Quality of Reflections on Teaching: Approaches to Its Measurement and Low-Threshold Promotion
The present study introduces a multidimensional approach to describing the composi tion of reflections, addressing previous inconsistencies in operationalization. The multi dimensional approach was empirically explored in comparison to unidimensional re flection quality measures and examined in relation to reflection-related dispositions.
Given the challenges of promoting reflection in teachers’ daily practice, low-threshold
interventions—repeated practice and structuring prompts—were investigated regarding
their potential to foster reflective competence. Using an integrated model of reflection, five
proposed dimensions—describing, evaluating, naming alternatives, justification, and self reference—were identified and assessed alongside the quality measures reflection depth
and breadth, and holistic grading. N = 29 teachers reflected verbally on two teaching videos,
first openly and then with structuring prompts. The transcribed reflections underwent
qualitative and evaluative content analysis to extract quantitative data. Compared to exist ing quality measures, the multidimensional approach provided deeper insights into the
complexity of the reflections. The correlations between different approaches to reflection
measurement and the reflection-related dispositions were only partially as expected. While
repeated practice did not yield a training effect, structuring prompts improved reflection
quality. Overall, the findings confirm the usefulness of the multidimensional approach as a
valuable means to describe the composition of reflections and highlight its potential for
quality measurement
How green will the green-steel production be?

This paper explores the technological pathways to achieving net-zero emissions in the steel sector, highlighting the potential for emissions reductions in other industries through the use of steel co-products. It discusses the primary and secondary steel production routes, the generation and utilization of by-products, and the integration of carbon capture and storage technologies. The paper also addresses the challenges of scrap availability and quality, the role of hydrogen in decarbonization, and the economic and regulatory factors influencing the industry's transition. A new preclinical sheep model of medial meniscus anterior root repair: Part 2-Surgical strategy, technical considerations, pearls and pitfalls
Purpose
To address a gap in translational research by developing a preclinical sheep model of medial meniscus anterior root (MAR) repair in vivo and to compare probabilities of potential pitfalls and difficulties with humans.
Methods
Preoperative planning and surgical procedures applied to patients were adapted to adult sheep. Eight healthy, skeletally mature, female Merino ewes between 2 and 4 years of age underwent a mini-open medial parapatellar approach to both stifle joints without luxating the patella. Next, the MAR was transected in 16 knees (8 sheep) resulting in a subtype 2A tear according to the LaPrade classification, followed by a transtibial pull-out repair through a 3.2 mm diameter bone tunnel with a reinforced Mason–Allen suture and non-absorbable suture material. Animals were followed until 21 days after surgery.
Results
The surgery time per knee ranged between 30 and 50 min (mean, 40.0 ± 7.8 min). The surgical technique was safe without intra- or post-operative complications. Solid repair is most likely if the following surgical principles are respected: (1) Selection of the MAR and the open technique allow for elegant tunnel positioning and less post-operative loading stress due to the normal extension deficit of sheep; (2) careful preparation of the MAR is mandatory; (3) considering the oval shape of the MAR attachment (MARA) results in anatomic tunnel placement; (4) robust suture placement and configuration avoids suture cut out. The probabilities of potential pitfalls and difficulties differ from the human situation.
Conclusion
A clinically adapted MAR repair model in adult sheep was developed following its complete transection close to the MARA, followed by an open transtibial pull-out repair. The surgical technique was safe without intra- or short-term post-operative complications. This model may be suitable to study the biomechanics and pathophysiology of meniscal root injuries and their repair.

Investigation of the binding kinetics and electrochemical properties of in situ reconstructed apo-GOx using electrodes with electrodeposited FAD cofactor
Herein, the electroanalytical properties of in situ reconstructed apo-glucose oxidase (apo-GOx) on elec trodes modified with electrodeposited cofactor flavin adenine dinucleotide (FAD) were studied in the
absence of any diffusional electron mediators. In the electrodeposited sensing layer, the order of enzyme
assembly using the same building blocks (e.g., spatially separated apo-GOx with FAD cofactor or native
holo-GOx with entrapped cofactor) significantly affected the electrodes’ electroanalytical performance.
