Prevalence of Plasmodium falciparum Resistance Markers to Sulfadoxine-Pyrimethamine among Pregnant Women Receiving Intermittent Preventive Treatment for Malaria in Uganda
This study was to assess the prevalence of mutations in Plasmodium falciparum dihydrofolate reductase (Pfdhfr) and dihydropteroate synthase (Pfdhps) genes among pregnant women using sulfadoxine-pyrimethamine (SP) as an intermittent preventive treatment (IPTp).The aim of this study was to assess the prevalence of mutations in Plasmodium falciparum dihydrofolate reductase (Pfdhfr) and dihydropteroate synthase (Pfdhps) genes among pregnant women using sulfadoxine-pyrimethamine (SP) as an intermittent preventive treatment (IPTp). A molecular epidemiological study of P. falciparum parasite resistance markers to SP was conducted from August 2010 to February 2012 in Mukono district in central Uganda. DNA was extracted from 413 P. falciparum-positive samples. Real-time PCR, followed by melting curve analysis, was used to characterize point mutations in the Pfdhfr and Pfdhps genes that are associated with SP resistance. The prevalence of the single-nucleotide mutations in Pfdhfr at codons 51I, 59R, and
108N and in Pfdhps at codons 437G and 540E was high (>98%), reaching 100% fixation after one dose of SP, while the prevalence of 581G was 3.3% at baseline, reaching 12.5% after one dose of SP. At baseline, the prevalence of Pfdhfr and Pfdhps quintuple mutations was 89%, whereas the sextuple mutations (including 581G) were not prevalent (3.9%), reaching 16.7% after one dose of SP. However, the numbers of infections at follow-up visits were small, and hence there was insufficient statistical power to test whether there was a true rise in the prevalence of this allele. The overall high frequency of Pfdhfr and Pfdhps quintuple mutations throughout pregnancy excluded further analyses of possible associations between certain haplotypes and the risk of lower birth weight and anemia. However, women infected with P. falciparum had 1.3-g/dl-lower hemoglobin levels (P_0.001) and delivered babies with a 400-g-lower birth weight (P_0.001) compared to nonparasitemic women. Despite this, 44 women who were P. falciparum positive at baseline became negative after one or two doses of SP (i.e., 50.5%), implying that SP-IPTp still has some efficacy. P. falciparum resistance markers to SP are high in this population, whereas P. falciparum infection was associated with poor birth outcomes

“I am alone and isolated”: a qualitative study of experiences of women living with genital fistula in Uganda
The study was conducted on women with a genital fistula seeking treatment at Mulago Hospital, Uganda. Data were transcribed and analysed using qualitative content analysis.Background: Globally, 2–3 million women are estimated to have a genital fistula, with an annual incidence of 50,000–100,000 women. Affected women remain silent within their communities, and their experiences often go unnoticed. Our objective was to explore the experiences of Ugandan women living with genital fistulas to understand how their lives were affected and how they coped with the condition.
Methods: We conducted 8 focus group discussions (FGDs) with 56 purposively selected women with a genital fistula seeking treatment at Mulago Hospital, Uganda. Data were transcribed and analysed using qualitative content analysis.
Results: Women with a fistula were living a physically changed and challenging life, living socially deprived and isolated, living psychologically stigmatised and depressed, and living marital and sexual lives that were no longer joyful. The women’s experiences were full of life changes and coping strategies, and they used both problem- and emotion-focused coping strategies to deal with the challenges. They devised ways to reduce the smell of urine to reduce the stigma, rejection and isolation. While trying to cope, the women found themselves alone and isolated. Women either isolated themselves or were isolated by society, including by close relatives and their husbands. Their sex lives were no longer enjoyable, and generally, women felt a loss of their marital and sexual rights.
