursement decisions, and kidney disease formed The REnal Surrogacy accEpTance in Chronic Kidney Disease (RESET CKD) Collaboration. This scientific steering committee held several meetings to develop a roadmap of activities with the aim of enabling the appropriate consideration of surrogate endpoints through structured multi-stakeholder engagement involving regulators, clinicians, HTA bodies, payers, industry, and patients.
Results
The strategic roadmap focuses on four areas: identifying the need for evidence; engaging stakeholders; collaborating in regulatory and reimbursement processes; and disseminating evidence. The RESET CKD collaboration is currently implementing the roadmap in the field of CKD through collating relevant evidence for a CKD-relevant surrogate endpoint in a scientific playbook, conducting economic evaluations, developing a position paper, and engaging patient groups.
Conclusions
Disparities between regulatory and reimbursement processes and decisions underscore the need for a structured approach to enhancing transparency, consistency, and timeliness in the use of surrogate endpoint evidence in healthcare decision-making. The roadmap developed through the RESET CKD Collaboration addresses this need and is already demonstrating practical value in its implementation. Although initially focused on CKD, the framework is designed to be transferable to other therapeutic areas. Key challenges remain, including the integration of surrogate endpoints into adaptive pricing models and performance-based agreements
Physical effort and task errors influence the choice for cognitive offloading
Minimizing effort is a principle widely accepted to govern human behavior. We revisited this principle in an extended rotation paradigm. Participants freely chose to solve an object comparison task by either mental or manual rotation of one of two simultaneously presented objects. We manipulated the required force of manual rotation, stimulus complexity, as well as the angular mismatch between both objects, while carefully eliminating confounds of physical effort with time. Our study revealed that both physical and mental effort affect strategy choice. Additionally, strategy choice in a certain trial was influenced by error commissions in the previous trial. These error-induced strategy switches were asymmetric: participants were more inclined to switch following mental than manual rotation errors. Our results suggest that human reactions to error commission when using external tools may differ in a qualitative manner from reactions to error commission during mental endeavors. Theoretical implications are discussed especially in the context of the entanglement of physical effort and time.
Public Significance Statement
Humans possess the ability to solve problems using either mainly mental or physical resources. For example, objects can be rotated in the mind or physically. Here, we show that when comparing the effort associated with both means, they are both considered for decision making. Furthermore, making a mistake seems to evoke different reactions depending on the means with which the error was made. Considering such different costs and benefits is vital for both designing and working in tech-infused environments that support problem solving
Persönliches Wissens- und Informationsmanagement. Eine unterschätzte Kompetenz für (angehende) Musiklehrpersonen?
To ensure the usability of knowledge and information (K&I) in times of rapid change and K&I accumulation, the management of K&I has been regarded increasingly relevant—also for teachers. Although this presumably also applies to preservice music teachers (PMTs), they have not yet been studied as a group that (needs to) manage their personal K&I. Driven by the goal of improving initial music teacher education (IMTE), this work builds a foundation to investigate PKIM education as one possible pathway to achieve this. Therefore, PKIM is defined by its goals, objects, and activities, supported by examples from the field of IMTE. To investigate possibilities to improve IMTE, it is conceptualised as a system whose function is to enable PMTs to offer good music teaching—represented by the variable “music teaching quality” (MTQ). A combined-offer-usage model of IMTE and music education (ME) in school shows the location of MTQ and music teaching competence (MTC) at the interface between those two systems.
