    Geschlechtsunterschiede bei Gangparametern im Verlauf der Heilung von Frakturen des Unterschenkels und des oberen Sprunggelenks

    Trotz vieler Gemeinsamkeiten unterscheiden sich Männer und Frauen aufgrund genetischer, hormoneller und sozioökonomischer Differenzen in vielen für die medizinische Behandlung relevanten Feldern. In den letzten Jahren wurden Geschlechtsunterschiede in der Frakturheilung und im Schmerzempfinden bekannt, viele Themen sind in diesem Kontext aber noch unerforscht. Diese Unterschiede begründen sich unter anderem wahrscheinlich auf der verschiedenartigen Ausprägung von Hormonen, aber auch Faktoren wie das Körpergewicht spielen eine Rolle. Bezüglich der Unterschiede in der Frakturheilung bei Männern und Frauen ist jedoch noch vieles unbekannt. Weitere Forschung in diesem Bereich ist wünschenswert, um mögliche Konsequenzen für Diagnostik und Therapien nicht zu übersehen und allen den bestmöglichen Therapiestandard anbieten zu können. Ziel dieser Studie ist es, zu untersuchen, ob während der Heilung von Frakturen des Unterschenkels und des oberen Sprunggelenkes (OSG) Geschlechtsunterschiede im Gangbild bestehen, und ob sich der Rehabilitationsverlauf zwischen Frauen und Männern unterscheidet. Die vorliegenden Ergebnisse lassen potenziell in der Zukunft die Entwicklung neuer individueller Therapiemöglichkeiten sowie eine adäquate Interpretation von Daten aus Ganganalysen, insbesondere bei prognostischer Anwendung, erwarten. Die in dieser Arbeit identifizierten Geschlechtsunterschiede sollten berücksichtigt werden, wenn Ganganalysen im Heilungsverlauf in der Klinik oder Forschung eingesetzt werden. Die Datenanalyse und Interpretation müssen aufgrund der Unterschiede geschlechtsspezifisch erfolgen. Für diese Studie wurden insgesamt 44 Patienten mit Frakturen des Unterschenkels aus der Klinik für Unfall-, Hand- und Wiederherstellungschirurgie des Universitätsklinikum des Saarlandes eingeschlossen (25 Männer und 19 Frauen). Mithilfe eines 3D Kamerasystems und mit Sensoren ausgestatteten Einlegesohlen wurden bei jedem Patienten Ganganalysen durchgeführt. Die dabei erhobenen kinematischen und kinetischen Gangparameter umfassen die Bewegungsabläufe sowie die Bodenreaktionskräfte. Die Messungen wurden sechs Wochen, drei Monate sowie sechs Monate postoperativ durchgeführt. Dabei erfolgte bei jeder Messung zusätzlich die Erfassung der Schmerzintensität mittels Fragebögen. Die Datenanalyse erfolgte nach Abschluss der Messungen. Die Ergebnisse der Studie zeigten, dass die Ganggeschwindigkeit bei Männern zu Beginn der Untersuchung höher lag als bei Frauen und im weiteren Verlauf noch weiter ansteigt. Bei Frauen konnte eine ähnliche Steigung beobachtet werden, die jedoch mit einer generell niedrigeren Ganggeschwindigkeit einherging. Unterschiede in der Heilungsgeschwindigkeit zwischen Männern und Frauen konnten anhand der Gangparameter nicht festgestellt werden. Ebenso zeigte das Schmerzempfinden keinen geschlechtsspezifischen Unterschied. Zusammenfassend spielt das Geschlecht eine Rolle im Gangbild während der Heilung von Frakturen des Unterschenkels und des OSGs und beeinflusst den Heilungsverlauf. Die in dieser Arbeit identifizierten Geschlechtsunterschiede sollten berücksichtigt werden, wenn Ganganalysen im Heilungsverlauf in der Klinik oder Forschung eingesetzt werden. Die Datenanalyse und Interpretation müssen aufgrund der Unterschiede geschlechtsspezifisch erfolgen.Despite many similarities, men and women differ in many areas relevant to medical treatment due to genetic, hormonal and socio-economic differences. In recent years, gender differences in fracture healing and pain perception have become known, but many topics are still unexplored in this context. These differences are probably due to the different expression of hormones, among other things, but factors such as body weight also play a role. However, much is still unknown about the differences in fracture healing in men and women. Further research in this area is desirable in order not to overlook possible consequences for diagnostics and therapies and to be able to offer everyone the best possible standard of treatment. The aim of this study is to ascertain whether there are gender differences in gait during the healing of fractures of the lower leg and upper ankle joint and to determine whether the rehabilitation process differs between men and women. The results of this study could potentially lead to the development of new individualised treatment options in the future as well as an adequate interpretation of data from gait analyses, particularly when used for prognostic purposes. The gender differences identified in this study should be taken into account when gait analyses are used in clinical or research settings during the healing process. Data analysis and interpretation must be gender-specific due to these differences. A total of 44 patients with fractures of the lower leg from the Department of Trauma, Hand and Reconstructive Surgery at Saarland University Hospital were included in this study (25 men and 19 women). Each patient underwent gait analysis using a three-dimensional camera system and insoles equipped with sensors. The kinematic and kinetic gait parameters recorded included the movement sequences and ground reaction forces. The measurements were taken six weeks, three months and six months postoperatively. The intensity of pain was also recorded using questionnaires for each measurement. The data was analysed after completion of the measurements The results of the study indicated that gait speed was higher in men than in women at the outset of the study and continued to increase as the study progressed. A comparable increase was observed in women, although gait speed was generally lower. The gait parameters did not permit the determination of differences in healing speed between men and women. Similarly, there was no gender-specific difference in pain perception. In conclusion, gender is a factor in the gait pattern during the healing of fractures of the lower leg and the ankle joint and exerts an influence on the healing process. The gender differences identified in this paper must be considered when gait analyses are conducted in clinical or research settings during the healing process.     Veränderungen der plantaren Druckverteilung im Stand im Verlauf der Heilung von Frakturen des Unterschenkels und des oberen Sprunggelenks

