Toepassing van innovatieve 3D visualisatie bij embryologie voor vroedkunde studenten
Het onderwijs in de embryologie vormt een uitdaging. Dit vak maakt deel uit van het curriculum van de opleiding vroedkunde en wordt door de studenten als moeilijk ervaren. De reden hiervoor is de nood aan ruimtelijk inzicht en ook het visualiseren van de
opeenvolging van dynamische embryonale ontwikkelingen in de 4de dimensie van tijd. In het traditioneel onderwijs (hoorcollege) zijn de mogelijkheden voor interactie en activatie geringer en is de cognitieve belasting hoger.
Tijdens een workshop, waarin diverse aspecten van de organogenese gelinked werden de interactieve pdf’s, werd een verkennend mixed -method, interventioneel effectonderzoek uitgevoerd aan de hand van kwantitatieve bevraging van de leeruitkomsten (MC-vragen) met een voor- en een nameting en een kwantitatieve/kwalitatieve nameting voor de leerervaring. Door de kleine steekproefgrootte en het ontbreken van de controlegroep was een statistische bewerking niet mogelijk. In de beschrijvende data-analyse brengen we wel naar voren, dat de studenten die het slechtst scoorden op de voormeting de grootste respons vertoonden. Bovendien vonden de studenten de workshop wel zinvol. Echter verder onderzoek ter optimalisatie van het embryologieonderwijs is noodzakelijk met een grotere steekproefgrootte en een controlegroep
The CONDUCT-AF trial: Rationale and design of a prospective, randomized, multicentre study comparing conduction system and biventricular pacing in patients undergoing atrioventricular node ablation for heart failure with atrial fibrillation
Aims There is a lack of data from randomized clinical trials comparing treatment outcomes between conduction system pacing (CSP) modalities and biventricular pacing (BVP) in symptomatic patients with refractory atrial fibrillation (AF) scheduled for atrioventricular node ablation (AVNA). The CONDUCT-AF investigates whether CSP is non-inferior to BVP in improving left ventricular ejection fraction (LVEF) and clinical outcomes in heart failure (HF) patients with symptomatic AF undergoing AVNA. Methods This study is an investigator-initiated, prospective, randomized, multicentre clinical trial conducted across 10 European centres, enrolling 82 patients with symptomatic AF, HF with reduced LVEF, and narrow QRS. Participants will be randomized 1:1 to CSP or BVP with subsequent AVNA and followed for at least 24 months. The primary endpoint is the change in LVEF after 6 months. Secondary endpoints will include time to the first occurrence of worsening HF or cardiovascular death and its individual components, total number of HF hospitalizations, change in quality of life, N-terminal pro-B-type natriuretic peptide, 6-min walk test distance, and safety outcomes. Conclusions The CONDUCT-AF trial will provide critical insights into the optimal pacing modality for patients with HF and refractory AF undergoing AVNA. Recruitment is expected to conclude in 2025, with the first study results anticipated in 2026.The CONDUCT-AF trial is an investigator-initiated, independent clinical trial. Het proces naar een duurzame implementatie van een prehabilitatieproject voor personen met blaaskanker die een radicale cystectomie zullen ondergaan
Introductie: Patiënten die een radicale cystectomie (RC) ondergaan hebben een hoog risico op postoperatieve complicaties, ondanks het gebruik van ERAS-protocol. Multimodale prehabilitatieprogramma’s kunnen de postoperatieve weerbaarheid verhogen. Binnen het Jessa Ziekenhuis werd prehabilitatie al opgenomen, maar de fysieke component kent uitdagingen in zijn implementatie.
Doel: Deze studie onderzoekt de barrières en mogelijkheden voor de implementatie van fysieke prehabilitatie als onderdeel van een multimodaal traject bij patiënten met niet-gemetastaseerde spierinvasieve blaaskanker die een RC zullen ondergaan.
