Modeling moist air effects and shape memory alloys in elastocaloric devices
Elastocaloric systems, which leverage shape memory alloys (SMAs) to achieve
efficient, eco-friendly thermal management, offer a promising alternative to
conventional air conditioning technologies. This study presents a simulationbased approach to modeling the effects of moist air and condensation
phenomena within these systems. Here, we examine key factors affecting
SMA performance, including mechanical behavior and the material’s latent
heat characteristics. Moist air, particularly under conditions where temperatures
fall below the dew point, introduces condensation and latent heat release,
which can influence thermal output in elastocaloric systems. This work
develops a comprehensive model that couples the thermomechanical behavior
of SMAs with the thermodynamics of moist air, incorporating condensation
heat transfer, mass balance, and moisture transport. Through simulations,
we quantify the impact of condensation on device level and assess how
ambient moisture conditions affect overall heat exchange. The findings enhance
our understanding of elastocaloric system performance under real-world
conditions, contributing to the advancement of sustainable and modern
technologies
Monitoring of Layered Thermoplastic Composites Using Shape Memory Alloys as Integrated Sensors for Multifunctional Lightweight Structures
Since lightweight design and construction safety is a crucial element in different sectors of
industry, the use of SMA wires in composites could improve the monitoring and adjustment
of mechanical properties starting from the product development process through to field
use. This work shows how embedded SMA wires can lead to a better understanding of
applied forces to a composite structure made of GFRP laminates. To achieve this, different
methods will be addressed. Besides mechanical testing of the GFRP-samples with embed ded SMA wires, NDT-methods like active thermography, high-frequency ultrasonic testing,
and computer tomography are used to detect the SMA wires, whereby thermography and
computer tomography are best suited. In this study, the location and the amount of the
applied force on GFRP composites with embedded SMA wires could be characterized
with relative resistance changes. It is shown that SMA wires with a diameter of 250 µm
are preferred to wires with a diameter of 100 µm due to production process and better
performance under load (4N force plateau for 100 µm in contrast to 25N force plateau for
250 µm wires). Furthermore, Young’s modulus of the GFRP composites with embedded
SMA wires was measured and is similar for various samples with 30.8 GPa on average
Mikronährstoffkonzentrationen im Zusammenhang mit anthropometrischen Messungen, Anämie und sozioökonomischen Faktoren bei syrischen Kindern im Vorschulalter
Besonders in Entwicklungsländern und Ländern in Krisengebieten sind die Prävalenzen von
Anämie und Unterernährung und daraus folgend einer gestörten Wachstumsentwicklung bei
Kindern besonders hoch. Wachstumsverzögerung, Untergewicht und Anämie haben nicht nur
gesundheitliche Auswirkungen auf die betroffenen Individuen, sondern als Folge erhöhter
Prävalenzen dieser Gesundheitszustände kann es auch zu negativen Auswirkungen auf die
sozioökonomische Entwicklung einer Gesellschaft kommen. Mehrere Studien deuten bereits
daraufhin, dass die Nährstoffe des C1-Metabolismus, Folat, Vitamin B12 und deren Marker,
für das fetale Wachstum und die sich anschließende kindliche Entwicklung eine wichtige Rolle
spielen. Ein Mangel dieser Mikronährstoffe führt zu einer Störung des Stoffwechsels und
scheint das longitudinale Wachstum und die kognitive Entwicklung eines Kindes negativ zu
beeinflussen. Ebenso spielen die Nährstoffe des C1-Stoffwechsels auch eine entscheidende
Rolle für die Entstehung einer Anämie. Sowohl eine beeinträchtigte Wachstumsentwicklung
bei Kindern als auch Anämie stellen Entwicklungsländer und Länder mit Krisengebieten vor
große gesundheitliche Probleme. Die Bekämpfung von Anämie und eingeschränktem
Wachstum bei Kindern ist in vielen Teilen der Welt bereits Ziel von Studien zur
Bevölkerungsintervention.