    Nach einer Fraktur des oberen Sprunggelenks, des Tibiaschaftes oder des Tibiakopfes verändern Patienten schmerz- und therapiebedingt ihr Geh- und Stehverhalten. Auch die plantare Druckverteilung im Gehen scheint sich aktuellen Studien zufolge bei diesen Patienten zu ändern. Bisher wurden Änderungen der plantaren Druckverteilung im Stehen im Heilungsverlauf noch nicht untersucht. Womöglich könnten diese jedoch einen diagnostischen und prognostischen Wert haben. Ziel dieser Studie war es deshalb, die Änderung der plantaren Druckverteilung im Stehen im Heilungsverlauf nach Frakturen des Unterschenkels und des oberen Sprunggelenks zu untersuchen. Hierfür wurden mittels Pedographie (instrumentierte Einlegesohlen mit je 16 Drucksensoren) verschiedene plantare Parameter prospektiv longitudinal bei 61 Patienten und 44 Probanden im zehnsekündigen Stand erhoben. Unterschiede wurden bei Patienten im Vergleich zu gesunden Probanden und zwischen den Frakturtypen untersucht. Die Messung wurde im Heilungsverlauf erstmals innerhalb von vierzehn Tagen nach der operativen Versorgung (V1) durchgeführt und anschließend 30 bis 60 Tage (V2), 70 bis 110 Tage (V3) und 130 bis 230 (V4) Tage nach der Operation wiederholt. Es wurden hierbei die folgenden Parameter analysiert: Die Kraft auf der Fußsohle in % des Körpergewichts, die Belastung des Vorfußes (Zehen), des Mittelfußes, des Rückfußes (Ferse), des Vorfußes, des Rückfußes/dorsalen Stützstrahles, des lateralen Stützstrahles, des medialen Stützstrahles, der lateralen Seite, der medialen Seite, des Metatarsus I, des Metatarsus II, des Metatarsus III, des Metatarsus IV und des Metatarsus V jeweils in Prozent der Belastung des gesamten Fußes und die Lage des Center of pressure (Cop) in anteroposteriorer und mediolateraler Richtung. Zusätzlich wurde über Schmerzfragebögen der Zusammenhang zwischen dem Schmerz und der Kraftbelastung des verletzten Beines untersucht. Die wichtigsten Ergebnisse der Studie sind: 1. In den Zeiträumen zwischen V1 und V2, sowie zwischen V2 und V3 stieg die Kraft auf dem verletzten Bein signifikant an. Zwischen V3 und V4 erfolgte kein weiterer Kraftanstieg. 2. Im Zeitraum V4 zeigten die Patienten eine höhere Belastung der lateralen Fußsohle, während bei den gesunden Probanden eine signifikant höhere Belastung der medialen Seite bestand (p = 0,024). Passend hierzu lag das Cop bei den Patienten weiter lateral als bei gesunden Probanden (p = 0,048). 3. Im Zeitraum V2 belasteten Patienten mit einer Tibiakopffraktur den Vorfuß signifikant mehr als Patienten mit einer oberen Sprunggelenks- oder Tibiaschaftfraktur (p = 0,002). Dementsprechend wurde der Rückfuß bei Patienten mit Tibiakopffrakturen in diesem Zeitraum signifikant weniger belastet als bei den anderen beiden Patientengruppen (p = 0,002). Auch die Belastung des medialen Stützstrahles war in diesem Zeitraum bei Patienten mit einer Tibiakopffraktur signifikant höher als bei Patienten mit Tibiaschaft- oder Sprunggelenksfrakturen (p = <0,001). 4. Im Zeitraum V3 lag das Cop in mediolateraler Richtung bei Patienten mit Tibiaschaftfrakturen signifikant weiter lateral als bei Patienten mit Sprunggelenksfrakturen (p = 0,049). 5. Im zeitlichen Verlauf zwischen V1 und V2 nahm bei Patienten mit einer Tibiakopffraktur die Belastung des medialen Stützstrahls signifikant mehr zu als bei Patienten mit einer Fraktur des oberen Sprunggelenks (p = 0,047). Auch die Belastung des Metatarsus I stieg bei Patienten mit einer Tibiakopffraktur signifikant mehr als bei Patienten mit einer Sprunggelenksfraktur (p = 0,003). Die Lage des Cops in anteroposteriorer Richtung veränderte sich in diesem Zeitraum bei Patienten mit Tibiakopffrakturen signifikant weiter nach anterior als bei Patienten mit Sprunggelenksfrakturen (p = 0,008). 6. Zwischen dem empfundenen Schmerz und der Kraftbelastung des verletzten Beines bestand im zeitlichen Verlauf ein geringer negativer Zusammenhang (p = 0,141). Zusammenfassend lässt sich schlussfolgern, dass Frakturen des oberen Sprunggelenks, des Tibiaschaftes oder des Tibiakopfes die plantare Druckverteilung im Stehen charakteristisch beeinflussen. Messungen der Druckverteilung sind in der Klinik schnell und einfach durchführbar und können, wie die Ergebnisse dieser Studie erstmals zeigen, Veränderungen im Heilungsverlauf darstellen.After a fracture of the upper ankle joint, the tibial shaft or the tibial plateau, patients change their walking and standing behavior