Conclusie: Een vroege doorverwijzing naar fysische geneeskunde/revalidatie is een haalbare en direct toepasbare oplossing om het aanbod van prehabilitatie te verbeteren. De inhoud van de fysieke interventie dient afgesteld te worden op de biopsychosociale noden van de patiënt. Het includeren van patiëntenperspectieven in een volgende fase is mede daardoor sterk aangeraden
Kwaliteit en bruikbaarheid van gezondheidsdata in de eerste lijnszorg: een analyse van de diabetesbarometer als instrument voor kwaliteitsverbetering
De masterproef analyseert de kwaliteit en bruikbaarheid van gezondheidsdata in de eerstelijnszorg, met de diabetesbarometer als casus. De hypothese is dat de implementatie van dit datakwaliteitsinstrument leidt tot een significante toename in de registratiegraden van kernparameters voor diabetes type 2.
De studie maakt gebruik van een mixed-methods casestudy in een Vlaamse groepspraktijk met zeven huisartsen, twee verpleegkundigen en een huisarts in opleiding. De methodologie omvat een longitudinale analyse van EMD-registraties over zes meetmomenten, gecombineerd met observaties van interdisciplinaire teamoverleggen.
De resultaten tonen aan dat de registratiegraden significant zijn gestegen. De grootste relatieve toename werd waargenomen bij eerder ondergeregistreerde parameters, zoals het voetonderzoek (van 14% naar 44%). Onbedoelde effecten waren onder meer initiële weerstand, seizoensschommelingen en een tijdelijk plateau in een labo-ratio door een fout in een sjabloon.
De conclusie is dat de implementatie van de diabetesbarometer leidde tot substantiële en duurzame verbeteringen in dataregistratie8888. De onderliggende succesfactoren — technische optimalisatie, taakdelegatie en een feedbackcultuur — zijn breed toepasbaar binnen de eerstelijnszorg en tonen aan dat betrouwbare EMD-data een hefboom zijn voor kwaliteitsverbeterin
Automated compartmental modelling of chemical reactors
Het ontwerpen van optimale reactorconfiguraties voor exotherme reacties is complex en vereist het balanceren van vaak conflicterende doelstellingen. Traditionele methoden zijn tijdrovend en verkennen de grote ruimte aan configuraties vaak niet efficiënt, zeker wanneer criteria zoals reactorvolume, tussenkoeling en procescomplexiteit samen overwogen moeten worden.
Deze thesis ontwikkelt een tweetraps genetisch algoritme voor het optimaliseren van adiabatische reactorconfiguraties. Het doel is de efficiëntie van de configuratie te maximaliseren, terwijl het totale reactorvolume, de tussenkoeling en het aantal adiabatische stappen geminimaliseerd worden, met als einddoel een totale conversie van 0,95.
