Long-Term Health Consequences of SARS-CoV-2- Trigger for Still Disease in Adults
Still disease is a rare inflammatory disorder often triggered by infections or malignancies, with SARS‐CoV‐2 as a possible factor. This paper explores a 56‐year‐old patient who developed adult‐onset Still disease after COVID‐19. Symptoms included fever, arthralgia, rash, hepatomegaly, and joint swelling. Diagnosis followed Yamaguchi criteria and treatment with glucocorticoids and immunosuppressants led to recovery. The paper discusses similarities between Still disease and severe COVID‐19 that suggest shared inflammatory mechanisms, particularly IL‐1 activation. Early diagnosis and treatment are vital. Further research is needed to clarify the link between Still disease and SARS‐CoV‐2 and improve treatment strategies
Funkcionalne naprave - baza ispitanika
Cilj ove studije bio je procijeniti utjecaj terapije Twin Block aparatom na karakteristike skeleta, dentoalveolarnih i mekih tkiva lica. Studija je obuhvatila 18 sudionika sa skeletnom malokluzijom klase II koji su liječeni Twin Block aparatom. Lateralni cefalogrami i 3D skenovi lica analizirani su prije i nakon terapije za svakog sudionika. Rezultati ovisne t-testa pokazali su značajno povećanje položaja donjih sjekutića u odnosu na apikalnu bazu mandibule (p < 0,001), nagiba donjih sjekutića u odnosu na bazu mandibule (p = 0,021), SNB (p = 0,005), Ls:E (p = 0,040) i efektivne duljine mandibule (p < 0,001) te značajno smanjenje ANB (p = 0,003), witsa (p = 0,001), ANPG-a (p = 0,001), predgriza (p = 0,001) i položaja gornjih sjekutića u odnosu na apikalnu bazu maksile. Terapija dvostrukim blokom uzrokovala je promjene u karakteristikama skeleta i mekog tkiva. Povećanje kuta SNB-a i smanjenje vrijednosti ANB-a i witsa smanjilo je skeletnu diskrepanciju. Smanjenje ANPG-a ukazalo je na poboljšanje skeletnog profila. Osim toga, povećanje efektivne duljine mandibule i udaljenosti između gornje usne i E-linije pridonijelo je poboljšanim estetskim karakteristikama profila mekog tkiva.Mjerenje latero-lateralnih cefalograma.

Double Trouble on the Lower Leg—Unique Human Coinfection with Echinococcus granulosus and Echinococcus multilocularis Without Liver Involvement
The tapeworms Echinococcus granulosus and Echinococcus multilocularis cause two different clinical manifestations in humans: cystic echinococcosis (CE) and alveolar echinococcosis (AE), respectively. Both forms of echinococcosis manifest primarily in the liver, while other organs or tissues are less frequently affected. Simultaneous occurrence of CE and AE is extremely rare, and all previously reported patients exhibited affected livers, while simultaneous infection without liver involvement has not yet been described. Herein, we present an exclusively extrahepatic E. granulosus and E. multilocularis coinfection localized between the calf muscles of a patient. Due to progressive painful local swelling, an abscess was suspected, but there was no improvement after the administration of multiple courses of antibiotics. When imaging diagnostics suggested a parasitic origin of the two identified cystic lesions, positive serology for both species indicated a dual infection. Albendazole therapy was started, and extensive surgical excision was performed. Both species were confirmed using PCR and sequencing from intraoperative samples. The current case shows that coinfection without liver involvement can occur, even in patients from low-incidence regions, which should be considered in the differential diagnosis of patients with unusual clinical presentation
The Influence of Environmental Requirements on the Design and Production Costs of Distribution Transformers
EU želi biti svjetski predvodnik u borbi protiv klimatskih promjena. Ta politika, između
ostalog, počiva na povećanju energetske učinkovitosti sustava, ali i na povećanju
održivosti integriranih elemenata u vidu trajnosti, potencijalom popravljivosti i reciklaže
te minimiziranjem utjecaja na okoliš.
