    2005-2021 Yılları Arasında Tobb Etü Tıp Fakültesi Hastanesi Merkez Laboratuvarına Ekinokokkozis Ön Tanıyla Başvuruların Değerlendirilmesi

    8.7, 4.8, 2.3, 0.4, and 0.1 (x103/µL), for leukocyte, neutrophil, lymphocyte, eosinophil and basophil counts. The average IgE value of negatives is 155.4 IU/mL. Conclusions: The positivity rate of 3.6% in high-income patients supports that this disease still important in Turkey. Female patients, especially in the middle age group, presenting with gastrointestinal system complaints, should be evaluated for echinococcosis, even if their biochemical/hematological parameters are normal.leukocyte, neutrophil, lymphocyte, eosinophil, and basophil values were 5.9, 3.4, 1.8 and 0.2 (x103/µL). The mean values for ALT, AST, urea, creatinine and CRP of negative patients were 30.0U/L, 27.9U/L, 16.5mg/dL, 0.9mg/dL and 14.2mg/dL27 (49.9%) females. Only 2 (3.6%) Turkish female patients were found positive. The positivity rate in women was 7.4%, and their diagnosis was confirmed radiologically (ultrasonography and computed tomography). The patients’ ages range from 1 to 81 (mean 41.2), whereas the seropositives' average age is 31.2. Six (10.1%) of patients were children and they were referred from Pediatrics and Pediatric Surgery clinics. Including one of the positive patients, 67.4% (33/49) of adults, applied to Gastroenterology Clinic. The other positive case was referred from Orthopedics and Traumatology Polyclinic. Mean ALT, AST, urea, creatinine, and CRP values of positive patients were 16.0U/L, 16.5U/L, 8.6mg/dL, 0.6mg/dL and 6.1mg/dL28 (50.1%) malestiters of 1/160 and above were considered as positive. Results: Our patients belong to the middle-upper and upper economic income group: 16.4% from foreign countriesObjectives: Serological, biochemical, and hemogram test results provide important clues in diagnosis and follow-up of echinococcosis. In this context, the results of the patients who were admitted to our hospital with a preliminary diagnosis of echinococcosis were evaluated retrospectively. Materials-Methods: Between 01 January 2005-31 December 2021, 55 patients were included in this study. The samples were analyzed by indirect hemagglutination method (Hydatidose, Fumouze Laboratoires, France)ve 0,1 (x103/µL)’dir. Negatif olanların IgE değeri ortalaması 155,4IU/mL olup, pozitif hastaların bu değeri bilinmemektedir. Sonuç: Hastanemize başvuran üst ekonomik gelir grubundaki hastalarda %3,6’lık pozitiflik oranı saptanması, Türkiye’de halen bu hastalığın önemini koruduğu görüşünü desteklemektedir. Gastrointestinal sistem şikayetleri ile başvuran özellikle ortaya yaş grubundaki kadın hastaların biyokimyasal/hematolojik parametreleri normal dahi olsa ekinokokkozis açısından değerlendirilmesi önerilir.0,42,34,80,9mg/dL ve 14,2mg/dL, lökosit, nötrofil, lenfosit, eozinofil ve bazofil için 8,716,5 mg/dL27,9U/L1,8 ve 0,2 (x103/µL) olarak belirlenmiştir. Negatif hastaların ortalama değerleri ise ALT, AST, üre, kreatinin ve CRP için 30,0U/L3,40,6mg/dL ve 6,1mg/dL, lökosit, nötrofil, lenfosit, eozinofil ve bazofil değerleri 5,98,6mg/dL16,5U/LAmaç: Ekinokokkozisin tanısı temel olarak görüntüleme yöntemlerine, tedavisi ise cerrahiye dayanmaktaysa da hastaların tanısı ile izleminde seroloji, biyokimya ve hemogram sonuçları da önemli ipuçları sunmaktadır. Bu kapsamda Hastanemize ekinokokkoz ön tanısıyla başvuran hastaların sonuçları retrospektif olarak değerlendirilmiştir. Gereç ve Yöntem: Çalışmaya 01. Ocak. 2005-31.Aralık.2021 tarihleri arasında TOBB-ETÜ Hastanesi Merkez Laboratuvarına ekinokokkoz ön tanısıyla çeşitli kliniklerden gelen 55 hastaya ait çoklu serum örnekleri alınmış, hastaların verileri hastane bilgi sisteminden çekilmiştir. Örnekler, indirekt hemaglütinasyon (IHA) yöntemiyle ticari kit (Hydatidose, Fumouze Laboratoires, France) ile çalışılmış1/160 ve üzeri titreler pozitif olarak kabul edilmiştir. Bulgular: Hastanemize başvuran hastalar orta-üst ve üst ekonomik gelir grubunda yer almakta olup %16,4’ü yabancı uyruklu, 28’i (%50,1) erkek, 27’si (%49,9) kadındır. Sadece iki (%3,6) Türk kadın hastada pozitiflik saptanmıştır. Kadınlardaki pozitiflik oranı %7,4 olup tanıları radyolojik olarak da (ultrasonografi ve bilgisayarlı tomografi) onaylanmıştır. Başvuranların yaşları 1-81 (ortalama 41,2) arasında iken seropozitiflerin yaş ortalaması 31,2’dir. Başvuranların 6’sı (%10,1) çocuk olup Pediatri ve Çocuk Cerrahisi polikliniklerinden yönlendirilmiştir. Pozitif hastalardan birisi de dahil olmak üzere erişkinlerin %67,4’ü (33/49) Gastroenteroloji Polikliniğine başvurmuştur.     