In search of the meaning-making student : Notes on the lack of third-order concepts in the Finnish history curriculum Att söka efter den meningsskapande studenten : Iakttagelser om bristen av third-order begrepp i de finländska läroplanerna för historia
In this article, the current Finnish history curricula for basic education and upper secondary school are analysed from the perspective of what space they give to the students’ personal meaning-making activity in teaching and learning of history. This is a pertinent question, considering that history is often mentioned as a subject where students process the issues of identity, worldview and values. The research question is: Where and how is the students’ processing of their personal relationship with history addressed in the curriculum? The material consists of the history section in the national core curricula for basic education (2014) and upper secondary school (2019) and in the criteria for final assessment in basic education (2020). The method is qualitative theory-driven content analysis. The result is that the students’ personal meaning-making activity is referred to in the general task and the learning objectives of history teaching and learning and in the assessment criteria of history learning but not in the content of history teaching. As the student as a meaning-making agent does not appear where the content of teaching is outlined in concrete terms, there may be only little inspiration for the history teachers to discuss history’s personal meaning in the classroom. This can explain why Finnish students have had difficulty to understand, for example, why questions of historical wrongs can be experienced as transgenerationally relevant and alive.I denna artikel analyseras vilket utrymme ges åt studenters personliga meningsgivande aktivitet i historieundervisning i de finländska läroplanerna för historia som nu är i kraft.
Sustainability-driven regime shifts in Complex Adaptive Systems: The case of animal production and food system
The role of animal production in sustainability transitions has become the subject of a heated societal debate, and a variety of discourses delineating the role that animal production should take in the future prevail. Such discourses can act as attractors that configure the organisation of Complex Adaptive Systems, such as food systems. The evolution of food systems seems to follow a cyclical pattern with occasional regime shifts, which can be driven by the system swapping attractors. In this study, alternative regimes and regime shift dynamics were illustrated for the Finnish food system facing pressures for sustainability transition. Two questions were asked. First, what could be the attractors capable for facilitating a regime shift and from where could they emerge? Second, how the regime shift could happen and what would be the role of animal production in the alternative regimes? Discourse analysis and systems science methodology were used in a participatory foresight process. Five prominent new basins of attraction were identified: ethics, environment, health, national food security and global market. All these manifested a specific conceptualisation of sustainability and resulted in radically different roles for animal production in the food system. Each of the new regimes was accompanied by some new landscape level pressures for change, emphasising the importance of holistic system analysis to avoid unintended or unexpected outcomes of sustainability transitions. Insights for the difficulty of planned regime shifts, use of Multi-Level Perspective (MLP) as an empirical mapping tool, and the utilisation of societal discourses as a source for new attractors were novel elements in the approach of this study
Single Center Experience with a 4-Week 177Lu-PSMA-617 Treatment Interval in Patients with Metastatic Castration-Resistant Prostate Cancer
Background: 177Lu-PSMA-617 is a promising theragnostic treatment for metastatic castration-resistant prostate cancer (mCRPC). However, both the optimal treatment dose and interval in mCRPC and the rate of identification of responders from non-responders among possible treatment candidates are unknown. Methods: 62 men with mCRPC who were treated with 177Lu-PSMA-617 during 1/2017-2/2019 were included in the study. Treatment responses, overall survival (OS) and progression free survival (PFS) were determined. The median follow-up time was 1.4 years (IQR 0.5-2.2). Tumor volume of metastases (MTV), SUVmax and tumor lesion activity (TLA) were quantitated from pre-treatment PSMA PET/CT images together with pre-treatment PSA.Results: An average of three treatment cycles (2-5) were given within a four-week interval. PFS was 4.9 months (2.4-9.6) and OS was 17.2 months (6-26.4). There were no major adverse events reported. A significant PSA response of >50% was found in 58.7% of patients, which was significantly associated with longer OS, p Conclusions: 177Lu-PSMA-617 treatment in four-week intervals was safe and effective. Almost 60% of patients had a significant PSA response, which was associated with better OS. Pre-treatment PSA kinetics or staging PSMA PET/CT-derived parameters were not helpful in identifying treatment responders from non-responders; better biomarkers are needed to aid in patient selection.</p
The Role of Digital Technology within the Business Model of Sustainable Entrepreneurship
The adoption and usage of digital technologies is considered one of the most promising transformations for sustainability, and sustainable entrepreneurship is considered key for providing solutions for the grand social and environmental challenges. The purpose of our study is to shed light on the role of digital technologies in value creation, value delivery, and value capture within the business models of sustainable entrepreneurship. This is important for understanding how digital technologies leverage sustainability. In fact, we have little knowledge on the impact of digital technologies within the business models of sustainable entrepreneurship. We studied the adoption and usage of digital technologies within the business models of 10 born-sustainable new ventures in Mexico applying a comparative case study approach. We found that context matters. Extant research claims that digital technologies promote social inclusion, increase connectivity, and lead to broader stakeholder integration. Despite these claims, this is the most challenging part for entrepreneurs. Our main findings, therefore, emphasize the constraints of the implementation of digital technologies within sustainable business models. This adds to the academic discourse a cross-geographic perspective from the Global South