Zahtjevi od strane kupaca na ekološke performanse transformatora su sve veći. Kupci, s
jedne strane pritisnuti ciljevima Europskog zelenog plana, a s druge strane sve većim
priljevom novih, potencijalnih dobavljača transformatora, zahtjeve su počeli formirati
kroz opsežne i složene specifikacije. Bodovanje na natječajima ne donose pretežno,
između ostalog, stavke tehničkog rješenja i cijene, već i rješenja koja opisuju ekološke
performanse transformatora, njegovu održivost, ulogu u kružnoj ekonomiji,
reciklabilnost, itd. Ovaj rad prikazuje primjer na kojem se obrađuju takvi i slični zahtjevi
kupca.
Transformatori kao elementi elektroenergetskog sustava imaju veliku ulogu u
vrednovanju njegove efikasnosti i održivosti. Kako je primjena transformatora zbog
potrebe tržišta sve veća, postaje jasno kako se svi uključeni akteri u lancu (kupci,
proizvođači transformatora, proizvođači komponenti i sirovina) moraju osvijestiti o
ekološkim performansama, ali i o posljedicama koje zahtjevi na te performanse mogu
prouzročiti.
U radu su predstavljene glavne komponente distributivnog transformatora. Analiziran je
njihov razvoj i utjecaj na ukupne ekološke performanse transformatora, s posebnim
naglaskom na GWP (engl. Global warming potential) vrijednost, tj. mjeru koja se koristi
za usporedbu učinaka različitih stakleničkih plinova na globalno zagrijavanje.
Analizirane su promjene dimenzija i masa distributivnog transformatora u posljednjih 10
godina. Za analizu izabrano je pet karakterističnih transformatora s obzirom na njihovu
snagu. Prikazani su detalji izvedbe kroz različite razine dopuštenih gubitaka, kao i utjecaj
na trošak materijala.
Kako je već navedeno, utvrđivanje utjecaja distributivnih transformatora na okoliš postaje
sve češći zahtjev kupca. Metodologije koje daju uvid u utjecaj proizvoda ili procesa na
okoliš su definirane i opisane u procjeni životnog ciklusa (engl. Life cycle assessment - LCA),
tj. smjernicama norme ISO 14040. Primjena metodologije je prikazana na primjeru
jednog transformatora.
Izbor materijala namota (bakar ili aluminij) ima utjecaj na dimenzije i masu
transformatora, te posljedično, na trošak materijala i ekološke performanse. Analizirane
su mogućnosti optimiranja elemenata konstrukcije i dodatnog smanjenja GWP vrijednosti
kotla transformatora.The EU wants to be a world leader in the fight against climate change. This policy is
based, among other things, on increasing the energy efficiency of the system, but also on
increasing the sustainability of integrated elements in terms of durability, repairability
and recycling potential, and minimizing the environmental impact.
Customers' demands for the environmental performance of transformers are increasing.
Customers, pressured on the one hand by the goals of the European Green Deal, and on
the other hand by the increasing influx of new, potential suppliers of transformers, have
started to formulate their requirements through extensive and complex specifications. The
scoring in tenders is not mainly based on, among other things, the items of the technical
solution and price, but also on solutions that describe the environmental efficiency of the
transformer, its sustainability, environmental impact, role in the circular economy,
recyclability, etc. This paper will present an example on which these and similar customer
requirements are addressed.
Transformers as elements of the power system play a major role in assessing its efficiency
and sustainability. As the use of transformers increases due to market needs, it becomes
clear that all actors involved in the chain (customers, transformer manufacturers,
suppliers of components and raw materials) must be aware of environmental
performance, but also the consequences that the requirements for this performance may
cause.
The main components of the distribution transformer are presented. Their development
and impact on the overall environmental efficiency of the transformer are analyzed, with
special emphasis on the GWP value (global warming potential), i.e. a measure used to
compare the effects of different greenhouse gases on global warming.
Changes in the dimensions and mass of the distribution transformer in the last 10 years
are analyzed. Five characteristic transformers were selected for analysis with respect to
their power. Design details are presented through different levels of allowable losses, as
well as the impact on material costs.