In this thesis, it is aimed to develop an active mode locked fiber laser with low phase noise at 4GHz frequency. Fiber lasers are divided into two part according to change of the light they emit over time as continuous wave or pulsed. In this study, a laser that produces a short pulse sequence is designed. In the first part of the thesis, general information about lasers is given and it is aimed to introduce the usage areas, development processes, concepts and used terms. In the second part of the thesis, the fiber optic technology and working principle used for the laser, which is aimed to be developed, are introduced, and the methods used to produce the pulse sequence of the laser are specified. Information is given about the doped fiber to be used as a gain medium in the light amplification of the laser to be developed and the light amplification process. In the third part of the thesis, the physically installed active mode locking fiber laser is shown. The materials used are introduced. All components in the laser are selected with polarization protection in order to avoid an unstable state in the laser. All fiber optic connectors have been chosen as APC type with 8 degrees inclination in order to prevent the Fresnel reflection that may be backwards in the laser. The characterization of the erbium doped fiber amplifier has been made and the unwanted ASE noise has been filtered out by using a band-pass filter after the optical amplifier in the laser due to the high ASE noise. The optical modulator has been characterized and it has been seen that the laser we designed works smoothly in the frequency range we want. The laser, which was developed according to the measurements obtained after the characterization of the components, was tested to operate at another frequency and worked without any problems in accordance with the results. In order to make active mode locking, the phase noises of the RF signal source connected outside the laser and the signal source coming from the laser were compared and it was determined that the mode locking laser filtered the phase noise. In the last part of the thesis, the results of the experimental studies carried out with the laser were summarized and suggestions were made for future studies in order to obtain better results.Bu tez çalışmasında 4GHz frekansında düşük faz gürültülü aktif kip kilitlemeli fiber lazer geliştirilmesi amaçlanmıştır. Fiber lazerler ürettiği ışığın zamanla değişimine göre sürekli dalga veya darbeli olmak üzere ikiye ayrılır. Bu çalışmada kısa süreli darbe dizisi üreten lazer tasarlanmıştır. Tezin ilk bölümünde lazerler hakkında genel bilgiler verilmiş olup kullanım alanları, gelişim süreçleri, kullanılan kavramların ve terimlerin tanıtılması hedeflenmiştir. Tezin ikinci bölümünde geliştirilmesi hedeflenen lazer için kullanılan fiber optik teknolojisi ve çalışma prensibi tanıtılmış, lazerin darbe dizisi üretebilmesi için kullanılan metodlar belirtilmiştir. Geliştirilecek lazerde ışığın yükseltilmesinde kazanç ortamı olarak kullanılacak katkılı fiber ve ışığın yükseltilme süreci hakkında bilgi verilmiştir. Tezin üçüncü bölümünde, fiziksel olarak kurulumu gerçekleştirilen aktif kip kilitleme yapan fiber lazer gösterilmiştir. Kullanılan malzemeler tanıtılmıştır. Lazerde kararsız durum oluşmaması için lazerdeki tüm bileşenler polarizasyon korumalı seçilmiştir. Lazerde geriye doğru olabilecek Fresnel yansımasının engellenmesi için tüm fiber optik konnektörler APC tipi 8 derece eğimli olarak seçilmiştir. Erbiyum katkılı fiber yükseltecin karakterizasyonu yapılmış ve yüksek ASE gürültüsünden dolayı lazerde optik yükselteçten sonra bant geçiren filtre kullanılarak istenmeyen ASE gürültüsü filtrelenmiştir. Optik modulatörün karakterizasyonu yapılmış olup tasarladığımız lazerde istediğimiz frekans aralığında sorunsuz çalıştığı görülmüştür. Bileşenlerin karakterizasyonları sonrası elde edilen ölçüere göre geliştirilen lazerin başka frekansta çalışması test edilmiş ve sonuçlarla uyumlu olarak sorunsuz bir şekilde çalışmıştır. Aktif kip kilitleme yapabilmek için lazer dışından bağlanan RF sinyal kaynağı ile lazerden çıkan sinyal kaynağı faz gürültüleri kıyaslanmış ve kip kilitleme yapan lazerin faz gürültüsünü filtrelediği tespit edilmiştir. Tezin son bölümünde kurulan lazerle yapılan deneysel çalışmaların sonuçları özetlenerek daha iyi sonuçların elde edilebilmesine yönelik ileride yapılacak çalışmalar için önerilerde bulunulmuştur

