    Thoracic CT Angiographies in Children Using Automated Power Injection with Bolus Tracking Versus Manual Contrast Injection: Analysis of Contrast Enhancement, Image Quality and Radiation Exposure

    Objectives: The purpose of this study was to analyze image quality and radiation exposure of thoracic computed tomography angiography (CTA) in children with congenital heart diseases (CHDs) using either manual contrast medium (CM) injection or automated power injectors with bolus tracking. Methods: A total of 137 thoracic CTAs of 120 consecutive pediatric patients were included in this retrospective study. We analyzed the method of CM administration (power injection with bolus tracking (PI) or manual injection (MI)), injection routes, volumes and flow rates of CM. For the evaluation of objective image quality, attenuation values in the heart chambers and great thoracic vessels were determined by region-of-interest (ROI) analysis and signal-to-noise (SNR) and contrast-to-noise (CNR) ratios calculated thereof. Visual image quality was assessed by two blinded readers (four-point Likert-scale) analyzing the presence of artifacts and the depiction of relevant anatomical structures. Effective radiation doses were calculated with dose length products and specific conversion factors. Results: CM administration was performed using PI in 119/137 CTAs, whereas MI was conducted in 18/137. The smallest size of peripheral venous cannulas was 24 gauge in 36/137 (26.3%) cases. Overall mean CM volume was 17 mL ± 16 mL (mean ± SD). In PI, the mean flow rate of CM was 1.52 ± 0.90 mL/s with a range between 0.5 and 5.0 mL/s. When comparing the overall PI population and an age-, size- and weight-matched PI subpopulation (18 cases) with the MI population, attenuation values in Hounsfield units (HU) and CNR values were significantly higher in the PI groups than in the MI group for each relevant cardiac structure (left ventricle, right ventricle, ascending aorta and pulmonary trunk, p = 0.02–0.001). Overall image quality and depiction of cardiac structures were rated significantly better in CTAs with PI (interquartile ranges: “good” to “excellent” (Likert 3–4)) in PI compared with CTAs acquired with MI (interquartile ranges: “fair” to “good” (2–3)) in MI by both readers (p < 0.001). The inter-observer reliability was strong, with a Kendall’s Tau-b correlation coefficient of τ = 0.802 (p < 0.001). The mean effective radiation dose (E) did not differ significantly when comparing the stratified samples (i.e., the matched PI subgroup and the MI group; 0.5 (±0.3) mSv in both, p = 0.76). There were no complications associated with the CM injections for both application approaches. Conclusions: Automated contrast agent applications with power injectors and bolus tracking ensure better image quality in pediatric CTA, even when low volumes and flow rates need to be applied. There is a slight increase in radiation associated with bolus tracking. This approach represents a suitable imaging technique for the work-up of congenital heart disease

    An Authentic Al=Si Double Bond

    Aluminatasilenes are the elusive heavier counterparts of the well-known borataalkenes in which the B=C double bond is formally replaced by an Al=Si moiety. We report the isolation of a four-membered anionic AlSi3 cycle with a bona fide Al=Si double bond, which is obtained by the reaction of 2 eq. of disilenide Tip2Si=SiTipLi with H2AlCl (Tip=2,4,6-triisopropylphenyl). X-ray crystallography, UV/Vis spectroscopy and computational analysis confirm the double bond character with a very short Al−Si bond of 2.283(1) Å and high Wiberg bond index (WBI) of 1.31. Intramolecular C−H activation by the Al=Si bond and the reactions with sulfur sources confirm typical double bond reactivity

    Student Reactions to Just-in-Time Formative and Summative Feedback in a Tablet-Based Family Medicine MCQ Exam