As already mentioned, determining the environmental impact of distribution transformers
is becoming an increasingly common request from customers. Methodologies that
provide insight into the environmental impact of a product or process are defined and
described in the Life Cycle Assessment (LCA), i.e. the guidelines of the ISO 14040
standard. The application of the methodology is shown on the example of one
transformer.
The choice of winding material (copper or aluminum) affects the dimensions and weight
of the transformer, and consequently, the cost of the material and the environmental
impact. The possibilities of optimizing the structural elements and further reducing the
GWP value of the transformer tank were analyzed
The Impact of Weighting Factors on Dual-Energy Computed Tomography Image Quality in Non-Contrast Head Examinations: Phantom and Patient Study
Background: This study aims to evaluate the impact of various weighting factors (WFs) on the quality of weighted average (WA) dual-energy computed tomography (DECT) non-contrast brain images and to determine the optimal WF value. Because they simulate standard CT images, 0.4-WA reconstructions are routinely used. Methods: In the initial phase of the research, quantitative and qualitative analyses of WA DECT images of an anthropomorphic head phantom, utilizing WFs ranging from 0 to 1 in 0.1 increments, were conducted. Based on the phantom study findings, WFs of 0.4, 0.6, and 0.8 were chosen for patient analyses, which were identically carried out on 85 patients who underwent non-contrast head DECT. Three radiologists performed subjective phantom and patient analyses. Results: Quantitative phantom image analysis revealed the best gray-to-white matter contrast-to-noise ratio (CNR) at the highest WFs and minimal noise artifacts at the lowest WF values. However, the WA reconstructions were deemed non-diagnostic by all three readers. Two readers found 0.6-WA patient reconstructions significantly superior to 0.4-WA images (p < 0.001), while reader 1 found them to be equally good (p = 0.871). All readers agreed that 0.8-WA images exhibited the lowest image quality. Conclusions: In conclusion, 0.6-WA reconstructions demonstrated superior image quality over 0.4-WA and are recommended for routine non-contrast brain DECT

Ovaj rad istražuje pomorsko tržište rasutog tereta, analizirajući ključne ekonomske, tehničke i
operativne aspekte s posebnim naglaskom na 2023. godinu. U radu su objašnjeni temelji
sezonalnosti i cikličnosti tržišta, karakteristike brodova te globalni tokovi sirovina poput
ugljena, željezne rude i žitarica. Posebna pažnja posvećena je poslovanju vodećih brodarskih
kompanija i njihovoj prilagodbi izazovima poput ekoloških standarda i volatilnosti tržišta.

operational aspects with a particular focus on the year 2023. The paper explains the basis of
seasonality and cyclicality of the market, the characteristics of ships and the global flows of
raw materials such as coal, iron ore and grain. Special attention is paid to the operations of
leading shipping companies and their adaptation to challenges such as environmental
standards and market volatility. In conclusion, the paper emphasizes the role of innovation,
sustainability and strategic planning for the future development of the sector and its resistance
to global changes
Chronic kidney disease and menopausal health: An EMAS clinical guide
Kidney diseases are related to the aging process. Ovarian senescence and the loss of estrogen's renoprotective effects are directly associated with a decline in renal function and indirectly with an accumulation of cardiometabolic risk factors. The latter can predispose to the development of chronic kidney disease (CKD). Conversely, CKD diagnosed during reproductive life adversely affects ovarian function. Aim: To set out an individualized approach to menopause management in women with CKD. Materials and methods: Literature review and consensus of expert opinion. Summary recommendations: Menopause hormone therapy can be given to women with CKD. The regimen should be selected on the basis of patient preference and the individual's cardiovascular risk. The dose of hormonal and non-hormonal preparations should be adjusted in accordance with the patient's creatinine clearance. The management of a postmenopausal woman with CKD should focus on lifestyle advice as well as regular monitoring of the main cardiovascular risk factors and evaluation of bone mineral density. Tailored multidisciplinary advice should be given to women with comorbidities such as diabetes, dyslipidemia, and hypertension. Management of osteoporosis should be based on the severity of the CKD