Um Faktoren zu ermitteln, die mit dem Wachstum und Anämie bei syrischen Kindern im Alter
zwischen 1 bis 5 Jahren in Zusammenhang stehen, führten wir eine Querschnittsstudie an
344 syrischen Kindern aus drei ambulanten Gesundheitszentren durch. Zwei der
Gesundheitszentren sind lokalisiert im Süden Syriens in Damaskus, ein Gesundheitszentrum
ist lokalisiert im Nordwesten Syriens in Hama. Mit Hilfe der gewonnenen Erkenntnisse können
langfristig Möglichkeiten der Intervention geschaffen werden. Nach unserem Wissen gibt es
derzeit nur wenige Studien, die potenzielle Risikofaktoren von Wachstumsverzögerung und
Anämie sowie den Zusammenhang zwischen Mikronährstoffen des C1-Stoffwechsels, deren
Markern, Anämie und Wachstumsentwicklung bei syrischen Kindern untersucht haben.
Mittels Fragebogen wurden sozioökonomische Einflussfaktoren und personenbezogene
Daten der Eltern sowie Daten zu Schwangerschaft und Stillzeit ermittelt. Neben
anthropometrischen Messungen der Kinder wurden Blutentnahmen zur Bestimmung des
Blutbildes und der Konzentrationen von Plasma-Homocystein, Plasma-Vitamin B12,
Plasma-Folat und deren Marker durchgeführt. - 6 -
Zusammenfassung
Die statistischen Ergebnisse ermittelten wir mit Hilfe des einseitigen Varianzanalyseverfahrens
(ANOVA), des Chi-Quadrat-Tests und logistischen Regressionsanalyseverfahren.
In der untersuchten Population betrug die Prävalenz von Anämie 30,2%, 88 von 104 Kindern
(84,6%) mit Anämie hatten gleichzeitig auch eine Mikrozytose, während in der gesamten
untersuchten Population 214 von 329 Kindern (62,2%) eine Mikrozytose hatten. 32% der
Kinder waren wachstumsverzögert (Height for Age Z-Score < -2), 16,3% waren untergewichtig
(Weight for Age Z-Score < -2). Die Plasmakonzentrationen von Homocystein, Folat und
Vitamin B12 betrugen im Median (10., 90. Perzentile) 8,4µmol/l (5,6µmol/l; 12,3µmol/l),
24,0nmol/l (9,3nmol/l; 34,7nmol/l) beziehungsweise 198pmol/l (123pmol/l; 367pmol/l). Die
Prävalenz erhöhter Homocystein-Plasma-Konzentrationen ( 8,5µmol/l), erniedrigter
Plasma-Folat- (< 12,0nmol/l) und -Vitamin B12-Konzentrationen (< 148pmol/l) betrug 49,6%,
18,4% und 23,4%. Die Vitamin B12-Plasma-Konzentration war ein stärkerer Prädiktor der
Plasma-Homocystein-Konzentration
als
die
Plasma-Folat-Konzentration
(Regressionskoeffizient β (95% KI): -0,219 (-0,288; -0,150) vs. 0,088 (0,009; 0,167)). Niedrige
Z-Scores (< -2 Standardabweichungen) für die Größe im Alter (Wachstumsverzögerung) und
das Gewicht im Alter (Untergewicht) wurden mit einem niedrigen Haushaltseinkommen, der
Rekrutierung während der Wintersaison und der geografischen Lage (Rekrutierung im
Nordwesten des Landes) in Verbindung gebracht. Anämie (50,5%) und erhöhte
Homocystein-Plasma-Konzentrationen (61,1%) traten bei Kindern im Alter von 12 bis 23
Monaten häufiger auf als bei Kindern höherer Altersgruppen. Das Vorhandensein einer
Anämie bei einem Kind war in der untersuchten Population nicht mit Wachstumsverzögerung
oder Untergewicht assoziiert. Erhöhte Plasma-Konzentrationen von Homocystein und
erniedrigte Folat- oder Vitamin-B12-Konzentrationen im Plasma erklärten nach Adjustierung
für Störfaktoren weder Anämie noch Wachstumsverzögerung oder Untergewicht.
