    Advocacy and Education for Children with Low Vision: Cortical Visual impairment

    This project addresses the gap in research and advocacy for children with low vision, particularly those diagnosed with Cortical Visual Impairment (CVI), the leading cause of visual impairment in children. Through a needs assessment and educational intervention, the project aimed to enhance occupational therapy (OT) students\u27 understanding of low vision in children and their role in addressing it. Pre- and post-surveys revealed a significant increase in knowledge among students, highlighting the need for specialized education in OT curricula.     Effect of the collaborative model on communication and clinical reasoning in DPT students’ clinical experiences.

    Introduction: The collaborative clinical experiences pair one physical therapist (PT) clinical instructor (CI) with one or more student(s) (Wolff-Burke et al, 2022).The collaborative model has been widely accepted among nursing and other healthcare professions. Benefits of the collaborative model include improved communication and clinical reasoning, enhanced teamwork, and reduced anxiety among students. Additionally, the collaborative model facilitates peer-to-peer learning and leadership. Despite the benefits, PT education has been slow to embrace this model. This study\u27s purpose was to determine if the collaborative model is effective in helping students obtain the required performance ratings required by the university during their clinical experiences. Subjects included student placement records were reviewed to determine inclusion and exclusion based on participation in a collaborative experience between Spring 2022 and Spring 2023. Methods: Clinical Performance Inventory (CPI) data was obtained and analyzed in communication and clinical reasoning. The professionalism and academic tracking systems were also reviewed to obtain student history related to communication and clinical reasoning (CR). Demographic information on gender and age was also included for comparison. Results: Every student passed their clinical experience. Students scored higher than the expected threshold for their respective clinical experiences, with 76.3% over performing on communication and 85.5% on clinical reasoning. Female students scored higher than male students on communication and CR for the Integrated Clinical Experience (ICE). Students referred to the professionalism tracking system scored 3% lower on communication and 2% lower on CR compared to those who were not referred to the professional tracking system. Discussion: The collaborative model appears to be an effective supervision model for clinical experiences. It may provide an effective way for students to develop their communication and clinical reasoning skills. Future studies should be designed to compare students’ performance and perceptions between those in traditional and collaborative models

    An Evidence-Based Bundle to Decrease Pressure Injuries (PIs) in Homecare

    Practice Problem: Pressure injuries are a significant problem in healthcare settings. The older adult patients in this home health agency have experienced an increased in incidences of pressure injuries which necessitates intervention. PICOT: The PICOT question that guided this project was In older adults 65years and above with pressure injuries in the home healthcare setting(P), does a pressure injury care bundle(I), compared to the current practices of standard care of repositioning alone( C),reduce the incidence of pressure injuries( O ) in 10 weeks (T)? Evidence: The evidence used to guide this evidence-based change project was from relevant health literature from databases including CINAHL, ProQuest, PubMed and Google Scholar on pressure injury care bundle implementation to reduce pressure injury on older adult patients in home care settings. Intervention: The Intervention used was the pressure injury care bundle consisting of the Braden scale tool for risk assessment, The Bates-Jasen Wound Assessment(BWAT) tool for skin assessment, nutrition, repositioning, hydration, proper support surfaces, and patients and caregivers involvement were used to reduced the incidence of pressure injury on older adults of the home health agency. The involvement of multidisciplinary clinicians of the home health care agency in implementing the pressure injury care bundle, as well as their feedback, monitoring, and documentation, was effective pressure injuries and reducing pressure injuries. Outcome: The intellectus statistics tool was used to analyzed the difference between standard care and the use of the Pressure injury care bundle. The findings revealed a 98% improvement in older adults\u27 pressure injuries within 10 weeks of using pressure injury care bundle. Conclusion: The pressure injury care bundle achieved an improvement in the patients\u27 pressure injuries and a reduction in patients\u27 pressure incidence. The proportion of standard care was significantly lower than the proportion of pressure injury care bundle

    Manualized Occupational Therapy Intervention For Children And Adolescents With Post-Traumatic Stress Disorder

    The capstone poster demonstrates the purpose for a manualized occupational therapy (OT) intervention for OT practitioners to treat children with post-traumatic stress disorder. The intervention uses an occupation-based approach informed by behavioral activation therapy and the model of human occupation principles. The manual includes two parts. Part one demonstrates the foundational principles for OT practitioners to obtain knowledge, skills, and tools before implementing the intervention. Part two outlines the treatment process beginning with evaluation, introducing manualized OT intervention to the child and the family member(s) and/or caregiver(s), treatment application, progress note, and discharge. The manualized OT intervention will allow OT practitioners to use an occupation-based approach involving graded occupation and to track treatment progress and outcomes.     Outdoor Recreation for People with Spinal Cord Injuries: An Educational Video Series

