    Probabilistic learning inference of boundary value problem with uncertainties based on Kullback-Leibler divergence under implicit constraints

    International audienceThis paper deals with a probabilistic learning inference that allows for integrating data (target set) into predictive models for which the target set is constituted of statistical moments of the quantity of interest (QoI) and for which the training set is constituted of a small number of points. In a first part, we present a mathematical analysis of a general methodology that allows for estimating a posterior probability model for a stochastic boundary value problem from a prior probability model of a control parameter. The given targets are statistical moments of the QoI for which the underlying realizations are not available. Under these conditions, the Kullback-Leibler minimum principle is used for estimating the posterior probability measure. The constraints are represented by an implicit nonlinear mapping for which a statistical surrogate model is introduced. The MCMC generator and the necessary numerical elements are given to facilitate the implementation of the methodology. In a second part, an application is presented to illustrate the proposed theory and is also, as such, a contribution to the three-dimensional stochastic homogenization of heterogeneous linear elastic media in the case of a non-separation of the microscale and macroscale. Consequently, the macroscale is another mesoscale at larger scale with random effective/apparent elastic properties. For the construction of the posterior probability measure by using the probabilistic learning inference, in addition to the constraints defined by given statistical moments of the random effective/apparent elasticity tensor, the second-order moment of the random normalized residue of the stochastic partial differential equation has been added as a constraint. This constraint guarantees that the algorithm seeks to bring the statistical moments closer to their targets while preserving a small residue

    Epilepsy and Associated Factors Among Adults Hospitalized for Attempted Suicide

    International audienceBackground: Suicidal behaviors are known to be increased in people with epilepsy compared to the general population. However, few studies have explored the frequency of epilepsy in a large sample of suicide attempters, and scarce data exist about differences and similarities between epileptic attempters (EA) and nonepileptic attempters (NEA). The aim of this study was to explore the frequency of epilepsy as well as psychopathological and somatic factors among suicide attempters.Methods: In this multicenter cross-sectional study, 1,229 adults hospitalized for attempted suicide were included during the period between July 2001 and December 2015. They were assessed with the Mini-International Neuropsychiatric Interview for DSM-IV Axis I mental disorders. Data concerning sociodemographic and somatic diseases, including epilepsy, were collected.Results: Sixty-five patients (5.3%) had epilepsy. EA had significantly fewer mean ± SD years of education compared with NEA (11.2 ± 3.2 vs 12.1 ± 2.9; P = .011) as well as increased rates of head trauma (29.2% for EA vs 16.2% for NEA; P = .007), antiepileptic use (35.4% for EA vs 23.8% for NEA; P = .036), and lifetime substance abuse and/or dependance (49.2% for EA vs 36.1% for NEA; P = .034). Multivariate analyses showed that years of education, head trauma, and panic disorder with agoraphobia predicted belonging to the EA group.Conclusions: These results suggest that epilepsy is overrepresented among suicide attempters. Few psychopathological differences as well as differences in somatic comorbidities except head trauma were observed between EA and NEA in this sample. These results contribute to draw a clinical profile of people with epilepsy in the population of suicide attempters

    Neoadjuvant nivolumab plus ipilimumab and adjuvant nivolumab in patients (pts) with localized microsatellite instability-high (MSI)/mismatch repair deficient (dMMR) oeso-gastric adenocarcinoma (OGA): The GERCOR NEONIPIGA phase II study