due to pain and therapy. According to recent studies, the plantar pressure distribution during walking also appears to change in these patients. Changes in plantar pressure distribution during standing have not yet been investigated during the healing process. However, these could possibly have diagnostic and prognostic value. The aim of this study was therefore to investigate the change in plantar pressure distribution during standing in the course of healing after fractures of the lower leg and upper ankle joint. For this purpose, pedography (instrumented insoles with 16 pressure sensors each) was used to prospectively record various plantar parameters over time in 61 patients and 44 test subjects in a ten-second stance. Differences were investigated in patients compared to healthy subjects and between fracture types. The measurements were taken for the first time during the healing process within fourteen days after surgical treatment and then repeated 30 to 60 days, 70 to 110 days and 130 to 230 days after the operation. The following parameters were analyzed: The force on the sole of the foot in % of the body weight, the load on the forefoot (toes), the midfoot, the hindfoot (heel), the forefoot, the hindfoot/dorsal support beam, the lateral support beam, the medial support beam, the lateral side, the medial side, metatarsus I, metatarsus II, metatarsus III, metatarsus IV and metatarsus V, each as a percentage of the load on the entire foot and the position of the center of pressure in the anteroposterior and mediolateral directions. In addition, pain questionnaires were used to investigate the relationship between the pain and the force load on the injured leg. This study is the first to investigate changes in plantar pressure distribution after fractures of the lower leg and upper ankle joint in patients. The most important results of the study are: 1. in the periods between V1 (0 to 14 days postoperatively) and V2 (30 to 60 days postoperatively), and between V2 and V3 (70 to 110 days postoperatively), the force on the injured leg increased significantly. There was no further increase in strength between V3 and V4 (130 to 230 days postoperatively). 2. in the period V4 (130 to 230 days postoperatively), the patients showed a higher load on the lateral sole of the foot, while the healthy subjects had a significantly higher load on the medial side (p = 0.024). In line with this, the Cop was further lateral in the patients than in healthy subjects (p = 0.048). 3. in period V2 (30 to 60 days postoperatively), patients with a tibial plateau fracture loaded the forefoot significantly more than patients with an upper ankle or tibial shaft fracture (p = 0.002). Accordingly, the hindfoot was loaded significantly less in patients with tibial plateau fractures during this period than in the other two patient groups (p = 0.002). The load on the medial support beam was also significantly higher in patients with a tibial plateau fracture than in patients with tibial shaft or ankle fractures during this period (p = <0.001). 4. in period V3 (70 to 110 days postoperatively), the center of pressure in the mediolateral direction was significantly more lateral in patients with tibial shaft fractures than in patients with ankle fractures (p = 0.049). 5. in the time course between V1 (0 to 14 days postoperatively) and V2 (30 to 60 days postoperatively), the load on the medial support beam increased significantly more in patients with a tibial plateau fracture than in patients with a fracture of the upper ankle joint (p = 0.047). The load on metatarsus I also increased significantly more in patients with a tibial plateau fracture than in patients with an ankle fracture (p = 0.003). The position of the center of pressure in the anteroposterior direction changed significantly more anteriorly in patients with tibial plateau fractures than in patients with ankle fractures (p = 0.008). 6. there was a small negative correlation between the perceived pain and the force load on the injured leg over time (p = 0.141). In summary, it can be concluded that fractures of the upper ankle joint, the tibial shaft or the tibial plateau characteristically influence the plantar pressure distribution during standing. Pressure distribution measurements can be carried out quickly and easily in the clinic and, as the results of this study show for the first time, can show clear changes in the healing process