Allereerst bepaalt een multi-objectief NSGA-II-algoritme de optimale volgorde van adiabatische stappen. In de tweede fase selecteert een single-objectief algoritme het meest geschikte reactortype per stap. De methode werd getest op vier casestudies met verschillende doelstellingsgewichten. Resultaten tonen dat het algoritme zich aanpast aan prioriteiten: focus op volumevermindering leidt tot meer stappen, minder tussenkoeling bevordert hogere starttemperaturen, en minder adiabatische stappen resulteren in grotere reactors. Het algoritme levert efficiënte configuraties op in circa 10 minuten per gewichtscombinatie wat veel sneller is dan handmatige methoden, die een dag kunnen duren en vaak suboptimaal blijven. Dit maakt het project tot een handige tool voor geautomatiseerd modelleren en besluitvorming in de chemische technologie
Intergenerational Formative Epistemic Injustice: A Wittgensteinian Critique of Dominant World-Pictures
This article critically examines dominant formulations of epistemic injustice, focusing on Miranda Fricker's tradition and its broader influence. We argue that much of the literature on epistemic injustice is formulated within a specific Western world-picture, and we discuss its implications. A significant source of this confinement is the view of language as merely a tool for communication. Drawing on Wittgenstein, we argue that this way of conceiving language ignores its deeper connections to our forms of life and that the generalization of epistemic injustice definitions, intended to be universally applicable, perpetuates intergenerational formative epistemic injustice, which has persisted since colonial times. This, in turn, limits epistemic capacity formation and the theory's applicability in different contexts, such that claims of including marginalized ways of knowing may, in fact, facilitate their co-optation and assimilation within a Western framework. Given the global diversity of epistemic harm and its intergenerational character, this article argues that the disaggregation of the scholarship and the recognition of diverse world-pictures as constitutive of its conceptual formation are essential to disrupting epistemic injustice
Radiomics Quality Score 2.0: towards radiomics readiness levels and clinical translation for personalized medicine
Radiomics is a tool for medical imaging analysis that could have a relevant role in precision oncology by offering precise quantitative support for clinical decision-making. The Radiomics Quality Score (RQS) is a tool developed to assess the rigour of radiomics studies that has now been widely adopted by researchers. Although RQS version 1.0 established a benchmark, an updated framework is required to account for evolving knowledge and ensure optimal evaluation of the quality of radiomics studies through the inclusion of fairness, explainability, rigorous quality control and harmonization. In this Review, we introduce the updated RQS 2.0, which maintains the scientific rigour of its predecessor and addresses these contemporary needs, and therefore could potentially accelerate clinical translation. Moreover, we introduce the radiomics readiness levels, inspired by the technology readiness level framework, which are integrated in RQS 2.0 and reflect nine distinct levels of incremental improvement in radiomics research with the ultimate aim of clinical implementation. Atherosclerosis after pre-eclampsia: systematic review and meta-analysis
Objective Pre-eclampsia complicates up to 8% of pregnancies and is associated with increased risk of ischemic cardiac and cerebral disease, which may be prevented through management of cardiovascular risk when early disease stages are detected. This meta-analysis aimed to determine the prevalence of clinical and subclinical atherosclerosis in women after pre-eclamptic vs non-pre-eclamptic pregnancy with advancing maternal age.Methods A systematic search of the literature was conducted in PubMed, Embase and Web of Science for studies reporting on the prevalence of atherosclerosis in women with a previous pre-eclamptic pregnancy and those with a previous uncomplicated pregnancy. Any systemic atherosclerosis documented using ultrasound or computed tomography was included. Random-effects meta-analysis was used to compute the odds ratio (OR) with 95% CI for the association between pre-eclampsia and the presence of atherosclerosis. Subgroup analysis was conducted according to average maternal age at evaluation.Results A total of 11 articles were included (13 217 participants). The average maternal age at evaluation ranged from 32 to 60 years. Within this age range, the pooled OR for the presence of atherosclerotic plaque after pre-eclampsia was 1.57 (95% CI, 1.39-1.78). The pooled OR of developing atherosclerotic plaque after a pre-eclamptic vs non-pre-eclamptic pregnancy increased gradually with advancing maternal age. The OR was not significant in the 30-39-year-old group (0.64 (95% CI, 0.10-4.15)), but the odds of finding an atherosclerotic plaque were significantly increased after pre-eclamptic pregnancy in the 40-49-year-old group (OR, 1.59 (95% CI, 1.34-1.89)) and 50-60-year-old group (OR, 2.00 (95% CI, 1.30-3.08)). At any given age, the percentage plaque prevalence in formerly pre-eclamptic women was roughly equal to that seen 10 years later in women with a previous non-pre-eclamptic pregnancy.Conclusions Women with a previous pre-eclamptic pregnancy exhibit atherosclerosis more frequently and approximately 10 years earlier compared with women with a previous non-pre-eclamptic pregnancy. Targeted primary prevention is required to reduce morbidity and mortality from premature cardiovascular disease in women after pre-eclampsia. (c) 2025 The Author(s).