These electrodes exhibited a pronounced real time current difference (i), and the work of in situ recon structed apo-GOx, compared with a native electrodeposited holo-GOx analogue, was not limited by
oxygen depletion (ii). This knowledge was subsequently used to profile several types of apo-GOx
Pre-existing endometriotic lesions promote the development of new lesions in the abdomen of mice
Endometriosis is a gynecological disease, which is defined by the development of endometriotic lesions outside the uterine cavity. Up to 20% of women of reproductive age are affected and show symptoms like dysmenorrhea, dyspareunia and infertility. This markedly reduces their quality of life. The reliable diagnosis of the disease can only be confirmed by laparoscopic surgery and histopathological analysis of tissue samples. Hence, several years can pass until patients are diagnosed correctly. The development of endometriosis is widely believed to be promoted by the process of retrograde menstruation according to the implantation theory of Sampson. This theory suggests that endometrial tissue fragments are transported to the abdominal cavity through the fallopian tubes and attach to the peritoneum, where they finally develop into endometriotic lesions. These lesions have been shown to change the peritoneal micromilieu.
This thesis analyzed if the presence of pre-existing endometriotic lesions supports the growth of new lesions. It also investigated if this is caused by an altered peritoneal environment or by an exchange of cells between individual lesions.
To test this, uterine tissue samples from FVB/N wild-type donor mice were transplanted simultaneously or time-delayed by 14 days with samples from transgenic FVB-Tg(CAG-luc-GFP)L2G85Chco/J donor mice into the abdomen of FVB/N wild-type recipient mice. The development of the lesions was monitored by using high-resolution ultrasound, bioluminescence imaging, histology and immunohistochemistry. Next, a peritoneal lavage was performed in additional FVB/N wild-type recipient mice in order to examine the peritoneal conditions after the induction of endometriotic lesions. For this, uterine tissue samples from transgenic FVB-Tg(CAG-luc-GFP)L2G85Chco/J donor mice were transplanted into the abdomen of recipient mice and peritoneal lavages were performed before and 14 days after transplantation. Flow cytometry and a cytokine array were used to assess the fraction of immune cells and levels of inflammatory cytokines in the peritoneal fluid.
The performed analyses showed that the growth of newly developing endometriotic lesions is promoted by the intraperitoneal presence of pre-existing ones. In fact, the time-delayed transplantation of uterine tissue samples resulted in higher growth rates of newly developing endometriotic lesions when compared to simultaneous transplantation. However, this cannot be explained by a cellular exchange between both lesion types (luciferase-positive lesions derived from FVB-Tg(CAG-luc-GFP)L2G85Chco/J mice and luciferase-negative lesions derived from FVB/N wild-type mice), because no bioluminescent signals could be detected outside the luciferase-positive lesions. Therefore, this observation was rather caused by peritoneal inflammation, which was induced by the pre-existing lesions.
In line with this view, the analysis of peritoneal fluid after the induction of endometriotic lesions showed increased levels of several cytokines, such as B-lymphocyte chemoattractant (BLC), tissue inhibitor of metalloproteinases 1 (TIMP-1), interleukin (IL)-5 and IL-23, when compared to baseline conditions (before the induction of endometriotic lesions). Additionally, the performed flow cytometry showed a high fraction of Gr1-positive granulocytes in the peritoneal fluid after 14 days since the induction of endometriotic lesions.