    Clinical Outcomes of Minced Cartilage Treatment (AutoCart™) for Medial Osteochondral Lesions of the Talus: A Prospective One-Year Follow-Up Study

    Background/Objectives: This prospective study aims to assess the clinical outcomes of the AutoCart™ technique for the treatment of medial osteochondral lesions of the talus (OLT). Methods: 29 consecutive patients treated for medial OLT were included. Demographic char acteristics and preoperative imaging (MRI and CT) were reviewed, and patient-reported outcome measures (PROMs)—including a Visual Analog Pain Scale (VAS), Foot Function In dex (FFI), Foot and Ankle Ability Measure (FAAM), and the Veterans RAND 12-Item Health Survey (VR-12)—were assessed preoperatively and at 6 and 12 months post-treatment. Results: In the cohort, 14 (48%) were female, 13 (45%) were male, and 2 (7%) did not disclose their gender. Median age was 35.5 years (interquartile range: 23.0–49.5). Mean defect size was 121.95 ± 84.46 mm2 . Three patients were treated entirely arthroscopically, while 26 patients underwent medial malleolar osteotomy with cancellous bone grafting from the calcaneus for cartilage fragment placement. At one-year follow-up, there were significant improvements in pain and functional outcomes. The VAS score showed a mean reduction of 1.3 points (95% CI: −2.6 to −0.1; p = 0.036). Strong improvement was observed in the FFI, with a mean reduction of 13.3 points (95% CI: −21.0 to −5.6; p = 0.001). The FAAM Sports subscale showed a significant increase of 18.6 points (95% CI: 7.0 to 30.1; p = 0.002). Conclusions: Patients demonstrate clinical improvement after minced cartilage implantation with the AutoCart™ technique. These findings suggest that the minced cartilage procedure is a viable treatment option for medial OLTs, though further studies are needed to assess long-term efficacy