    (1) Background: While the benefits of digital assessments for universities and educators are well documented, students’ perspectives remain underexplored. (2) Methods: This study employed an exploratory mixed-methods approach. Three examination cohorts were included (winter semester 2022/23, summer semester 2023, and winter semester 2023/24). Written emotional responses to receiving just-in-time summative or formative feedback were analyzed, as well as examining the impact of formative feedback on learning attitudes. All cohorts completed qualitative open-ended research questions. The responses were coded using Kuckartz’s qualitative content analysis. Descriptive statistics were generated using jamovi. (3) Results: Students generally responded positively to formative and summative feedback. The majority expressed a desire to receive feedback. The categories created for formative feedback indicate a tendency toward self-reflection and supported the learning processes. In contrast, the summative feedback categories suggest that students primarily value feedback’s transactional aspect. (4) Conclusions: Integrating formative and summative feedback in digital just-in-time assessment offers the potential to capitalize on the “sensitive periods” of study reflection that occur during assessment. This approach enables assessment for learning while simultaneously reducing emotional distress for students

    Femtosecond Laser-Assisted Donor and Recipient Preparation for Bowman Layer Transplantation

    Objectives: A Ziemer LDV Z8 femtosecond laser (FSL) was used to obtain optimal cutting parameters with precise settings for donor and recipient preparations for Bowman layer transplantation. Methods: Of 48 human research corneas examined, 32 were used for Bowman layer preparation (donor) and 16 for pocket preparation (recipient) using the LDV Z8 FSL. The cutting thickness of the Bowman layer, pocket depth, and corresponding laser settings were varied. The quality of sections was evaluated based on the occurrence of adhesions, bridges, or perforations. Histological specimens were prepared and analyzed. Results: Preparation of the Bowman layer and recipient pocket was possible using all selected settings. The thinner the Bowman layer and the more superficial the pocket preparation, the higher the risk of perforation was. Considering the fact that the Bowman layer was cut as thinly as possible, a Bowman layer thickness of 30 µm showed a 100% success rate. Bowman layers cut at 25 µm had a lower success rate (50%). The pocket depth of 150 µm showed a 100% success rate in the preparation. Histological processing revealed smooth, precisely cut edges of Bowman layers and pockets. Implantation into the pocket was successful in all cases. Conclusions: Both Bowman layer and pocket preparation were technically and surgically feasible using the LDV Z8 FSL, and the prepared Bowman layers were thinner than those reported in previous studies. The optimal Bowman layer thickness was 30 µm, and a resection depth of 150 µm was used to prepare the pockets safely

    Nanoparticle interactions with mucosal barriers

    Human pulmonary mucus represents a tenacious barrier for pulmonary drug administration. The design of an efficient inhalation therapy demands a detailed understanding of the mucus layer and well characterized mucus models for the investigation of the behavior of therapeutics in mucus. The developed strategies used to circumvent the mucus barrier should be evaluated in terms of efficacy and safety. The application of various microscopy techniques provided new insights revealing a non-porous, heterogenous and composite structure of mucus. Calu-3 in vitro mucus was characterized regarding mucins as macromolecules, small metabolites, biophysical properties and compared to human pulmonary ex vivo mucus providing valuable input in terms of the interpretation of data obtained using these models. Calu-3 in vitro mucus was employed for the investigation of the impact of mucus modulation, as a mucus penetration enhancing strategy on the protective function of mucus. Cytotoxicity assessment of the impact of probing model particles revealed that modulation led to a reduced protective function of mucus emphasizing the importance of safety assessment of therapy protocols involving mucus modulation.Der menschliche pulmonale Mukus stellt eine Barriere für die Verabreichung von Medikamenten in die Lunge dar. Die Entwicklung einer effizienten Inhalationstherapie erfordert ein detailliertes Verständnis der Mukusschicht und gut charakterisierte Mukusmodelle zur Untersuchung des Verhaltens von Therapeutika im Mukus. Die entwickelten Strategien zur Umgehung der Mukusbarriere sollten im Hinblick auf ihre Wirksamkeit und Sicherheit bewertet werden. Die Mukus-Struktur wurde mittels verschiedener Mikroskopietechniken untersucht. Die Ergebnisse zeigten eine nicht poröse, heterogene Komposit-Struktur des Mukus. Calu-3 in vitro Mukus wurde in Bezug auf Muzine als Makromoleküle, kleine Metabolite und biophysikalische Eigenschaften charakterisiert und mit menschlichem pulmonalem ex vivo Mukus verglichen. Diese Untersuchung lieferte wertvolle Erkenntnisse für die Interpretation der mit diesen Modellen gewonnenen Daten. Calu-3 in vitro Mukus wurde verwendet, um den Einfluss der Mukusmodulation als Strategie zur Verbesserung der Mukuspenetration auf die Schutzfunktion des Mukus zu untersuchen. Die Zytotoxizitätsanalyse der Auswirkung von Modellpartikeln ergab, dass die Modulation zu einer verringerten Schutzfunktion des Mukus führte. Diese Ergebnisse betonen die Bedeutung von Sicherheitsuntersuchungen bei der Entwicklung der Therapieprotokolle, die eine Modulation des Mukus umfassen

