    Insecticide-treated net distribution in Western Kenya: impacts related to COVID-19 and health worker strikes

    We examined the impact of coronavirus disease (COVID) mitigation, supply and distribution interruptions on the delivery of long-lasting insecticide-treated nets (LLINs) in Western Kenya. The median monthly distribution of LLINs declined during COVID mitigation strategies (March-July 2020) and during the health worker strikes (December 2020-February 2021). Recovery periods followed initial declines, indicative of a 'catching up' on missed routine distribution. Mass community campaigns were delayed by 10 months. These observations offer encouragement for routine net distribution resilience, but complete interruptions of planned mass distributions require alternate strategies during pandemics

    A local filtering technique for robot skin data

    Algorithms for tactile data processing heavily rely on the generation of tactile images representing the contact distribution. This approach allows to exploit existing algorithms for image processing, but requires the integration of the tactile elements on a flat surface. Robotic skins technologies introduce challenges related to the non-regular distribution of the tactile elements and the non-planar surface over which they are integrated. In this paper we present a method to address these challenges by developing a local filtering technique directly applicable on large-area tactile sensing systems. The proposed filter can process the contact distribution without the need of intermediate steps (generation of a tactile image). We focus particularly on the design of a filter to detect sharp variations in the contact distribution, i.e. \textit{edges}. The approach is validated in a task of planar contour following performed using a robot equipped with two different end-effectors (planar and non-planar) sensorized with large-area tactile sensing technology. Additional experiments have been performed to evaluate strengths and limitations of the proposed approach with respect to tactile image-based data processing techniques

    Detection of a particle shower at the Glashow resonance with IceCube

    The Glashow resonance describes the resonant formation of a W- boson during the interaction of a high-energy electron antineutrino with an electron1, peaking at an antineutrino energy of 6.3 petaelectronvolts (PeV) in the rest frame of the electron. Whereas this energy scale is out of reach for currently operating and future planned particle accelerators, natural astrophysical phenomena are expected to produce antineutrinos with energies beyond the PeV scale. Here we report the detection by the IceCube neutrino observatory of a cascade of high-energy particles (a particle shower) consistent with being created at the Glashow resonance. A shower with an energy of 6.05 ± 0.72 PeV (determined from Cherenkov radiation in the Antarctic Ice Sheet) was measured. Features consistent with the production of secondary muons in the particle shower indicate the hadronic decay of a resonant W- boson, confirm that the source is astrophysical and provide improved directional localization. The evidence of the Glashow resonance suggests the presence of electron antineutrinos in the astrophysical flux, while also providing further validation of the standard model of particle physics. Its unique signature indicates a method of distinguishing neutrinos from antineutrinos, thus providing a way to identify astronomical accelerators that produce neutrinos via hadronuclear or photohadronic interactions, with or without strong magnetic fields. As such, knowledge of both the flavour (that is, electron, muon or tau neutrinos) and charge (neutrino or antineutrino) will facilitate the advancement of neutrino astronomy

    Relapse and post-discharge body composition of children treated for acute malnutrition using a simplified, combined protocol: A nested cohort from the ComPAS RCT

