    A critical appraisal of "Lower-extremity resistance training on unstable surfaces improves proxies of muscle strength, power and balance in healthy older adults: a randomised control trial"

    This paper offers a critical appraisal of a 2016 article published in BMC Geriatrics about the effects of lower extremity resistance training on unstable surfaces in healthy, older adults. An introduction to the clinical question which led to the selection of this work is followed by a description of the methods used to select the work. Included in the results section are a summary and appraisal of the article's introduction, methods, results, and discussion. Finally, a discussion is included about the clinical significance, relevance to the clinical question, potential arguments against the intervention, and application to future practice

    Evaluation and treatment of cervical radiculopathy: a case study

    This case report documents the evaluation and evidence-based treatment of a patient who had a presentation consistent with cervical radiculopathy. The patient highlighted in this report is a 30-year-old male diagnosed with bilateral carpal tunnel syndrome. He had numbness and tingling in both hands, especially the right, not limited to median nerve distribution of either hand. The physical therapy evaluation pointed to cervical radiculopathy as the primary cause of the symptoms following patient subjective report of his symptoms, special testing, and passive accessory intervertebral motion testing. Treatment was focused on decreasing intensity of numbness and tingling along with increasing grip and upper body strength. The therapist utilized 5 outcome measures: grip strength, cervical range of motion, manual muscle testing, the Neck Disability Index, and the QuickDASH. Of these outcome measures, the therapist was only able to re-evaluate grip strength and cervical range of motion. After the plan of care, the patient demonstrated increased cervical rotation ROM and grip strength objectively and reported decreased intensity of symptoms and increased time to symptom onset when working and exercising. This case report demonstrates an effective, evidence-based approach to physical therapy evaluation and treatment for a patient with cervical radiculopathy

    A critical appraisal of "Ultrasound treatment for treating the carpal tunnel syndrome: randomized ‘sham' controlled trial"

    This critical appraisal looks at a 1998 article published by the Department of Physical Medicine and Rehabilitation at the University of Vienna. The article is a randomized clinical trial aiming to investigate the clinical efficacy of pulsed ultrasound in the treatment of idiopathic carpal tunnel syndrome. An introduction explaining the purpose behind this critical appraisal is provided before describing the methods of my research. The results section of this paper breaks down my analysis of the article's introduction, methods, results, and discussion. To close out my analysis, the discussion describes the possible implementation of ultrasound intervention on physical therapy patients

    A critical appraisal of "Blood flow restriction training for the shoulder: a case for proximal benefit"

    This paper provides an appraisal of a 2021 article in the American Journal of Sports Medicine about the use of blood flow restriction training in conjunction with low intensity exercise in the upper extremity. The process of forming a clinical question and searching for an article that coincided with the question is presented. The article chosen for appraisal compares BFR with low intensity exercise to low intensity exercise alone through outcome measures of shoulder lean mass, upper extremity lean mass, rotator cuff strength, muscular endurance, and acute EMG amplitude. A summary of the research conducted and an assessment of introduction, methods, results, and discussion of the article is included as well. A discussion of the intervention of BFR and its clinical implications for different patient populations and injuries is offered. A conclusion of the overall use of BFR with low intensity exercise and the future use of the intervention is incorporated into the appraisal

    Seasonal variations in the diet of Myotis velifer using a molecular approach

    Insectivorous bats worldwide play an essential role as predators in ecosystems and serve as pest control for agriculture businesses. Characterizing diets of specific bat species is difficult using conventional methods that cannot capture detailed dietary information. New technologies have progressed to overcome these challenges, such as high-throughput sequencing. In this study, we used metabarcoding of the cytochrome oxidase I mitochondrial gene to analyze fecal samples of Myotis velifer and provide insight into the seasonal variation of diet from a known colony located at Fort Leaton State Historic Site in Presidio, Texas. After filtering sequence reads, we recovered and analyzed 706 unique diet items (UDI) in the diet of M. velifer. We found 483 taxa (species and genus level) belonging to 11 insect orders in fecal samples from 66 bats collected from March to October 2021. Based on our analyses, M. velifer experienced seasonal variation in diet. The orders containing the most unique diet items were Diptera (n= 353), Lepidoptera (n=160), and Blattodea (n=59). We identified important insect crop pests in their diet and substantial consumption of mosquitoes. Bats captured in June experienced the highest diversity of orders in their diet during the peak maternity season. Females were more abundant than males, indicating that the historic fort may serve as an essential maternity colony

