    Clinicians’ Perspectives and Methodological Application of Fluorescence in situ Hybridization (FISH) to Define Cytogenetic Risk in Multiple Myeloma: An Italian, Real-World, Survey-Based Report From the European Myeloma Network (EMN) Italy

    Background: Fluorescence in situ hybridization (FISH) is the standard technique for the prognostic detection of cytogenetic abnormalities (CA) in multiple myeloma (MM). In Italy, the application of practical guidelines for FISH testing in clinical studies and the degree of standardization of laboratory techniques are largely unknown. Methods: We conducted a survey from April to July 2023 among 70 MM-treating centers associated with the European Myeloma Network Italy and geographically well distributed across Italy. We aimed to record laboratory and clinicians’ perspectives about FISH application in Italy, with a focus on 1q alterations. Results: FISH was widely accessible across the country, with 71% of centers performing it locally, while the remaining centers (predominantly those with <30 newly diagnosed MM cases/year) sent samples to external laboratories. Variability in laboratory techniques, such as CD138+ cell purification and CA detection thresholds, was observed among centers. The centers analyzed del(17p) (100%), t(4;14) (100%), t(14;16) (98%), 1q+ (96%, with 70% distinguishing between gain and amplification), t(11;14) (90%), del(1p32) (88%), del(13q) (68%), and hyperdiploidy (52%). FISH emerged as a crucial prognostic technique, since 94% of centers used the Revised International Staging System (R-ISS) at diagnosis, and 69% implemented the recent R2-ISS. Most centers performed FISH at diagnosis in all patients, while others did not routinely perform FISH in some categories of patients (e.g., aged >80 years). At relapse, 53% of centers routinely repeated FISH testing, 9% did not, while others repeated it selectively. Conclusions: This overview of FISH use in Italy provides a basis for future standardization efforts

    If you blink at me, I’ll blink back. Domestic dogs’ feedback to conspecific visual cues

    Blinking, along with other facial expressions, has been suggested to play a role in dogs' intra- and interspecific communication, however the feedback this signal elicits from the audience is still poorly studied. In this study, we investigated the behavioural and physiological responses of 54 domestic dogs to videos of conspecifics performing blink. Based on existing literature, we hypothesized that dogs would show a higher rate of blinking when exposed to blink than to another facial expression (nose lick) and to an attentive still-looking face (control). Results showed that dogs blinked more during the blink video compared to the nose lick (NL) video, suggesting a mimicry phenomenon and implying a possible role of blinking in dogs' communication. Cardiac analyses showed increased heart rate variability values during the video sessions independently to the type of facial signal projected, suggesting that the stimuli were not perceived as stressful. The present results open the door to future investigation of blink synchronization, as this aspect was not directly addressed in the present study. Future research should also explore the effects of eye blink and NL in modulating intraspecific social interactions

    The effect of natural basil extract on oxidative stability and techno-functional properties of breadsticks

    This study investigates the effects of basil extract (BE) on the rheological, antioxidant, and sensory properties of breadsticks, offering insights into its potential as an alternative to synthetic antioxidants like butylated hydroxytoluene (BHT). Incorporating 0.5 % BE significantly improved dough rheological properties, as evidenced by the increase in storage (G′) and loss (G′′) moduli. The antioxidant capacity of BE powder was 9.47 ± 3.57 mmol/gdw, effectively reducing lipid oxidation during accelerated storage. The phenolic compound characterization of BE revealed the presence of antioxidant compounds. Samples with 0.5 % and 1 % BE exhibited lower peroxide values (15.18 ± 0.03 and 13.42 ± 1.79 meq/kg, respectively) compared to BHT, which displayed instability under UV exposure. The addition of BE also reduced moisture content while maintaining water activity levels comparable to those observed in the control and BHT samples. The sensory evaluation highlighted the superior acceptability of 1 % BE, associated with its lower oxidation intensity. However, BE altered the breadstick color, particularly at higher concentrations, indicating potential challenges in consumer appeal. Overall, 0.5 % BE proved to be effective in extending the shelf life of breadsticks, offering a clean-label alternative to synthetic antioxidants. Addressing color stability could further optimize its application in bakery products

    Il tema delle Case della Salute, poi ridenominate Case della Comunità attraverso gli strumenti normativi a supporto del PNRR, ha visto in questi ultimi anni molti contributi tesi alla definizione degli aspetti organizzativi, prestazionali, gestionali di natura assistenziale sia medica che sociale. In questo processo di caratterizzazione assai meno è stato prodotto riguardo al ruolo dell’architettura, da intendersi come espressione della qualità funzionale e formale degli spazi per operatori e fruitori in senso comunitario,all’interno degli edifici e dei luoghi pubblici abitati della città e del territorio, nei quartieri, a diretto contatto con la cittadinanza. In questo senso pareva necessario sottoporre al tecnico professionista ed al committente pubblico alcuni modelli di indirizzo per la progettazione dello spazio architettonico di questi presidi socio-sanitari che ci piace meglio definire Centri di Salute Comunitaria a sottolinearne la natura pubblica e collettiva dei servizi offerti, dove le pratiche per la salute e la comunità possano realizzare una piena oltre che virtuosa reciprocità anche grazie all’apporto dell’architettura

    Cow’s Milk Allergy in Breastfed Infants: What We Need to Know About Mechanisms, Management, and Maternal Role

    Cow’s milk allergy is one of the most prevalent food allergies in infancy. Exclusive breastfeeding is the recommended source of nutrition for the first six months of life, but some infants may develop cow’s milk allergy due to the transfer of milk proteins such as β-lactoglobulin through breast milk. There are still many uncertainties about cow’s milk allergy in breastfed babies. The purpose of this review is to summarize the latest findings mainly focused on immunological mechanisms and challenges in diagnosis. We pointed out which clinical signs in breastfed infants are associated with immediate IgE responses and which are linked to delayed non-IgE mechanisms or mixed IgE/non-IgE-mediated reactions. Since standard IgE tests are often useless in non-IgE cases, diagnosis typically involves dietary elimination and cow’s milk challenge. This study addresses the controversial topic of maternal elimination diets, assessing the nutritional risks to both mothers and infants in relation to their possible benefits. It has also been discussed whether the microbiota signature could be a potential factor in both tolerance development and the risk of cow’s milk allergy in early life. Overall, accurate diagnosis and personalized treatment plans are vital to prevent overdiagnosis and ensure proper growth while maintaining the practice of breastfeeding
