Psychotropic polyfarmacy among belgian nursing home residents with mild to moderate dementia
Objectives: Dementia is the most common reason for nursing home admission and is increasingly prevalent. Behavioral and psychiatric symptoms are common in dementia and are commonly treated with psychotropic medications. However, nursing home residents are especially vulnerable to the adverse effects of potentially inappropriate medication. This observational study aimed to determine the rate of psychotropic drug use and psychotropic polypharmacy among Belgian nursing home residents with mild to moderate dementia.
Design: This is a cross-sectional study examining the prevalence of psychotropic medication use.
Setting and participants: Data were collected from the charts of 494 residents with dementia in 23 Belgian nursing homes.
Methods: Medications were categorized according to the Belgian Centre for Pharmacotherapeutic Information. The total number of psychotropic medications and prevalence of psychotropic polypharmacy (≥2 daily psychotropic medications) were calculated, and the effects of age, sex, duration of nursing home stay, dementia severity, and severity of neuropsychiatric symptoms were examined.
Results: A total of 68.6% of residents used at least 1 kind of psychotropic medication, 40.3% used ≥2, and 14% used ≥3. Hypnotics, sedatives, and anxiolytics were the most prescribed category (45.3%), followed by antidepressants (33.0%) and antipsychotics (29.6%). Younger age and female sex were associated with a higher number of psychotropic medications used. More severe polypharmacy was also associated with higher severity of neuropsychiatric symptoms.
Conclusions and implications: There is a high prevalence of psychotropic drug use among Belgian nursing home residents with dementia. This is comparable with findings in other countries. Considering the risk of adverse effects, we advise further efforts to review the use of potentially inappropriate medications.status: Publishe
The Fate of Behavioral Modernity
Over the past twenty years, empirical and theoretical advances have significantly reshaped the research agenda on “behavioral modernity” and its conceptualization. Major shifts involve the material signatures of the phenomenon, the general archaeological pattern, the proposed mechanisms of evolutionary change, and species attribution. In response, some researchers have called for eliminating the concept altogether. In this paper, I contend that, while eliminativist arguments are gaining traction, their justification remains unsatisfactory. I consider three angles of attack: (1) the relationship between “anatomical” and “behavioral” modernity and their purported shared fate, and two explicit eliminativist positions as advanced by (2) Shea (2011) and (3) Garofoli (2016). I suggest that major concerns can be addressed through conceptual “housekeeping” and, possibly as a result, “relabeling”, while contending that the ongoing debate has not yet erased a fundamental explanandum—a transition—that still warrants further investigation. This pertains to a conceptual core that has not yet been defeated. The effect of publicly co-funded industry-science collaboration on scientific production
Competitive industry-science collaboration programs encourage academic scientists to co-develop innovation projects with firms. These programs combine attributes of competitive research funding and science commercialization policies. Because they demand more time and effort than traditional science funding, and may push applicants toward projects with higher commercial potential, the question arises whether they come at the expense of scientific productivity or alter the direction of research. Using data from a large-scale, cross-country R&D policy, we find no evidence of negative impacts on science. On the contrary, our analysis shows an increase in joint scientific publications with industrial partners, while the overall direction of research remains unchanged.sponsorship: Authors are listed alphabetically. Open Data and Equity: Naming the Issues, Blaming the Causes, and Claiming Solutions. An Exploratory and Interdisciplinary Systematic Literature Review
Open data fall short of their goal to empower all social groups equally. Although the literature examines this issue through the concept of inclusion, substantial gaps remain in defining and understanding the implications of open data for equity in public administration, with research on this topic scattered across disciplines. This fragmentation hinders the possibility of evaluating public policies. To address this gap, we ask: What is the state of the art (naming) on equity in relation to open data, particularly regarding the causes and effects of inequities (blaming) and the strategies to address them (claiming)? Our interdisciplinary review of 69 studies finds that open data serve as a valuable tool for detecting inequities. However, they also raise concerns related to data justice, as inequities in open data arise from epistemic injustice, commodification, capability gaps, financial constraints, and governance structures reinforcing power asymmetries. To address these issues, we suggest balancing data pluralism with standardization and shifting research data practices toward reflexivity. Other strategies focus on governance and encompass stewardship and the adoption of collective benefit models. Dielectric waveguides can be used as a low-loss, high-bandwidth channel for mm-Wave signals, making them suitable for high-speed, short-range wired communication links. The increasing performance of digital CMOS technologies has enabled the design of low-cost integrated mm-Wave transceivers. In addition, polymer fibers are cheap and easy to manufacture. This makes dielectric waveguide technology a cost-effective solution compared to optical links. Furthermore, the superior channel characteristics enable faster and longer links compared to copper wireline.
This work focuses on noncoherent communication, making the transceiver circuits simpler and more power-efficient, as well as obviating the need for carrier recovery. The introduction of a more linear envelope detection strategy based on rectification enables higher-order modulations. In addition, envelope detectable single-sideband signals are proposed as a means of further increasing the spectral efficiency.
On the system level, reflections on electrically-long structures are examined and found to severely degrade the performance. To remedy this, a low-reflection coupler structure is proposed, in addition to using transceivers with well-matched mm-Wave and baseband interfaces.
The mm-Wave amplifying circuits themselves can be made stable, robust and isolated by unilateralizing the individual transistor stages. Where matching networks are often designed using ad-hoc methods, this work introduces a systematic approach based on traditional filter-design techniques which offers improved insight into the trade-offs and allows better control over the resulting network performance.
Three dielectric waveguide links were designed:
▪ A link operating at 125 GHz using amplitude-shift keying, achieving an unequalized data rate of 27 Gb/s over 1 m of plastic fiber. By applying signal processing techniques, up to 50 Gb/s PAM4 was possible over 3 m.
▪ A dual-band PAM4 link, with bands at 117.5 and 152.5 GHz, combined and separated by an on-chip diplexer. The combined bands reach 100 Gb/s over a 3-m fiber. The low-band operating stand-alone, achieves up to 50 Gb/s over 7 m.
▪ A receiver for an envelope-detectable single-sideband link. The increased spectral efficiency enables higher symbol rates, with NRZ data rates of 36 Gb/s over 1 m and 32 Gb/s over 5 m of fiber being demonstrated.status: Publishe
Pure Deep Red Electroluminescence in Mixed Phase Quasi-2D Tin Iodide Perovskite Heterostructure
Mixed cation quasi-2D tin iodide perovskites are promising materials for deep-red to NIR emitting light-emitting diode (LED) applications. However, phase purity remains the major concern to avoid mixed emissions from two or more wavelengths. This work presents the evolution of pure deep red electroluminescence (EL) centred at ≈700 nm corresponding to the emission of pure n = 2 2D crystalline phase. However, structural and optical studies evidenced the formation of mixed-phase heterostructures consisting of n = 1, n = 2, and n = ∞ phase (i.e., FASnI3). It is observed that altering the mixed cation ratio, phase distribution is modified, affecting the EL behavior, causing the observation or absence of the n = 1 2D phase EL emission peak. Selective charge transport and their cascade mechanism illustrates the pure color EL evolution with the support from transient absorption spectroscopic studies. In this regard, a case study is provided along with the supportive rationalities behind the exceptional evolution of pure deep red EL even from the mixed-phase perovskite, reported for the first time in case of quasi-2D tin iodide perovskite.sponsorship: B.Z. and Q.S. would like to acknowledge MEXT KAKENHI Grant (24K01295, 26286013).