    Deceptive attacks in modern web

    Deception on the Web remains a persistent threat, as attackers increasingly exploit not only psychological and visual manipulation but also the technological complexity of modern platforms. Despite growing attention to phishing and malware, there is limited understanding of how emerging technologies enable new, harder-to-detect forms of deception--posing challenges for both detection and remediation. In this thesis, we examine how recent web advancements, specifically browser capabilities and social platforms, can be exploited to mount deception-based attacks, and analyze the operational challenges in mitigating them. We first study link previews on 20 social platforms, uncovering how inconsistencies in preview generation and a lack of safeguards allow attackers to craft and distribute benign-looking previews for malicious links. We then investigate clickbait PDFs, documents mimicking web content and leading to web attacks. Analyzing 176,208 real-world samples, we find that attackers primarily rely on SEO abuse and that most files evade antivirus detection. To understand how attackers maintain such campaigns, we monitor 4,648,939 PDF links hosted on 177,835 domains, revealing systemic infrastructure abuse. Finally, interviews with 24 hosting providers show that abuse remediation is often deprioritized, not due to technical limits but to misaligned business responsibilities, cost considerations, and the perceived lack of risk posed by compromised customer instances.Täuschung im Web bleibt eine anhaltende Bedrohung, da Angreifer*innen zunehmend nicht nur psychologische und visuelle Manipulationen nutzen, sondern auch die technologische Komplexität moderner Plattformen. Trotz wachsender Aufmerksamkeit für Phishing und Schadsoftware besteht nur ein begrenztes Verständnis dafür, wie neue Technologien neuartige, schwerer zu erkennende Formen der Täuschung ermöglichen--was sowohl die Erkennung als auch die Abwehr erschwert. In dieser Arbeit wird untersucht, wie aktuelle Entwicklungen im Web, insbesondere Browser-Funktionalitäten und soziale Plattformen, für täuschungsbasierte Angriffe ausgenutzt werden können, und die operativen Herausforderungen bei deren Eindämmung werden analysiert. Zunächst werden Link-Vorschauen auf 20 sozialen Plattformen untersucht und aufgezeigt, wie Inkonsistenzen bei der Generierung von Vorschauen und das Fehlen von Schutzmechanismen es Angreifer*innen ermöglichen, harmlos wirkende Vorschauen für schädliche Links zu erstellen und zu verbreiten. Anschließend werden Clickbait-PDF-Dokumente analysiert, die Webinhalte nachahmen und zu Webangriffen führen. Anhand von 176.208 realen Beispielen wird festgestellt, dass Angreifer*innen hauptsächlich auf den Missbrauch von SEO setzen und dass die meisten Dateien nicht von Antivirensoftware erkannt werden. Um zu verstehen, wie Angreifer*innen solche Kampagnen aufrechterhalten, werden 4.648.939 PDF-Links auf 177.835 Domains überwacht und systematischer Missbrauch von Infrastruktur aufgedeckt. Abschließend zeigen Interviews mit 24 Hosting-Anbietern, dass Missbrauchsbekämpfung oft nachrangig behandelt wird – nicht aufgrund technischer Beschränkungen, sondern wegen unklarer Zuständigkeiten, Kostenüberlegungen und der Wahrnehmung, dass kompromittierte Kundeninstanzen ein geringes Risiko darstellen

