Increasing efficacy of contact-tracing applications by user referrals and stricter quarantining
We study the effects of two mechanisms which increase the efficacy of contact-tracing applications (CTAs) such as the mobile phone contact-tracing applications that have been used during the COVID-19 epidemic. The first mechanism is the introduction of user referrals. We compare four scenarios for the uptake of CTAs — (1) the p% of individuals that use the CTA are chosen randomly, (2) a smaller initial set of randomly-chosen users each refer a contact to use the CTA, achieving p% in total, (3) a small initial set of randomly-chosen users each refer around half of their contacts to use the CTA, achieving p% in total, and (4) for comparison, an idealised scenario in which the p% of the population that uses the CTA is the p% with the most contacts. Using agent-based epidemiological models incorporating a geometric space, we find that, even when the uptake percentage p% is small, CTAs are an effective tool for mitigating the spread of the epidemic in all scenarios. Moreover, user referrals significantly improve efficacy. In addition, it turns out that user referrals reduce the quarantine load. The second mechanism for increasing the efficacy of CTAs is tuning the severity of quarantine measures. Our modelling shows that using CTAs with mild quarantine measures is effective in reducing the maximum hospital load and the number of people who become ill, but leads to a relatively high quarantine load, which may cause economic disruption. Fortunately, under stricter quarantine measures, the advantages are maintained but the quarantine load is reduced. Our models incorporate geometric inhomogeneous random graphs to study the effects of the presence of super-spreaders and of the absence of long-distant contacts (e.g., through travel restrictions) on our conclusions
A habenula-insular circuit encodes the willingness to act
The decision that it is worth doing something rather than nothing is a core yet understudied feature of voluntary behaviour. Here we study “willingness to act”, the probability of making a response given the context. Human volunteers encountered opportunities to make effortful actions in order to receive rewards, while watching a movie inside a 7 T MRI scanner. Reward and other context features determined willingness-to-act. Activity in the habenula tracked trial-by-trial variation in participants’ willingness-to-act. The anterior insula encoded individual environment features that determined this willingness. We identify a multi-layered network in which contextual information is encoded in the anterior insula, converges on the habenula, and is then transmitted to the supplementary motor area, where the decision is made to either act or refrain from acting via the nigrostriatal pathway

Matrix-bound growth factors are released upon cartilage compression by an aggrecan-dependent sodium flux that is lost in osteoarthritis
Articular cartilage is a dense extracellular matrix-rich tissue that degrades following chronic mechanical stress, resulting in osteoarthritis (OA). The tissue has low intrinsic repair especially in aged and osteoarthritic joints. Here, we describe three pro-regenerative factors; fibroblast growth factor 2 (FGF2), connective tissue growth factor, bound to transforming growth factor-beta (CTGF-TGFβ), and hepatoma-derived growth factor (HDGF), that are rapidly released from the pericellular matrix (PCM) of articular cartilage upon mechanical injury. All three growth factors bound heparan sulfate, and were displaced by exogenous NaCl. We hypothesised that sodium, sequestered within the aggrecan-rich matrix, was freed by injurious compression, thereby enhancing the bioavailability of pericellular growth factors. Indeed, growth factor release was abrogated when cartilage aggrecan was depleted by IL-1 treatment, and in severely damaged human osteoarthritic cartilage. A flux in free matrix sodium upon mechanical compression of cartilage was visualised by 23Na -MRI just below the articular surface. This corresponded to a region of reduced tissue stiffness, measured by scanning acoustic microscopy and second harmonic generation microscopy, and where Smad2/3 was phosphorylated upon cyclic compression. Our results describe a novel intrinsic repair mechanism, controlled by matrix stiffness and mediated by the free sodium concentration, in which heparan sulfate-bound growth factors are released from cartilage upon injurious load. They identify aggrecan as a depot for sequestered sodium, explaining why osteoarthritic tissue loses its ability to repair. Treatments that restore matrix sodium to allow appropriate release of growth factors upon load are predicted to enable intrinsic cartilage repair in OA
The radio loudness of SDSS quasars from the LOFAR Two-metre Sky Survey: ubiquitous jet activity and constraints on star formation
