    Currency Under War Communism: An Example of Gresham's Law?

    This article covers the consequences of the Bolsheviks' coup d'état in 1917 for the Soviet monetary system. I argue that the episode illustrates Gresham's Law, as three different currencies made an appearance in the Russian economy. This monetary disorder is scarcely mentioned by Marxist authors. Este año, la exposición colectiva se basa en la noción de que nuestro hogar, nuestro medio ambiente y nuestro planeta están bajo presión, instándonos a trabajar juntos para explorar un camino sostenible hacia adelante. Un total de 217 proyectos se presentan del 20 de mayo al 26 de noviembre de 2023 en Palazzo Bembo, Palazzo Mora y los Jardines de Marinaressa, en el corazón de Venecia, Italia. / The project is selected in the catalog and book of the European Cultural Center (ECC) that presents the sixth edition of its extensive biennial architecture exhibition entitled Time Space Existence.     Effects of MIdazolam versus MOrphine in acute cardiogenic pulmonary edema and chronic obstructive pulmonary disease: An analysis of MIMO trial

    Aims: Chronic obstructive pulmonary disease (COPD) is an important comorbidity in heart failure. The MIMO trial showed that patients with acute cardiogenic pulmonary edema (ACPE) treated with midazolam had fewer serious adverse events than those treated with morphine. In this post hoc analysis, we examined whether the presence/ absence of COPD modifies the reduced risk of midazolam over morphine. Methods: Patients >18 years old clinically diagnosed with ACPE and with dyspnea and anxiety were randomized (1:1) at emergency department arrival to receive either intravenous midazolam or morphine. In this post hoc analysis, we calculated the relative risk (RR) of serious adverse events in patients with and without COPD. Calculating the CochranMantel-Haenszel interaction test, we evaluated if COPD modified the reduced risk of serious adverse events in the midazolam arm compared to morphine. Results: Overall, 25 (22.5%) of the 111 patients randomized had a history of COPD. Patients with COPD were more commonly men with a history of previous episodes of heart failure, than participants without COPD. In the COPD group, the RR for the incidence of serious adverse events in the midazolam versus morphine arm was 0.36 (95%CI, 0.1-1.46). In the group without COPD, the RR was 0.44 (95%CI, 0.22-0.91). The presence of COPD did not modify the reduced risk of serious adverse events in the midazolam arm compared to morphine (p for interaction =0.79).     Dolor torácico secundario al consumo de drogas atendidos en urgencias hospitalarias en España

    Objective To determine the characteristics of patients with chest pain (CP) associated with recent drug use. Methods Study of cases from the REUrHE registry attended in the emergency department of 11 Spanish hospitals for CP following recreational drug use. Results CP accounted for 8.97% of attendances (males 82.9%, p < 0.001). Cocaine was present in 70% of cases, followed by cannabis (35.7%) and amphetamines and derivatives (21.4%). The most frequent initial symptoms were: palpitations (45.5%, p < 0.001), anxiety (42.5%, p < 0.001), hypertension (13.6%, p < 0.001) and arrhythmias (5.9%, p < 0.001). Patients with TD received more treatment (81.9% vs 74.1%; p < 0.001), although they were admitted less (7.6%, p = 0.0), with no differences in terms of CPR manoeuvres, sedation, intubation, or admission to intensive care (1.9%). Conclusions In CP following acute drug intoxication, cocaine use predominates, although cases of cannabis use are increasing.Objetivo Conocer las características de los pacientes con dolor torácico (DT) asociado al consumo reciente de drogas. Método Estudio de los casos del Registro REUrHE atendidos en Urgencias de 11 hospitales españoles por DT tras el consumo de drogas de uso recreacional. Resultados El DT supuso el 8,97% de las asistencias (varones 82,9%, p < 0,001). La cocaína estaba presente en el 70% de los casos, seguida del cannabis (35,7%) y las anfetaminas y derivados (21,4%). La clínica inicial más frecuente fue: palpitaciones (45,5%, p < 0,001), ansiedad (42,5%, p < 0,001), hipertensión (13,6%, p < 0,001) y arritmias (5,9%, p < 0,001). Recibieron más tratamiento los pacientes con DT (81,9% vs 74,1%; p < 0,001), aunque ingresaron menos (7,6%, p = 0,0), sin diferencias en cuanto a maniobras de RCP, sedación, intubación, o ingreso en cuidados intensivos (1,9%).     Jorge clinical study: 10-year outcomes of risk-adapted radiotherapy defined by multiparametric MRI for prostate cancer