These findings indicate that, in addition to other pathomechanisms, the development of endometriosis is driven by a lesion-induced peritoneal inflammation, which creates favorable conditions for the growth of new lesions.Die Endometriose ist eine gynäkologische Erkrankung, die durch die Entstehung von
Endometrioseherden außerhalb der Uterushöhle gekennzeichnet ist. Bis zu 20% der Frauen
im gebärfähigen Alter sind davon betroffen und zeigen Symptome wie Dysmenorrhoe,
Dyspareunie und Infertilität. Dies beeinträchtigt ihre Lebensqualität deutlich. Die sichere
Diagnose der Krankheit kann nur durch einen invasiven laparoskopischen Eingriff und eine
histopathologische Untersuchung von Gewebeproben gestellt werden. Daher können
mehrere Jahre vergehen, bis die Patientinnen die richtige Diagnose erhalten. Es wird
allgemein angenommen, dass die Entstehung einer Endometriose durch retrograde
Menstruation gemäß der Implantationstheorie von Sampson gefördert wird. Diese Theorie
besagt, dass Fragmente von Endometriumgewebe durch die Eileiter in die Bauchhöhle
transportiert werden und sich am Bauchfell festsetzen, wo sie sich schließlich zu
Endometrioseherden entwickeln. Es wurde somit gezeigt, dass die Herde das peritoneale
Mikromilieu verändern.
In der vorliegenden Arbeit wurde untersucht, ob bereits bestehende Endometrioseherde das
Wachstum neuer Herde fördern. Außerdem wurde überprüft, ob dies auf ein verändertes
peritoneales Milieu oder auf einen Zellaustausch zwischen einzelnen Herden zurückzuführen
ist.
Zu diesem Zweck wurden Uterusgewebeproben von FVB/N-Wildtyp-Spendermäusen
gleichzeitig oder um 14 Tage zeitversetzt mit Proben von transgenen FVB-Tg(CAG-luc-
GFP)L2G85Chco/J-Spendermäusen in die Bauchhöhle von FVB/N-Wildtyp-
Empfängermäusen transplantiert. Die Entwicklung der Herde wurde mittels hochauflösendem
Ultraschall, Biolumineszenz-Bildgebung, Histologie und Immunhistochemie analysiert.
Darüber hinaus wurde bei weiteren FVB/N-Wildtyp-Empfängermäusen eine Peritoneallavage
durchgeführt, um die peritonealen Bedingungen nach der Induktion von Endometrioseherden
zu untersuchen. Dazu wurden Uterusgewebeproben von transgenen FVB-Tg(CAG-luc-
GFP)L2G85Chco/J Spendermäusen in die Bauchhöhle der Empfängertiere transplantiert und
Peritoneallavagen vor und 14 Tage nach der Transplantation durchgeführt. Um den Anteil von
Immunzellen und Entzündungsmediatoren in der Peritonealflüssigkeit zu bestimmen, erfolgte
eine Durchflusszytometrie und ein Zytokin-Array.
Diese Untersuchungen zeigten, dass das Wachstum neuer Endometrioseherde durch das
intraperitoneale Vorhandensein bereits bestehender Herde gefördert wird. Tatsächlich führte
die zeitlich verzögerte Transplantation von Uterusgewebeproben zu höheren Wachstumsraten von sich neu entwickelnden Endometrioseherden im Vergleich zur
gleichzeitigen Transplantation. Dies konnte jedoch nicht durch einen Zellaustausch zwischen
beiden Herdtypen (Luciferase-positive Herde aus FVB-Tg(CAG-luc-GFP)L2G85Chco/JGewebe
und Luciferase-negative Herde aus FVB/N-Gewebe) erklärt werden, da außerhalb
der Luciferase-positiven Herde keine Biolumineszenz-Signale nachgewiesen werden
konnten. Daher war diese Beobachtung eher auf eine peritoneale Entzündung
zurückzuführen, die durch die bereits bestehenden Herde ausgelöst wurde.
In Übereinstimmung damit zeigte die Analyse der Peritonealflüssigkeit nach der Induktion von
Endometrioseherden erhöhte Konzentrationen von Zytokinen, wie B-Lymphozyten-
Chemoattraktant (BLC), Gewebe-Inhibitor von Metalloproteinasen 1 (TIMP-1), Interleukin (IL)-
5 und IL-23 im Vergleich zu den Ausgangsbedingungen (vor der Induktion von
Endometrioseherden). Darüber hinaus zeigten durchflusszytometrische Analysen einen
hohen Anteil an Gr1-positiven Granulozyten in der Peritonealflüssigkeit 14 Tage nach der
Induktion von Endometrioseherden.