Bei syrischen Vorschulkindern im Alter von 1 bis 5 Jahren sind die Prävalenzen für Anämie,
Wachstumsverzögerung und Untergewicht hoch. Die Beseitigung von Anämie,
eingeschränktem Wachstum bei Kindern und Defiziten in der Versorgung mit Mikronährstoffen
sollte auch in Syrien, insbesondere bei Kindern im Vorschulalter aus sozioökonomisch
schwachen Familien und ihren Müttern als wichtige Aufgabe zur Prävention mangelhafter
Gesundheitszustände in der Bevölkerung angesehen werden.1.2. Abstract
The prevalence of anemia and malnutrition and the resulting impaired growth development in children is particularly high in developing countries and countries with ongoing crisis. Stunting, underweight and anemia not only have an impact on the health of the affected individuals, increased prevalence of these health conditions can also have a negative impact on the socio-economic development of a society. Several studies already indicate that the nutrients of C1 metabolism, folate, vitamin B12 and their markers, play an important role in fetal growth and subsequent child development. The deficiency of these micronutrients leads to a metabolic disorder and appears to have a negative impact on a child's longitudinal growth and cognitive development. The nutrients of C1 metabolism also play a decisive role in the development of anemia. Both impaired growth in children and anemia pose major health problems for developing countries. Combating anemia and stunted growth in children is already the target of population intervention studies in many parts of the world.
To identify factors associated with growth and anemia in Syrian children aged 1 to 5 years, we conducted a cross-sectional study of 344 Syrian children from three public health centers to create long-term intervention opportunities using the knowledge gained. Two of the health centers are located in the south of Syria in Damascus and one health center is located in the northwest of Syria in Hama. To our knowledge, there are currently only a few studies that have investigated potential risk factors of growth retardation and anemia and the relationship between micronutrients of C1 metabolism, their markers, anemia and growth development in Syrian children.
Questionnaires were used to determine socio-economic factors and personal data of the parents as well as data on pregnancy and breastfeeding. In addition to anthropometric measurements of the children, blood samples were taken to determine the small blood count and the concentrations of plasma homocysteine, plasma vitamin B12, plasma folate and their markers. We determined the statistical results using the one-way analysis of variance (ANOVA) test, the chi-square test and logistic regression analysis methods.
In the population studied, the prevalence of anemia was 30.2%, 88 of 104 children (84.6%) with anemia also had microcytosis, while in the entire population studied, 214 of 329 children (62.2%) had microcytosis. 32% of the children were growth retarded (height for age z-Score < -2), 16.3% were underweight (weight for age z-Score < -2). The median (10th, 90th percentile) plasma concentrations of homocysteine, folate and vitamin B12 were 8.4μmol/l (5.6μmol/l, 12.3μmol/l), 24.0nmol/l (9.3nmol/l, 34.7nmol/l) and 198pmol/l (123pmol/l, 367pmol/l), respectively.The prevalence of increased plasma homocysteine concentrations ( 8.5μmol/l), decreased plasma folate (< 12.0nmol/l) and vitamin B12 concentrations (< 148pmol/l) was 49.6%, 18.4% and 23.4%, respectively.
The plasma concentration of Vitamin B12 was a stronger predictor of plasma homocysteine concentration than the plasma concentration of folate (regression coefficient β (95% CI): -0.219 (-0.288, -0.150) vs. 0.088 (0.009, 0.167)). Low z-scores (< -2 standard deviations) for height for age (stunting) and weight for age (underweight) were associated with lower household income, recruitment during the winter season, and geographic location (recruitment in the northwest of the country). Anemia (50.5%) and elevated plasma homocysteine concentrations (61.1%) were more common in children aged 12 to 23 months than in children of older age groups. The presence of anemia in a child was not associated with stunting or underweight in this study. Elevated plasma concentrations of homocysteine and decreased plasma folate or vitamin B12 concentrations did not explain anemia, stunting or underweight after adjustment for confounding factors.