    Purpose: To analyze the 10-year biochemical relapse-free survival (BRFS), locoregional relapse-free survival (LRFS), metastasis-free survival (MFS), and overall survival (OS) in patients diagnosed with localized prostate adenocarcinoma treated with radiotherapy (RT) ± androgen deprivation therapy (ADT), according to the risk groups based on multiparametric magnetic resonance imaging (mpMRI) instead of digital rectal exam (DRE). Methods: We retrospectively evaluated 140 consecutive patients diagnosed with localized prostate adenocarcinoma, stratified into different risk groups-low (LR), intermediate (IR), and high (HR) by mpMRI results. Results: After a median follow-up of 104 months, in LR group (n = 15), 10-year BRFS was 86.7%, 10-year LRFS was 86.7%, 10-year MFS was 93.3%, and 10-year OS was 100%. In IR group (n = 80), 10-year BRFS was 80.5%, 10-year LRFS was 86.1%, 10-year MFS was 92.6%, and 10-year OS was 76%. In HR group (n = 45), 10-year BRFS was 72.8%, 10-year LRFS was 78.7%, 10-year MFS was 82.1%, and 10-year OS was 77% (2 deaths from prostate cancer). According to mpMRI results, 36 (25.7%) patients change the risk group and 125 (89.28%) patients change the TNM stage. There was a trend for higher metastatic relapse in patients who switched from IR to HR (due to mpMRI) versus the patients who remained in the IR (20%, vs. 1.81% p = 0.059). Multivariate analysis showed that locoregional relapse was strongly associated with distant relapse (OR = 9.28; 95%CI: 2.60-33.31). There were no cases of acute grade 3 toxicity. Late grade 3 genitourinary, gastrointestinal, and sexual toxicity were 2.8%, 0.7%, and 1.2%, respectively. Conclusion: This is the first study with a 10-year median follow-up of patients diagnosed with localized prostate cancer treated with radiotherapy according to the risk groups established by mpMRI.     Introducción. El estudio de la importancia de la familia en el rendimiento académico de los estudiantes sigue siendo uno de los temas de investigación más relevantes, a pesar de décadas de estudio. Es un tópico que ha sido más explorado en la educación primaria que en la educación secundaria. Método. Con la técnica de análisis de árboles de decisión y con una muestra seleccionada aleatoriamente, los objetivos de este estudio son explorar la importancia de variables familiares sobre el rendimiento académico del alumnado de Educación Secundaria Obligatoria y caracterizar al alumnado con mejor y peor rendimiento a partir de sus factores familiares. La muestra ha sido seleccionada mediante un muestreo aleatorio por conglomerados. Los participantes fueron 1078 estudiantes pertenecientes a 38 institutos de educación secundaria públicos. En cuanto a las variables analizadas, de cada progenitor se tuvo en cuenta su nivel educativo, su situación laboral y las expectativas que tenían sobre el futuro académico del estudiante. Además, también se incluyeron el tipo de familia, el apoyo familiar afectivo y si el estudiante se encontraba declarado en situación de riesgo por los servicios de protección a la infancia y a la adolescencia. Resultados. Se comprobó la influencia de algunas de las variables familiares, especialmente la declaración de situación de riesgo, el apoyo familiar afectivo y las vinculadas a la madre del alumnado. Se obtuvieron grupos claramente diferenciados por su promedio de rendimiento académico a partir de la influencia de factores familiares. Discusión y conclusiones. La importancia de que las dos variables sociofamiliares más relevantes para el rendimiento académico sean las expectativas de los progenitores y el apoyo familiar afectivo plantea la necesidad de que desde las escuelas se intervenga en ellas.Introduction. The study of the importance of the family in the academic performance of students remains one of the most relevant research topics, despite decades of study. It is a field that has been more explored in primary education than in secondary education. Method. using the decision trees analysis and with a randomly selected sample, the goals of this research are to explore the importance of family variables on the academic performance of compulsory secondary education students and to characterize the students with the best and worst performance based on their family factors. The sample has been selected through a random cluster sampling. The participants were 1078 students, belonging to 38 public secondary education institutes. Regarding the variables analyzed, each parent considered their educational level, their employment situation and the expectations they had about the academic future of the student. In addition, the type of family, affective family support and whether the student was declared at risk by the child and adolescent protection services were also included. Results. By means of the decision trees, the influence of some of the family variables was verified, especially the declaration of a risk situation, affective family support and those related to the mother of the students. Groups clearly differentiated by their average academic performance were also obtained from the influence of family factors. Discussion or Conclusion.     Mineralocorticoid receptor antagonists for nephroprotection and cardioprotection in patients with diabetes mellitus and chronic kidney disease