    Validierung des Bewertungsbogens für Point-of-Care Ultraschalluntersuchungen (VALPOCUS)

    Hintergrund und Zielsetzung: In den vergangenen Jahren hat die Point-of-Care Ultraschalluntersuchung (POCUS) im Bereich der Intensiv- und Notfallmedizin an Stellenwert gewonnen. Dabei gibt es zunehmend Evidenz für die Vorteile der POCUS-Anwendung. Beispielsweise wird die Zeit bis zur Diagnosestellung verkürzt und das Patientenoutcome verbessert. Die Ausbildungs- und Evaluationsstrukturen sind jedoch heterogen. Das Ziel der Arbeit war die Erstellung und Validierung eines Bewertungsbogens für POCUS-Anwender. Methodik: Nach einer Literaturrecherche wurde mittels Delphi-Methode ein Bewertungsbogen erstellt. Zur Validierung des Bewertungsbogens führten Ärzte und Studierende eine POCUS- Untersuchung durch. Diese Untersuchung wurde mit Hilfe des Bewertungsbogens und - davon unabhängig - durch Experten beurteilt. Diese Bewertungen wurden im Anschluss miteinander korreliert. Zur Bestimmung der Objektivität des erstellten Bewertungsbogens wurde die Inter- Rater-Reliabilität zwischen zwei unabhängigen Bewertern erhoben. Ergebnisse: Im Rahmen der Delphi-Umfrage konnte für zwölf Bewertungskriterien ein Konsens erreicht werden. Insgesamt wurden 70 Point-of-Care Untersuchungen eingeschlossen. Davon wurden 19 Untersuchungen durch Ärzte und 51 durch Studierende durchgeführt. Für den Bewertungsbogen konnte eine Inter-Rater-Reliabilität nach Cohens Kappa von 0.778 (linear gewichtet, SEM 0.37, p < 0.001) und nach Krippendorff‘s alpha von 0.895 gezeigt werden. Die Mittelwerte der Ergebnisse des adaptierten Bewertungsbogens und der Expertenbewertungen differierten um 2.6% (46.9% Bogen vs. 44.2% Experten). Die Korrelation zwischen den beiden Bewertungsmethoden betrug nach Pearson 0.691 (p < 0.001). Schlussfolgerung: Für den Bewertungsbogen konnte eine gute Validität und Objektivität demonstriert werden. Die Validität spiegelt sich in der starken Korrelation der Bewertungen des Bogens mit den Expertenbewertungen wider. Darüber hinaus unterschieden sich die Ergebnisse des Bewerbungsbogens und der Experten nur geringfügig. Die hohe Inter-Rater- Reliabilität weist auf eine objektive Bewertung hin. Ob der Bogen auf andere POCUS- Protokolle und Szenarien übertragbar ist, muss in weiteren Studien untersucht werden. Der Bewertungsbogen stellt eine gute Möglichkeit dar, Point-of-Care Ultraschall Anwender zu bewerten und könnte beispielsweise in der POCUS Aus- und Fortbildung eingesetzt werden.Title: Validation of the Assessment Tool for Point-of-Care Ultrasound (VALPOCUS) Background and Objective: In recent years, point-of-care ultrasound (POCUS) has gained increasing importance in intensive care and emergency medicine. There is growing evidence supporting the benefits of POCUS application, such as a reduced time to diagnosis and improved patient outcomes. However, training and evaluation structures remain heterogeneous. The aim of this study was to develop and validate an assessment tool for POCUS users. Methods: Following a literature review, a Delphi-method was used to develop an assessment tool. To validate the tool, physicians and medical students performed a POCUS-examination. These examinations were evaluated using the newly developed assessment tool and, independently, by expert reviewers. The correlation between these evaluations was afterwards analyzed. To determine the objectivity of the assessment tool, the inter-rater reliability between two independent evaluators was assessed. Results: Consensus was reached for twelve evaluation criteria in the Delphi-survey. A total of 70 point-of-care ultrasound examinations were included, with 19 performed by physicians and 51 by medical students. The assessment tool demonstrated an inter-rater reliability of 0.778 (linearly weighted Cohen’s kappa, SEM 0.37, p < 0.001) and a Krippendorff’s alpha of 0.895. The mean scores obtained using the adapted assessment tool and those given by experts differed by 2.6% (46.9% assessment tool vs. 44.2% experts). The correlation between both evaluation methods was 0.691 (Pearson, p < 0.001). Conclusion: The assessment tool demonstrated good validity and objectivity. The strong correlation between the assessment tool scores and expert evaluations supports its validity. Moreover, the results from the assessment tool and expert ratings showed only minor differences. The high inter-rater reliability suggests an objective evaluation process. Further studies are needed to determine whether the tool can be applied to other POCUS protocols and scenarios. The assessment tool provides a valuable opportunity to evaluate POCUS users and could be implemented in POCUS training and continuing education programs

