    Effect of 3D printing technology and print orientation on the trueness of additively manufactured definitive casts with different tooth preparations.

    OBJECTIVES To evaluate the fabrication trueness of additively manufactured maxillary definitive casts with various tooth preparations fabricated with different 3-dimensional (3D) printers and print orientations. METHODS A maxillary typodont with tooth preparations for a posterior 3-unit fixed partial denture, lateral incisor crown, central incisor and canine veneers, first premolar and second molar inlays, and a first molar crown was digitized with an industrial scanner. This scan file was used to fabricate definitive casts with a digital light processing (DLP) or stereolithography (SLA) 3D printer in different orientations (0-degree, 30-degree, 45-degree, and 90-degree) (n = 7). All casts were digitized with the same scanner, and the deviations within each preparation site were evaluated. Generalized linear model analysis was used for statistical analysis (α = 0.05). RESULTS The interaction between the 3D printer and the print orientation affected measured deviations within all preparations (P ≤ 0.001) except for the lateral incisor crown and canine veneer (P ≥ 0.094), which were affected only by the main factors (P < 0.001). DLP-90 mostly led to the highest and DLP-0 mostly resulted in the lowest deviations within posterior tooth preparations (P ≤ 0.014). DLP-30 led to the lowest deviations within the first premolar inlay and DLP-45 led to the lowest deviations within the central incisor veneer preparation (P ≤ 0.045). CONCLUSIONS Posterior preparations of tested casts had the highest trueness with DLP-0 or DLP-30, while central incisor veneer preparations had the highest trueness with DLP-45. DLP-90 led to the lowest trueness for most of the tooth preparations. CLINICAL SIGNIFICANCE Definitive casts with tooth preparations fabricated with the tested DLP 3D printer and the print orientation adjusted on tooth preparation may enable well-fitting restorations. However, 90-degree print orientation should be avoided with this 3D printer, as it led to the lowest fabrication trueness

    Intra-aneurysmal contrast agent stasis during intraoperative digital subtraction angiography may predict long-term occlusion after clipping.

    PURPOSE The routine use of intraoperative digital subtraction angiography (iDSA) increases detection of intracranial aneurysm (IA) remnants after microsurgical clipping. Spontaneous thrombosis of IA remnants after clipping is considered a rare phenomenon. We analyse iDSA characteristics to find predictors for IA remnant thrombosis. METHODS IA with intraoperative detection of a remnant after clipping were identified and divided into remnants experiencing spontaneous thrombosis, and remnants with long-term patency and/or remnant growth. Angiographic features of iDSA were analysed and compared between the two groups. RESULTS Of 37 IAs with intraoperative remnant on 3D-iDSA, five sustained a spontaneous remnant thrombosis and remained occluded in long-term follow-up. In all five cases, iDSA revealed delayed inflow and consequent stasis of the contrast agent until the late venous phase. On the other hand, in all cases with persistent long-term IA remnants (n = 32) iDSA demonstrated timely arterial contrast inflow and wash-out without stasis of intra-aneurysmal contrast agent. CONCLUSIONS Contrast stasis in IA remnants during iDSA appears to predict long-term IA occlusion, indicating that clip correction manoeuvres or even attempted endovascular treatment of the remnant IA may be avoided in these patients

    Robust optimization and assessment of dynamic trajectory and mixed-beam arc radiotherapy: apreliminary study.

    Dynamic trajectory radiotherapy (DTRT) and dynamic mixed-beam arc therapy (DYMBARC) exploit non-coplanarity and, for DYMBARC, simultaneously optimized photon and electron beams. Margin concepts to account for set-up uncertainties during delivery are ill-defined for electron fields. We develop robust optimization for DTRT&DYMBARC and compare dosimetric plan quality and robustness for both techniques and both optimization strategies for four cases.&#xD;Approach. Cases for different treatment sites and clinical target volume (CTV) to planning target volume (PTV) margins, m, were investigated. Dynamic gantry-table-collimator photon paths were optimized to minimize PTV/organ-at-risk (OAR) overlap in beam's-eye-view and minimize potential photon multileaf collimator (MLC) travel. For DYMBARC plans, non-isocentric partial electron arcs or static fields with shortened source-to-surface distance (80 cm) were added. Direct aperture optimization (DAO) was used to simultaneously optimize MLC-based intensity modulation for both photon and electron beams yielding deliverable PTV-based DTRT&DYMBARC plans. Robust-optimized plans used the same paths/arcs/fields. DAO with stochastic programming was used for set-up uncertainties with equal weights in all translational directions and magnitude δ such that m= 0.7δ. Robust analysis considered random errors in all directions with or without an additional systematic error in the worst 3D direction for the adjacent OARs. &#xD;Main results. Electron contribution was 7%-41% of target dose depending on the case and optimization strategy for DYMBARC. All techniques achieved similar CTV coverage in the nominal (no error) scenario. OAR sparing was overall better in the DYMBARC plans than in the DTRT plans and DYMBARC plans were generally more robust to the considered uncertainties. OAR sparing was better in the PTV-based than in robust-optimized plans for OARs abutting or overlapping with the target volume, but more affected by uncertainties. &#xD;Significance. Better plan robustness can be achieved with robust optimization than with margins. Combining electron arcs/fields with non-coplanar photon trajectories further improves robustness and OAR sparing

    The prioritisation of curable sexually transmitted infections among pregnant women in Zambia and Papua New Guinea: Qualitative insights.