    Investigation of Dimensionality Reduction Methods for Chatter Detection With Svm

    Chatter vibrations significantly affect the quality and efficiency of machining operations. Machine learning algorithms for intelligent chatter detection are viable options when there is sensor data available at the machine tool level. Most machine learning methods require a feature engineering phase where the most valuable data should be extracted and prepared as input for a machine learning classifier. The selection of the proper dimensionality reduction method at this early stage enhances the performance of the classifier. This study aims to investigate the effectiveness of several dimensionality reduction methods when using Support Vector Machine (SVM) as a classifier. Vibration signals collected during slot milling are binary labeled as stable (0) and chatter (1). Signals were reshaped to 0.5second segments and 0.1-second segments. Ten-dimensional (10D) statistical time-domain features extracted from signals were reduced to three-dimensional (3D) feature space with Principal Component Analysis (PCA), t-Distributed Stochastic Neighbor Embedding (t-SNE), and Autoencoder (AE) dimensionality reduction methods. Signals were classified by SVM classification with various training distributions. The effectiveness of different dimensionality reduction techniques and different training distributions were compared for chatter detection. Furthermore, it was observed that dimensionally reduced features were classified quicker and more accurately than statistical time-domain features

    Search for Heavy Particles in the B-Tagged Dijet Mass Distribution With Additional B-Tagged Jets in Proton-Proton Collisions at P Root S=13 Tev With the Atlas Experiment

    A search optimized for new heavy particles decaying to two b-quarks and produced in association with additional b-quarks is reported. The sensitivity is improved by b-tagging at least one lower-pT jet in addition to the two highest-pT jets. The data used in this search correspond to an integrated luminosity of 103 fb-1 collected with a dedicated trijet trigger during the 2017 and 2018 pffisffi = 13 TeV proton-proton collision runs with the ATLAS detector at the LHC. The search looks for resonant peaks in the b-tagged dijet invariant mass spectrum over a smoothly falling background. The background is estimated with an innovative data-driven method based on orthonormal functions. The observed b-tagged dijet invariant mass spectrum is compatible with the background-only hypothesis.     Geçerli Fesih Nedeni Olarak İşçinin Yetersizliği