To explore how PKIM education might improve MTQ, hypotheses on necessary dispositions for PMT to teach well are derived from MTC models. Accordingly, changes in IMTE can be considered improvements if they support PMTs’ acquisition of these MTC. An analysis of the relations between PKIM competences and MTCs reveals that PKIM competences function both as meta-competences and as intersecting components of MTCs. The analysed MTC models focus on possession of knowledge as a predictor for MTQ. However, given that music teaching—according to the proposed definitions—can be seen as sharing of information, we also need to focus on possession of information as another important predictor for MTQ. On the whole, the accessibility, quality, quantity, and relevance of K&I that PMT possess is logically connected with their MTQ and might be positively influenced by PKIM education. Further research concerning specific interventions could be based on the proposed concepts and hypotheses.Um die Nutzbarkeit von Wissen und Informationen (K&I) in Zeiten von Beschleunigung und Informationsüberfluss zu gewährleisten, wird K&I-Management zunehmend als relevant angesehen. Dies gilt vermutlich auch für angehende Musiklehrpersonen (PMTs). Diese wurden jedoch bisher noch nicht als Manager von K&I untersucht.
Diese Arbeit schafft eine Grundlage für die Erforschung von Bildung in PKIM als möglichem Weg zur Verbesserung des Musiklehramtsstudiums (IMTE). Dazu wird PKIM anhand seiner Ziele, Objekte und Aktivitäten definiert. Um Möglichkeiten zur Verbesserung der IMTE zu untersuchen, wird IMTE als System beschrieben, mit der Funktion, PMTs zu gutem Unterrichten zu befähigen – dargestellt durch die Variable „Musikunterrichtsqualität“ (MTQ). Ein kombiniertes Angebots-Nutzungs-Modell von IMTE und schulischem Musikunterricht (ME) zeigt die Position von MTQ und Musiklehrkompetenz (MTC) an der Schnittstelle zwischen beiden Systemen.
Um Potenziale von PKIM-Bildung für die MTQ zu untersuchen, werden aus MTC-Modellen Hypothesen über die notwendigen Dispositionen für gutes Musikunterrichten zusammengetragen. Veränderungen in IMTE können entsprechend als Verbesserungen betrachtet werden, wenn sie einen besseren Erwerb dieser MTC durch PMTs ermöglichen. Eine Analyse der Beziehungen zwischen PKIM-Kompetenzen und MTCs zeigt, dass PKIM-Kompetenzen sowohl Metakompetenzen als auch Überschneidungsmenge von MTCs sind. Die analysierten MTC-Modelle konzentrieren sich auf Wissensbesitz als Prädiktor für MTQ. Da Unterrichten aber – gemäß den vorgeschlagenen Definitionen – als Teilen von Informationen betrachtet werden kann, muss der Informationsbesitz als weiterer wichtiger Prädiktor für MTQ beachtet werden. Insgesamt ist die Zugänglichkeit, Qualität, Quantität und Relevanz des K&I von PMTs logisch verknüpft mit MTQ, und könnte durch PKIM-Bildung positiv beeinflusst werden. Weitere Forschungen zu spezifischen Interventionen kann auf den vorgeschlagenen Konzepten und Hypothesen aufbauen

In der Inklusions- und Sonderpädagogik ist eine zunehmende Thematisierung von Vulnerabilität wahrzunehmen. Die Zusammenführung von Vulnerabilität und Vulneranz einerseits und Verhaltensauffälligkeiten andererseits stellt eine Leerstelle dar, die es für einen umfassenden Diskurs in der Pädagogik zu füllen gilt. Die Beiträge des vorliegenden Bandes bieten interdisziplinäre Zugänge zum skizzierten Themenbereich und richten sich an alle, die in der pädagogischen Arbeit mit Kindern und Jugendlichen mit Verhaltensauffälligkeiten tätig sind.
Der Band gliedert sich in drei Teilbereiche: Vulnerabilität und Vulneranz werden erstens im Kontext biographischer Erfahrungen betrachtet, zweitens einer fundierten theoretischen und disziplinären Betrachtung unterzogen sowie drittens unter Perspektive verschiedener intersektionaler Zugänge diskutiert
vs . The variation between genitive and as prepositional governments in German
Die vorliegende Arbeit beschäftigt sich mit der Variation zwischen Genitiv- und von-Rektion bei Präpositionen im Deutschen wie unweit des Bahnhofs vs. unweit vom Bahnhof. Das Ziel der vorliegenden Arbeit liegt in der Beantwortung der Fragen, welche Präpositionen diese Variation haben und wie diese Variation erklärt werden kann. Mit Grammatikalisierung deutscher Präpositionen als theoretische Grundlage wird die Hypothese aufgestellt, dass es sich bei der Variation um ein Phänomen handelt, dem der Sprachwandel zu Grunde liegt und dass es bei den Rektionsverläufen zwei unterschiedliche Phänomene gibt.