    Development of biomimetic glycosaminoglycan-gelatine hydrogel systems for the controlled delivery of pro- and anti-angiogenic factors

    The demographic change in western countries leads to an increasing number of chronic wounds which require adequate health care as age and age-associated diseases, including diabetes, are major risk factors for their development. Chronic wounds are in a state of persistent inflammation which inhibits the proliferation and angiogenesis necessary for physiological wound healing. A promising approach to improve angiogenesis in chronic wounds is the application of bioactive wound dressings mimicking the extracellular matrix and releasing pro-angiogenic factors into the surrounding tissue. Hydrogels based on collagen and derivatives of naturally occurring glycosaminoglycans, including sulphated hyaluronan (sHA), showed promising results for the delivery of pro-angiogenic factors as well as limiting their rapid degradation in vivo. Furthermore, tissue inhibitor of matrix metalloproteinase 3 (TIMP-3) could limit the degradation of vascular endothelial growth factor A (VEGF-A) without significant inhibition of angiogenesis. However, the current collagen-based hydrogels are limited because of the chemical heterogeneity of collagen resulting in heterogenous hydrogel properties, whereas gelatine offers an alternative with homogenous chemical properties as well as the potential use in bioprinting. This thesis developed a biomimetic glycosaminoglycan-gelatine hydrogel system and examined it for its mechanical properties and for the controlled release of the pro-angiogenic factor VEGF-A and the inhibitor of matrix degradation TIMP-3. Hydrogels based on 10% methacrylated gelatine (GelMA) showed promising results regarding their mechanical stability and therefore were chosen to conduct further experiments. Compared to GelMA and GelMA/hyaluronan (HA) hydrogels, GelMA/sHA hydrogels showed an improved hydrogel stiffness and swelling which are necessary for a drug-delivery system. All hydrogel types were able to bind and release both VEGF-A and TIMP-3. Initially, all hydrogels bound similar amounts of VEGF-A and/or TIMP-3 but only GelMA/sHA hydrogels showed a constant release of VEGF-A and/or TIMP-3 for 21 days whereas both GelMA and GelMA/HA hydrogels rapidly released large amounts of VEGF-A and/or TIMP-3. The released VEGF-A and TIMP-3 was examined for its remaining bioactivity. VEGF-A released from GelMA/sHA hydrogels showed an increased VEGF receptor stimulation on KDR/NF AT-RE HEK293 cells compared to VEGF-A released from GelMA and GelMA/HA hydrogels. Similar results were found for TIMP-3 after release from GelMA/sHA hydrogels as it still showed inhibitory potential towards two major enzymes in extracellular matrix remodelling, collagenase and matrix metalloproteinase 9. In summary, the present thesis shows the potential of GelMA/sHA hydrogels for the controlled delivery of bioactive VEGF-A and TIMP-3 over a prolonged period. This highlights the promising potential of GelMA/sHA hydrogels as off-the-shelf bioactive wound dressings in future.Der demographische Wandel in westlichen Ländern