In Syrian preschool children aged 1 to 5 years, the prevalence of anemia, stunting and underweight is high. The elimination of anemia, stunted growth in children and micronutrient deficiencies should be considered an important task for the prevention of poor health conditions in the population in Syria, especially in preschool children from socioeconomically disadvantaged families and their mothers
Advancing security protocol verification: A journey along the boundaries of the symbolic model
Cryptographic protocols underpin modern digital security, yet their formal verification remains a significant challenge. The symbolic model of cryptography addresses this, representing bitstrings as terms and cryptographic operations as semantics of function symbol applications. This approach allows a more abstract representation of protocols, improving scalability and automation during analysis. However, this high level of abstraction can overlook attacks that exploit the subtleties of underlying cryptographic operations. This thesis advances the accuracy of symbolic analysis by refining the abstraction of cryptographic primitives and by assessing the limits of leading verification tools. First, we propose more detailed symbolic models for cryptographic hash functions, authenti- cated encryption with associated data (AEAD), and key encapsulation mechanisms (KEMs). We integrate these models into Tamarin, a leading symbolic verification tool, and demonstrate their ability to automatically detect both known and novel attacks in real-world security protocols. Second, we present the first formal analysis of the Security Protocol and Data Model (SPDM), a widely deployed industry security standard. Our work results in one of the largest Tamarin models to date, pushing the boundaries of symbolic analysis and revealing a severe authentication vulnerability. This discovery led to our proposed fixes being included in both the specification and the reference implementation.Kryptographische Protokolle sind die Grundlage moderner digitaler Sicherheit, doch ihre formale Verifikation bleibt eine große Herausforderung. Aus diesem Grund wurde das symbolische Modell entwickelt, welches eine abstraktere Darstellung von Protokollen ermöglicht und dadurch höhere Skalierbarkeit und Automatisierung erlaubt. Diese hohe Abstraktionsebene kann jedoch Angriffe übersehen, die die Feinheiten der zugrunde liegenden kryptografischen Operationen ausnutzen. Diese Dissertation verbessert die Genauigkeit der symbolischen Analyse durch die Verfeinerung der Abstraktion von kryptographischen Primitiven und wertet währenddessen die Grenzen von aktuellen Verifikationstools aus. Zunächst erstellen wir detailliertere symbolische Modelle für kryptografische Hash Funktionen, authentifizierte Verschlüsselung mit zugehörigen Daten, und Schlüsselkapselungsmechanismen. Wir integrieren diese Modelle in Tamarin, ein führendes symbolisches Verifikationstool, und demonstrieren ihre Fähigkeit, automatisch bekannte und neue Angriffe in realen Sicherheitsprotokollen zu erkennen. Zweitens präsentieren wir die erste formale Analyse des Security Protocol and Data Model (SPDM), einem weit verbreiteten Industriesicherheitsstandard. Diese Analyse resultiert in einem der bisher umfangreichsten Tamarin-Modelle, das die Grenzen der symbolischen Analyse erweitert und eine schwerwiegende Schwachstelle in der Authentifizierung aufgedeckt
Robustness of large language models in moral judgements
With the advent of large language models (LLMs), there has
been a growing interest in analysing the preferences encoded
in LLMs in the context of morality. Recent work has tested
LLMs on various moral judgement tasks and drawn conclu sions regarding the alignment between LLMs and humans.
The present contribution critically assesses the validity of the
method and results employed in previous work for eliciting
moral judgements from LLMs. We find that previous results
are confounded by biases in the presentation of the options
in moral judgement tasks and that LLM responses are highly
sensitive to prompt formulation variants as simple as changing
‘Case 1’ and ‘Case 2’ to ‘(A)’ and ‘(B)’. Our results hence indi cate that previous conclusions on moral judgements of LLMs
cannot be upheld. We make recommendations for more sound
methodological setups for future studies
Model-Based Robust Position Control of an Underactuated Dielectric Elastomer Soft Robot
Achieving accurate closed-loop position control of soft
robots remains an ongoing research problem, due to the challenges
posed by underactuation, elastic nonlinearities, and material creep.