We examine the distribution of radio emission from ∼42 000 quasars from the Sloan Digital Sky Survey, as measured in the LOFAR Two-metre Sky Survey (LoTSS). We present a model of the radio luminosity distribution of the quasars that assumes that every quasar displays a superposition of two sources of radio emission: active galactic nuclei (jets) and star formation. Our two-component model provides an excellent match to the observed radio flux density distributions across a wide range of redshifts and quasar optical luminosities; this suggests that the jet-launching mechanism operates in all quasars but with different powering efficiency. The wide distribution of jet powers allows for a smooth transition between the ‘radio-quiet’ and ‘radio-loud’ quasar regimes, without need for any explicit bimodality. The best-fitting model parameters indicate that the star formation rate of quasar host galaxies correlates strongly with quasar luminosity and also increases with redshift at least out to z ∼ 2. For a model where star formation rate scales as Lαbol(1+z)β, we find α = 0.47 ± 0.01 and β = 1.61 ± 0.05, in agreement with far-infrared studies. Quasars contribute ≈0.15 per cent of the cosmic star formation rate density at z = 0.5, rising to 0.4 per cent by z ∼ 2. The typical radio jet power is seen to increase with both increasing optical luminosity and black hole mass independently, but does not vary with redshift, suggesting intrinsic properties govern the production of the radio jets. We discuss the implications of these results for the triggering of quasar activity and the launching of jets
Maternal depression is the predominant persistent risk for child cognitive and social-emotional problems from early childhood to pre-adolescence: A longitudinal cohort study
Rationale
Brain development occurs rapidly during early childhood and continues throughout middle childhood. Early and later windows of opportunity exist to alter developmental trajectories. Few studies in low- and middle-income countries have examined the importance of the timing of exposure to risks for poor pre-adolescent cognitive and social-emotional outcomes.
Methods
We assessed 359 children who participated in two follow-up studies of the Supplementation with Multiple Micronutrients Intervention Trial conducted in Indonesia in 2001–2004: at 3.5 years in 2006 and 9–12 years in 2012–2014. Using structural equation models, we examined indicators of early childhood (3.5 y) and pre-adolescent (9–12 y) exposure to risks (child height-for-age z-score [HAZ], hemoglobin [Hb], maternal depressive symptoms [MDS], home environment [HOME]), with two developmental outcomes: cognitive ability and social-emotional problems. We characterized patterns of change by calculating residuals of indicators measured earlier (3.5 y) predicting the same indicators measured later (9–12 y), for example, the residual of 3.5 y MDS predicting 9–12 y MDS (rMDS).
Results
Three early risk indicators (HOME, Hb, and MDS) were indirectly associated with pre-adolescent cognitive scores through early cognitive scores (HOME: 0.15, [95% CI 0.09, 0.21]; Hb: 0.08 [0.04, 0.12], MDS: −0.07 [-0.12, −0.02]). Pre-adolescent cognitive scores were also associated with change in MDS (rMDS: −0.13 [-0.23, −0.02]) and Hb (rHb: 0.10 [0.00, 0.20]) during middle childhood. For pre-adolescent social-emotional problems, both early childhood MDS (0.31 [0.19, 0.44]) and change in MDS during middle childhood (rMDS: 0.48 [0.37, 0.60]) showed strong direct associations with this outcome.
Conclusions
Our findings confirm those of previous studies that prevention of risk exposures during early childhood is likely to support long-term child development. It also adds evidence to a previously scarce literature for the middle childhood period. Prevention of maternal depressive symptoms and child anemia during middle childhood should be assessed for effectiveness to support child development
Transformation of primary care during the COVID-19 pandemic: experiences of healthcare professionals in eight European countries
Background: Primary care has a crucial role in responding to the COVID-19 pandemic as the first point of patient care and gatekeeper to secondary care. Qualitative studies exploring the experiences of healthcare professionals during the COVID-19 pandemic have mainly focused on secondary care. Aim: To understand the experiences of European PCPs working during the first peak of the COVID-19 pandemic. Design and Setting: An exploratory qualitative study, using semi-structured interviews in primary care in England, Belgium, the Netherlands, Ireland, Germany, Poland, Greece and Sweden, between April and July 2020. Method: Interviews were audio recorded, transcribed and analysed using a combination of inductive and deductive thematic analysis techniques. Results: Eighty interviews were conducted with PCPs. PCPs had to make their own decisions on how to rapidly transform services in relation to COVID-19 and non-COVID-19 care. Despite being overwhelmed with guidance, they often lacked access to practical training. Consequently, PCPs turned to their colleagues for moral support and information to try to quickly adjust to new ways of working, including remote care, and deal with uncertainty. Conclusion: PCPs rapidly transformed primary care delivery despite a number of challenges. Representation of primary care at policy level and engagement with local primary care champions will facilitate easy and coordinated access to practical information on how to adapt services, ongoing training and access to appropriate mental health support services for PCPs. Preservation of autonomy and responsiveness of primary care are critical to preserve the ability for rapid transformation in any future crisis of care delivery
