    Identification of Problems or Barriers in Medicine Procurement Process in Low- and Lower-Middle-Income Countries: A Narrative Review

    Medicine procurement at LLMIC still becomes a challenge regarding to low medicine availability. There is no study yet that identifies the problems of medicine procurement at LLMIC countries so that it becomes the objective of this narrative review. PUBMED, Science Direct, Google Schoolar, PLOS from May 14th, 2021 to May 22nd, 2021 were used. The problems of procurement were found since the processes of selection, planning, and procurement. Factors of human resources, organization, management information system, and parties excluding health facilites (government, distributor, and pharmacy industry) also contribute to hinder the medicine procurement. The effort to solve this problem should include multisectors with the aim to strengthen the medicine management and the supply chain.     Bioinformatic and Molecular Interaction Studies Uncover That CCA-1.1 AND PGV-1 Differentially Target Mitotic Regulatory Protein and Have a Synergistic Effect against Leukemia Cells

    Numerous studies demonstrated CCA-1.1, the modified compound from PGV-1, inhibits tumor cells growth in breast and colorectal cancer. This time, we used bioinformatics and molecular interaction approaches to ascertain the potential CCA-1.1 activity target focusing on leukemia, along with the cytotoxic test in leukemia cells. Genomics data expression was collected through the COSMIC database by selecting gene sets from K562 cells as a model for chronic myelogenous leukemia (CML).  We identified CCA-1.1 and PGV-1 predicted targets through SwissTargetPrediction. The overlapping genes between the CCA-1.1, PGV-1, and K562 cells were chosen for further analysis. We narrowed down the potential targets by using the list of genes involved in the cell cycle and mitosis collected through GeneCards. A molecular docking study was applied to determine the molecular interaction between CCA-1.1 or PGV-1 and the predicted protein target. We carried out a cytotoxic test using a trypan blue exclusion assay. We treated K562 cells with CCA-1.1 and PGV-1 in single and combination treatment to determine the half concentration of growth inhibitory (GI50) and combination index (CI) score. CCA-1.1 and PGV-1 shared similar predicted target genes in mitosis, and interestingly CCA-1.1 were mainly targeted in Aurora A (AURKA) in K562 cells with lower docking scores against the inhibitor in molecular docking analysis. Moreover, each compound exhibited an inhibitory effect similarly, and the co-treatment resulted in a synergistic effect in K562 cells. Collectively, we indicated that CCA-1.1 and PGV-1 possibly targeted mitosis in cell cycle progression, and along with their specific targets, led to their synergistic activity in CML. These findings should be validated through experimental studies to provide more pharmacological activities of CCA-1.1 to cure CML

    The Effects of Media and Blanching Time on the Antioxidative Properties of Curcuma aeruginosa Roxb.

    Black saffron (Curcuma aeruginosa Roxb.) belongs to the family of Zingiberaceae is one of rhizomes widely used as raw material in Indonesian Traditional Medicines. Black saffron (BS) contains some bioactive compounds responsible for certain biological activities including antioxidant. Blanching has been reported to increase the antioxidant activity of BS. This study aims to formulate BS containing high antioxidant activity. This research was performed by varying blanching medium (citric acid and aquadest) and blanching time (0; 2.5; 5; 7.5 and 10 min). The fabrication stage of BS powder included peeling, cleaning, blanching, slicing drying, grinding, and sieving. After that, the treated BS was analyzed for the antioxidant activity, total phenolic, total flavonoid, tannin, crude fiber, and water contents. BS powder subjected to blanching process showed better antioxidant activity than that without blanching process. Blanching using citric acid media 0.05% for 5 min showed the best antioxidant activities, as indicated by high contents of total phenolic, total flavonoid, and tannins. Powdered BS is potential to be used as materials to be applied as fortifying agents in food products

    Favourable drug-lead pharmacokinetic features for designing gallic acid-standardized Syzygium polyanthum aqueous extract-based product

