Daily time spent on screens and psychological well-being: Cross-sectional association within the SUN cohort
Background: The impact of various lifestyles on psychological well-being (PWB) remains under-studied. We aimed to explore the cross-sectional association between daily screen use (television, tablet and mobile phone) and PWB within the SUN cohort.
Methods: PWB was assessed using the 29-item Ryff scale (ranging from 29 to 174), and participants with scores >75th percentile were considered as having optimal PWB. Participants were categorized based on their self-reported weekly screen usage hours. Postestimation logistic regression models assessing the prevalence likelihood of not achieving optimal PWB were adjusted for sociodemographic, psychological, personality and lifestyles factors. Isotemporal substitution models explored the potential impact on PWB resulting from replacing 1 h/day of screen time with 1 h/day of exercise.
Results: The study included 3051 participants (55.8% women, mean age 57.3 ± 11.1 years, mean Ryff's score: 139.1 ± 17.4 points). Daily screen use for ≥2 h was associated with a higher prevalence likelihood of not achieving an optimal PWB (Prevalence Ratio [PR]:1.09; 95% CI:1.01-1.18). Among PWB dimensions, screen use ≥2 h/day was linked to an increased likelihood of not achieving optimal scores in environmental mastery (PR:1.11; 95% CI:1.02-1.20), life purpose (PR:1.10; 95% CI:1.02-1.18), and personal growth (PR:1.09; 95% CI:1.01-1.18). Replacing 1 h of daily screens time with 1 h of exercise may lead to potential improvements in environmental mastery (Odds Ratio [OR]:0.87; 95% CI:0.76-0.99), purpose in life (OR:0.86; 95% CI: 0.76-0.98), personal growth (OR:0.84; 95% CI:0.73-0.96) and positive interpersonal relationships (OR:0.86; 95% CI:0.75-0.99).
Conclusions: These findings highlight the importance of reducing screen use activities and increasing physical exercise for achieving optimal PWB
Interventions to increase the quality of life in Amyotrophic Lateral sclerosis patients
Introduction: Amyotrophic lateral sclerosis (ALS) is recognized as a rare disease due to its lack
of cure and limited research. While pharmacological treatments for symptoms often result in
adverse effects, non-pharmacological interventions do not and additionally enhance the quality
of life (QoL).
Objective: The purpose of this review is to identify interventions to increase quality of life in
amyotrophic lateral sclerosis patients.
Methodology: This systematic review of the literature performed its search in three databases
(PubMed, PsycINFO and CINHAL). With terms “amyotrophic lateral sclerosis”, “intervention” and
“quality of life” it elected 176 articles published since 2013, the number was reduced to 11 final
articles.
Results: Four main categories related were found. Firstly, respiratory interventions. Secondly,
communications interventions and abilities. Thirdly, strategies regarding patient’s mental health
and emotional. Finally, physical and rehabilitation therapies.
Conclusion: Exploring strategies aimed at improving respiratory function, addressing mental
health concerns, enhancing communication abilities, and promoting physical independence
significantly benefit patients. Further research into interventions aimed at enhancing the quality
of life for individuals with amyotrophic lateral sclerosis should be done.Introducción: La esclerosis lateral amiotrófica (ELA) se reconoce como una enfermedad rara
debido a su falta de cura e investigación limitada. Mientras que los tratamientos farmacológicos
para los síntomas a menudo resultan en efectos adversos, las intervenciones no farmacológicas
no lo hacen y además mejoran la calidad de vida.
Objetivo: El propósito de esta revisión es identificar intervenciones para aumentar la calidad de
vida en pacientes con esclerosis lateral amiotrófica.
Metodología: Esta revisión sistemática de la literatura realizó su búsqueda en tres bases de
datos (PubMed, PsycINFO y CINHAL). Con los términos "esclerosis lateral amiotrófica",
"intervención" y "calidad de vida", se seleccionaron 176 artículos publicados desde 2013, y el
número se redujo a 11 artículos finales.
Resultados: Se encontraron cuatro categorías principales relacionadas. En primer lugar,
intervenciones respiratorias. En segundo lugar, intervenciones y habilidades de comunicación.
En tercer lugar, estrategias relacionadas con la salud mental y emocional del paciente.
Finalmente, terapias físicas y de rehabilitación.