Although soft driving technologies relying on tendons and smart
material transducers (e.g., dielectric elastomers, shape memory
alloys) offer more ease of controllability compared to pneumat ics, the corresponding controller design problem becomes even
more challenging because of additional nonlinear effects. Those in clude a configuration-dependent actuation matrix, that stems from
the kinematics of the actuation, and control input saturation, which
is especially critical for smart material actuators. In this article,
we investigate for the first time the closed-loop position control
of a soft-robotic system driven by dielectric elastomer actuators.
The objective is to regulate the robot state to a constant setpoint,
accounting for the effects of open-loop instability, underactuation,
control input saturation, and constant external disturbances. To
achieve this goal, we propose a model-based feedback scheme,
which combines a stabilizing energy-shaping controller with a ro bustifying PI-like law. After presenting the general theory, a linear
matrix inequalities algorithm is proposed to practically address
the controller design in spite of strong model nonlinearities. Ex perimental validation conducted on a prototype of the soft-robotic
system confirms the effectiveness of the proposed control approach
A Comprehensive CYP2D6 Drug-Drug-Gene Interaction Network for Application in Precision Dosing and Drug Development
Conducting clinical studies on drug–drug-gene interactions (DDGIs) and extrapolating the findings into clinical dose recommendations is challenging due to the high complexity of these interactions. Here, physiologically-based pharmacokinetic (PBPK) modeling networks present a new avenue for exploring such complex scenarios, potentially informing clinical guidelines and handling patient-specific DDGIs at the bedside. Moreover, they provide an established framework for drug–drug interaction (DDI) submissions to regulatory agencies. The cytochrome P450 (CYP) 2D6 enzyme is particularly prone to DDGIs due to the high prevalence of genetic variation and common use of CYP2D6 inhibiting drugs. In this study, we present a comprehensive PBPK network covering CYP2D6 drug–gene interactions (DGIs), DDIs, and DDGIs. The network covers sensitive and moderate sensitive substrates, and strong and weak inhibitors of CYP2D6 according to the United States Food and Drug Administration (FDA) guidance. For the analyzed CYP2D6 substrates and inhibitors, DD(G)Is mediated by CYP3A4 and P-glycoprotein were included. Overall, the network comprises 23 compounds and was developed based on 30 DGI, 45 DDI, and seven DDGI studies, covering 32 unique drug combinations. Good predictive performance was demonstrated for all interaction types, as reflected in mean geometric mean fold errors of 1.40, 1.38, and 1.56 for the DD(G)I area under the curve ratios as well as 1.29, 1.43, and 1.60 for DD(G)I maximum plasma concentration ratios. Finally, the presented network was utilized to calculate dose adaptations for CYP2D6 substrates atomoxetine (sensitive) and metoprolol (moderate sensitive) for clinically untested DDGI scenarios, showcasing a potential clinical application of DDGI model networks in the field of model-informed precision dosing
Gromomycins: An Unprecedented Class of Triterpene Antibiotics Produced by a Novel Biosynthetic Pathway
The current situation with drug-resistant microbial pathogens is critical, dictating an acute need for novel
efficient antibiotics. Herein, we report a new class of antibiotics named gromomycins with significant activity, especially
against drug-resistant Gram-positive pathogens, including methicillin- and daptomycin-resistant Staphylococcus aureus.