    The article 18 of the Labour Law states that, for the working places employing 30 or more employees, employer has to rely on a valid reason stemming from employee's competence or misconduct or the requirements of work, working place or enterprise if it aims to terminate a contract of an employee having at least six months of seniority. Valid reason is the term which has a negative effect on the duty of working but not as much intense as just reason. Being a sub-category of the valid reasons, the incompetence of an employee focuses on the termination of the employee working under some standards while carrying out its duty of working. The incompetence of an employee doesn't require the presence of fault, while the misconduct of employee requires the presence of that element. Incompetence also is divided into two sub-categories as physical and occupational incompetence. Physical incompetence is the situation when the employee is under some standards regarding its duty due to agedness or illness. On the other hand, occupational incompetence can be related to lack of enough occupational knowledge or competence. Performance evaluation systems are useful tools for assessing both incompetence categories and constitute a solid ground for termination. At the end of this study, termination under third party pressure and termination on ground of suspicion which are sources of discussion in the doctrine will be tried to be enlightened. While trying to focus on valid reason term and putting forward some suggestions on specific issues, this study benefits from actual jurisprudence and some foreign law systems.İş Kanunu m.18'de, otuz veya daha fazla işçi çalıştıran işyerlerinde en az altı aylık kıdemi olan işçinin belirsiz süreli iş sözleşmesini fesheden işverenin, işçinin yeterliliğinden veya davranışlarından ya da işletmenin, işyerinin veya işin gereklerinden kaynaklanan geçerli bir nedene dayanmak zorunda olduğu düzenlenmektedir. İşçinin davranışlarından kaynaklanan geçerli fesih nedenleri işçinin kusurlu bir davranışını gerekli kılmaktayken, işçinin yetersizliği kaynaklı geçerli fesih nedenleri açısından kusur gerekli bir şart değildir. Bu yetersizlik nedenleri fiziki ve mesleki yetersizlik başlıkları altında toplanabilir. Fiziki yetersizlik, işçinin yaşı veya hastalanması gibi nedenlerle iş görme borcunu yerine getirmesi esnasında yetersiz kalmasını ifade eder. Mesleki yetersizlik ise daha çok işçinin mesleki bilgisi, yetkinliği ve donanımı ile ilişkilendirilebilir. Objektif ölçütler içeren performans değerlendirme sistemleri, her iki yetersizlik halinin ölçülmesi ve feshe dayanak oluşturması açısından büyük öneme sahiptir. Çalışmanın son kısmında ise şüphe feshi ve baskı feshi olarak adlandırılan ve öğretide yoğun tartışmalar yaratan konu başlıkları incelenmekte olup, çalışmanın tamamımda güncel yargı kararları ve yabancı hukuk sistemlerindeki örneklerden yararlanılmaya çalışılmıştır

    Self-Controlling Resource Management Model for Electric Vehicle Fast Charging Stations With Priority Service

    Along with the increasing number of electric vehicles (EVs) on the roads, the demand for public fast charging stations is growing. Long charging times for EVs may lead to congestion in charging stations, queues, and increased waiting times. Different vehicle owners have different sensitivities to waiting time and price. User prioritization is an effective solution for satisfying users with different requests. In this study, we develop a self-controlling resource management model for EV fast-charging stations that provide prioritized service. The model aims to control the delay times of express and normal vehicle classes such that the ratio of their average delay times tracks a target relative delay rate in real time. Each station can determine and change its target relative delay rate according to its policy. The model manages the allocation of resources to user classes in real time through a control mechanism to track the target. The control mechanism uses a simulation model to predict the outcomes of its actions. Numerical studies demonstrate that the model successfully achieves the relative delay target in both steady state and real time under different conditions. The model is applicable to most systems with a dynamically varying workload and a priority-based service goal. (c) 2021 Elsevier Ltd. All rights reserved