Recent Case-Law of the Court of Justice of the European Union on Liability for Damages Due to Violation of the Right to Protection of Personal Data
U radu se prikazuju recentne odluke Suda Europske unije o tumačenju čl. 82. Uredbe o zaštiti podataka kojim je propisana odgovornost za štetu. Neposredna primjena Uredbe o zaštiti podataka pred sudovima država članica otvara brojna pitanja o usklađenosti pravila nacionalnih prava s pravilima Uredbe, posebno onih koja se odnose na pojam neimovinske štete i utvrđivanje njezine visine, vrstu i pretpostavke odgovornosti za štetu te teret dokaza u postupcima radi njezine naknade. Sud je Europske unije tijekom posljednje godine dana sustav privatne provedbe Uredbe o zaštiti podataka svojim tumačenjima značajno upotpunio, ali su brojna pitanja i dalje otvorena. Među njima se svakako ističu ona koja su posljedice činjenice da je pojam neimovinske štete autonoman i da mora imati ujednačeno tumačenje u cijeloj Europskoj uniji. Budući da Uredba o zaštiti podataka ne propisuje kriterije za utvrđivanje visine naknade za tu štetu niti daje smjernice nacionalnim zakonodavcima i sudovima u tom pogledu, upitno je koliko su uz trenutačno važeće uređenje ostvarivi njezini ciljevi.This paper presents the most recent decisions of the Court of Justice of the European Union on the interpretation of specific terms in Article 82 of the General Data Protection Regulation, which provides for liability for damages. The direct application of the Regulation before the Member States’ courts raises numerous questions about the compatibility of national law provisions with those of the Regulation. This includes issues related to non-material damage and the determination of its amount, the nature and conditions of liability for damage and the burden of proof in compensation proceedings for such damage. In the previous year, the Court’s interpretations have improved the private enforcement system of the Regulation to a certain extent, but many questions remain unanswered. Among them, those that stand out are certainly the ones resulting from the fact that the concept of nonmaterial damage is autonomous and must have a uniform interpretation throughout the European Union. Since the Regulation neither prescribes criteria for determining the amount of compensation for such damage nor it provides guidance to national legislators and courts in this regard, it is uncertain to what extent its objectives can be achieved with the current regulation
REMOTE WORK AND WORK AT A DESIGNATED SEPARATE WORKPLACE
Rad na daljinu predstavlja sve prisutniji oblik rada u suvremenom društvu, osobito potaknut tehnološkim razvojem i globalnim krizama poput pandemije Covid-19. Ovaj diplomski rad analizira rad na daljinu i rad na izdvojenom mjestu rada kao fleksibilne oblike zapošljavanja, s naglaskom na njihovu pravnu regulaciju, prednosti i izazove. Prikazani su ključni elementi ugovora o radu u tim slučajevima, prava i obveze poslodavaca i radnika, kao i aktualni pravni okviri u hrvatskom, europskom i međunarodnom zakonodavstvu. Posebna pažnja posvećena je pravu na isključivanje te potrebi za dodatnim normiranjem određenih aspekata kako bi se osigurala zaštita prava radnika. Rad donosi zaključak da rad na daljinu može biti koristan i održiv model rada, pod uvjetom da se provodi uz jasna pravila, odgovornu primjenu i kontinuirano unaprjeđenje zakonodavnog okvira.Remote work is becoming an increasingly widespread form of employment in modern society, particularly driven by technological development and global crises such as the Covid-19 pandemic. This thesis examines remote work and work at a designated separate workplace as flexible forms of employment, focusing on their legal regulation, benefits and challenges. It provides an overview of the key contractual elements in such employment arrangements, the rights and obligations of employers and employees, and the current legal frameworks in Croatian, European and international labour law. Special attention is given to the right to disconnect and the need for further legal regulation to ensure the protection of workers' rights. The thesis concludes that remote work can be a useful and sustainable model if implemented with clear rules, responsible practices and continuous legal development