    International audienceBackground: In pts with resectable gastric adenocarcinoma or OGA, radical surgery is the only curative option and perioperative chemotherapy seems worthless for those with MSI/dMMR tumors. Methods: We conducted phase II study evaluating the efficacy of neo-adjuvant nivolumab and ipilimumab followed by adjuvant nivolumab in pts with resectable MSI/dMMR, T2-T4 NxM0 tumors. Treatment consisted of nivolumab 240 mg every 2 weeks (6 infusions) and ipilimumab 1 mg/kg every 6 weeks (2 infusions), followed by radical surgery 5 weeks (±1 week) after last injection of nivolumab. Pts with Becker tumor regression grade (TRG) < 3 were treated with adjuvant nivolumab 480 mg every 4 weeks (9 infusions). The primary objective was pathological complete response (pCR) rate. Results: From 10/2019 to 06/2021, a total of 32 pts were enrolled (all had dMMR±MSI-H status; 16 with gastric cancer and 16 with OGA). The median age was 65.5 years (range, 40-80). Initial stage was: usT3Nx = 22, usT2Nx = 4, and usTxNx = 6 (not evaluable by ultrasound endoscopy). At data lock, 32 pts had terminated neo-adjuvant period and 27 (84%) completed all 6 neo-adjuvant cycles. 8 pts (32%) experienced G3/4 adverse events (AEs) during neo-adjuvant treatment. 29 pts underwent surgery; 2 refused surgery and had complete endoscopic response with tumor-free biopsies, 1 had surgery not performed in relation with a metastatic progression. The median delay between last injection and surgery was 35 days (extreme, 25-170). Overall surgical morbidity (54%, n = 14) was related to: anastomotic leakage = 4, pancreatitis = 2, pneumonia = 2, and other = 6. The incidence of post-operative mortality was 3.8% (n = 1) due to cardiovascular AE at post-operative day 3. All the 29 pts who underwent surgery had R0 resection: 17 (59%) had pCR (i.e., TRG 1a as per Becker grade, ypT0N0) and 4 (14%) had TRG 1b as per Beker grade (< 10% residual tumor per tumor bed). 1 pt had TRG 2 (10-50% of residual tumor) and 7 pts had TRG 3 (˃50% of residual tumor) as per Becker grade, per local pathological assessment (central pathological review is awaited). With a median follow-up of 10.9 months, 1 pt had metastatic relapse, 1 pt died without relapse and 30 pts were recurrence/progression-free. Conclusions: Neo-adjuvant therapy with nivolumab and ipilimumab is feasible and is associated with a high pCR rate in pts with MSI/dMMR resectable OGA. Data will be updated for the congress.     Soil-water adaptive management process: the case of On-site Wastewater Treatment Systems in peri-urban areas in France

    International audienceThis paper proposes a contribution to tackle urban sanitation issues giving some hindsight on a place-based science-practice collaborative project. Insights from a French public service in charge of On-site Wastewater Treatment Systems (OWTS) highlighted the need to deepen the understanding of soil infiltration assessments and to clarify their relation to some misunderstandings between actors. The initial aim was to explore how a knowledge based approach can offer an original perspective to historical “septic tanks” and overcome technical and organizational difficulties. In this work we consider OWTS as hybrid infrastructures necessary to collect, transport, treat and dispose of domestic wastewater on the plot where it is generated. We suppose that OWTS adaptive management (sludge not included) offer many opportunities regarding water circulation (ecological function), infrastructure diversity (redundancy), and soil-based decision making (spatial planning). The main objective of this paper is to document the French sociotechnical configuration through the interplay between soil and water actors, OWTS technics and local institutions. The originality is to look at OWTS through the lens systemic perspective of Nature-Based Solutions (NBS). We first contextualize OWTS implementation and planning by presenting technical design studies (water-soil interactions, indicators assessment) and the diversity and capacity of actors. Second, we build on the application of a soil-based methodology in a peri-urban district to provide a place-based outlook on the influence of soil infiltration rates variability in day-to-day management. While the current consideration given to soil infiltration rate in OWTS design studies created a situation of conflicts and contestations between actors, a sociotechnical transition is taking place with new regulations and innovative energy-driven device. Moreover, collective infrastructure for domestic wastewater is currently challenged by densification constraints in metropolitan areas (urban sprawl regulation). As a consequence, resolving binding measures for indicators assessment would support OWTS integration into mainstream urban development. Careful investigation of soil hydraulic functions is thus a steppingstone in the search for an adaptive strategy both at the plot and the neighborhood scales. Not only OWTS are opportunities to support the adaptive management process in the water sector, but OWTS have also the potential to improve soil-based decision-making in urban areas. Finally, valuating OWTS as useful NBS for wastewater management in urban areas supports the evaluation of soil capacities to deliver ecosystem services and contributes to justify land-use changes on a functional knowledge basis

    Le projet « Expérimentation du Véhicule routier Autonome » (EVRA), financé dans la cadre du programme d’investissements d’avenir (PIA), et plus généralement l’effervescence médiatique et industrielle des dernières années autour du véhicule automatisé ont entraîné l’émergence d’un réseau de jeunes chercheurs en France.Le LVMT a eu le plaisir d’accueillir le 31 janvier 2022 un atelier regroupant les doctorant.e.s et postdoctorant.e.s engagé.e.s dans un travail de recherche sur le véhicule automatisé. Il avait pour objectif de formaliser le réseau existant et de discuter des travaux en cours.La séance a consisté en la présentation succincte des travaux de chacun et chacune et s’est poursuivie par une discussion sur la manière de traiter du véhicule automatisé dans un travail de recherche. Les échanges ont ainsi notamment porté sur les méthodes et concepts mobilisés et sur les difficultés rencontrées. Cette synthèse relate de manière structurée cette discussio