    Sensör, amacı ortamdaki değişiklikleri tespit etmek ve bu de?ğişiklikleri bilgisayar gibi elektronik cihazlara göndererek veri elde etmek olan bir cihazdır. Sensörler bir moleküler tanıma sistemi (re?septör), bir fizikokimyasal dönüştürücü ve sinyal işlemcisinden olu?şur. Biyosensör ise, temel olarak enzim, nükleik asit, hücre gibi bir biyo-tanıma materyalinden, elektrokimyasal, optik, kütle tabanlı veya kalorimetrik sinyal dönüştürücüden ve bir sinyal işlemcisinden oluşan, biyolojik ve kimyasal maddelerin belirlenmesinde kullanılan cihazdır. Biyosensörler, biyoreseptörlerin türlerine göre enzimatik biyosen?sörler, protein reseptör bazlı biyosensörler, immünosensörler, DNA?aptamer bazlı biyosensörler ve tam hücre biyosensörleri olarak sınıflandırılabilir. İlk üretilen biyosensörlerden sonra büyük gelişmeler kaydedilmiştir. Biyosensörlerin tasarlanması, üretimi ve optimizasyonu için kimya, fizik, mühendislik, biyoloji, tıp, eczacılık gibi farklı di?siplinlerin katkıları olmuştur. Ayrıca, günümüzde mikroakışkanlar ile nanoteknoloji gibi yeni teknolojilerin ışığında hızlı bir ilerleme kay?dedilmiştir. Bu derlemede biyosensörlerin yapıları, çalışma prensipleri ve biyosensör çeşitleri hakkında bilgiler özetlenecek, biyosensörlerin gıda sektöründeki uygulamaları, gıda toksikolojisi açısından önemi tar?tışılacaktır.     The Efficacy of Biofeedback Therapy in Patients With Solitary Rectal Ulcer and Dyssynergic Defecation

    Background: The treatment of solitary rectal ulcer (SRU) is challenging and controversial; generally, no response to conventional treatments can be obtained, particularly in patients with dyssynergic defecation (DD). We assessed the efficiency of biofeedback therapy (BFT) in patients who did not respond to conservative treatments and had coexistence of SRU and DD. Methods: BFT responses, as well as anorectal manometry and rectoscopy results of 20 patients with the coexistence of SRU and DD, were assessed. Results: Mean age was 32.5 years. Of the patients, 12 were female, and 8 of them were male. An average of 12 sessions of BFT was performed on the patients. Ulcer disappeared in 11 patients (55%) after BFT, and the ulcer size decreased in 3 patients (15%). However, ulcers healed in 9 (90%) of 10 patients whose DD pattern disappeared following BFT, and ulcers healed in only 20% of patients whose DD pattern continued (p = 0.005). The change in anal resting pressure after BFT was significant (p = 0.016). Ulcers were healed in 87.5% (7/8) of the patients whose anal resting pressure decreased after BFT and whose DD disappeared, while ulcers remained untreated in 85.7% of the patients whose anal resting pressure decreased, but the DD pattern continued (p = 0.005). Conclusions: SRU patients with DD are typically unresponsive to medical treatments. Ameliorating anorectal dyssynergia should be the priority of treatment in these patients. BFT is an effective treatment for DD. BFT enhances the healing of ulcers in patients with SRU by restoring coordination of the pelvic floor. © 2022 S. Karger AG, Basel.     Evaluation of Serum Progranulin Levels in Patients With Psoriasis: a Case-Control Study

    Objectives: T cells, keratinocytes, dendritic cells, inflammatory cytokines including tumor necrosis factor alpha are involved in the pathogenesis of psoriasis. The anti-inflammatory effect of progranulin (PGRN) is known by inhibiting the effects of Tumor Necrosis Factor (TNF- alpha). We aimed to evaluate the PGRN levels and the factors affecting PGRN levels in patients with psoriasis. Materials and Methods: The study enrolled 44 patients with psoriasis and 44 age- and sex- matched healthy controls. Serum PGRN levels were measured in all participants and compared between the two groups. PGRN levels were also evaluated in terms of demographic data, duration of disease, age of onset, previous treatments, smoking and drinking habits, PASI scores, and presence of nail and joint involvement in the psoriasis group. Results: Mean serum PGRN levels were significantly higher (10.70 +/- 2.56ng/ml) in the psoriasis group than in the control group (3.16 +/- 1.02ng/ml) (P 0.001). There was no significant relationship between serum PGRN levels and clinical characteristics of psoriasis including the presence of joint and nail involvement, PASI scores, duration of disease, age of onset. Conclusions: In conclusion, psoriasis patients have elevated serum levels of PGRN irrespective of patient and clinical characteristics. To increase knowledge on the effect of PGRN in the pathogenesis of psoriasis can lead to new therapeutic options for the disease
