Anthropogenic influences on Rhodnius ecuadoriensis populations and nesting behaviors in sylvatic areas of southern ecuador.
Chagas disease, caused by the parasite Trypanosoma cruzi, remains a major public health concern. While Rhodnius ecuadoriensis, a key vector, is traditionally sylvatic, environmental disturbances have driven its adaptation to human-influenced habitats. This study explores a novel factor: how anthropogenic waste affects vector ecology by altering nest compositions. Unlike prior research, this study examines whether human-derived materials in nests influence triatomine colonization. Given rising pollution, particularly post-COVID-19, understanding its role in disease transmission is essential for developing innovative vector control strategies.
Nest records were collected and analyzed in 2018, 2022, and 2023, across eight communities in Loja province, Ecuador. Nests were categorized as peridomestic if found 30 m away. The number of insects found in these nests was quantified using entomological indices. Pearson correlation analysis was applied to assess relationships between nest location and the presence of anthropogenic materials.
A total of 389 nests were examined, yielding 1,089 individuals of Rhodnius ecuadoriensis (including both nymphs and adults). The infestation index in peridomestic areas dramatically decreased from 33.3% in 2018 to 0% in 2022, highlighting an intriguing temporal shift that warrants further investigation. In contrast, sylvatic areas showed fluctuating infestation rates (27.5% in 2018, 16.5% in 2022, and 22.2% in 2023). The study uniquely identified a significant association between triatomine infestation and mammal nests, with 50.1% of infested nests located within mammal habitats. Notably, 35.2% of these nests contained anthropogenic materials, particularly near human-altered landscapes such as roads and paths. The weak to moderate negative correlation between the presence of anthropogenic materials and proximity to roads or rivers (r = -0.361, p = 0.039) highlights an innovative exploration of the influence of human environmental changes on vector ecology.
This study offers a novel perspective on the dual impact of increasing pollution levels on wildlife. It highlights how anthropogenic waste not only reduces vector populations but also increases mortality rates through entanglement in waste materials. These findings underscore the urgent need for environmental education programs focused on waste management within local communities. Furthermore, the study paves the way for further research to assess the rate of T. cruzi infection in relation to environmental and anthropogenic factors, offering a critical foundation for understanding and potentially mitigating Chagas disease transmission
Case Report: Early Spontaneous Subcapsular Hematoma in a Transplanted Kidney: The Importance of High Suspicion and Timely Intervention
Subcapsular hematoma (SH) is a rare but serious complication after renal transplantation, potentially leading to graft dysfunction. Through external compression and activation of the renin-angiotensin-aldosterone system a SH can lead to a serious condition known as Page kidney. Early diagnosis and timely intervention are crucial to prevent graft loss.
We describe a 67-year-old male who underwent kidney transplantation and developed an SH in the immediate postoperative phase. Despite being hemodynamically stable and asymptomatic, the patient exhibited persistent anuria and rising serum creatinine levels (349 μmol/L). Routine ultrasound (US) revealed a SH (2.5 cm × 4.3 cm × 7.8 cm) compressing the graft. Doppler findings showed normal renal arterial resistive indices. A subsequent non-contrast CT scan confirmed the diagnosis. Given the risk of graft ischemia, surgical decompression via longitudinal and transverse capsular incisions was performed. Postoperatively, diuresis resumed, and renal function improved, with a decline in creatinine levels. The patient was discharged on postoperative day 25 with preserved graft function.
SH in transplant recipients is uncommon and often linked to surgical trauma, anticoagulation, or allograft biopsy. While small hematomas may resolve conservatively, larger ones can lead to graft dysfunction. This case highlights the importance of routine postoperative US for early detection, as well as the role of timely surgical intervention in preserving renal function. Given the potential for irreversible ischemia and graft loss, proactive surgical decompression should be favored over conservative management in transplant recipients with significant SH.

