Evaluation of the accuracy of an artificial intelligence in identifying contraindications to exercise therapy : Comparison with and interrater reliability of physical therapists judgments
Purpose
The study validates a rule-based system for identifying contraindications to exercise therapy in a medical context. It evaluates accuracy and performance by comparing it with physical therapists’ assessments and patients' characteristics.
Method
The dataset included 80 patient cases with clinical characteristics assessed by 20 physical therapists for contraindications to exercise therapy. Fleiss kappa and pooled kappa values measured agreement between physical therapists and AI. AI performance was assessed by sensitivity, specificity, accuracy and F1 score. Clinical characteristics were compared between therapists' votes using ANOVA and Bonferroni post-hoc test.
Results
The physical therapists had a mean age of 40.85 (8.23) years and a mean experience of 14.53 (8.20) years. Out of 64 patient cases, there was consensus on 35 cases with no contraindication and 29 cases with a consensus on “contraindication exists” for exercise therapy. In 16 cases there was no consensus between therapists. Overall, therapists had 87.5% agreement with Fleiss Kappa κπ = .43. The pooled kappa value between therapists and AI was κpooled = .63. AI achieved perfect values (1) for sensitivity, specificity, accuracy and F1 score. Statistically, consensus-based comparisons by therapists revealed significant differences in pain intensity, duration, timing, and quality.
Conclusion
The study shows significant agreement between physical therapists and the AI, consistent with similar musculoskeletal studies. Various clinical characteristics highlight the importance of clinical reasoning and contraindication detection. In conclusion, advanced technologies such as decision support and expert systems could have a profound impact on clinical practice, improving accuracy, personalized exercises and telemedicine referrals for efficient care and improved patient decisions

Gender diverse people’s psychological wellbeing and identity in the context of gender affirming speech pathology practice : A qualitative study protocol
Background
Gender diverse people may experience discomfort with the sound of their voice. Additionally, their psychological wellbeing, closely connected to a person’s identity, or perspective of themselves, is often reduced. A primary goal of gender affirming healthcare is to support clients’ psychological wellbeing. Speech pathology practices assist clients to alter their voice to develop authentic self-presentation. These practices have been shown to have positive outcomes related to clients’ voices but have not yet been shown to improve overall client psychological wellbeing. Assisting gender diverse people to transform a negative identity perspective into a positive one could have a beneficial impact on their psychological wellbeing. Therefore, to support clients’ psychological wellbeing, gender affirming speech pathology care could benefit from focusing on gender diverse clients’ identity more broadly, i.e., beyond gender. For this type of care, speech pathologists would need to see themselves as responsible for supporting client psychological wellbeing and identity. However, some clinicians may be hesitant to incorporate more holistic approaches to provide such care. This study aims to explore gender diverse speech pathology clients’ views on psychological wellbeing and identity and speech pathologists’ perspectives and actions in supporting clients in that regard.
Methods
This qualitative study approaches the research topic through insights from gender diverse people’s and speech pathologists’ subjective perspectives and experiences on psychological wellbeing and identity. Gender diverse people will participate in one-on-one episodic interviews, whereas speech pathologists providing gender affirming care will participate in focus group discussions. Data will be analysed with reflexive thematic analysis. The study is supported by collaborators from the gender diverse community. Study findings will be disseminated in an accessible manner to healthcare professionals providing gender affirming care, mainly speech pathologists, and to the gender diverse community.
Implications
Study findings are anticipated to contribute to further understanding gender diverse people’s psychological wellbeing and identity in a gender affirming speech pathology context to tailor practices to the unique needs of gender diverse clients
Political Science Perspectives on Consumer Responsibility
Consumers regularly make choices about specific products or services that have a range of consequences such as production conditions, environmental damage, or climate impacts. Responsible consumer decisions could therefore be politically desirable, raising the question of the role of responsibility in consumer policy. To systematically approach various facets of consumer responsibility, this chapter first deals with concepts of responsibility that may be relevant for consumer policy. On this basis, the chapter is dedicated to the concrete assumption of responsibility by the consumer, as this can ultimately lead to political action. It also focuses on consumer responsibility as an expectation for political decisions. The chapter concludes that even responsible consumers in complex consumer societies are dependent on support and provides arguments as to why government interventions in the market are still—or perhaps increasingly—necessary

Neben grundsätzlichen fachlichen und juristischen Aspekten der Vorbehaltsaufgaben in der Pflege stellen sich Fragen zur Gestaltung des Pflegeprozesses in den unterschiedlichen Bereichen der pflegerischen Praxis. Dies liegt daran, dass die fachlich zu bearbeitenden Pflegeprobleme unterschiedlich gelagert, die leistungsrechtlichen Bedingungen sowie die Personen und Institutionen, mit denen die professionelle Pflege im jeweiligen Setting kooperiert, andere sind. Der Beitrag befasst sich mit der Gestaltung des Pflegeprozesses in der ambulanten Pflege und zeigt auf, welche Gegebenheiten die dortigen Pflegeprozesse beeinflussen und wodurch die Wahrnehmung der Vorbehaltsaufgabe „Steuerung des Pflegeprozesses“ erschwert wird

Influence of materiality in professional geriatric care : Conceptual, methodological and empirical insights
Most people become more reliant on care and support as they age. The constitution of ageing people in the context of nursing support thus represents a material aspect in the daily life of these people and must be taken into account in the science of gerontology. However, theories of (geriatric) care have previously been predominantly human-centric. In light of the material turn, the goal of this paper is to highlight the potential to be found in using agential realism to critically examine geriatric care. It will begin by detailing previous perspectives on geriatric care and any use of material aspects to be found in it. It will then present a conceptual-methodical approach that allows for an examination of the act of caring, taking material aspects into account. The application of this approach to empirical material drawn from an example of acute care in Germany will, in conclusion, illustrate significant elements that, in light of agential realism, must also be taken into account when investigating what it means to provide good geriatric care