Higher physical activity levels are related to faecal microbiota diversity and composition in young adults
Increasing physical activity (PA) is recognised as an efficacious approach for preventing and treating cardiometabolic diseases. Recently, the composition of microorganisms living within the gut has been proposed as an important appropriate target for treating these diseases. Whether PA is related to faecal microbiota diversity and composition in humans remains to be ascertained. Thus, we examined the association of the time spent in objectively measured PA with faecal microbiota diversity and composition in young adults. A cross-sectional study enrolled 88 young adults aged 22.0 ± 2.3 years (72.7% women), whose time spent in PA at different intensities was objectively measured with a wrist-worn accelerometer for 7 consecutive days. Faecal microbiota diversity and composition were analysed with hypervariable tag sequencing of the V3–V4 region of the 16S rRNA gene. The mean Euclidean Norm of the raw accelerations Minus One (mg) during waking time, considered as overall PA, and the time spent in vigorous PA were positively correlated with alpha diversity indexes (all rho ≥ 0.23, P ≤ 0.034). Regarding faecal microbiota composition, participants with low time spent in vigorous PA had higher relative abundance of the Gammaproteobacteria class (q = 0.021, FDR = q-value) compared to the participants with high time spent in vigorous PA, and lower relative abundance of the Porphyromonadaceae family (q = 0.031) and the Alistipes genus (q = 0.015) compared to the individuals with high and intermediate time spent in vigorous PA, respectively. Our results suggest that PA, especially of vigorous intensity, is related to faecal microbiota diversity and the Gammaproteobacteria class and Porphyromonadaceae family in young adults
Evaluation of pre- and in-hospital workflows and time intervals with acute ischemic stroke patients
Objective: Stroke is one of the leading causes of death and disability globally. Rapid recanalization therapy for acute ischemic stroke (AIS) patients is critical for improving outcome. While in-hospital time intervals have decreased and treatment methods have improved over the past decade, pre-hospital time intervals have remained unchanged. This study aims to develop a comprehensive understanding of the prognosis of AIS patients based on pre-hospital and in-hospital data. Methods: A retrospective study was conducted in Southwest Finland covering the period of 01/01/2022-31/12/2022. The study included a total of 174 AIS patients who were conveyed by the emergency medical services (EMS) to Turku University Hospital (TUH) and given intravenous thrombolysis (IVT) and/or endovascular treatment (EVT). Pre- and in-hospital care records of AIS patients were analyzed to evaluate workflows, time intervals, and the impacts of these time intervals on patient outcome. Binary logistic regression analysis was utilized to identify predictors of on-scene time (OST) and favorable outcome. Results: The median OST for EMS was 19 minutes. Analysis indicated that the scene of the stroke event being an apartment building, barriers to care such as a locked door, vertigo as a symptom, and EMS taking measurements on-scene were predictive of longer OST. Longer OST was observed to negatively impact patient outcome, along with symptom severity and gender. Using the stroke code as the dispatch code and positive FAST signs were associated with shorter OST. In-hospital median door to needle time (DNT) was 14 minutes in the IVT group and 11 minutes in the IVT+EVT group. EMS prenotification was associated with shorter in-hospital time intervals. Conclusions: The presence of vertigo as a symptom poses challenges to identification by EMS. Pre-hospital OST meets national targets, but EMS workflows could be optimized to reduce OST and thereby positively influence patient outcome. These findings underscore the need for targeted interventions in EMS protocols to improve stroke care outcomes.</p

Potilaiden kokemuksia unta tukevista hoitotyön toiminnoista sairaalan vuodeosastolla
Vuodeosastolla potilaan unta voivat häiritä erilaiset sairaalaympäristöstä johtuvat tekijät kuten hoitotoimenpiteet, osaston äänet, valaistus, lämpötila sekä vuoteen epämukavuus. Potilaiden unen tukeminen on tärkeä osa potilaiden terveyden, hyvinvoinnin ja toipumisen edistämistä vuodeosastohoidon aikana. Tutkimus on osa laajempaa hanketta, jossa kehitettiin vuodeosastoille potilaiden unen tukemisen toimintamalli. Potilaat toivat niukasti esille tunnetiloihin ja psyykkiseen hyvinvointiin liittyviä asioita, eivätkä he myöskään aina olleet maininneet hoitajille näihin liittyvästä tuen tarpeesta. Hoitotyön keinoin voidaan tukea potilaiden nukkumista vuodeosastolla. Tämä edellyttää hoitajilta tietämystä unen edistämisestä ja että he pitävät unen tukemista tärkeänä.</p
Charles Baudelaire's The Swan and the vanishing Cityspace
Altti Kuusamo’s article deals with the theme of loss and melancholy in Charles Baudelaire’s famous poem The Swan (Le Cygne, 1859) taking seriously into account one of the central subject matters, the image of the La Place du Carrousel, which referred to the courtyard between the Louvre and the Tuileries Palace and which had turned out to be a wasteland during Baudelaire’s mature work, in 1850s. In this way my article does not concentrate on the favourite subject of the flaneur in Charles Baudelaire’s poems, not even in the manner which Walter Benjamin made so fashionable. Instead, I’ll draw attention to the mental topography of the Carrousel site in Paris which seemed to be important to Baudelaire and also to his friends during 1840s. The feeling of the deserted quarters is in a way “humming” in the backyard of the Baudelaire’s poem. The motivation for my article lies in pondering why so many literary analysts of the poem, for example Walter Benjamin, Jean Starobinski, Yves Bonnefoy, Jonathan Culler, Stephanie Bundy and Ross Chambers, just to name some central literary figures, have not taken into account those images and metaphors which surround the name Carrousel in the poem. In the poem metaphors which are connected to ancient myths sweep the deserted paving stones of Place du Carrousel and illustrate the situation in which the demolition of the houses and streets of the western side of Louvre had taken place. Also the themes of loss and exile are closely tied to the central imagery of Carrousel close to Louvre.</p