    Introduction Severe and moderate acute malnutrition (SAM and MAM) affect more than 50 million children worldwide yet 80% of these children do not access care. The Combined Protocol for Acute Malnutrition Study (ComPAS) trial assessed the effectiveness of a simplified, combined SAM/MAM protocol for children aged 6–59 months and found non-inferior recovery compared to standard care. To further inform policy, this study assessed post-discharge outcomes of children treated with this novel protocol in Kenya. Methods Six ‘combined’ protocol clinics treated SAM and MAM children using an optimised mid-upper arm circumference (MUAC)-based dose of ready-to-use therapeutic food (RUTF). Six ‘standard care’ clinics treated SAM with weight-based RUTF rations; MAM with ready-to-use supplementary food (RUSF). Four months post-discharge, we assessed anthropometry, recent history of illness, and body composition by bioelectrical impedance analysis. Data was analysed using multivariable linear regression, adjusted for age, sex and allowing for clustering by clinic. Results We sampled 850 children (median age 18 months, IQR 15–23); 44% of the original trial sample in Kenya. Children treated with the combined protocol had similar anthropometry, fat-free mass, fat mass, skinfold thickness z-scores, and frequency of common illnesses 4 months post-discharge compared the standard protocol. Mean subscapular skinfold z-scores were close to the global norm (standard care: 0.24; combined 0.27). There was no significant difference in odds of relapse between protocols (SAM, 3% vs 3%, OR = 1.0 p = 0.75; MAM, 10% vs 12%, OR = 0.90 p = 0.34). Conclusions Despite the lower dosage of RUTF for most SAM children in the combined protocol, their anthropometry and relapse rates at 4 months post-discharge were similar to standard care. MAM children treated with RUTF had similar body composition to those treated with RUSF and neither group exhibited excess adiposity. These results add further evidence that a combined protocol is as effective as standard care with no evidence of adverse effects post-discharge. A simplified, combined approach could treat more children, stretch existing resources further, and contribute to achieving Sustainable Development Goal Two

    X-ray free-electron laser studies reveal correlated motion during isopenicillin N synthase catalysis

    Isopenicillin N synthase (IPNS) catalyzes the unique reaction of L-δ-(α-aminoadipoyl)-L-cysteinyl-D-valine (ACV) with dioxygen giving isopenicillin N (IPN), the precursor of all natural penicillins and cephalosporins. X-ray free-electron laser studies including time-resolved crystallography and emission spectroscopy reveal how reaction of IPNS:Fe(II):ACV with dioxygen to yield an Fe(III) superoxide causes differences in active site volume and unexpected conformational changes that propagate to structurally remote regions. Combined with solution studies, the results reveal the importance of protein dynamics in regulating intermediate conformations during conversion of ACV to IPN. The results have implications for catalysis by multiple IPNS-related oxygenases, including those involved in the human hypoxic response, and highlight the power of serial femtosecond crystallography to provide insight into long-range enzyme dynamics during reactions presently impossible for nonprotein catalysts

    A new combination testing methodology to identify accurate and economical point-of-care testing strategies

    Background: Quick, cheap and accurate point-of-care testing is urgently needed to enable frequent, large-scale testing to contain COVID-19. Lateral flow tests for antigen and antibody detection are an obvious candidate for use in community-wide testing, because they are quick and cheap relative to lab-processed tests. However, their low accuracy has limited their adoption. We develop a new methodology to increase the diagnostic accuracy of a combination of cheap, quick and inaccurate index tests with correlated or discordant outcomes, and illustrate its performance on commercially available lateral flow immunoassays (LFIAs) for Sars-CoV-2 antibody detection. Methods and Findings: We analyze laboratory test outcomes of 300 serum samples from health care workers detected with PCR-confirmed SARS-Cov-2 infection at least 21 days prior to sample collection, and 500 pre-pandemic serum samples, from a national seroprevalence survey, tested using eight LFIAs (Abbott, Biosure/Mologic, Orientgene-Menarini, Fortress, Biopanda I, Biopanda II, SureScreen and Wondfo) and Hybrid DABA as reference test. For each of 14 two-test combinations (e.g., Abbott, Fortress) and 16 three-test combinations (e.g., Abbott, Fortress, Biosure/Mologic) used on at least 100 positive and 100 negative samples, we classify an outcome sequence – e.g., (+,–) for (Abbott, Fortress) – as positive if its combination positive predictive value (CPPV) exceeds a given threshold, set between 0 and 1. Our main outcome measures are the sensitivity and specificity of different classification rules for classifying the outcomes of a combination test. We define testing possibility frontiers which represent sensitivity and false positive rates for different thresholds. The envelope of frontiers further enables test selection. The eight index tests individually meet neither the UK Medicines and Healthcare Products Regulatory Agency’s 98% sensitivity and 98% specificity criterion, nor the US Center for Disease Control’s 99.5% specificity criterion. Among these eight tests, the highest single-test LFIA specificity is 99.4% (with a sensitivity of 65.2%) and the highest single-test LFIA sensitivity is 93.4% (with a specificity of 97.4%). Using our methodology, a two-test combination meets the UK Medicines and Healthcare Products Regulatory Agency’s criterion, achieving sensitivity of 98.4% and specificity of 98.0%. While two-test combinations meeting the US Center for Disease Control’s 99.5% specificity criterion have sensitivity below 83.6%, a three-test combination delivers a specificity of 99.6% and a sensitivity of 95.8%. Conclusions: Current CDC guidelines suggest combining tests, noting that “performance of orthogonal testing algorithms has not been systematically evaluated” and highlighting discordant outcomes. Our methodology combines available LFIAs to meet desired accuracy criteria, by identifying testing possibility frontiers which encompass benchmarks, enabling cost savings. Our methodology applies equally to antigen testing and can greatly expand testing capacity through combining less accurate tests, especially for use cases needing quick, accurate tests, e.g., entry to public spaces such as airports, nursing homes or hospitals