Conclusión: Explorar estrategias destinadas a mejorar la función respiratoria, abordar
preocupaciones de salud mental, potenciar habilidades de comunicación y fomentar la
independencia física beneficia significativamente a los pacientes. Se debe llevar a cabo una
investigación adicional sobre intervenciones dirigidas a mejorar la calidad de vida de las
personas con esclerosis lateral amiotrófica

Postoperative management and nursing care after implantation of a total artificial heart: Scoping review
Introduction: End-stage heart failure (HF) is a condition whose only successful long-term treatment, with a survival of more than 10 years, is heart transplantation. However, limited organ availability and the progressive increase in the number of patients with advanced HF have served as an impetus for the development of implantable mechanical assistive devices.
Aim: To provide an overview of postoperative management and nursing care after the implementation of a Total Artificial Heart (TAH).
Methods: A scoping review was carried out by consulting the PUBMED, CINAHL, and COCHRANE databases. From all the documents located, information was extracted on the date of publication, country of publication, type of study, and results of interest to answer the research question. In addition, the degree of recommendation was identified.
Results: Twenty-three documents were included in the scoping review. Results were classified in relation to: 1) description of the CAT SynCardia®; 2) nursing care in the immediate postoperative period (management of the device and management of hematological, infectious, nephrological, nutritional complications, related to immobilization, sleep-rest disturbances, psychological disorders, and patient and family education); and 3) follow-up at home.
Conclusions: The complexity of implantation of the TAH, the multiple related complications that can arise during this process, both in the immediate post-operative and late, require a standardised and multidisciplinary management. The absence of standardised protocols raises the need for future studies to measure the effectiveness of care in patients with TAH. A multidisciplinary approach is crucial. Nurses must acquire autonomy and involvement in decision-making and develop competencies to address the patient's and family's physiological and psychosocial needs.Introducción: La insuficiencia cardíaca (IC) terminal es una afección cuyo único tratamiento exitoso a largo plazo, con una supervivencia de más de 10 anos, es el trasplante cardíaco (TC). Sin embargo, la disponibilidad limitada de órganos y el aumento progresivo del número de pacientes con IC avanzada han servido de impulso para el desarrollo de dispositivos de asistencia mecánica implantables y corazón artificial total (CAT).
Objetivo: Proporcionar una visión general del manejo postoperatorio y los cuidados de enfermería después de la implantación de un CAT.
Métodos: Se realizó una revisión de alcance consultando las bases de datos, PUBMED, CINAHL y cochrane. De todos los documentos se extrajo información sobre la fecha de publicación, país de publicación, tipo de estudio y resultados de interés para responder a la pregunta de investigación. Además, se identificó el grado de recomendación.
Resultados: Se incluyeron 23 documentos en la revisión. Los resultados se clasificaron en relación con: 1) la descripción del CAT SynCardia; 2) la atención de enfermería en el postoperatorio inmediato (manejo del dispositivo y manejo de las complicaciones hematológicas, infecciosas, nefrológicas, nutricionales, relacionadas con la inmovilización, alteraciones del sueno-descanso, alteraciones psicológicas y la educación al paciente y familia); y 3) el seguimiento en domicilio.
Conclusiones: La complejidad de la implantación del CAT, las múltiples complicaciones relacionadas que pueden surgir durante este proceso, tanto en el postoperatorio inmediato como tardío, requieren un manejo estandarizado y multidisciplinario. La ausencia de protocolos estandarizados plantea la necesidad de futuros estudios para medir la efectividad de los cuidados en pacientes con CAT. El abordaje multidisciplinar es crucial. Las enfermeras deben implicarse y adquirir autonomía en la toma de decisiones y desarrollar competencias para atender las necesidades fisiológicas y psicosociales del paciente y la familia
Wearable voice dosimetry system.
The objective quantification of voice acoustic parameters is used for the diagnosis, monitoring, and treatment of voice disorders. Such assessments are carried out with specialised equipment within the doctor's office. The controlled conditions employed are usually not those of the real environment of the patient. The results, although very informative, are specific to those measurement conditions and to the time when they were performed. A wearable voice monitoring system, based on an accelerometer to ensure the message, can overcome these limitations. We present a miniaturised, low-power, and low-cost wearable system to estimate and record voice fundamental frequency (F0), intensity and phonation time for long intervals in the everyday environment of the patient. It was tested on two subjects for up to two weeks of recording time. It was possible to identify distinct periods in vocal activity, such as normal, professional, demanding or hyperfunctional. It provided information on the workload that the vocal cords needed to cope with over time and when and to what extent that workload was concentrated. The proposed voice dosimetry system enables the extraction and recording of voice parameters for long periods of time in the everyday environment of the patient, allowing the objectification of vocal risk situations and personalised treatment and monitoring
Fluid Theodicy: God as Representations in Patterns of Empathy, Emotions, and Conceptual Believing
The term theodicy was coined by the philosopher Gottfried Wilhelm Leibniz (1646-1716) and is inherent in the question of how evil can exist if an intrinsically good God guides everything. The publication of this oeuvre initiated intense philosophical and theological discourse in the subsequent centuries, during which many issues that bare upon human well-being were articulated. Also, Leibniz’s rational approach to the relationship between God and evil raised a number of issues related to the topic of belief. This topic has entangled discourses on theodicy with a long-lasting debate on beliefs, which goes back to Antiquity. Recently, a paradigm-shift shed new light on the understanding of belief. Science has begun to address the neurophysiological mechanisms of the processes that underpin belief formation, modulation, and change. The term credition was coined in order to capture and reflect this new and innovative understanding of the fluidity of beliefs and believing. Florecimiento, familia y situación económica: un estudio longitudinal con personas adultas emergentes en España.
