Get PDF心停止蘇生後の病態生理に，患者の状態を悪化させる病因物質（mediator）の関与が知られており，特に炎症性cytokineの血中濃度が高い状態として“Cytokine Storm”の存在が知られている．又，心停止蘇生後の病態生理において，ミトコンドリア機能障害が重要な役割を果たしていることが分かってきた．ミトコンドリアは酸素を消費し細胞内でATPを生成することで，細胞の機能を支える重要な細胞小器官である．一方で，ATPが細胞外にもれ出ることにより，そのATPが細胞死や炎症反応を惹起することが分かっている．心停止という致死的病態における病態生理の解明は喫緊の課題である．そのキーとなる，炎症の主役であるcytokine，病態の根幹となりえるミトコンドリア機能について，最新の知見と自験例をふまえつつ解説する．departmental bulletin pape

Get PDF放射線治療の目標は有害事象を生ずることなく局所制御を図ることにあり，空間的線量分布の改善（腫瘍への線量集中性の向上），時間的線量配分の工夫（線量分割法の最適化），薬物相互作用の活用（腫瘍における放射線感受性の向上）の３つの領域で開発が進められてきた．近年著しい発達を見せたのが空間的線量分布の改善で，外照射技術と画像誘導技術の進歩がこれを支えた．現在では体幹部定位放射線治療（SBRT）と強度変調放射線治療（IMRT）は多くの癌腫において重要な役割を担っている．また多くの施設で画像誘導放射線治療（IGRT）が実施されている．今後はがんの生物学的背景およびその周辺薬剤を活用した治療法の開発が求められる．シリーズ 最新のがんdepartmental bulletin pape
Case report of port-site hernia in laparoscopic surgery and a review of the causes and preventive measures for its development
Get PDF緒言：腹腔鏡手術は低侵襲手術であり，近年増加傾向であり，合併症の報告も同じく増加している．その合併症の一つに，トロッカー創部におけるポートサイトヘルニアがある．発症した場合，イレウスが併発し，緊急手術を要する場合がある．その頻度は，0.1～0.5％程度であり，比較的稀な合併症の一つである．当科では，2013年から2023年の2,819例の婦人科腹腔鏡手術のうち5例のポートサイトヘルニアを経験した．今回われわれはその5例の症例報告及びその発症原因と予防策を文献的考察も合わせて報告する．
症例：5症例の平均年齢は61歳（範囲46-76歳）であった．腹腔鏡手術を施行した後，術後平均7日（範囲7-14日）にそれぞれイレウス症状が悪化し，緊急手術を実施した．すべての症例で12mmトロッカー創部よりポートサイトヘルニアが起きており，腸管完納後に創部を閉創した．また，症例4では筋膜は閉じており，腹膜と筋膜の間にヘルニアが生じ，発症した．それ以外の4症例では，筋膜が開いており，縫合不全で発症した．症例3，5に関しては，ヘルニアの修復時に小腸切除術も余儀なくされた．
考察：ポートサイトヘルニアは，患者要因と手術操作に伴う様々な要因が挙げられる．これらの症例を経験した後に，当科では縫合糸誘導針を用いて，創部を全層で縫合する対策を行った．その後ヘルニア，イレウスの発症はなくなったため，大多数の症例は予防できると考えられる．departmental bulletin pape
Long-Term Clinical Outcomes of the Patients with Chronic Myeloid Leukemia Treated with Nilotinib as the 1st-Line Tyrosine Kinases Inhibitor at the Kindai university hospital
Get PDFBackground: Nilotinib has been shown to be a more potent tyrosine kinase inhibitor of BCR::ABL1 than imatinib. We evaluated long-term efficacy and safety based on the 7-year follow-up data in patients with newly diagnosed chronic myeloid leukemia (CML) who were treated with nilotinib as the 1st-line therapy (1L-NIL) at the Kindai university hospital.
Methods: In this single-center, observational study, we enrolled patients with newly diagnosed CML in chronic phase (CML-CP) who received 1L-NIL since December 2010, when nilotinib received regulatory approval for CML-CP in Japan. The primary endpoint was overall survival (OS) of the 1L-NIL patients. In addition, we analyzed molecular responses and safety profiles as the key secondary endpoints.
Results: The median observation period was 6.8 (range: 1.2-12.3) years. The estimated OS and progression-free survival rates of the 1L-NIL patients (n=25) were both 95.5%. By 18 months, the cumulative achievement rates of major molecular response (MMR) and MR4.5 were 81.3% and 25.0%, respectively. Adverse events (AEs) occurred in 21 of 23 patients (91.3%) with Grade ≥3 in 8 of 23 patients (34.8%). We observed no unexpected serious AEs in this research.
Conclusions: Our data showed that the efficacy of nilotinib persisted over the 7-year follow-up and long-term administration of nilotinib was not associated with unacceptable late toxic effects at the Kindai university hospital.departmental bulletin pape
Study on machine interface using gestures
Get PDFSeveral attempts to realize friendly and espressive machine interface using gesture recognition and usages are described. We employed the published software libraries based on Python language for speech recognition, gesture recognition, and controlling of humanoid robot and game machine for the purpose. Four examples of the systems are reported which have been considered in our lab.: (1) Improving expressiveness with Pepper™ and a robot hand. (2) Remote control of Pepper using a conference system. (3) Realizing presentations with Pepper. (4) Controlling Nintendo Switch™ with gestures. Thanks to the software libralies and devices of high performances achieved, these example systems work well. The site was 100m x 300m and had 35 buildings. The main building had a large hall of 200 tatami mats and a guest room. There was also a Sukiya-style tearoom. Additionally, there were six sake breweries and a Western-style building. The per capita levy of the municipal tax and Local government finance
Get PDF本稿では、国と地方の財政関係を見直して資金の移転を縮小する案として個人住民税における均等割の増額の検討をおこなった。本稿で得られた結果として市町村民税均等割を現在の標準税率である3,500円から7,000円に増額した場合、4,132.8億円の増収となることがわかった。10,000円とした場合、7,703.7億円の増収となることがわかった。均等割を3,500円にしても不交付団体数は増加することはなく、10,000円にしても不交付団体数は10減少するにすぎないことがわかった。地方の財源をより拡充させるためのいまひとつの案として地方消費税が考えられる。地方税の原則の1つである安定性の見地から地方消費税の割合引き上げも一考すべきだろう。The paper examines the proposal to reduce the transfer of funds by revisiting the fiscal relationship between the national and local governments, specifically through the option of increasing the per capita levy of the municipal tax. The findings indicate that raising the per capita levy of the municipal tax from the current standard rate of 3,500 yen to 7,000 yen would result in an additional revenue of 413.28 billion yen. If increased to 10,000 yen, the additional revenue would amount to 770.37 billion yen. It was also found that even with the per capita levy set at 3,500 yen, the number of non-subsidized municipalities does not increase, and with the levy set at 10,000 yen, the number only decreases by 10. Another proposal to enhance local financial resources is the introduction of a local VAT. Firstly, previous foreign studies of destination marketing are reviewed and their background, basic logical structure, and expected outcomes are discussed.