Viral detection and identification in 20 min by rapid single-particle fluorescence in-situ hybridization of viral RNA
The increasing risk from viral outbreaks such as the ongoing COVID-19 pandemic exacerbates the need for rapid, affordable and sensitive methods for virus detection, identification and quantification; however, existing methods for detecting virus particles in biological samples usually depend on multistep protocols that take considerable time to yield a result. Here, we introduce a rapid fluorescence in situ hybridization (FISH) protocol capable of detecting influenza virus, avian infectious bronchitis virus and SARS-CoV-2 specifically and quantitatively in approximately 20 min, in virus cultures, combined nasal and throat swabs with added virus and likely patient samples without previous purification. This fast and facile workflow can be adapted both as a lab technique and a future diagnostic tool in enveloped viruses with an accessible genome
Rossby wave dynamics over South America explored with automatic tropical–extratropical cloud band identification framework
During austral summer, persistent tropical-extratropical cloud bands, such as the SouthAtlantic convergence zone (SACZ) over South America (SAm), link the tropical humid areas to the subtropics. In this study, we utilize an automatic objectbased methodology to identify synoptic cloud band events occurring over SAm that are responsible for almost 60% of the precipitation during the rainy season (November-March). In addition to identifying SACZ events as cloud bands persisting 4 or more days, the framework also highlights the relevance of transient events (i.e., events persisting for 3 days or less) to the climatology. The location and persistence of the cloud band events are modulated by the propagation of synoptic-scale extratropical disturbances interacting with intraseasonal variability in the basic-state upper-level zonal wind. During persistent events (i.e., lasting 4 or more days), upper-level westerly anomalies over the subtropics favor the propagation extratropical disturbances deeper into the tropics. Conversely, transient events occur when the Bolivian high is displaced/expanded southeastward, bringing upper-level easterly winds into subtropical latitudes and blocking the propagation of Rossby waves into lower latitudes. Subsequent anomalous subtropical convection from the cloud bands result in sources of Rossby waves that interact with the basic flow, resulting in downwind enhancement or damping of the extratropical disturbances. The adopted methodology proved to be a powerful framework in demonstrating this interaction between scales, with the basic state influencing and being modified by the synoptic disturbances
Association of peripartum management and high maternal blood loss at cesarean delivery for placenta accreta spectrum (PAS): A multinational database study
INTRODUCTION: Placenta accreta spectrum (PAS) carries a high burden of adverse maternal outcomes, especially significant blood loss, which can be life-threatening. Different management strategies have been proposed but the association of clinical risk factors and surgical management options during cesarean delivery with high blood loss is not clear. MATERIAL AND METHODS: In this international multicenter study, 338 women with PAS undergoing cesarean delivery were included. Fourteen European and one non-European center (USA) provided cases treated retrospectively between 2008 and 2014 and prospectively from 2014 to 2019. Peripartum blood loss was estimated visually and/or by weighing and measuring of volume. Participants were grouped based on blood loss above or below the 75th percentile (>3500 ml) and the 90th percentile (>5500 ml). RESULTS: Placenta percreta was found in 58% of cases. Median blood loss was 2000 ml (range: 150-20 000 ml). Unplanned hysterectomy was associated with an increased risk of blood loss >3500 ml when compared with planned hysterectomy (adjusted OR [aOR] 3.7 [1.5-9.4], p = 0.01). Focal resection was associated with blood loss comparable to that of planned hysterectomy (crude OR 0.7 [0.2-2.1], p = 0.49). Blood loss >3500 ml was less common in patients undergoing successful conservative management (placenta left in situ, aOR 0.1 [0.0-0.6], p = 0.02) but was more common in patients who required delayed hysterectomy (aOR 6.5 [1.7-24.4], p = 0.001). Arterial occlusion methods (uterine or iliac artery ligation, embolization or intravascular balloons), application of uterotonic medication or tranexamic acid showed no significant effect on blood loss >3500 ml. Patients delivered by surgeons without experience in PAS were more likely to experience blood loss >3500 ml (aOR 3.0 [1.4-6.4], p = 0.01). CONCLUSIONS: In pregnant women with PAS, the likelihood of blood loss >3500 ml was reduced in planned vs unplanned cesarean delivery, and when the surgery was performed by a specialist experienced in the management of PAS. This reinforces the necessity of delivery by an expert team. Conservative management was also associated with less blood loss, but only if successful. Therefore, careful patient selection is of great importance. Our study showed no consistent benefit of other adjunct measures such as arterial occlusion techniques, uterotonics or tranexamic acid
