    Bridging the skills gap: apprentice assistant practitioners having an impact in mental healthcare services

    It is important to have a well-trained workforce and this is especially so in mental healthcare services. This article is the first of four looking at mental healthcare services and the apprentice assistant practitioner (AAP). The background to the introduction of assistant practitioners (AP), both in generic terms and more specifically their potential in mental health provision, will be explored.The present situation in mental health services will be examined, along with how the AP role might help mitigate some of the critical challenges faced in this area of health. The development of higher apprenticeships will be discussed in relation to standardisation of the AP role and how universally recognised apprenticeship standards around knowledge, behaviours and values will aid a more consistent understanding of the AP position and their consequent deployment in services

    The challenges of prioritising low carbon in public sector Flood and Coastal Erosion Risk Management (FCERM) construction

    Purpose – Low carbon solutions in infrastructure have been well documented and promoted in most areas of the UK except in the context of public sector Flood and Coastal Erosion Risk Management (FCERM) infrastructure. With the UK Government providing £2.5bn capital investment to reduce risk of flooding and coastal erosion between 2015 and 2020, the carbon impact of this construction programme will have a significant impact on the UK’s carbon targets. The purpose of this paper is to provide a comprehensive literature review focusing on the effect of carbon on climate change, the role of UK public sector FCERM construction and organisational cultural challenges in promoting low carbon.Design/methodology/approach – An electronic survey of practising professionals in a leading government agency that procures major FCERM construction projects has been undertaken. The survey covers participants from the whole value chain within the project life cycle, since many authoritative sources call for integration, and for change to be implemented in partner organisations.Findings – The survey shows that although carbon is considered, it is not yet at the level of importance nor is it prioritised to the extent at which cost is. This is for both public and private sector supply chain organisations. Low carbon, although included in discussions, does not feature as prominently throughout all project stages.Research limitations/implications – The utilisation of a survey for this research is limited as it merely supports current industry findings, albeit having focused on a specific infrastructure area. Further qualitative research is required to fully explore the findings within the survey, and to establish whether the implementation of a new whole life carbon calculator within FCERM construction will have an impact on the organisational culture and future successful implementation of low carbon construction.Practical implications – The results of this research identify the specific areas in which industry practitioners involved in promoting and prioritising low carbon could focus on to facilitate the change required to fully embed low carbon into FCERM construction.Social implications – This research supports industry knowledge specifically for public sector FCERM construction, and the changes to organisational culture required to fully embed low carbon solutions in public sector construction. These changes may have an impact on the amount of carbon being used, which can positively affect climate change as a whole.Originality/value – The literature review shows that carbon has a clear impact on climate change, and organisational culture and leadership can facilitate the successful implementation of new initiatives. However, previous attempts to embed low carbon into infrastructure construction practice have had limited success to date. The survey findings support the view that organisational culture and leadership can influence the successful embedding of low carbon solutions, and why this has been a challenge

    In 2008 we published an article in this journal entitled Seven Strong Claims about Successful School Leadership. The article was based on a major literature review that was summarised in a paper published by the National College for School Leadership in England. Both the National College paper and our subsequent article proved to be far more popular than we anticipated and both have been extensively cited over the past 10 years. This article revisits each of the seven claims, summarising what was said about each in the original publication

    Trimethoxysilyl end-capped hyperbranched polyglycidol/polycaprolactone copolymers for cell delivery and tissue repair: synthesis, characterisation and aqueous solution properties

    New PCL-HBPG/SiHBPG triblock copolymers composed of a polycaprolactone (PCL) core with two outer blocks of trimethoxysilyl end-capped hyperbranched polyglycidol (HBPG/SiHBPG), of varying molecular weight, have been successfully synthesised and characterised. The effect of copolymer composition and structure upon the crystallization, melting, thermal degradation and aqueous solution behaviour were investigated at various temperatures. In aqueous solution, at concentrations above their corresponding critical aggregation concentration the PCL-HBPG/SiHBPG copolymers readily self-assemble into large multicore structures composed of PCL domains and corona consisting of HBPG/SiHBPG branches. The multicore structures were stabilized by numerous hydrogen-bonds from the HBPG moieties as well as via formation of siloxane crosslinks (i.e. Si-O-Si bonds). As the formation of the siloxane linkages is irreversible the PCL-HBPG/SiHBPG-based particles will be covalently crosslinked at higher concentrations in vivo and form injectable gels scaffolds that will be biocompatible and capable of invoking cell attachment and differentiation without the need for exogenous biological stimuli

    Kooth is an online mental health service for children and young people*. Since its launch in 2004, it has grown with the children and young people who use it, its humanistic approach shaped by their demand and our clinical expertise.Today it is a large, thriving and diverse community of young people who use it for multifarious reasons and in multifarious ways; importantly, they have the power to direct support themselves. They and are in control of their therapeutic journey. The service is full of choices in terms of how to get help, but also in terms of the therapeutic support available. As such, it is an extremely complex entity, perhaps reflecting the very nature of wellbeing and mental health.As well as making this an extremely rich environment where young people actively choose to spend their time, it has also taken the service well beyond what today’s quantitative outcomes measures of mental health services can capture.This paper signals the beginning of an important, ground-breaking journey to define the role and the impact of Kooth. We have explored, mapped and categorised activity within the service with a specific goal in mind: to define our Theory of Change. This describes the Kooth experience and the many routes in and out of the service, each reliant on our understanding of an individual’s unique building blocks for wellness.The Theory of Change (ToC) is not a clinical model and nor does it seek to be. It is not there to show how we can address specific standalone symptoms. Instead it is a characterisation of the broad, person-centred, holistic work Kooth does every day.It is also the foundation for a future set of mental health outcome measures, which will represent each child or young person’s journey

    Supporting adolescents with type 1 diabetes during the transition from child to adult services: A literature review

    This literature review considers the personal and practice implications of type 1 diabetes amongst adolescents. The review is part of a wider Post Graduate study and here 25 largely qualitative articles are critically considered. The selection process and thematic review lead to the identification of the themes: ‘social and emotional transition’ and ‘service related transition’. Social and emotional transition relates to the experiences of adolescents diagnosed with type 1 diabetes. These included increased risk of anxiety, depression and eating disorders which were exasperated during the challenging adolescent period. The service related transition refers to this vulnerable group not fitting well into child or adult services and the added pressures of managing the move from one to another.This included expectation of increased autonomy and problems of poor self-management. There are some examples of positive practice found in this review largely grounded in improved personal and service transition. The review considers problems encountered and informs professionals of gaps in practice, encompassing the distinct difficulties adolescents with type 1 diabetes may experience

    Increasing the use of simulation in training

    The focus of this paper is to explore the role and development of simulated learning within the apprenticeship assistant practitioner foundation degree programme. It will also discuss the challenge of influencing a change in practice within a competency-based learning programme.In order to shape and develop a supportive, effective and indeed inclusive teaching strategy, the use of the simulated learning environment will be incorporated across a number of mandatory nonclinical modules, such as communication, group dynamics and principles of care, among others.