    Diabetic kidney disease develops in about 40% of patients with diabetes and is the commonest cause of chronic kidney disease worldwide. Patients with chronic kidney disease, especially those with diabetes mellitus, are at high risk of both developing kidney failure and cardiovascular death. The use of renin-angiotensin system blockers to reduce the incidence of kidney failure in patients with diabetic kidney disease dates back to studies that are now 20 or more years old. During the last few years sodium-glucose co-transporter-2 inhibitors have shown beneficial renal effects in randomized trials. However, even in response to combined treatment with renin-angiotensin system blockers and sodium-glucose co-transporter-2 inhibitors, the renal residual risk remains high with kidney failure only deferred, but not avoided. The risk of cardiovascular death also remains high even with optimal current treatment. Steroidal mineralocorticoid receptor antagonists reduce albuminuria and surrogate markers of cardiovascular disease in patients already on optimal therapy. However, their use has been curtailed by the significant risk of hyperkalaemia. In The FInerenone in reducing kiDnEy faiLure and dIsease prOgression in Diabetic Kidney Disease (FIDELIO-DKD) study comparing the actions of the non-steroidal mineralocorticoid receptor antagonist finerenone with placebo, finerenone reduced the progression of diabetic kidney disease and the incidence of cardiovascular events with a relatively safe adverse event profile.     Multi-joint isometric measurement for the evidence-based assessment of upper limb strength impairment in wheelchair athletes with different health conditions: a preliminary study

    The present study presents a novel specific multi-joint isometric test to assess upper limb strength impairment for evidence-based classification in wheelchair sports. Sixteen wheelchair athletes participated in this study and were classified according to their type of physical impairment and health condition as follows: athletes with neurological impairment (ANI, n = 5) and athletes with impaired muscle power (IMP, n = 11). In addition, six non-disabled participants formed a control group (CG, n = 6). All the participants performed the isometric propulsion strength test (IPST), evaluating pushing and pulling actions, and two wheelchair performance tests. Excellent relative intra-session reliability scores were obtained for strength values for the ANI, IMP and CG groups (0.90 < ICC < 0.99) and absolute reproducibility showed acceptable scores of SEM (< 9.52%) for IPST pushing action. The ANI had significantly lower scores in strength and wheelchair performance than the IMP and the CG, while no differences were found between the IMP and the non-disabled participants. In addition, no correlations were found for wheelchair athletes between the isometric upper limb strength measure and wheelchair performance. Our findings suggest that the IPST is a valid test for strength measurement in upper limb impairment wheelchair athletes with different health conditions, which must be used in combination with a performance test to obtain a holistic assessment of this population.Sin financiación1.9 Q4 JCR 20220.649 Q2 SJR 2022No data IDR 2022UE
