Self-Regulation and Sensory Processing Education Program for Educators
Children with low self-regulation skills may face many challenges in the classroom environment that can impact their performance in school-based activities. As teachers play a vital role in the development of young children, this educational program aims to improve teacher competence (knowledge, skills, and attitudes) in applying sensory processing and self-regulating concepts to the school environment.https://soar.usa.edu/otdcapstonesspring2024/1034/thumbnail.jp
Evidence-Based Practice for Terminal Extubation: A Guideline Development for Critical Care
Practice Problem: In the critical care unit it was identified that providers utilized various practices in managing patients who were to be terminally extubated (TE). The variations in TE practice resulted in healthcare providers engaging in the TE process feeling unsure of the care they were providing and questioning if best practice measures were being implemented.
PICOT: The PICOT question guiding this project was: “In acute care adult patients who are to be TE (P), how does the development of an evidence-based TE guideline (I) compared to current TE practices (C) improve critical care healthcare workers perception and ability to provide quality end-of-life care practices during the TE process (O) over ten weeks? (T)”
Evidence: An exhaustive literature search identifying best practice measures for TE resulted in 55 peer- reviewed articles. Ten studies were included after filtering for duplicates, keywords, and evaluating for strength and quality of the evidence.
Intervention: The project’s intervention was to develop and implement a TE guideline in a critical care unit. The Johns Hopkins Evidence-Based Practice Model for Nursing and Havelock’s Change Theory guided each step in the development and implementation of a three-phase TE guideline.
Outcome: A pre and post-survey was conducted to evaluate nurses’ change in perception and medication administration during the TE process. A two-tailed paired sample t-test was conducted resulting in both t- tests being not significant. However, the change in the mean score (0.27 difference in the standard deviation (SD) of pre and post-perception of TE and 0.22 difference in the SD of the pre and post-comfort in giving medications during TE) indicates clinical impact.
Conclusion: The ambiguity a healthcare provider has in their work can result in errors and frequent questioning of one\u27s actions, bringing about feelings of inadequacy and workplace burnout. Developing a multidisciplinary, evidence-based terminal extubation guideline can offer healthcare providers guidance and confidence in their role during the TE process and ensure quality care is provided to patients and families as best practice methods are being provided

Policy Review and Recommendation: Full Practice Authority for Psychiatric Mental Health Nurse Practitioners in Georgia
Citizens of the state of Georgia, particularly those in rural or underserved communities, experience a lack of mental health resources. Extensive research evidence underscores the urgent need for enhanced access to mental health services, especially in rural areas where individuals face significant barriers such as limited provider availability and long travel distances. Through a thorough review of existing policy related to scope of practice, this scholarly work advocates for policy changes related to the practice authority of Psychiatric Mental Health Nurse Practitioners (PMHNPs). Advancement of legislative priorities to promote full practice authority for PMHNPs in the state of Georgia empowers the practitioners to address access barriers and deliver essential mental health care to underserved communities. This DNP scholarly project underscores the critical importance of evidence-based policy changes to address healthcare disparities and improve public health outcomes. By advocating for the removal of restrictive practice barriers for PMHNPs, this project sought to ensure timely and quality mental health care access across Georgia
Toward An Identity for the Field of Doctoral Education in Health Sciences
The Association of Doctoral Programs in Health Sciences (ADPHS) was informally established in November 2019, officially incorporated in August 2021, and is currently a 501(c)(3) non-profit organization comprised of the directors of member doctoral programs of health sciences. The ADPHS grew from informal discussions among program directors who agreed that a major problem in the field of doctoral education in health sciences was the lack of a clearly defined and easily articulable identity. The discussions led to the drafting of an informal and nonscientific survey used to help clarify the current health sciences education environment, relevant emerging trends, and the educational philosophies adopted by the directors of health sciences doctoral programs nationally. The results of the survey and follow-up discussions revealed a strong consensus among program directors that the field of doctorate education in health sciences is uniformly characterized by its interdisciplinary nature. In this position paper, we provide the rationale for the formal position of the ADPHS that the identity of the field of doctoral education in health sciences is based on its interdisciplinary approach to education
Adapted Cheerleading Program for the Pediatric Population
There are limited adapted programs with educated professionals that exist for children with childhood disabilities to be able to adapt activities to the specific needs of the child. Due to the limited opportunities for participation in leisure activities, these children have limited opportunities to improve physical, verbal, or emotional skills with their peers. The objective of this capstone was to create an adapted cheerleading program for the pediatric population to improve their physical, emotional, and verbal abilities as well as improve their mental health and overall quality of life.https://soar.usa.edu/otdcapstonessummer2024/1011/thumbnail.jp
Implementing a Patient Education Bundle to Improve Patient Satisfaction
Practice Problem: Patients on the intervention unit reported via HCAHPS surveys a lack of understanding of the nurse provided education on their medications’ side effects and purpose after discharge home from the hospital. Nurses at the intervention unit’s organization do not use any formal or standardized method of patient education to ensure patient comprehension.
