    Establishing Epidemic and Intensity Thresholds After a Major Change in Respiratory Virus Surveillance in Spain in 2020

We retrospectively extracted diagnostic codes from primary healthcare databases between Weeks 30/2010 and 39/2021, using methods similar to SiVIRA. Epidemic and intensity thresholds were estimated using the moving epidemic method (MEM). The epidemic threshold for the 2024-2025 season was 400 cases per 100,000 persons for ARI and 36 for ILI, and medium, high, and very high intensity thresholds were 953, 1079, and 1139 for ARI and 167, 218, and 246 for ILI, estimating low intensity. Reconstructing the historical series using electronic health records was possible and allowed the estimation of thresholds. Experience gained will guide the choice of MEM parameters in the future

    Consum de drogues; Consum d’alcohol; EstadístiquesDrugs consumption; Alcohol consumption; StatisticsConsumo de drogas; Consumo de alcohol; EstadísticasEl Programa d’enquestes domiciliàries sobre alcohol i altres drogues a Espanya (EDADES) és un programa biennal d’enquestes domiciliàries sobre el consum de drogues, promogut per la Delegació del Govern per al Pla Nacional sobre Drogues (DGPNSD) en col·laboració amb les comunitats autònomes, que es va iniciar l’any 1995 i que, actualment, té resultats de quinze enquestes (dels anys 1995, 1997, 1999, 2001, 2003, 2005, 2007, 2009, 2011, 2013, 2015, 2017, 2019, 2022 i 2024). Es disposa, així, d’una sèrie temporal cada vegada més dilatada que permet analitzar l’evolució de les prevalences del consum d’alcohol, tabac, hipnosedants i drogues de comerç il·legal, i també sobre altres aspectes relacionats amb el consum de drogues, com els perfils dels consumidors, la percepció de risc que té la població davant de determinades conductes de consum, la percepció de disponibilitat de les substàncies psicoactives il·legals, la visibilitat dels problemes associats amb el consum de drogues i les mesures que la població considera més efectives per resoldre’l. Addicionalment, les últimes edicions d’aquestes enquestes han anat incorporant alguns continguts relacionats amb les addiccions sense substància, especialment el joc amb diners i l’ús compulsiu d’internet

    Role of VATS-US in identifying and characterizing pulmonary nodules: a narrative review

    Echogenic pattern; Nodule identification; Pulmonary nodulesPatró ecogènic; Identificació de nòduls; Nòduls pulmonarsPatrón ecogénico; Identificación de nódulos; Nódulos pulmonaresThe aim of this study was to show the efficacy described in the scientific literature of lung ultrasound (LU) during video-assisted thoracic surgery (VATS) to determine the location and characterization of pulmonary nodules. The results showed that intraoperative LU is especially useful in localizing lung lesions by VATS with a sensitivity close to 100%. It was also shown to be useful in planning very precise lung resections, reducing the resection of healthy lung tissue. Although general criteria could be established for the degree of benignity/malignancy of lung lesions based on the ultrasound patterns obtained, the great variability observed in these patterns does not offer sufficient guarantees to make a reliable diagnosis. In this sense, the application of Doppler ultrasound or the utilization of Intraoperative Contrast-Enhanced Ultrasound (Io-CEUS) in a completely collapsed lung can be advantageous for the accurate localization and characterization of pulmonary lesions. Even so, at present, definitive confirmation of the nature of lung lesions usually requires biopsy and histopathological study. The development of artificial intelligence algorithms that integrate the results of histological analyses with various types of ultrasonographic patterns (based on pulmonary aeration obtained, as well as Doppler and Io-CEUS records) will likely represent the future of ultrasonographic differentiation of these lesions

    Family Shopping Basket Intervention: A Strategy to Reduce Obesity in Prepubertal Children

Family-based interventions are demonstrating positive results, especially in prepubertal children. The aim of our study was to evaluate the effectiveness of a family grocery basket intervention for the treatment of childhood obesity in a Spanish primary care office. Methods: A randomized controlled trial comparing a family grocery basket intervention through the analysis of unhealthy products included in the grocery receipts that families bring to the primary care office, in comparison with the usual interventions. Results: Ninety-one children participated in the study (intervention group: n = 60, control group: n = 31). After one year of follow-up, a relevant weight loss with a decrease of ≥0.5 SDS in the z-IMC was obtained in 60.6% of the total sample. In the intervention group, there was a significant decrease in the number of unhealthy products in the family grocery basket and a lower percentage of hypertension and severe obesity than in the control group. In families in which there was a significant decrease in the consumption of unhealthy products, a higher percentage of weight loss was observed. Conclusions: Intervention in the family grocery basket through receipts is an original, simple and effective tool for family-based treatment in childhood obesity

Unfortunately, radiotherapy can be associated with late toxicities in survivors. This systematic review provides an overview of the quality of evidence related to late effects following upper abdominal radiotherapy in survivors of Wilms tumor and neuroblastoma. Method A systematic search was conducted using the PubMed database to address clinical questions regarding late effects on the liver, pancreas, vessels, kidney(s), musculoskeletal structures, second malignancy induction, spleen, and intestines. The Quality In Prognosis Studies (QUIPS) tool was utilized to assess the risk of bias in individual studies. The Grading of Recommendations Assessment, Development, and Evaluation (GRADE) tool was applied to evaluate the overall quality of evidence concerning late toxicity risks after upper abdominal radiotherapy. Results Out of 3080 records, 55 studies were included. We identified high levels of evidence for the prevalence of metabolic syndrome, diabetes, and functional asplenia as late toxicities following upper abdominal radiotherapy. A moderate level of evidence was found for an increased risk of secondary malignant neoplasms (renal cell carcinomas and colorectal carcinomas) and chronic kidney disease. Very low evidence for prevalence of scoliosis/spinal deformity and aortic growth abnormalities was observed. Conclusion This systematic review highlights the different levels of evidence of a spectrum of late toxicities, associated with upper abdominal radiotherapy in survivors from a Wilms tumor and neuroblastoma. Within the multi-disciplinary approach, modern radiotherapy has the potential to limit these late effects.This study was supported by the Children Cancer-free Foundation (KiKa) and the Maarten van der Weijden Foundation (KiKa grant number 463)