    Predicting prosocial tendencies among college students

    Prosocial behaviors inherently benefit oneself, others, and the larger society. Therefore, further investigation as to which factors influence positive social acts is beneficial to better understand what motivates prosocial behavior as well as how it may be promoted. Much of the literature on prosocial behavior targets the construct of empathy. However, little research has been done to differentiate between cognitive and affective empathy, and their relationship with prosocial behaviors. Moreover, proponents of rational compassion, consisting of rational thinking and compassion, contradict the positive assessment of empathy's contributions to positive social acts by proposing that empathy may not be the best predictor of prosocial behavior. Consequently, this project aimed to determine which among the constructs of empathy and compassion in addition to fairness and kindness best predict prosocial tendencies in a variety of contexts. The current study assessed prosocial tendencies, cognitive and affective empathy, rational compassion, fairness, and kindness among college-aged participants. The resulting data were analyzed using a hierarchical regression with empathy, compassion, fairness, and kindness as predictors, and prosocial tendencies as the outcome variable. Results indicated that cognitive empathy and kindness were the strongest predictors of positive social behavior. Sex was also explored as a moderator and indicated that the effect of kindness on prosocial tendencies depended on participant sex. Overall, this study elucidates the factors that influence prosocial behavior

    Boston Marathon athlete with gluteal tendinopathy: a case report

    This case report demonstrates the importance of progressive loading in our older adult athletes who deal with gluteal tendinopathy. This condition most often affects people ages 40 and older. As the demographics of the nation are shifting towards an older population, the prevalence of this condition might also increase. Despite some goals not being met, this case study shows that common goals such as strength do not always translate into improvement in function and quality of life. As a profession, physical therapists need to be prepared to deal with the sport-specific goals that our older athletes have, while managing the challenges of an often-slow rehabilitation process. The patient is a 55 year old Boston Marathon participant. During training he started to experience right lateral hip pain described as achy and sometimes sharp. He performed in the Boston Marathon, however he was unable to finish the race without walking intermittently due to the pain. The patient was seen for 8 sessions, which comprised of gluteal isometric and isotonic strengthening exercises, followed by neuromuscular control exercises. The patient continued to run and increased his volume while receiving treatment. The patient was able to reduce pain from 7/10 to 3/10 while increasing their volume. They improved in pain, and function as seen in two self-reported outcome measures (Lower Extremity Functional Scale, and Hip Outcome Score). The patient decreased in strength according to data collected by a handheld dynamometer, but he achieved his sport-specific goal of decreasing pain while increasing running distance

    A critical appraisal of "Comparative efficacy of routine physical therapy with and without neuromobilization in the treatment of patients with mild to moderate carpal tunnel syndrome"

    This paper offers a critical appraisal of a 2022 article about a manual therapy treatment, neuromobilization, used to treat mild to moderate carpal tunnel syndrome (CTS) patients. The clinical question that resulted in the selection of the article is introduced along with the techniques used to find the article. The article's background information along with the purpose of the critical appraisal of this article is given. A summary of the research article is presented in addition to an appraisal of each of the article's sections; introduction, methods, results, and discussion. A discussion of the clinical significance of the intervention used in reference to current PT practice is provided. It also discussed how the study was relevant to the clinical question presented. Lastly, an argument not in favor of the intervention is given and the application of the results for future CTS patients