PICOT: Among adult inpatients (18 years or older) on a step-down unit in an acute care facility (P), does implementing an education bundle related to patient medication education (I), compared to current practice of no formal education process (C), increase patient satisfaction as measured by HCAHPS scores on patient medication (O) in eight weeks (T)?
Evidence: Through rigorous literature review, evidence-based methods proven to assist patients’ learning were discovered. The literature states that to enhance comprehension, education should be provided using plain language, in patients’ preferred language, and utilizing their preferred learning method. Healthcare providers can then ensure their understanding with the use of the teach-back method.
Intervention: Nurses on the intervention unit were educated on an education bundle and implemented the bundle as new practice throughout patients’ hospitalization. Use of this bundle was promoted for each alert and oriented patient during medication administration and at discharge.
Outcome: The intervention of a medication education bundle resulted in a statistically significant increase of the intervention unit’s patients’ satisfaction of provision of medication education HCAHPS scores over time.
Conclusion: When used together, multiple education interventions are effective in increasing patient satisfaction of education and reported understanding of medications’ side effects and purpose
Exploration of Club Motorcycle Riding for Veteans
There is significantly limited research regarding the occupation of club motorcycle riding, and even more limited with its application to the veteran population
The study yielded 17 survey participants and 7 interview participants from the VFW MC. The study aimed to discover the benefits of club motorcycle riding for the veteran population through a Doing, Being, Becoming, and Belonging Framework. The data consisted of both quantitative and qualitative components which revealed four overall themes.
The four main themes that came from the data analysis were (1) Camaraderie, (2) Mental and Emotional Health Through Occupation, (3) Reconstruction of Identity, and (4) Altruistic Contributions. From trust to brotherhood, each veteran seemed to have a connection within the VFW MC that they have not experienced since their time in the service. There was a collective sense of occupation that underlined each members answers when explaining the importance of the club.
Having a role within a group occupation can be a benefit to those going through life transitions, such as retirement. There needs to be a further emphasis on the importance of discovering the ways occupational therapy can promote a collectivistic mindset within the veteran population.https://soar.usa.edu/otdcapstonessummer2024/1039/thumbnail.jp
Using Home Telehealth to Reduce Hospital Readmission Rates in Heart Failure Patients in the Emergency Room
Practice Problem: In one hospital, the readmission rates and emergency room visit for patients who are diagnosed with heart failure (HF) was increasing.
PICOT: Among veterans who are hospitalized for HF (P) does the enrollment into a care coordination program for these veterans (I) compared to current practices (C), reduce the hospital readmission rates and emergency department visits (O) within 10-weeks (T)?
Evidence: The literature supports the idea that home telehealth as a home monitoring program for patients who have chronic diseases such as heart failure (HF) and has had success in patients with hypertension amongst patients. Transition strategies from inpatient to home are important to prevent hospital re-admissions.
Intervention The project included enrolling patients who have HF, in an evidence-based home telehealth care coordination program, intensive case management or specialty case management programs within 48 hours of being discharged from the hospital or the emergency department.
Outcome: Those who enrolled in the case management program reduced the emergency room visits and hospitalizations by 75.86 %, with a total of 29 visits prior to enrollment and down to seven post intervention. The result of the two-tailed paired samples t-test was significant based on an alpha value of .05, t(20) = 2.85, p = .010, showing a significant difference in the 8-week period.
Conclusion: This EBP project shows that enrolling patients who have recently been hospitalized for HF, in a home telehealth or a case management program was effective at reducing readmissions in an 8-week period. The results were both statistically and clinically significant
Occupational Therapy\u27s Role: Transition to College for Students with Learning Differences
The project aimed to improve the prospect of success for students with learning differences transitioning to college, allowing them to accomplish their educational and career goals better. Additionally, it may provide educational opportunities for educators, occupational therapists, families, and these persons. When these individual\u27s occupational demands are identified, occupational therapy programs can be designed to meet their requirements. OT professionals may have evidence and programs to assist these students and other interprofessional teams by providing unique assessments, interventions, and relevant findings. Raising awareness of the need for additional resources and accessibility can be aided by OT\u27s increased presence in the community for students with learning differences. Furthermore, the program can be utilized as a future reference to optimize outcomes and effectiveness for students with learning differences.https://soar.usa.edu/otdcapstonessummer2024/1043/thumbnail.jp