Diese Ergebnisse deuten darauf hin, dass die Entwicklung einer Endometriose neben anderen
Pathomechanismen durch eine herdinduzierte peritoneale Entzündung vorangetrieben wird,
die günstige Bedingungen für das Wachstum neuer Herde schafft
Einfluss von Simvastatin auf die Frakturheilung im murinen Ischämiemodell
Simvastatin findet als Reduktase-Hemmer des Enzyms 3-Hydroxy-3-methyl-glutarylCoenzym-A-Reduktase (HMG-CoA) in der Klinik breite Anwendung zur Therapie der
Hypercholesterinämie sowie zur Senkung der kardiovaskulären Mortalität. Darüber hinaus hat
Simvastatin einen nachgewiesenen aniinflammatorischen, vaskuloprotektiven und
angiogenen sowie osteogenen Effekt. Diese Prozesse sind u.a. essentieller Bestandteil
während einer Frakturheilung. Der Einfluss von Simvastatin auf die Knochenheilung wurde
bisher kontrovers diskutiert. Inwiefern Simvastatin die Frakturheilung unter ischämischen
Bedingungen beeinflusst, ist bisher nicht bekannt. Das Ziel dieser Studie war es, den Einfluss
von Simvastatin auf die Frakturheilung in einem murinen Ischämiemodell zu untersuchen.
Bei 47 CD-1 Mäusen wurde durch Ligatur der Arteria profunda femoris eine milde Ischämie
der rechten unteren Extremität induziert. Nach geschlossener Frakturierung wurde das Femur
mit einer intramedullären Zugschraube osteosynthetisch versorgt. Die Mäuse der
Simvastatingruppe (n=22) erhielten nachfolgend täglich 30 mg/kg Körpergewicht (KG)
Simvastatin per os. Der Kontrollgruppe (n=25) wurde eine äquivalente Menge isotone
Natriumchloridlösung per os täglich appliziert. 2 und 5 Wochen postoperativ wurden nach
Explantation der Femora biomechanische, röntgenologische, computertomographische,
histomorphometrische und proteinbiochemische Untersuchungen durchgeführt.
In der biomechanischen und röntgenologischen Untersuchung zeigten sich keine
Unterschiede zwischen den Gruppen. In der Computertomographie war 2 und 5 Wochen
postoperativ in der Simvastatingruppe ein signifikant erniedrigtes absolutes Knochenvolumen
und ein erniedrigter relativer Knochenanteil mit gleichzeitig verminderter Trabekelanzahl
feststellbar. Korrespondierend dazu war nach 2 Wochen der Trabekelabstand signifikant
erhöht und nach 5 Wochen die Trabekeldicke signifikant erniedrigt. In der
histomorphometrischen Analyse ergab sich nach 5 Wochen in der Simvastatingruppe eine
signifikant erniedrigte Gesamtkallusfläche. In der proteinbiochemischen Analyse erwies sich
die Expression von Cluster of differentiation 31 (CD31) und Phosphatidylinositol-3-kinase
(PI3K) im Rahmen einer stimulierten Angiogenese und Proliferation signifikant erhöht. Die
Expression des Osteogenesemarkers Bone Morphogenetic Protein (BMP)-2 zeigte sich
signifikant erniedrigt. Die Expressionen von Osteoprotegerin (OPG) und Receptor Activator of
NF-κB Ligand (RANKL) ergaben keine signifikanten Unterschiede. Zusammenfassend zeigen die Daten der vorliegenden Arbeit, dass Simvastatin die Frakturheilung unter ischämischen Bedingungen am Mausmodell beeinträchtigt ohne jedoch
die Knochenheilung insgesamt zu verhindern. Dieser Effekt ist am ehesten auf ein
Missverhältnis angiogener und osteogener Expressionsmarker im Kallusgewebe
zurückzuführen. Darüber hinaus ergeben sich Hinweise, dass die Wirkung von Simvastatin auf
die Frakturheilung dosisabhängig ist, sodass künftig weitere Studien den Effekt von
Simvastatin und neuester Derivate hiervon auf ihr osteoanaboles Potenzial unter hypoxischen
Bedingungen noch weiter untersuchen sollten.As a reductase inhibitor of the enzyme 3-hydroxy-3-methyl-glutaryl-coenzyme A reductase
(HMG-CoA), Simvastatin is widely used in the clinic to treat hypercholesterolemia and to
reduce cardiovascular mortality. In addition, Simvastatin has a proven aniinflammatory,
vasculoprotective, angiogenic and osteogenic effect. These processes are essential
components during fracture healing. The influence of Simvastatin on bone healing has been
the subject of controversial debate. The extent to which Simvastatin influences fracture healing
under ischemic conditions is not yet known. The aim of this study was to investigate the
influence of Simvastatin on fracture healing in a murine ischemia model.