Conclusion: Women with a fistula make adjustments in their lives to cope with the physical, social, psychological and sexual challenges. They use both problem- and emotion-focused coping to minimise their sense of isolation, as well as the rejection and stigma associated with fistula. These findings are essential for counselling patients, families and community members affected by a fistula. In similar contexts, health programmes should go beyond fistula closure and target communities and families to reduce the stigma and isolation faced by women with genital fistula
Optimization of waste collection and disposal in Kampala city
This is a research paper on Optimization of waste collection and disposal in Kampala cityWaste collection and transportation is an important municipal service that involves high expenditures if not handled efficiently. This has hindered waste management in many Sub-Saharan African countries. In this study, Geographic Information System (GIS) tools were used to optimize travel distances, trips and collection time, which leads to maximizing total waste collection, yielding large savings and keeping the environment clean. The study suggested the best waste collection routes, and determined a suitable vehicle fleet and capacity to be used by Kampala Capital City Authority (KCCA), which is the body responsible for waste management in Kampala. The use of the GIS tools led to the reduction in the total number of trips and travel distances, which decreased fuel consumption and vehicle emissions. In addition, the model can be used by the various outsourced private operators, collecting and disposing of solid wastes. Since the current municipal landfill for Kampala city is almost full, the GIS tool was used to identify the optimum location of a new proposed landfill site, based on optimized travel distances. The results of this study can help KCCA to decrease costs of managing wastes and environmental as well as social impact
Effects of public private partnerships on education service delivery in Uganda: A comparative analysis of technical efficiency of secondary schools in Mukono district.
This paper was presented in the Annual SME Conference hosted by Uganda Christian University, Faculty of Business and Administration; October 2016.The paper examined whether secondary schools under Public Private Partnership (PPP) arrangement are more efficient in regard to UCE pass rate and enrollment when compared to schools which are not under partnership.The 2015 Public Private Partnership Act makes provision for policy stability in order to reduce private sector uncertainty on investment, develop institutional capacities for technical analysis, monitoring and management of public private partnerships (PPP Act, 2015). Through the use of a comparative research design, this paper examined whether secondary schools under Public Private Partnership (PPP) arrangement are more efficient in regard to UCE pass rate and enrollment when compared to schools which are not under partnership. The study further examined whether management styles impact efficiency of secondary schools. Using primary data from a sample of 95 secondary schools in Mukono district we find that Non-PPP schools are more efficient than those under PPP in regards to pass rate. However, in respect to enrollment, schools under PPP were found to be more efficient and management was found to be significantly impacting on the efficiency of schools. On this ground a number of recommendations are made to ensure that PPP is of greater benefit to Ugandans

Antecedents and outcomes of employee engagement in soft drink industry in Uganda
A dissertation presented to the College of Higher Degrees and Research - Kampala International University Kampala, Uganda; in partial fulfillment of the requirements for the degree Doctor of Philosophy in Business Management.This study sought to establish the relationship between antecedents and outcomes of employee engagement in the soft drink industry in Uganda. The specific objectives of this study were (1) to ascertain the relationship between engagement antecedents and employee engagement; (2) to determine the relationship between employee engagement and the work outcomes; (3) to establish whether after controlling for the antecedents of engagement, employee engagement will predict unique variance in work outcomes. Adopting a pragmatic philosophy with a sequential mixed methods strategy starting with quantitative cross-sectional survey design (N = 210) followed by key informant interviews with managers the researcher examined the relationship between nine antecedents, employee engagement and four outcomes - job satisfaction, organizational commitment, OCB and turnover intent. Self-administered Questionnaire of four scales were administered to sampled employees of soft drink industry Kampala, Mukono and Buikwe districts of Uganda. Hypotheses were tested through correlation and hierarchical regression analysis techniques. All the nine antecedent variables studied were significantly correlated with employee engagement and employee engagement was significantly correlated with job satisfaction, organizational commitment, OCB and turnover intention. For the job satisfaction model, the hierarchical regression analysis results suggested that employees who experienced a high degree of role clarity, high degree of compensation fairness, and high opportunities for development, also reported being more likely to be satisfied with their place of employment. For the organizational commitment model, the hierarchical regression analysis results shows that three antecedent variables demonstrated a significant relation with organizational commitment, suggesting that employees who experienced a high degree of role clarity, high degree of job security, and high opportunities for development, also reported being more likely to be committed to their organization. For the OCB model, the hierarchical regression analysis results shows that five antecedent variables demonstrated a significant relation with OCB - role clarity, collaboration, job security, compensation fairness, and development. For the turnover intent model, the hierarchical regression analysis found the significant antecedents to be collaboration, social support, reward/recognition, compensation fairness and development. This research has validated and extended the engagement model and social exchange theory in the Uganda context. The only antecedent that is significant in all the four outcome models is development. The recommendation is that for businesses to improve their employee outcomes (job satisfaction, organizational commitment, OCB and reduction of turnover intent) they should develop and nurture strategies for improvement of the significant antecedents and employee engagement by concentrating on employee development, compensation fairness, role clarity, job security, collaboration and organizational support. Amazingly, reward/recognition as well as job design characteristics and material resources were found to be insignificant in the outcome models.