    TRAP mediated conformational changes of the human Sec61 channel

    The integral membrane pore Sec61 catalyzes the translocation of many secretory precursor proteins into the endoplasmic reticulum, as well as the insertion of transmembrane proteins into the cell and organellar membranes. Precursor proteins that possess weak signal peptides frequently require presence of the accessory membrane protein TRAP. Structural biology has recently established that TRAP shares several contact sites with Sec61, though not by an extended binding interface. However, how TRAP mechanistically supports the translocation of precursor proteins is still partially unresolved.     Alterations of Photoreceptor Synaptic Ribbons in the Retina of a Human Patient With Oculocutaneous Albinism Type 1 (OCA1)

    PURPOSE. Albino (Tyrc-2J/Tyrc-2J) C57BL/6J mice carry a mutation in the tyrosinase gene and are known to display alterations of photoreceptor synaptic ribbons. In the present study, we wanted to test whether similar alterations exist in oculocutaneous albinism type 1 (OCA1), a human disease that also results from mutations in the tyrosinase gene. METHODS. In the present study, we assessed the morphology of a human OCA1 retina in comparison to control human retinas. We analyzed the retina of a 35-year-old OCA1 patient by immunolabeling at light and electron microscopic levels, conventional trans mission electron microscopy, and by genomic DNA sequencing of the RIBEYE/CtBP2 gene in comparison to normal human controls. RESULTS. The morphological analyses revealed an overall surprisingly normal appear ance of the retina, except for the presence of strikingly abnormal photoreceptor synaptic ribbons. Synaptic ribbons are presynaptic specializations of the continuously active reti nal ribbon synapses and mainly consist of the RIBEYE protein. In the OCA1 patient, photoreceptor synaptic ribbons were very small and reduced to small fragments that were either still associated with the active zone transmitter release site or floating in the cytosol. The RIBEYE gene appeared to be unaltered in the OCA1 patient, except for some single nucleotide polymorphisms (SNPs) that were also present in controls. CONCLUSIONS. The OCA1 patients displayed similar defects of photoreceptor synaptic ribbons as previously observed in the albinotic mice with a defect in the tyrosinase gene. The observed alterations of synaptic ribbons are not due to mutations in the RIBEYE gene but are likely indirect consequences of the deficient melanin biosynthesis in the OCA1 patient

    Toward the development of a specific non-enzymatic amperometric sensor for determining uric acid in fermentation samples

    The development is proposed of a specific non-enzymatic amperometric sensor based on electrodeposited copper nanoparticles (Cu-NPs) for the determination of uric acid (UA) in fermentation samples. Through optimization of the Cu-NPs-containing sensing layer, it was demonstrated that copper(II)-induced oxidation (catalytic effect) in the presence of molecular oxygen is more effective for determining UA than the adsorption of UA on Cu and Cu-oxide surfaces. More importantly, simply changing the sensing layer's surface chemistry by increasing the defect CuxOy on the surface of Cu-NPs after heating at 70 °C for only 20 min significantly improved the specificity of UA determination in both model and real fermentation samples (viz. supernatants of S. cerevisiae and E. coli). This study can be used as a guideline for the future assembly of functional electrodeposited sensing layers for the specific determination of target electroactive bioanalyte(s)

