Metabolic Syndrome and Subclinical Hypothyroidism: A Type 2 Diabetes-Dependent Association

    No full text
Introduction. Subclinical hypothyroidism (ScH) is an endocrine alteration that is related to cardiovascular risk factors, including those categorized as components of the Metabolic Syndrome (MS). However, findings in prior reports regarding an association between these alterations are inconsistent. The purpose of this study was to determine the relationship between both entities in adult subjects from Maracaibo City, Venezuela. Materials and Methods. The Maracaibo City Metabolic Syndrome Prevalence Study is a descriptive, cross-sectional study with random and multistage sampling. In this substudy, 391 individuals of both genders were selected and TSH, free T3, and free T4 tests were performed as well as a complete lipid profile, fasting glycaemia, and insulin blood values. ScH was defined according to the National Health and Nutrition Examination Survey (NHANES) criteria: high TSH (≥4.12mUI/L) and normal free T4 (0.9-1,9 ng/dL) in subjects without personal history of thyroid disease. MS components were defined according to IDF/AHA/NHLBI/WHF/IAS/IASO-2009 criteria. A multiple logistic regression analysis was used to assess the relationship between MS components and ScH diagnosis. Results. Of the evaluated population, 10.5% (n=41) was diagnosed with ScH, with a higher prevalence in women (female: 13.6% versus male: 7.7%; χ2=3.56, p=0.05). Likewise, 56.1% (n=23) of the subjects with ScH were diagnosed with MS (χ2=4.85; p=0.03), being hyperglycemia the main associated criterion (χ2=11.7; p=0.001). In multivariable analysis, it was observed that the relationship was exclusive with the presence of type 2 diabetes mellitus (T2DM) OR: 3.22 (1.14-9.14); p=0.03. Conclusion. The relationship between ScH and MS in our population is dependent on the presence of hyperglycemia, specifically T2DM diagnosis, findings that vary from those previously reported in Latin American subjects

    No full text
Background: The aim of this study is to determine the polymorphism of the CYP2C19 gene in the Lebanese population living in Colombia due to the lack of information and the importance of establishing its behavior which can predict a diminished or increased metabolism of medication related to it. Methods and Results: 109 Lebanese volunteers residents of Colombia were included, genotypes for the CYP2C19 were detected by polymerase Chain Reaction-PCR finding that the most frequent allele was *1 (73.4%), followed by *2 (26.6%) the allele *3 was not found in the studied population. Only fast metabolites were found because of the *1/*1 (73.4%) and *1/*2 (26.6%) genotypes, any *1/*3, *2/*2 or *3/*3 genotype were found. Conclusions: The results obtained show a similar behavior with the alleles frequencies of the previous studies made in Colombia, Africa, Europe and other American population. Knowing the genotype of the population is important for the selection and adjustment of the doses of the best medication to give the optimal treatment in medical practice. The pharmacogenetics will reduce the adverse reactions during medical treatment for a better and more accurate clinical approach

Communication Process for the Strengthening of the Care of the Minor in a Pediatric Unit

    No full text
Introduction: The reflection about the elements of communication that health professionals use strengthens the empathic relationship and therefore the actions that seek self-care in patients. Nursing schools must teach how to acquire competences from what should be (moral) to fully consider the emotions and needs of hospitalized children so that they can attend to the call of care. Nursing should tend to social and communication skills to offer a humanized care when children appeal to crying, to silence, to play, to attract attention. Objective: Propose a framework of moral management of communication from the design of a functional communication structure model for the pediatric patient to come to the call of their care in a positive way. Methodology: Nature of qualitative study of descriptive type and symbolic interactionism (perceptual regression). The population was 44 nurses. The data was obtained by interviews analyzed by the STATGRAPHICS XVII Centurión Plus® and Atlas Ti 8.0 ® programs. Conclusion: The practices of nurses respond to the dynamics so that the child comes to the care when they are aware of the purpose of communication in strengthening the moral development of the child. A model of functional communication structure for pediatric patient care adjusted to the axioms of Paul Watzlawick is proposed

Presence of Extrinsic Motivational Factors for Entrepreneurship in the Electronic Governments of the Countries of the Andean Community

    No full text
The purpose of the study is to detect the presence of extrinsic motivational factors for entrepreneurship, from the perspective of the role of communication in the portals of Electronic Governments (EG), of countries that form the Andean Community (CAN), for the support of the aspiring entrepreneur. An evolutionary/incremental methodology was used to reveal the underlying procedural sequence in the initial phases of an enterprise, by means of a group of implicit variables in each dimension/phase chosen a priori. These variables account for: a) the presence of motivational factors, b) relative information of these factors and c) government/citizen communication. The results indicate both a high presence and information of motivational factors on CAN government websites. Nevertheless, the sequence of the process is affected in the ‘communication’ dimension between governmental entities and citizens due to the reduced presence of online services and active tools that facilitate communication in real time. The study concludes that the EG of CAN countries, understood as an integral concept, contribute to the empowerment of citizens and may have a relational impact on the population through the improvement of its entrepreneurial culture

Consecutive renal biopsy in a cohort of patients with lupus nephritis of the Colombian Caribbean

    No full text
Background: Renal biopsy is the gold standard for the diagnosis and classification of lupus nephritis (LN). However, a consecutive biopsy can predict the clinical course and optimize the therapeutic strategies. Objectives: To compare the histopathological findings with clinical responses. Patients and Methods: Thirty patients with active LN were included. Renal biopsies were performed at the time of diagnosis and subsequently under clinical criteria according to consensus of Spanish Society of Nephrology. The response to treatment was defined as complete response, partial responder or non-responder. The histological change in second biopsy towards LN classes I, II or III/IV-C was defined as histological response (HR). Results: In initial renal biopsy, 28 (93%) patients showed proliferative LN; III-A or A/C (n; 7), IV-A or A/C (n: 19) and mixed; III+IV/V (n; 2). LN class V was presented in two cases. The clinical response was; complete response (10%), partial response (20%), and non-response (70%). HR was manifested in 37% and non-histologic response in 63% of patients. Around 33% of patients with complete response/partial response showed active lesions in the consecutive renal biopsy. Conclusions: In Colombian Caribbean, LN is aggressive and refractory to treatment. The consecutive renal biopsy allowed to demonstrate the persistence of the activity of the lesion in almost half of the patients, which may provide additional information to create better response criteria. The consecutive renal biopsy is a tool that allows improving the evaluation of the response to treatment in the LN