    Evaluating clinical characteristics studies produced early in the Covid-19 pandemic: a systematic review

    Objectives: Clinical characterisation studies have been essential in helping inform research, diagnosis and clinical management efforts, particularly early in a pandemic. This systematic review summarises the early literature on clinical characteristics of patients admitted to hospital, and evaluates the quality of evidence produced during the initial stages of the pandemic. Methods: MEDLINE, EMBASE and Global Health databases were searched for studies published from January 1st 2020 to April 28th 2020. Studies which reported on at least 100 hospitalised patients with Covid-19 of any age were included. Data on clinical characteristics were independently extracted by two review authors. Study design specific critical appraisal tools were used to evaluate included studies: the Newcastle Ottawa scale for cohort and cross sectional studies, Joanna Briggs Institute checklist for case series and the Cochrane collaboration tool for assessing risk of bias in randomised trials. Results: The search yielded 78 studies presenting data on 77,443 people. Most studies (82%) were conducted in China. No studies included patients from low- and middle-income countries. The overall quality of included studies was low to moderate, and the majority of studies did not include a control group. Fever and cough were the most commonly reported symptoms early in the pandemic. Laboratory and imaging findings were diverse with lymphocytopenia and ground glass opacities the most common findings respectively. Clinical data in children and vulnerable populations were limited. Conclusions: The early Covid-19 literature had moderate to high risk of bias and presented several methodological issues. Early clinical characterisation studies should aim to include different at-risk populations, including patients in non-hospital settings. Pandemic preparedness requires collection tools to ensure observational studies are methodologically robust and will help produce high-quality data early on in the pandemic to guide clinical practice and public health policy.     Post-traumatic stress disorder symptoms among journalists repeatedly covering COVID-19 news

    The coronavirus pandemic has resulted in many journalists repeatedly covering stories related to human suffering. This study investigates whether these journalists experienced higher rates of psychological distress, post-traumatic stress disorder (PTSD) and depressive symptoms than those who have been working during the pandemic yet covering stories other than COVID-19 and aims to identify what factors may protect journalists from developing trauma-related symptoms. We assessed journalists (n = 120) working during the COVID-19 pandemic using self-report measures. Journalists repeatedly covering COVID-19 stories had significantly higher psychological distress (η2 = 0.04) and PTSD symptoms (η2 = 0.08), but not depression, compared to journalists who did not report on COVID-19. Rumination and numbing in response to unwanted memories predicted PTSD symptoms (R2 = 0.53) and may be risk factors for PTSD in this population. Unhelpful resilience appraisals distinguished journalists who reported on COVID-19 and who developed distressing re-experiencing symptoms from those who similarly reported on distressing material and who did not develop symptoms. Targeting resilience appraisals may be helpful in reducing re-experiencing symptoms after trauma exposure