A 50-year-old man with no significant medical history presents to the ED with a 2-day history of headache and fever. He also reports severe fatigue, diffuse myalgias, abdominal pain, diarrhea, nausea, and occasional vomiting. Initial examination shows no neck stiffness and negative Kernig's and Brudzinski's signs. Empiric antibiotic treatment with ceftriaxone and metronidazole is started for suspected abdominal infection. The patient rapidly develops hearing loss. On reassessment, marked neck stiffness is noted. Ceftriaxone is intensified, and amoxicillin and acyclovir are added to cover infectious meningitis. Violent Partners or a Specific Class of Offenders? A Criminal Career Approach to Understanding Men Involved in Intimate Partner Sexual Violence
The current study investigates the criminal career of individuals involved in intimate partner sexual violence (IPSV). Specifically, the goal is to determine whether men who engage in IPSV can be distinguished from those who engage in intimate partner non sexual violence (IPNSV) only and whether criminal trajectories in the resulting subgroup are heterogeneous. The sample comes from a Canadian database including a total of 12,458 individuals involved in IPSV and 32,474 individuals involved in IPNSV). Bivariate and multivariate analyses are performed to examine the differences in the two groups while latent profile analysis allows examining the heterogeneity of characteristics of men who engaged in IPSV. Findings indicate that the criminal career of men who engage in IPSV follows a pattern that is clearly distinct from that of men who engage in IPNSV only and is more specialized in terms of sexual offenses. Results also show that the criminal trajectories followed by the men who engage in IPSV are heterogeneous. Four profiles of different trajectories were identified. Both theoretical and practical implications are discussed
Impact of maternal stress and COVID-19 exposure during pregnancy on offspring neurodevelopment: signature cohort 12 month follow-up
The COVID-19 pandemic brought unprecedented global challenges. Amid the crisis, the potential impact of COVID-19 exposure on the neurodevelopment of offspring born to infected mothers emerged as a critical concern. This is a prospective cohort study of pregnant women and their offspring enrolled in the Signature project at Hospital Universitario Virgen del Rocio in Seville, Spain, between 01/01/2024 and 08/31/2022. Participants were subsequently classified into SARS-CoV-2 exposed (mild or severe COVID-19) and non-exposed groups. Maternal stress was assessed through the Perceived Stress Scale; offspring neurodevelopment was screened through Ages & Stages Questionnaire, 3rd Edition (ASQ-3) at 6 months and 12 months. Association between COVID-19 severity and neurodevelopmental outcomes were analyzed with univariate and multivariate analysis. The cohort included 689 women categorized into three distinct groups based on COVID-19 severity: non-infected mothers (N = 277), mothers with mild COVID-19 (N = 358), and mothers with severe COVID-19 (N = 54). The reference category for the odds ratio (OR) analyses was the non-infected group. In utero exposure to mild or severe COVID-19, as well as maternal stress, was not significantly associated with risk of developmental concern in any ASQ-3 subdomain at 12-month follow-up. This study highlights the need for further research with longer follow-up periods to better understand the potential long-term impact of prenatal COVID-19 exposure on offspring neurodevelopment.

L’analyse dialogique du discours appliquée à l’entretien clinique: un point de rencontre interdisciplinaire
Centré sur une approche interdisciplinaire croisant sciences du langage et psychologie, ce chapitre a pour but de montrer en quoi une approche dialogique inspirée de Bakhtine contribue à la compréhension du déroulement de l’entretien clinique à but thérapeutique et, plus largement, à une théorie du fonctionnement psychologique. Après une brève présentation des particularités de l’entretien clinique, on présente certains éléments essentiels de l’approche dialogique adoptée. Celle-ci met l’accent sur le langage en tant que discours vivant, adressé à quelqu'un, ancré dans un certain type d’activité, traversé par différentes voix et pris dans une chaîne dialogique. Dans la version de l’approche dialogique présentée ici, le mot « dialogue » désigne à la fois le dialogue en présence (entre deux ou plusieurs individus), le dialogue en absence avec des locuteurs absents et l’autodialogue, le dialogue avec soi-même. Sont ainsi mis en évidence l’hétérogénéité de l’espace discursif et les tensions dialogiques qui s’exercent aussi bien dans cet espace que dans le discours de chaque locuteur. Se pose alors la question de l’application méthodologique de cette théorie à certains corpus interactifs, en l’occurrence l’entretien clinique. Pour répondre à cette question, on présente quatre facettes du dialogisme : le contexte, les voix, le positionnement énonciatif et la co-construction des significations dans les mouvements du discours. Chaque facette est introduite puis illustrée par un ou plusieurs exemples tirés de divers entretiens cliniques entre un patient (ou plusieurs patients) et un thérapeute. En conclusion, on montre que tout en offrant une théorie du langage mettant l’accent sur le discours en usage, les tensions dialogiques qui le traversent et les mouvements du discours, l’approche dialogique contribue à une théorie de la psyché. Mettant au jour la dimension intersubjective de l’esprit humain, elle invite à considérer le langage, non pas comme la traduction directe d’un vécu psychique, mais comme un discours adressé, situé, se présentant dans une forme matérielle qui doit être soumise à une analyse fine et appuyée sur une théorie du langage. Dans ses développements pratiques, cette approche requiert la collaboration entre analystes du langage et praticiens de la psychothérapie. Sur le plan méthodologique, elle demande une collaboration interdisciplinaire entre psychologie et sciences du langage