Mild ischemia of the right lower limb was induced in 47 CD-1 mice by ligation of the profunda
femoris artery. After closed fracturing, the femur was osteosynthesized with an intramedullary
screw. The mice in the Simvastatin group (n=22) subsequently received 30 mg/kg body weight
of Simvastatin per os daily. The control group (n=25) received an equivalent amount of isotonic
sodium chloride solution per os daily. Biomechanical, radiographic, computed tomographic,
histomorphometric and protein biochemical examinations were performed 2 and 5 weeks
postoperatively after explantation of the femora.
There were no differences between the groups in the biomechanical and radiographic
examinations. Computed tomography showed a significantly reduced absolute bone volume
and a reduced relative bone proportion in the Simvastatin group at 2 and 5 weeks
postoperatively, with a simultaneous reduction in the number of trabeculae. Correspondingly,
the trabecular spacing was significantly increased after 2 weeks and the trabecular thickness
was significantly reduced after 5 weeks. The histomorphometric analysis revealed a
significantly reduced total callus area in the Simvastatin group after 5 weeks. In the protein
biochemical analysis, the expression of cluster of differentiation 31 (CD31) and phosphatidylinositol-3-kinase (PI3K) was found to be significantly increased in the context of
stimulated angiogenesis and proliferation. The expression of the osteogenesis marker bone
morphogenetic protein (BMP)-2 was significantly decreased. The expressions of
osteoprotegerin (OPG) and receptor activator of NF-κB ligand (RANKL) showed no significant
differences.
In summary, the data of the present study show that simvastatin impairs fracture healing under
ischemic conditions in the mouse model without, however, preventing bone healing altogether.
This effect is most likely due to an imbalance of angiogenic and osteogenic expression markers
in callus tissue. In addition, there are indications that the effect of simvastatin on fracture
healing is dose-dependent, so that further studies should continue to investigate the effect of
simvastatin and its latest derivatives on their osteoanabolic potential under hypoxic conditions
Piezo1 Channel Activators Yoda1 and Yoda2 in the Context of Red Blood Cells
Piezo1 is a mechanosensitive non-selective cation channel. Genetic alterations of the
channel result in a hematologic phenotype named Hereditary Xerocytosis. With Yoda1 and,
more recently, Yoda2, compounds to increase the activity of Piezo1 have become available.
However, their concrete effect depends on the nano environment of the channel and hence
on the cell type. Here we compare the potency of Yoda1 and Yoda2 in red blood cells (RBCs).
We investigate the effect of the compounds on direct channel activity using automated
patch clamp, as well as the secondary effects of channel activation on signalling molecules
and cellular response. In terms of signalling, we investigate the temporal response of the
second messenger Ca2+, and in terms of cellular response, the activity of the Gárdos channel.
