Although our envi-ronment is constantly changing and offers countless opportunities for interactions, this usually comes naturally to us. However, complex and unconscious brain processes are taking place in the background. In my thesis, I investigated a number of these processes. In the first part of my thesis, I studied how our brains select and prepare reaching movements. The results show that, when we have to choose between a movement with the left hand or the right hand, our brain prepares both movements. The brain therefore seems to start preparing movements even before the movement to be performed has been selected. In the second part of my dissertation, I studied how our brains ensure the smooth control of movements while driving a vehicle. The results show that the brain uses information from the senses for this. In addition, it seems to be able to learn the relationship between the steering movement and the feedback from the senses.     Low dose CT perfusion: filtering the way to the clinic

    Contains fulltext : 318247.pdf (Publisher’s version ) (Open Access)CT perfusion is a method in which multiple CT scans are taken in succession, after contrast medium has been administered to the patient. This allows the radiologist to see where and when blood enters body parts. The disadvantage of this method is that many CT scans must be made, which results in a lot of radiation for patients. To limit the total amount of radiation, the radiation per CT scan must remain small. However, this leads to low image quality. This dissertation shows that a filter method specific for CT perfusion can improve this low image quality without losing much resolution. This confirms the potential of this filter method. As a follow-up study, the filter was adapted for use in CT perfusion for diagnosis of brain infarcts. The filter showed promising results for better visualization of the infarct.     Inherited Dyslipidemic Splenomegaly: A Genetic Macrophage Storage Disorder Caused by Disruptive Apolipoprotein E (APOE) Variants.

    Contains fulltext : 318043.pdf (Publisher’s version ) (Open Access)BACKGROUND: Persistent splenomegaly, often an incidental finding, can originate from a number of inherited metabolic disorders (IMDs). Variants of APOE are primarily known as risk factors in terms of cardiovascular disease; however, severe dysfunction of APOE can result in a disease phenotype with considerable overlap with lysosomal storage disorders (LSDs), including splenomegaly and gross elevation of N-palmitoyl-O-phosphocholine-serine (PPCS). METHODS: A case study (deep phenotyping, genetic and FACS analysis) and literature study was conducted. RESULTS: The index patient, with a family history of early-onset cardiovascular disease, presented with splenic infarctions in a grossly enlarged spleen. The identified genetic cause was homozygosity for two APOE variants (c.604C>T, p.(Arg202Cys) and c.512G>A, p.(Gly171Asp); ε1/ε1), resulting in a macrophage storage phenotype resembling an LSD that was also present in the brother of the index patient. A FACS analysis of the circulating monocytes showed increased lipid content and the expression of activation markers (CD11b, CCR2, CD36). This activated state enhances lipoprotein intake, which eventually converts these monocytes/macrophages into foam cells, accumulating in tissues (e.g., spleen and vascular wall). A literature search identified seven individuals with splenomegaly caused by APOE variants (deletion of leucine at position 167). The combined data from all patients identified male gender, splenectomy and obesity as potential modifiers determining the severity of the phenotype (i.e., degree of triglyceride increase in plasma and/or spleen size). Symptoms are (partially) reversible by lipid-lowering medication and energy restricted diets and splenectomy is contra-indicated. CONCLUSIONS: Inherited dyslipidemic splenomegaly caused by disruptive APOE variants should be included in the differential diagnoses of unexplained splenomegaly with abnormal lipid profiles. A plasma lipid profile consistent with dysbetalipoproteinemia is a diagnostic biomarker for this IMD

    Health psychology and climate change: Time to address humanity's most existential crisis

It threatens the health and wellbeing of billions of people, through extreme weather events, displacement, food insecurity, pathogenic diseases, societal destabilisation, and armed conflict. Climate change dwarfs all other challenges studied by health psychologists. The greenhouse gas emissions driving climate change disproportionately originate from the actions of wealthy populations in the Global North and are tied to excessive energy use and overconsumption driven by the pursuit of economic growth. Addressing this crisis requires significant societal transformations and individual behaviour change. Most of these changes will benefit not only the stability of the climate but will yield significant public health co-benefits. Because of their unique expertise and skills, health psychologists are urgently needed in crafting climate change mitigation responses. We propose specific ways in which health psychologists at all career stages can contribute, within the spheres of research, teaching, and policy making, and within organisations and as private citizens. As health psychologists, we cannot sit back and leave climate change to climate scientists. Climate change is a health emergency that results from human behaviour; hence it is in our power and responsibility to address it.31 p

    Transient bacteraemia secondary to imiquimod use as a treatment for cervical high-grade squamous intraepithelial lesions - A case report.

    Contains fulltext : 317856.pdf (Publisher’s version ) (Open Access)Imiquimod is an immunomodulating cream used in the treatment of cervical high-grade squamous intraepithelial lesions (cHSIL). Side-effects are common and mostly well tolerated, but can be severe. A woman using imiquimod for recurrent cHSIL presented with signs of a distributive shock nearly three weeks after the start of imiquimod treatment, for which she was admitted to a medium care unit. No infectious source was identified. The woman recovered well with conservative treatment. This case demonstrates the importance of monitoring patients for side-effects during imiquimod treatment. Side-effects, including the possibility of severe side-effects, should be discussed during patient counselling on therapy options for cHSIL treatment