    In this study, Syzygium polyanthum was standardized against gallic acid (GA), and a complete pharmacokinetic test was conducted using in vitro and in vivo models against this phytochemical. High-performance liquid chromatography showed that GA is a major phytochemical in aqueous extract of S. polyanthum. It exhibited a low equilibrium solubility and physiochemical stability at pH 2.0 and 7.4, but it deteriorated rapidly at pH 9.2. It showed low permeability toward Caco-2 intestinal absorption with eight times slower absorption in oral than intravenous administration. GA was unstable in mouse, rat, and dog plasma sera with in vitro half-lives (t1/2) of 60, 53, and 56 min, respectively, but was relatively stable in human plasma serum (t1/2 = 185 min). Approximately 5.6% of GA (10 µM) bound to the human plasma proteins. GA was stable in mouse, rat, dog, and human microsomal extracts with in vitro microsomal intrinsic clearance values of 72, 68, 6, and 22 µL/min/mg, respectively. GA selectively inhibited or stimulated the activity of the tested CYP450 enzymes. The in vivo oral bioavailability of GA was 54%, with short elimination half-life and a high volume of distribution. Thus, the mention pharmacokinetic features of GA must be considered during the development of GA-based products to yield the optimum dosage and pharmacological effect

    Exploring the Potency of Jatropha Seed Meal (Jatropha curcas) as a Chemoprevention Agent through Metastatic Inhibition

    Jatropha (Jatropha curcas) is often used as biodiesel because of the oil content in its seeds. The production of jatropha oil generates a byproduct in the form of jatropha seed meal, which contains compounds with cytotoxic activity and phorbol esters, as co-carcinogens and tumor promoters. Meanwhile, metastasis is one of the characteristics of cancer where the cells spread to another tissue. This study aimed to determine the potential of jatropha seed meal as a chemoprevention agent, particularly an antimetastatic one, under bioinformatic study and molecular docking. Genecards and DAVID were performed to explore the protein involved in the metastatic process and its gene ontology. The prediction target protein was caught by SwissTargetPrediction. Jatropha seed meal showed the presence of isoamericanol A, myricetin, daidzein, gallic acid, and rutin. There are 11 prediction target proteins correlated to metastatic in extracellular matrix components. Then we were docked to a protein involved in metastasis, matrix metalloproteinase (MMP)-9 (PDB ID: 6ESM) using MOE software. The docking score determined the interaction properties. The docking analysis revealed that isoamericanol A, daidzein, and myricetin exhibited better binding affinity than native ligands and other compounds. Moreover, based on our literature study, the jatropha seed meal contains isoamericanol A, rutin, myricetin, daidzein, and gallic acid, which present anticancer properties by inhibition of cell invasion and migration, cell cycle arrest induction, and suppression of the MMP-9 activity. Overall, jatropha seed meal has potential as an antimetastatic agent. A comprehensive study is needed to explore the possibility of developing it as a supportive agent in combination with a chemotherapeutic agent

    Effects of Maltodextrin on Sauropus androgynus Leaf Extract Characteristics

    Katuk (Sauropus androgynus) leaves have been traditionally used in Indonesia for increasing human breast milk production. In the previous research, katuk leaves have high moisture content, therefore katuk leaves extract were being prepared as spray dried S. androgynus extract. The freeze dried leaves were extracted with ultrasonic assisted extraction method using ethanol 80% as a solvent. Then, katuk extract was dried with spray drying method using maltodextrin as a drying aid. To improve the physical characteristics of this extract, it was mixed with mannitol, spray dried lactose, and crospovidone into S. androgynus extract powder. The results showed better physical characteristics, especially on moisture content and flow properties of powders. Metabolic profiles of all samples were analysed by thin layer chromatography (TLC) densitometry method, while the dried extract was dissolved in a suitable solvent and then spotted on GF254 and the plate was developed using a mixture of n-butanol:acetic acid:water (60:22:1.2). Based on TLC profiles, there are three different spots can be seen clearly at 366 nm on chromatogram of S. androgynus leaves, freeze dried leaves, spray dried leaves extract, and leaves extract powder. There is one spot (S3) at Rf 0.80 which is a stable chemical compound that is not affected by all factors in the entire process in S. androgynus extract powder formulations from extraction, drying, to formulation

    Validation of UV-Vis Spectrophotometric Method to Determine Drug Release of Quercetin Loaded-Nanoemulsion

    A simple validated UV/Vis spectrophotometric method to analyze quercetin released from the nanoemulsion matrix has been developed to study the drug release profile of quercetin loaded-nanoemulsion. Quercetin analysis was carried out at a maximum wavelength of 254 nm. The method showed linearity with a correlation coefficient (r) value of 0.9998 in the range of 4 - 12 μg/mL. The results also demonstrated that the procedure is accurate and precise, with recovery (%) in the range of 99.65 - 100.1312 % and RSD (%) as ≤ 2%. The limit of detection (LOD) and limit of quantitation (LOQ) values were found to be 4.0535 μg/mL and 13.5118 μg/mL, respectively. The developed method was found to be valid to analyze quercetin released in nanoemulsion preparations

    Tyrosinase Inhibitory and Antioxidant Activity of Paederia foetida L.