Empirisch wird in der diachronen Untersuchung im Zeitraum 1465–1999 der Rektionsverlauf ausgewählter Präpositionen erforscht, woraus sich zwei Gruppen von Präpositionen ergeben. Präpositionen mit ursprünglicher Kasusrektion regieren ursprünglich entweder ausschließlich den Genitiv oder den Genitiv und den Dativ, während die von-Rektion als neue Rektion auftritt, die die grammatische Funktion der Präposition von darstellt. Im Gegensatz dazu haben Präpositionen mit ursprünglicher von-Rektion oder Variation der Rektion anfangs entweder ausschließlich die von-Rektion oder sowohl die von-Rektion als auch die Kasusrektion, wobei die von-Rektion als lexikalisch motiviert gilt. Im Laufe der Zeit ändert sich die Rektion in Richtung Genitivrektion. Da der diachrone Rektionswechsel bei der jeweiligen Gruppe von Präpositionen noch nicht abgeschlossen ist, besteht eine Variation zwischen Genitiv- und von-Rektion in der geschriebenen Gegenwartssprache, die weitestgehend morphosyntaktisch motiviert ist.
Darüber hinaus werden in der Arbeit weitere Aspekte in Betracht gezogen. Einerseits stellt sich die Frage, ob die von-Rektion eine umgangssprachliche Variante darstellt. Die Befunde der zusätzlichen Untersuchung zur Variation in der mündlichen Gegenwartssprache zeigen, dass dies nicht bei allen Präpositionen der Fall ist. Andererseits wird der sprachnormative Umgang mit der Variation analysiert. Es stellt sich heraus, dass der Genitiv im Allgemeinen in den früheren Zeiträumen einen höheren Status gegenüber der Dativ- und der von-Variante einnimmt. In den späteren Zeiträumen und in der Gegenwartssprache wird die Variation jedoch von der Sprachnorm deskriptiver und sprachwissenschaftlich fundierter als in der Zeit davor beschrieben, sodass oft eine freie Variation angegeben wird.This dissertation concerns the variation between genitive and von as prepositional governments in German, such as the following variation: unweit des Bahnhofs vs. unweit vom Bahnhof. The purpose of this dissertation is to find out which German prepositions have this variation and consequently to explain this variation. Using the grammaticalization of German prepositions as a theoretical basis, this dissertation proposes the hypothesis that the variation between genitive and von as prepositional governments is based on language change and that it involves two different diachronic changes of prepositional governments.
These changes are then empirically researched on selected prepositions in the diachronic study in the period of 1465–1999. This results in two groups of prepositions. The first group consists of prepositions which originally govern either only genitive or genitive and dative at the same time, whereas von arises as the new government, which shows the grammatical function of the preposition von itself. In contrast, prepositions in the second group originally govern either exclusively von or von and genitive or a variation of genitive and dative at the same time, whereas von is lexically motivated. Over time, they tend to prefer the genitive case. However, since the diachronic change of prepositional government has not yet been completed for the respective group of prepositions, there is a variation between genitive and von as prepositional governments in contemporary German, which is largely morphosyntactically motivated.
Other aspects are also taken into account in this dissertation. On the one hand the question arises as to whether the von-government represents a colloquial variant in contemporary German. The results of the additional study on variation in contemporary spoken German show that this does not apply to every preposition. On the other hand this dissertation also analyses how German linguistic norm handles this and related variations. It turns out that the genitive case generally receives a special, higher status over the dative and the von-variants in the earlier periods. In the later periods and in contemporary German, however, the linguistic norm describes variations in a more descriptive and linguistically reasoned manner than the period before, so that free variation is often given