The opening of the Gárdos channel leads to a hyperpolarisation of the RBCs, which is
measured by the Macey–Bennekou–Egée (MBE) method. Although the interpretation of the
data is not straightforward, we discuss the results in a physiological context and provide
recommendations for the use of Yoda1 and Yoda2 to investigate RBCs
ArthroRad-Studie: Multizentrische prospektive randomisierte einfach verblindete Studie zur Effektivität von niedrigdosierter Strahlentherapie bei schmerzhaften Arthrosen -Daten nach 3-monatigem Follow-up-
Zielsetzung
Seit langem ist der analgetische Effekt der niedrigdosierten Strahlentherapie bei
schmerzhaften Arthrosen auf der Grundlage zahlreicher retrospektiver Daten bekannt,
hierbei ist der Therapiestandard die Applikation von 3 Gy bis zu 6 Gy Gesamtdosis bei zwei-
bis dreimal wöchentlichen Einzeldosen von 0,5 Gy bis 1 Gy. In multiplen tierexperimentellen
Studien sowie in vitro-Studien konnte der überlegene antiinflammatorische Effekt bei
Gesamtdosen zwischen 0,3 Gy und 0,7 Gy nachgewiesen werden. Daher wurde diese
klinische prospektive randomisierte Studie konzipiert, um die Wirksamkeit der aktuellen
Standard-Gesamtdosis von 3 Gy mit einer niedrigeren experimentellen Gesamtdosis von
0,3 Gy im Hinblick auf Schmerzreduktion und Lebensqualitätsverbesserung zu vergleichen.
Patienten, Material und Methoden
Es wurden insgesamt 244 Gelenke (Hand-, Finger-, Kniegelenke) von 133 Patienten in die
Studie eingeschlossen, hiervon konnten beim follow-up nach drei Monaten noch 221
Gelenke ausgewertet werden. Nach Studieneinschluss erfolgte die Randomisierung
entweder in die Standarddosisgruppe (3 Gy Gesamtdosis, 0,5 Gy Einzeldosen zweimal
wöchentlich) oder in die experimentelle Dosisgruppe (0,3 Gy Gesamtdosis, 0,05 Gy
Einzeldosen zweimal wöchentlich). Die Studie war einfach-verblindet, die Patienten erfuhren
die applizierte Dosis nicht. Die Nachsorge erfolgte drei und zwölf Monate nach Abschluss der
Radiatio, wobei dieser Dissertation die Auswertung der Daten des follow-ups nach drei
Monaten zugrunde liegt. Studienendpunkte waren die Schmerzreduktion nach dem VAS
Score, die Funktionsverbesserung nach dem KOOS-PS-Score (Kniegelenke) oder SF
SACRAH-Score (Finger-/Handgelenke) sowie die Verbesserung der Lebensqualität mittels
SF-12-Fragebögen in Eigen- und Fremdbeurteilung.
Ergebnisse
Bei den 221 ausgewerteten Gelenken (64 Knie, 172 Hand-/Fingergelenke) wurde die
Gesamtdosis von 3 Gy bei 33 Knien und 77 Händen appliziert, die Gesamtdosis von 0,3 Gy
bei 81 Händen und 30 Knien. In beiden Gruppen zeigte sich ein gutes analgetisches
Ergebnis anhand der Differenz des VAS-Scores, allerdings ohne statistisch signifikanten
Unterschied. Ähnliche Ergebnisse konnten auch für die funktionellen Scores und die
Lebensqualitätsfaktoren nachgewiesen werden, aber auch hier ohne statistische Signifikanz.
Der einzige prognostische Parameter mit statistischer Signifikanz war der Ausgangsschmerz:
Hier zeigte sich, dass Patienten mit einem höheren Ausgangsschmerz vor Therapiebeginn
eine stärkere Schmerzlinderung durch die Radiatio erzielten. Radiogene Nebenwirkungen
waren nicht zu verzeichnen. Die Studie wurde aufgrund der langsamen Rekrutierung
vorzeitig beendet.