    Paederia foetida L. is a tropical Asian plant containing bioactive compounds and often used as functional foods. The aim of this study was to determine the antioxidant capacity and inhibitory activity of P. foetida leaves extract on tyrosinase activity. In addition, the total phenolic content (TPC) and total flavonoid content (TFC) were determined. TPC and TFC was evaluated by the Folin-Ciocalteu and the aluminum chloride (AlCl3) colorimetric method, respectively. The antioxidant capacity was determined by 2,2-diphenyl-1-picrylhydrazyl (DPPH), ferric reducing antioxidant power (FRAP), and cupric reducing antioxidant capacity (CUPRAC) method. The antioxidant capacity was expressed as trolox equivalent antioxidant capacity (TEAC) dan ascorbic acid equivalent capacity (AAEC). The tyrosinase inhibitory activity was conducted by enzyme-linked immunosorbent assay (ELISA) using L-tyrosine as a substrate and measured at 490 nm by ELISA reader. TPC in the crude extract, fraction A, and fraction B was 173.18 ± 3.99, 553.95 ± 5.70 and 405.37 ± 33.90, respectively. TFC in the crude extract, fraction A, and fraction B was 12.79 ± 0,25, 143.16 ± 9.27, 143.50 ± 6.90, respectively. The best antioxidant capacity of the extract was shown in the DPPH method (15.71 ± 1.6 mg TEAC/g and 100.77 ± 8.5 mg AAEC/g). Meanwhile, fraction B showed the best antioxidant capacity by the FRAP (11.48 ± 1.5 TEAC/g and 8.39 ± 1.2 mg AAEC/g ) and CUPRAC (116.34 ± 1.9 mg TEAC/g and  66.11 ± 1.3 mg AAEC/g) methods. Tyrosinase inhibitory activity exhibited that the IC50 of fraction A and B was 13.67 μg/mL and 13.37 μg/mL, respectively

    Stress during the COVID-19 pandemic affects the physiological and immunological response to women's reproductive health. Eucalyptus globulus and Apium graveolens are immunomodulators related to women's reproductive health. This study aimed to investigate E. globulus and A. graveolens towards the expression of Heat Shock Protein-70 (HSP70) and tumor necrosis factor-alpha (TNF-α) as stress markers, luteinizing hormone (LH), and Growth Differentiation Factor 9 (GDF-9) as folliculogenesis markers. This study was an experimental randomized controlled trial using a pre-posttest control group design. Sixty women of reproductive health age who had stress based on the DASS-21 questionnaire were divided into two groups at area of Nusukan Health Center, Indonesia. The intervention group was given 300 mg A. graveolens capsules orally and E. globulus essential oil for 14 days. The control group was given a placebo. Blood samples and stress levels were evaluated before and after the intervention to determine the effect of the intervention. There was no difference in stress levels in the control and treatment groups at pre-test. The intervention group decreased HSP70, TNF-α, and stress levels (p<0.001). A significantly decreased TNF-α increased LH level (p<0.001). Moreover, LH and GDF-9 levels increased in the intervention group compared to the control group (p<0.001). In summary, A. graveolens supplementation and E. globulus essential oil decreased stress and protected folliculogenesis markers on women's reproductive health due to stressful conditions during pandemic COVID-19

    Systematic Literature Review on Methodology and Manufacturing Process of Personalized Tablet using Selective Laser Sintering Technology

    3-dimensional printing (3DP) technology has garnered interests as a novel candidate for future pharmaceutical manufacturing. Since the first drug product (Spritam®) has been approved for commercialization by the Food and Drug Administration (FDA), there has been an enormous opportunity for printing custom drugs using 3DP. Many 3DP methods have been documented for pharmaceutical applications in the literature. However, selective laser sintering (SLS) printing remains the least studied for pharmaceutical applications. There are many advantages and challenges in adopting an SLS method to fabricate personalized medicines, such as accurate, cheaper, and simpler ways to configure dosing for certain patient groups. In this study, we systematically reviewed all available literature investigating the technique of personalized printlets using SLS printing, and further discussed the method used in its process. A systematic searching strategy was performed in Scopus, PubMed, and Google Scholar databases using predetermined search strings. Of the 122 articles, only eight articles completely met the inclusion criteria, and they were subsequently used for data synthesis. The results showed that the printing process, spectrophotometry analysis, thermal analysis, X-ray powder diffraction and characterization of the printlet were the vital parameters in the printing method, leading to potential pharmaceutical applications in personalized medicine
