Effectiveness of the credit-line approach for support of CD4 equipment functionality in northern Uganda
This article examined the effectiveness of the credit-line approach in improving laboratory service delivery by using the functionality of CD4 test equipment as a proxy indicator.Background: Improving laboratory service delivery requires a functioning logistics and supply system. Uganda’s Ministry of Health uses the credit-line approach to provide laboratory supplies including commodities for CD4 test equipment.
Objectives: We examined the effectiveness of the credit-line approach in improving laboratory service delivery by using the functionality of CD4 test equipment as a proxy indicator.
Method: A cross-sectional survey was conducted at 7 level-three health centres (HC IIIs), 18 level-four health centres (HC IVs), and 10 hospitals in 15 districts of mid-northern Uganda, including the Lango (17 facilities) and Acholi sub-regions (18 facilities), between July 2013 and August 2013. Functionality, was determined through self- and interviewer-administered questionnaires. The chi-squared test was used to assess differences in functionality by sub-region, facility type, and equipment type.
Results: A total of 38 CD4 test analysers were assessed. Of these, 26 (68%) were functional. In hospitals, 85% of CD4 analysers were functional, in HC IVs, 67% were functional and in HC IIIs, 43% were functional. The differences did not reach statistical significance. In the Lango sub-region, 72% of analysers were functional; in the Acholi sub-region, 65% were functional. Non-functionality was mainly due to lack of reagents and cartridges, as well as low staffing levels of laboratory technicians with the skills necessary to operate the equipment.
Conclusion: The credit-line approach supported the functionality of CD4 equipment in the surveyed facilities. However, there is a need to address issues of staffing and availability of reagents to enhance the functionality of CD4 equipment and improve patient care, especially at HC IIIs

Lack of Effectiveness of Antiretroviral Therapy in Preventing HIV Infection in Serodiscordant Couples in Uganda: An Observational Study.

We examined the real-world effectiveness of ART as an HIV prevention tool among HIV serodiscordant couples in a programmatic setting in a low-income country.
Methods
We enrolled individuals from HIV serodiscordant couples aged >18 years of age in Jinja,
Uganda from June 2009 – June 2011. In one group of couples the HIV positive partner was receiving ART as they met clinical eligibility criteria (a CD4 cell count >250 cells/ μL or WHO Stage III/IV disease). In the second group the infected partner was not yet ARTeligible. We measured HIV incidence by testing the uninfected partner every three months.
We conducted genetic linkage studies to determine the source of new infections in seroconverting participants.
Results
A total of 586 couples were enrolled of which 249 (42%) of the HIV positive participants were receiving ART at enrollment, and an additional 99 (17%) initiated ART during the study. The median duration of follow-up was 1.5 years. We found 9 new infections among partners of participants who had been receiving ART for at least three months and 8 new infections in partners of participants who had not received ART or received it for less than three months, for incidence rates of 2.09 per 100 person-years (PYRs) and 2.30 per 100 PYRs, respectively. The incidence rate ratio for ART-use was 0.91 (95% confidence interval 0.31-2.70; p=0.999). The hazard ratio for HIV seroconversion associated with ART-use by the positive partner was 1.07 (95% CI 0.41-2.80). A total of 5/7 (71%) of the transmissions on ART and 6/7 (86%) of those not on ART were genetically linked.
Conclusion
Overall HIV incidence was low in comparison to previous studies of serodiscordant couples. However, ART-use was not associated with a reduced risk of HIV transmission in this study.The study was funded by CIHR (Grant numbers MOP-8970, HBF-112308 and MHI-I26391) and CANFAR (Grant number 023 502)
Beyond repair-family and community reintegration after obstetric fistula surgery: study protocol
The objectives for this study are to understand the process of family and community reintegration post fistula surgery and develop a measurement tool to assess long-term success of post-surgical family and community reintegration.Background: Obstetric fistula is a debilitating birth injury that affects an estimated 2–3 million women globally, most in sub-Saharan Africa and Asia. The urinary and/or fecal incontinence associated with fistula affects women physically, psychologically and socioeconomically. Surgical management of fistula is available with clinical success rates ranging from 65–95 %. Previous research on fistula repair outcomes has focused primarily on clinical outcomes without considering the broader goal of successful reintegration into family and community.
The objectives for this study are to understand the process of family and community reintegration post fistula surgery and develop a measurement tool to assess long-term success of post-surgical family and community reintegration.
Methods: This study is an exploratory sequential mixed-methods design including a preliminary qualitative component comprising in-depth interviews and focus group discussions to explore reintegration to family and community after fistula surgery. These results will be used to develop a reintegration tool, and the tool will be validated within a small longitudinal cohort (n = 60) that will follow women for 12 months after obstetric fistula surgery. Medical record abstraction will be conducted for patients managed within the fistula unit. Ethical approval for the study has been granted.
Discussion: This study will provide information regarding the success of family and community reintegration among women returning home after obstetric fistula surgery. The clinical and research community can utilize the standardized measurement tool in future studies of this patient population
Uganda’s Vision 2040 and Human Needs Promotion
The article discusses Uganda’s Vision 2040 launched by President Yoweri Kaguta Museveni in 2013, a thirty-year development master plan for Uganda.In 2013 the President of Uganda Yoweri Kaguta Museveni launched Uganda’s Vision 2040, a thirty-year development master plan which has received both praise and criticism from Ugandans. Although Vision 2040 has received both praise and criticism in almost equal measure, in this article I argue that Vision 2040 does not adequately promote the satisfaction of the human needs of Ugandans, yet the satisfaction of these needs is a prerequisite for achieving genuine development. Using a human needs framework in general, and Doyal and Gough’s intermediate needs in particular, I show that the Vision’s strategies adequately promote the satisfaction of only three intermediate needs of Ugandans, partially promote the satisfaction of seven intermediate needs of Ugandans, and that there is no single strategy to promote the satisfaction of one of the intermediate needs. This is because some of the Vision strategies do not adequately tackle the current and likely future challenges and bottlenecks to the satisfaction of the human needs of Ugandans. This is mainly due to the central role placed on the private capital (sector) in Vision 2040. I argue that in order for Vision 2040 to adequately promote the satisfaction of the human needs of Ugandans, all and not just some of the needs must be adequately satisfied since human needs are interrelated and interdependent. In addition, Vision 2040 should also be in position to satisfy the human needs of all Ugandans and not just some

Accounting Practices and Performance of SMEs: a case of Mukono Central Division
This was submitted as a partial fulfillment of the award of the Master of Business Administration (Finance & Accounting), Uganda Christian University.The study sought to establish the accounting practices utilized by the SMEs, the motivation for and against the utilization of these practices and whether the utilization of accounting practices of interest have performance implications for SMEs with same magnitude. The study adopted a survey research approach to achieve the three study objectives. The study also adopted purposive sampling method to get 90 SMEs who participated in the study. The study utilized correlation mean and standard deviation to address objectives one and two of the study and correlation and regression analysis to address the third objective of the study. The study found that an improvement in keeping and preparing purchases and sales ledgers to be associated with improvement in SME performance. The findings also revealed that most of the SMEs to a moderate extent are motivated to utilize accounting practices to keep track of their business activities. However, the study also discovered that most of the SMEs are constrained from utilizing these practices mostly because of lack of training in accounting. The results revealed that accounting practices have some significant implications on the performance of the SMEs in Mukono central division. This finding was based on the regression and correlation analysis. With these findings the study makes a contribution of providing empirical evidence that has been widely missing to channel the presently passionate discussion surrounding the issues of accounting practices and performance of SMEs in the study context
