    Navigating Life With Parkinson's Disease: A Focus Group Study on Coping Strategies and Considerations for Self-Management Support.

    Contains fulltext : 317632.pdf (Publisher’s version ) (Open Access)AIM: To investigate the experiences of people with Parkinson's disease in coping with and adapting to their disease and to identify considerations for a tailored self-management support program. DESIGN: A descriptive phenomenological focus group study. METHODS: Five semi-structured focus groups were conducted between April 2023 and June 2023 in the Netherlands, with 12 people with Parkinson's disease. Two researchers independently performed an inductive content analysis. RESULTS: Three principal categories emerged: (1) Rational realisation versus emotional experience: the coping strategy transition. This category includes three main coping strategies: denial or avoidance coping, acknowledging with less active coping and proactive and task-oriented coping. (2) Factors that influence coping, including mindset and skills, social circles and communication and access to support and care. (3) Considerations for successful self-management of Parkinson's disease, highlighting key areas such as psycho-emotional guidance, nutrition and lifestyle, peer support and maintaining autonomy and sense of identity. CONCLUSION: Coping and adaptation strategies are individual and dynamic processes, with multiple key or turning points during the transition between strategies. Tailored self-management support can enhance coping abilities during these transitions, fostering adaptation to a life with Parkinson's disease. IMPLICATIONS FOR THE PROFESSION AND/OR PATIENT CARE: A patient-focused version of an existing blended self-management support program for family caregivers will be developed, which will be delivered by healthcare professionals. IMPACT: This study can help healthcare professionals tailor support for people with Parkinson's disease, emphasising their role in facilitating coping and adaptation. Enhancing self-management can improve self-efficacy, quality of life and potentially reduce healthcare utilisation in people with Parkinson's disease. REPORTING METHOD: Findings are reported according to the COREQ guidelines. PATIENT OR PUBLIC CONTRIBUTION: Patients and Parkinson's disease experts participated in the preparation and implications of the findings.     Multicenter and inter-software evaluation of ablative margins after thermal ablation of colorectal liver metastases.

    Contains fulltext : 316991.pdf (Publisher’s version ) (Open Access)PURPOSE: To assess the association between minimal ablative margin (MAM) and local tumor progression (LTP) following CT-guided thermal ablation of colorectal liver metastases (CRLM) in a multicenter cohort and across two confirmation software. MATERIALS AND METHODS: This multicenter retrospective study included patients who underwent CT-guided radiofrequency or microwave ablation for CRLM between 2009 and 2021 in three institutions. Three-dimensional (3D) MAM was retrospectively assessed using dedicated ablation confirmation software by automatic non-rigid (Ablation-fit) or semi-automatic rigid co-registration (SAFIR) of intraprocedural pre- and post-ablation contrast-enhanced CT scans by two independent reader teams blinded to patient outcomes. LTP was assessed on a per-tumor basis. Factors associated with LTP-free survival were assessed using multivariable Cox regression analysis. RESULTS: Overall, 113 patients (mean age: 67 ± 10 years; 78 men) who underwent thermal ablation for 189 CRLM (mean diameter: 1.9 ± 1.1 cm) met the inclusion criteria. 173/189 (92%) CRLM could be successfully analyzed using both software. Over a median follow-up of 31 months (IQR: 22-47), 21 of 173 CRLM (12.1%) developed LTP. On multivariable analysis, 3D MAM was independently associated with LTP in both software (Ablation-fit: HR 0.47, 95% CI: 0.36-0.61, p < 0.001; SAFIR: HR 0.42, 95% CI: 0.32-0.55, p < 0.001). No LTP was observed in CRLM ablated with MAM ≥ 4 mm (Ablation-fit) and ≥ 5 mm (SAFIR). The per-tumor median absolute difference in MAM quantification between both software was 2 mm (IQR: 1-3). CONCLUSION: MAM was independently associated with LTP after thermal ablation of CRLM across multicenter data and two confirmation software. Ablations achieving a MAM ≥ 5 mm were associated with local control in both software. CLINICAL RELEVANCE STATEMENT: MAMs from intraprocedural contrast-enhanced CT were independently associated with LTP after thermal ablation of CRLM across multicenter data and two confirmation software, with a margin ≥ 5 mm associated with local control in both software. KEY POINTS: Sufficient ablative margins are critical for local control following thermal ablation of CRLM. Intraprocedural CT-derived MAM was the only independent factor associated with LTP across two confirmation software. No LTP was observed in CRLM ablated with a MAM ≥ 5 mm.01 februari 202

    The ongoing and contentious coverage of abortion in a progressive context: a long-term cross-outlet assessment of Dutch abortion news (2000-2022)