Schlussfolgerung
In beiden Studienarmen konnte eine gute Schmerzlinderung und eine begrenzte
Verbesserung der Funktions- und Lebensqualitätswerte erzielt werden, allerdings ohne
statistische Signifikanz. Der in laborexperimentellen Studien beschriebene überlegene
antiinflammatorische Effekt von Gesamtdosen zwischen 0,3 Gy und 0,7 Gy konnte in unserer
Studie nicht bestätigt werden. Somit ist die Durchführung weiterer klinischer Studien
notwendig, um die ideale Bestrahlungsdosis zu eruieren und um die Effekte der sehr niedrig
dosierten Strahlentherapie auf inflammatorische Erkrankungen weiter zu untersuchen.English title
ArthroRad trial: multicentric prospective and randomized single-blinded trial on the effect of
low-dose radiotherapy for painful osteoarthritis depending on the dose-results after 3 months'
follow-up
Purpose
The analgesic effect of low-dose radiotherapy for painful osteoarthritis has been recognized
on the basis of numerous retrospective data; the standard therapy is the application of 3 Gy
to 6 Gy total dose with single fractions of 0,5 Gy to 1 Gy two to three times a week. Multiple
animal experimental studies and in vitro studies demonstrated a superior anti-inflammatory
effect at total dose between 0,3 Gy and 0,7 Gy. This clinical prospective randomized trial was
designed to compare the effectiveness of the current standard total dose of 3 Gy with a lower
experimental total dose of 0,3 Gy in regard to pain reduction and quality of life improvement.
Patients, material and methods
A total of 244 joints (hand, finger, knee joints) from 133 patients were included in the study,
of which 221 joints will be evaluated at the three month follow up appointment. The joints
were randomized either into the standard dose group (3 Gy total dose, 0,5 Gy single doses
twice a week) or into the experimental dose group (0,3 Gy total dose, 0,05 Gy single doses
twice a week). The study was single-blinded and the patients were not informed about the
applied dose. There were follow-up examinations three and twelve months after completion
of the radiation treatment. This dissertation outlines the data from the follow-up after three
months. Study endpoints were pain reduction according to the VAS score (visual analogue
scale), functional improvement according to the KOOS-PS score (the knee injury and
osteoarthritis outcome score) or SF-SACRAH score (short form score for the assessment
and quantification of chronic rheumatic affections of the hands), as well as the improvement
in quality of life using SF-12 score (short form 12).
Results
In the 221 joints evaluated (64 knees, 172 hand/finger joints), the total dose of 3 Gy was
applied to 33 knees and 77 hands and the total dose of 0.3 Gy was applied to 81 hands and
30 knees. In both groups there was a positive analgesic result based on the difference in the
VAS score. However, there was no statistically significant difference. Similar results could
also be detected for the functional scores and the quality of life scores, but lack a statistically
significant difference. The only prognostic parameter with statistical significance was the
baseline pain: Showing that patients with higher baseline pain before the start of therapy
achieved greater pain relief from radiotherapy. Side effects were not recorded and the trial
was closed prematurely due to slow recruitment.
Conclusion
Pain relief and a limited improvement in functional and quality of life scores were achieved in
both study arms despite statistical significance. The superior anti-inflammatory effect of total
doses between 0,3 Gy and 0,7 Gy described in laboratory experimental studies could not be
confirmed in our study. Therefore, further clinical studies are necessary to determine the
ideal radiation dose and further investigate the effects of very low-dose radiation therapy on
inflammatory diseases
Preventing Childhood Sexual Abuse Related Mental Health Deterioration Using a Narrative Family Intervention in Burundi
Despite the severe impact of childhood sexual abuse (CSA) on trauma-related disorders, preventive interventions are scarce, especially in (post-)conflict regions. We developed and evaluated a narrative family communication approach for sexually abused Burundian female children and adolescents (N = 102). The intervention cohort (n = 55) received psychoeducation, parental skill training, and preventive narrative exposure therapy. Intervention participants reported improved parental acceptance at 3- and 12-month follow-ups (3mFUP, 12mFUP), and showed greater improvement in overall mental health between initial assessment (IA) and 3mFUP (drm = -0.70) and between IA and 12mFUP (drm = -1.36) compared to non-treated controls (drm = -0.33, drm = -0.02, respectively). Moreover, PTSD symptoms were significantly less pronounced in the intervention cohort than in the control cohort at both follow-ups. Our findings suggest that the preventive narrative family intervention might effectively buffer against devastating mental health consequences, including emerging PTSD symptoms, in the aftermath of CSA.