Gromomycins are pentacyclic triterpenes with a cyclic guanidino group forming the fifth six-membered ring. We have used
transposon mutagenesis to identify the gromomycin biosynthetic gene cluster, since it could not be assigned by any available
bioinformatics tools, highlighting its unique biosynthetic route. Using gene cluster engineering, feeding experiments, and
LC-MS and NMR analyses we have proposed the biosynthetic pathway for gromomycins, which are the first bacterial
triterpenes synthesized independently of the squalene pathway. They also exhibit a so far unprecedented cyclization route
that utilizes a hexaprenylguanidine linear precursor. Leveraging our understanding of their biosynthesis, we have identified
additional gromomycin producers, resulting in the isolation of novel bioactive derivatives
PI3K Inhibitors in the Treatment of PIK3CA-Mutated Breast Cancer: A Methodological Comparison of Molecular Pathology Analysis Techniques for the Detection of PIK3CA Variants in Long-Term Preserved Tumor Tissue
Einleitung
Das Mammakarzinom ist das häufigste Karzinom der Frau. Neben den konventionellen Therapieansätzen steht, nach der SOLAR1 Studie von Andre et al. aus dem Jahr 2019, mit dem PI3K-Inhibitor Alpelisib in Kombination mit Fulvestrant seit 2020 eine weitere Behandlungsmöglichkeit für das PIK3CA-mutierte, Hormonrezeptorpositive, HER2neu-negative, endokrin vortherapierte, lokal fortgeschrittene oder metastasierte Mammakarzinom zur Verfügung. Voraussetzung für den Einsatz dieser Medikamentenkombination ist der vorherige Nachweis einer PIK3CA-Mutation durch „liquid-biopsy“, oder aus einer Gewebeprobe. Aufgrund der Tatsache, dass es sich bei Alpelisib um eine Zweitlinientherapie handelt, welche erst 2020 zugelassen wurde, findet eine solche Testung aktuell vor allem nachträglich aus asserviertem, formalin-fixierten und paraffin-eingebetteten Tumorgewebe statt. Vor diesem Hintergrund ist es Ziel dieser methodischen Arbeit die Validität der drei gängigsten Methoden, Sequenzierung nach Sanger, Echtzeit-PCR und das „Next-Generation-Sequencing“, für den Nachweis oder Ausschluss von PIK3CA-Mutationen im Langzeit-archiviertem Tumorgewebe zu vergleichen und somit möglicherweise eine präferenzielle Empfehlung für die molekularpathologische Diagnostik zu geben.
Material und Methodik
Untersucht wurde in neutral-gepuffertem Formalin fixiertes und in Paraffin eingebettetes Überschussmaterial von Mamma-Resektaten aus den Jahren 2006 bis 2011. Das Untersuchungsmaterial besteht in dieser Arbeit ausschließlich aus DNA-Proben, welche aus nicht-spezifischer-Typ- (NST) lobulären und kombinierten Karzinomen der Mamma isoliert wurden. Das verwendete Probenmaterial wurde ursprünglich in der Klinik für Frauenheilkunde, Geburtshilfe und Reproduktionsmedizin (Leitung: Prof. Dr. E.-F. Solomayer) am Universitätsklinikum des Saarlandes (UKS) gewonnen und im Institut für Allgemeine und Spezielle Pathologie am UKS (Leitung: Prof. Dr. Rainer M. Bohle) nach leitliniengerechter Diagnostik untersucht. In dieser Arbeit wurden die Proben mittels Sequenzierung nach Sanger, Quantitativer Echtzeit-PCR und Next-Generation-Sequencing (NGS) untersucht. Anschließend wurden die molekulargenetischen Untersuchungsmethoden unabhängig voneinander im Hinblick auf die Mutationsfrequenz unter Berücksichtigung der Mutationsloki Exon 7, Exon 9 und Exon 20 des PIK3CA und der intrinsischen Subtypen Luminal A, Luminal B, HER2-positiv und Triple-negativ ausgewertet. Zusätzlich wurde mittels Cohens´s Kappa die Reliabilität der drei verwendeten PIK3CA-Mutationsanalyseverfahren statistisch ermittelt.