    Contains fulltext : 312234.pdf (Publisher’s version ) (Open Access)Utilising automated content analysis with transformer-based topic modelling (BERTopic), this study examines cross-outlet variation in volume and topics over two decades (2000–2022) of abortion coverage in seven major daily newspapers in the Netherlands. Our findings reveal an ongoing coverage of abortion. Volume of coverage was notably highest in religious newspapers, indicating that these newspapers constitute a prominent voice in Dutch abortion debate. We discerned four distinct frames used in abortion news coverage. The first emphasised political developments as well as protest and advocacy related to abortion, framing the issue as a controversial political and moral topic. The second emphasised uncommon and problematic aspects of abortion, neglecting to present it as a commonplace and safe reproductive healthcare procedure. Two other frames seek to undermine women's decision-making agency: the marginalisation of women's personal perspectives; and the highlighting of prerequisites for abortion care. The media’s emphasis on a hierarchy of deservedness may consequently reduce public support for particular reasons for choosing abortion. Overall, our findings show that abortion remains a regularly covered and contentious issue, even amidst progressive legislation. This study highlights the need for balanced reporting that engages with women’s diverse personal experiences and perspectives.18 p

    The Impact of Relocations Within Nursing Home Care on Long-Term Care Residents According to Stakeholders: A Qualitative Study.

We aimed to collect a broad stakeholder overview of observed and experienced impacts on residents and aspects that influence the impact. METHODS: We conducted a qualitative study using semistructured interviews followed by one focus group. We recruited participants from various stakeholder perspectives based on differences in roles while having an interest or involvement in relocations, and experience with relocations. The interviews and focus group were audiorecorded, transcribed verbatim and analysed using responsive and thematic analysis. RESULTS: In 17 interviews including one duo interview, participants described the impact on residents varying from very positive to very negative. In addition, stakeholders addressed differences in impact related to the relocation phase (before, during, after). Aspects influencing the impact of relocations were (1) mental resilience of residents, (2) organisation of relocations, (3) social connections of residents, and (4) the new (care) environment. The focus group with six participants added further insights in the subtheme 'organisation of relocations', emphasising the importance of clear and timely communication with residents and relatives and recognizability of (personal) items and professional caregivers from the former nursing home. CONCLUSION: Stakeholders described the impact of relocations within nursing homes to vary between and within nursing home residents. Aspects they identified to influence this impact provide incentives to reduce the negative impact on residents and foster positive impact. Further research needs to zoom-in on the perceived impact of relocations within nursing homes of residents themselves

    Advancing the chemotherapy of tuberculous meningitis: a consensus view.

    Contains fulltext : 314665.pdf (Publisher’s version ) (Open Access)Tuberculous meningitis causes death or disability in approximately 50% of affected individuals and kills approximately 78 200 adults every year. Antimicrobial treatment is based on regimens used for pulmonary tuberculosis, which overlooks important differences between lung and brain drug distributions. Tuberculous meningitis has a profound inflammatory component, yet only adjunctive corticosteroids have shown clear benefit. There is an active pipeline of new antitubercular drugs, and the advent of biological agents targeted at specific inflammatory pathways promises a new era of improved tuberculous meningitis treatment and outcomes. Yet, to date, tuberculous meningitis trials have been small, underpowered, heterogeneous, poorly generalisable, and have had little effect on policy and practice. Progress is slow, and a new approach is required. In this Personal View, a global consortium of tuberculous meningitis researchers articulate a coordinated, definitive way ahead via globally conducted clinical trials of novel drugs and regimens to advance treatment and improve outcomes for this life-threatening infection.01 januari 202

    Educational professionals' attitudes, self-efficacy, and classroom practices toward high-ability students: The role of collaborative school culture and schools' collective efficacy

    Contains fulltext : 313540.pdf (Publisher’s version ) (Open Access)This preregistered study examined how a collaborative school culture and schools' collective efficacy are related to educational professionals' attitudes, personal self-efficacy, and classroom practices concerning education for high-ability students based on survey data from 875 educational professionals (teachers, counselors, and school leaders). Multilevel analyses indicated that a collaborative school culture was positively related to educators’ personal self-efficacy, but negatively to their attitudes toward special provisions for high-ability students. Collective efficacy for teaching high-ability students positively related to educators’ personal self-efficacy and attitudes toward special provisions for high-ability students, as well as teachers’ differentiated classroom practices for high-ability students. Overall, a collaborative school culture and collective efficacy for teaching high-ability students were mostly associated with positive outcomes in educational professionals (teachers, managers, and school counselors).15 p