Abstract: El objetivo principal de este estudio fue examinar las asociaciones de las relaciones familiares (implicación parental y apoyo de la autonomía) y los ingresos familiares con el florecimiento durante la adultez emergente. Este objetivo se analizó desde una perspectiva longitudinal. La muestra estuvo compuesta por 400 jóvenes (n = 132 hombres, 268 mujeres en T1; n = 132 hombres, 266 mujeres en T2) de edades comprendidas entre los 18 y 29 años (M = 20.31, DT = 2.04 en T1; M = 23.66, DT = 2.08 en T2) que participaron en el Proyecto Transición a la Adultez en España (TAE). Los resultados indicaron que existe una asociación significativa y positiva entre florecimiento en T2 y las variables familiares (implicación parental y apoyo de la autonomía) analizadas en T1. Asimismo, los resultados indicaron que la implicación parental en los años iniciales de este periodo (T1) juega un rol predictor en el florecimiento de las personas adultas emergentes durante los años intermedios de la etapa (T2) y también predice el cambio en el florecimiento entre los dos tiempos analizados (T1 y T2). Finalmente, los resultados sugieren que los ingresos familiares y el apoyo de la autonomía en los años intermedios de la adultez emergente (T2) son variables predictoras del florecimiento de las personas adultas emergentes en esos mismos años (T2). El presente estudio presenta un trabajo longitudinal escasos en países como España. Nuestros resultados ofrecen un punto de partida acerca de cómo promocionar el florecimiento de las personas adultas emergentes y comprender las relaciones parento-filiales teniendo en cuenta el papel que desempeña la situación económica de las familias durante esta etapa de desarrollo en el territorio español
The Fundamental Argument without any garbage
Rik Peels's (2017) Fundamental Argument is an important argument against epistemic scientism. The crucial
premise of the Fundamental Argument is that if nonscientific sources of belief did not provide us with knowledge, neither could the sciences. But, the sciences do
provide us with knowledge. Thus, epistemic scientism
is false. This paper defends Peels's argument against recent criticisms. In particular, Hietanen and colleagues
criticize Peels's argument for resting on what they call
the “garbage in, garbage out” principle (GIGO). This
paper strengthens their attacks on the GIGO principle.
It shows, however, that we don't need the GIGO principle to motivate the crucial premise of the Fundamental
Argument. Instead, it argues that the crucial premise is
true for the following reasons: scientific knowledge is
the result of successful inquiry; at some point we lacked
scientific knowledge; and, we cannot successfully inquire without some prior knowledge
Practical issues related to immunocytochemistry on cytological smears: Tips and recommendations
Objective:
Immunocytochemistry (ICC) is essential for enhancing diagnostic accuracy and identifying markers for diagnosis, prognosis and targeted therapies. While cell blocks (CBs) are preferred for standardization and optimized staining, cytological smears are an alternative when CBs are unavailable. However, the literature on ICC protocols for smears is sparse. This review addresses preparation, fixation and protocols for nuclear and cytoplasmic antibodies on smears, drawing from our laboratory's experience.
Methods:
We reviewed procedures for ICC on cytological smears using existing literature and practical insights from our laboratory.
Results:
Commercially available antibodies were found to be reliable for ICC on smears if specimens are properly prepared and fixed. Protocols developed in our laboratory maintained antigenicity and provided clear staining results.
Conclusions:
Although ICC on CBs is the gold standard for standardization, cytological smears are a viable alternative when CBs are unavailable. Success in ICC on smears depends on proper preparation and fixation. This review offers practical protocols and insights to help laboratories optimize ICC on cytological smears. Further research and standardization are necessary to enhance reproducibility and reliability of ICC on smears. The practical information provided is based on personal experience in our laboratory