Ergebnisse
Von den 59 Patientinnen gehören 22 zu der Gruppe HR-positiv und HER2/neu-negativ. Das Vorhandensein eines PIK3CA-Pseudogens auf dem Chromosom 22 erschwerte die Mutationsanalyse mittels Sequenzierung nach Sanger. Bei circa 75% der Proben konnte mittels Sequenzierung nach Sanger das Exon 9 des PIK3CA nicht vollständig analysiert werden. In der Echtzeit-PCR konnte eine Probe nicht analysiert werden. Mit der NGS konnten alle Patientinnen-Proben analysiert werden. Die Sensitivität der Sequenzierung nach Sanger lag in dieser Arbeit bei 33%. Die Spezifität liegt in dieser Arbeit für die Sequenzierung nach Sanger bei 100%. Die Quantitative Echtzeit-PCR und das Next-Generation-Sequencing hatten nach Altman eine sehr gute und nach Landis und Koch eine fast perfekte Reliabilität.
Schlussfolgerung
Zusammenfassend ergibt sich, dass für eine primär separate PIK3CA-Mutationsanalyse aus mehrere Jahre als Paraffinblock asserviertem Tumorgewebe zunächst die Sequenzierung nach Sanger verwendet werden sollte. Falls die Sequenzierung nach Sanger den Wildtyp oder ein inkomplettes Ergebnis nachweist, sollte eine erneute Analyse mittels NGS stattfinden. Detektiert die Sequenzierung nach Sanger eine Mutation, ist keine weitere Analyse notwendig.Introduction
Breast cancer is the most common cancer in women. In addition to conventional treatment approaches, the PI3K inhibitor alpelisib in combination with fulvestrant has been available since 2020 as a further treatment option for PIK3CA-mutated, hormone receptor-positive, HER2-negative, endocrine-pretreated, locally advanced or metastatic breast cancer following the SOLAR1 study by Andre et al. in 2019. The prerequisite for the use of this drug combination is the prior detection of a PIK3CA mutation by liquid biopsy or from a tissue sample. Since alpelisib is a second-line therapy that was only approved in 2020, such testing is currently mainly carried out retrospectively from preserved excess tissue. Against this background, the aim of this methodological work is to practically compare the three established methods, sequencing according to Sanger, real-time PCR and next-generation sequencing, for this application and thus provide a recommendation for practice.
Methods and Material
Excess material from breast resections obtained between 2006 to 2011. The tissue analyzed was formalin-fixed and paraffin-embedded. The test material in this study consists exclusively of DNA samples isolated from lobular, NST and combined carcinomas of the breast. The sample material used was originally obtained at the Department of Gynaecology, Obstetrics and Reproductive Medicine (Head: Prof. Dr E.-F. Solomayer) at Saarland University Hospital (UKS) and examined at the Institute of General and Special Pathology at UKS (Head: Prof. Dr Rainer M. Bohle) according to guideline-based diagnostics. In this study, the samples were analysed using Sanger sequencing, quantitative real-time PCR and next-generation sequencing. The molecular genetic examination methods were then analysed independently of each other regarding detection of mutation frequency at loci in the PIK3CA gene in exon 7, exon 9 and exon 20 in the intrinsic breast cancer subtypes luminal A, luminal B, HER2-positive and triple-negative. In addition, the reliability of the three PIK3CA mutation analysis methods used was calculated and statistically analysed using Cohen's Kappa.
Results
Of the 59 patients, 22 belonged to the HR-positive and HER2/neu-negative group. The presence of a PIK3CA pseudogene on chromosome 22 made it difficult to analyse the mutation using Sanger sequencing. In approximately 75% of the samples, exon 9 could not be fully analysed by Sanger sequencing. One sample could not be analysed using quantitative real-time PCR. All patient samples could be analysed using NGS. The sensitivity of Sanger sequencing in this study was 33%. The specificity for Sanger sequencing in this study was 100%. Next-generation-sequencing and real-time PCR showed a very good reliability according to Altman and almost perfect reliability according to Landis and Koch.
Conclusion
In summary, Sanger sequencing should initially be used for a PIK3CA mutation analysis from long time preserved paraffin-embedded tumour tissue. If Sanger sequencing is negative or detects only the wild type gene, a new analysis should be performed using NGS. If Sanger sequencing detects a mutation, no further analysis is necessary