    Quantitative mass spectrometry-based (chemo-) proteomics for the characterization of MEP pathway inhibitors

    A critical bottleneck in early drug discovery of new agents for both bacterial and malarial pathogens is the challenge of accurately identifying protein targets, confirming target–engagement, and evaluating selectivity profiles. To that end, quantitative mass spectrometry-based proteomics (LC-MS/MS) is a powerful tool for acquiring comprehensive and unbiased information on protein expression and protein–drug interactions on a proteome-wide scale. As part of the MepAnti research program, global proteomic profiling was conducted on six human pathogens (Mycobacterium tuberculosis, Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa, Staphylococcus aureus, and Plasmodium falciparum) to assess the detectability and abundance of the 2-C-methyl-D-erythritol 4-phosphate (MEP) pathway enzymes using LC-MS/MS. Subsequently, compound–protein interactions were characterized across multiple chemical classes developed within the MepAnti Consortium. Initially, we employed conventional probe-based chemoproteomics. However, this method can present challenges, as the protein targets may not be effectively enriched due to potential losses in activity and selectivity during the probe-design process. To overcome these limitations, integral solvent-induced protein precipitation (iSPP) was evaluated and subsequently employed. This biophysical proteomics approach measures changes in protein stability in response to solvent-induced precipitation in the presence of ligands, facilitating the identification of compound–protein targets. iSPP is a modification-free approach, eliminating the need for compound functionalization, and is suitable for application to hard-to-culture pathogens characterized by slow growth rates and limited protein yields. Overall, this thesis underscores the pivotal role of chemo- and biophysical proteomics in advancing early-stage drug discovery for anti-infective agents.Ein entscheidender Engpass in der frühen Wirkstoffentwicklung für bakterielle und malariabedingte Krankheitserreger ist die präzise Identifizierung von Protein-Zielen, die Bestätigung der Zielbindung (Target Engagement) und die Bewertung der Selektivitätsprofile. Die quantitative, massenspektrometrie-basierte Proteomik (LC-MS/MS) ist ein effizientes Werkzeug, um umfassende und unverfälschte Informationen über Proteinexpression und Protein-Wirkstoff-Interaktionen auf proteomweiter Ebene zu gewinnen. Im Rahmen des MepAnti-Forschungsprogramms wurde eine globale Proteomprofilierung an sechs humanpathogenen Organismen (Mycobacterium tuberculosis, Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa, Staphylococcus aureus und Plasmodium falciparum) durchgeführt, um die Nachweisbarkeit und Häufigkeit der Enzyme des 2-C-Methyl-D-Erythritol-4-phosphat (MEP)-Stoffwechsels mittels LC-MS/MS zu bewerten. Im Anschluss daran wurden Wirkstoff-Protein-Interaktionen über mehrere chemische Klassen hinweg charakterisiert, die innerhalb des MepAnti-Konsortiums entwickelt wurden. Zunächst wurde die klassische, auf Sonden basierende Chemoproteomik eingesetzt. Allerdings können bei dieser Methode Herausforderungen auftreten, da die Zielproteine aufgrund möglicher Aktivitäts- und Selektivitätsverluste während des Sondendesigns nicht effektiv angereichert werden könnten. Um diese Einschränkungen zu überwinden, wurde die integral solvent-induced protein precipitation (iSPP) evaluiert und anschließend eingesetzt. Dieser biophysikalische proteomische Ansatz misst Änderungen in der Protein-Stabilität als Reaktion auf lösungsmittelinduzierte Präzipitation in Anwesenheit von Liganden und erleichtert die Identifizierung von Wirkstoff-Protein-Zielen. iSPP ist eine modifikationsfreie Methode, die keine Wirkstofffunktionalisierung erfordert und sich für schwer zu kultivierende Krankheitserreger mit langsamen Wachstumsraten und begrenzten Proteinausbeuten eignet.