    More than 15 million people globally are living with spinal cord injury (SCI). Only half of people with SCI engage in leisure activities and approximately 71% of the population reports dissatisfaction in their leisure participation. There are numerous intrinsic and extrinsic barriers that lead to these significant declines in leisure exploration and participation, including lack of knowledge and inaccurate accessible online information. In correlation with these barriers, occupational therapy practitioners underutilize leisure participation during the rehabilitation process for people with SCI and this is an underreported topic in the literature. Occupational therapy practitioners and people with SCI lack knowledge of adapted outdoor recreational activities that people with SCI can engage in. The purpose of this project was to create an educational video series to promote participation in adapted outdoor recreational activities among people with SCI. An educational video series containing 18 videos was created through the culmination of literature review, communication with stakeholders, and gathering data during capstone site experience. Topics incorporated in this video series include background of SCI and outdoor recreation, precautions, equipment, transfers, participation expectations for cervical and thoracic level SCI, and opportunities for outdoor recreation participation in Colorado.     Mild Traumatic Brain Injuries in Justice-Involved Individuals and Executive Function Consequences on Transition and Reentry into the Community ​

    The United States has the highest documented incarceration rate in the world (Carson, 2022), with an increasing number of justice-involved individuals (JII) potentially affected by mild traumatic brain injury (mTBI). Despite the high prevalence of mTBIs in the general population, limited research exists on the prevalence and impact of mTBIs in JII. Preliminary evidence suggests that the prevalence of mTBIs in incarcerated individuals is 50 times higher than in the general public (Siegler et al., 2017). This study aimed to explore the lived experiences of JII with mTBI, focusing on the origin of mTBIs, executive dysfunction, and the occupational barriers faced during the transition and reentry process. A mixed-methods, descriptive phenomenological approach was utilized to capture data from six study participants who were formerly incarcerated. Quantitative measures included demographics, comorbidity and social determinants of health survey (SDoH), the Rivermead Post-Concussion Symptoms Questionnaire (RPQ), and the Executive Skills Profile (ESP). Qualitative data was gathered via the Ohio State University Traumatic Brain Injury Identifier (OSU TBI-ID) and a 15-question semi-structured interview. Results indicate that JII experience several compounding disparities including socioeconomic status, comorbidities, and social determinants of health that are further exacerbated by persistent post-concussional symptoms and executive dysfunctions. This research demonstrated how mTBI and associated executive dysfunction complicate interactions for both those who have the injury and for professionals in the criminal legal system.     A Multifactorial Fall Prevention Program

    Falls among older adults are a pressing public health concern, leading to significant physical, psychological, and economic consequences. This capstone project aimed to design and implement a multifactorial fall prevention program to address unmet occupational needs and enhance safety and occupational participation among long-term care residents at a skilled nursing facility. The six-week program included targeted weekly sessions focusing on behavior modification, footwear safety, adaptive equipment training, strength and balance exercises, toileting strategies, and environmental modifications. Pre- and post-program assessments utilized the Fall Efficacy Scale International (FES-I) and participant surveys to evaluate changes in confidence, fear of falling, and satisfaction with fall prevention strategies.     Effect of Acute Care Discharge Delays on Inpatient Rehabilitation Facility (IRF) Utilization

    Discharge delays present hospitals with challenges in managing costs for Medicare admissions. Meanwhile, Inpatient Rehabilitation Facilities (IRF) are subject to strict pre-admission requirements that can complicate efficient transitions of care for patients with intensive rehabilitation needs. The purpose of this retrospective quantitative correlational study was to examine any relationship between acute care discharge delays and IRF utilization. This study investigated whether length of stay (LOS) and Medicare Advantage prior authorization requirements impact IRF referrals by reviewing 3,747 medical records of acute stroke patients aged 65 years and older at a regionally integrated healthcare system over a 3-year period (2020- 2022). Referral to IRF was strongly correlated with LOS, and Medicare financial class (fee-for-service vs. Medicare Advantage) was a significant moderator of that relationship. Patient age at admission and whether the referring hospital operated an IRF were also significantly related to IRF referral. Further, distinct differences in post-acute care pathways were noted for stroke patients between FFS and MA plans despite very similar clinical profiles

    The Occupational Performance of Student-Athletes Post-Concussion: A Guide for Coaches and Athletic Trainers

    Concussions are a rising concern amongst the sports community. Student-athletes with concussions can exhibit deficits in physical, psychosocial, cognitive, and visual-perceptual abilities (Centers for Disease Control, 2019). Occupational therapists are equipped to address the impacts of those deficits to improve occupational engagement and performance but are often overlooked in concussion management teams (Finn, 2019). A concussion management team typically consists of coaches, athletic trainers (ATs), physicians, and sometimes physical therapists, depending on the severity of the concussion. Due to growing concern and continued research, the concussion management team must be current with new information. Coaches and ATs may not keep up with new challenges and may not know how to identify subtle signs of concussions. This guidebook aimed to create a guidebook for coaches and ATs on working with student-athletes with concussions to educate them on concussions, how to treat them, understand symptoms, and more. When creating the guidebook, occupational therapy was rooted in the background by utilizing the Model of Human Occupation (MOHO) and the Cognitive-Behavioral Frame of Reference. The guidebook consists of six chapters that include background on concussions, what occupational therapy is and its role, how to identify, treat, and adapt to concussions, how concussions vary by person and sport, and more. This capstone document comprises five chapters that discuss the background, literature review, proposed methods of creating the guidebook, results, and discussion. This capstone project broadens the understanding of concussions in student-athletes and occupational therapy’s role in the concussion management team.