    Curable sexually transmitted infections (STIs) are neglected in public health policy, services and society at large. Effective interventions are available for some STI but seem not to be prioritised at global, regional or local levels. Zambia and Papua New Guinea (PNG) have a high burden of STIs among pregnant women but little is known about the prioritisation of STI treatment and care among this group. We undertook a qualitative study to explore how STIs are prioritised among pregnant women in local health systems in Zambia and PNG. Semi-structured interviews were conducted with 19 key informants-health care workers providing antenatal care, and policy and programme advisers across the two countries. Audio recordings were transcribed and translated into English and stored, managed, and coded in NVivo v12. Analysis used deductive and inductive thematic analysis. Findings were coded against the World Health Organization health system building blocks. Participants spoke about the stigma of STIs at the community level. They described a broad understanding of morbidity associated with undiagnosed and untreated STIs in pregnant women. The importance of testing and treating STIs in pregnancy was well recognised but many spoke of constraints in providing these services due to stock outs of test kits for HIV and syphilis and antibiotics. In both settings, syndromic management remains the mainstay for treating curable STIs. Clinical practice and treatment were not in alignment with current STI guidelines in either country, with participants recognising the need for mentorship and in-service training, as well as the availability of commodities to support their clinical practice. Local disruptions to screening and management of syphilis, HIV and other curable STIs were widely reported in both countries. There is a need to galvanise priority at national and regional levels to ensure ongoing access to supplies needed to undertake STI testing and treatment

    A new flow path: eDNA connecting hydrology and biology

    Environmental DNA (eDNA) has revolutionized ecological research, particularly for biodiversity assessment in various environments, most notably aquatic media. Environmental DNA analysis allows for non-invasive and rapid species detection across multiple taxonomic groups within a single sample, making it especially useful for identifying rare or invasive species. Due to dynamic hydrological processes, eDNA samples from running waters may represent biodiversity from broad contributing areas, which is convenient from a biomonitoring perspective but also challenging, as hydrological knowledge is required for meaningful biological interpretation. Hydrologists could also benefit from eDNA to address unsolved questions, particularly concerning water movement through catchments. While naturally occurring abiotic tracers have advanced our understanding of water age distribution in catchments, for example, current geochemical tracers cannot fully elucidate the timing and flow paths of water through landscapes. Conversely, biological tracers, owing to their immense diversity and interactions with the environment, could offer more detailed information on the sources and flow paths of water to the stream. The informational capacity of eDNA as a tracer, however, is determined by the ability to interpret the complex biological heterogeneity at a study site, which arguably requires both biological and hydrological expertise. As eDNA data has become increasingly available as part of biomonitoring campaigns, we argue that accompanying eDNA surveys with hydrological observations could enhance our understanding of both biological and hydrological processes; we identify opportunities, challenges, and needs for further interdisciplinary collaboration; and we highlight eDNA's potential as a bridge between hydrology and biology, which could foster both domains

    Infrared live imaging and navigated laser for nematode photocoagulation in a child with diffuse unilateral subacute neuroretinitis (DUSN).

    PURPOSE To report the application of an infrared fundus imaging and navigated laser system to photocoagulate a nematode in diffuse unilateral subacute neuroretinitis (DUSN). OBSERVATIONS A 14-year-old boy with DUSN was treated with systemic albendazole and corticosteroids. Laser photocoagulation of the visible nematode was performed using a navigated laser in live infrared fundus view (Navilas 577s, OD-OS GmbH, Berlin, Germany). While the localization of the nematode was difficult in regular fundoscopy due to the light-shy helminth, it could be well localized and targeted with the infrared live video mode and navigated laser system. No inflammatory flare-up was observed after the nematode was killed. CONCLUSIONS AND IMPORTANCE Laser photocoagulation and systemic antihelminthic therapy are an established treatment for DUSN. Infrared imaging and navigated laser systems seem useful in targeting and killing mobile nematodes

    Maternal manipulation of offspring size can trigger the evolution of eusociality in promiscuous species.

    Eusocial organisms typically live in colonies with one reproductive queen supported by thousands of sterile workers. It is widely believed that monogamous mating is a precondition for the evolution of eusociality. Here, we present a theoretical model that simulates a realistic scenario for the evolution of eusociality. In the model, mothers can evolve control over resource allocation to offspring, affecting offspring's body size. The offspring can evolve body-size-dependent dispersal, by which they disperse to breed or stay at the nest as helpers. We demonstrate that eusociality can evolve even if mothers are not strictly monogamous, provided that they can constrain their offspring's reproduction through manipulation. We also observe the evolution of social polymorphism with small individuals that help and larger individuals that disperse to breed. Our model unifies the traditional kin selection and maternal manipulation explanations for the evolution of eusociality and demonstrates that-contrary to current consensus belief-eusociality can evolve despite highly promiscuous mating