führt zu einer Zunahme der Zahl chronischer Wunden. Diese benötigen eine angepasste Gesundheitsversorgung, da besonders das Alter sowie altersassoziierte Erkrankungen, z. B. Diabetes mellitus, Risikofaktoren für die Entstehung von chronischen Wunden sind. Chronische Wunden befinden sich in einem Zustand der dauerhaften Entzündung, die die für die physiologische Wundheilung notwendige Proliferation und Angiogenese inhibiert. Ein vielversprechender Ansatz zur Verbesserung der Angiogenese in chronischen Wunden ist die Verwendung von bioaktiven Wundauflagen, die die extrazelluläre Matrix nachahmen. Hydrogele, die auf Kollagen und Glykosaminoglykanderivaten, wie zum Beispiel sulfatiertes Hyaluron (sHA), basieren, zeigten vielversprechende Ergebnisse für die Freisetzung von pro-angiogenen Faktoren. Des Weiteren können bioaktive Wundauflagen pro-angiogene Faktoren in das umliegende Gewebe abgeben und den schnellen in vivo-Abbau dieser begrenzen. Weiterhin könnte der Gewebsinhibitor der Metalloproteinasen 3 (TIMP-3) den Abbau von vaskulärem endothelialem Wachstumsfaktor A (VEGF-A) einschränken ohne signifikante Inhibition der Angiogenese. Allerdings sind die aktuellen, Kollagen-basierten Hydrogele durch die heterogenen chemischen Eigenschaften des Kollagens und den daraus resultierenden heterogenen Hydrogeleigenschaften eingeschränkt. Als Alternative bietet Gelatine homogene chemische Eigenschaften und die mögliche Nutzung im Rahmen des Bioprintings. In dieser Arbeit wurde ein biomimetisches Glykosaminoglykan-Gelatine Hydrogel entwickelt und dessen mechanische Eigenschaften sowie die kontrollierte Freisetzung von dem pro-angiogenen VEGF-A und dem Inhibitor des Matrixabbaus TIMP-3 untersucht. Hydrogele basierend auf 10% methacrylierter Gelatine (GelMA) zeigten vielversprechende Ergebnisse hinsichtlich ihrer machanischen Stabilität, sodass mit diesen weitere Experimente durchgeführt wurden. GelMA/sHA Hydrogele zeigten verbesserte Steifheit und Schwellung verglichen mit GelMA und GelMA/Hyaluron (HA) Hydrogelen. Dabei handelt es sich um notwendige Eigenschaften für sog. „drug-delivery“-Systeme. Alle Hydrogelarten waren in der Lage VEGF-A sowie TIMP-3 zu binden und freizusetzen. Initial banden alle Hydrogele ähnliche Mengen an VEGF-A und/oder TIMP-3, aber nur GelMA/sHA Hydrogele zeigten eine konstante Freisetzung von VEGF-A und/oder TIMP-3 über 21 Tage, wohingegen die Freisetzung aus GelMA und GelMA/HA Hydrogelen schnell große Mengen an VEGF-A und/oder TIMP-3 freisetzten. Die verbleibende biologische Aktivität von freigesetztem VEGF-A und TIMP-3 wurde untersucht. VEGF-A zeigte nach seiner Freisetzung aus GelMA/sHA Hydrogelen, im Vergleich zu GelMA und GelMA/HA Hydrogelen, eine verstärkte Aktivierung des VEGF-Rezeptors von KDR/NF AT-RE HEK293 Zellen. Ähnliche Ergebnisse fanden sich für TIMP-3 nach der Freisetzung aus GelMA/sHA Hydrogelen. TIMP-3 zeigte weiterhin eine inhibierende Wirkung gegenüber zwei wichtigen Enzymen des Remodellings der extrazellulären Matrix, Kollagenase und Matrixmetalloprotease 9. Zusammenfassend zeigt die vorliegende Arbeit das Potential von GelMA/sHA Hydrogelen zur kontrollierten Freisetzung von bioaktivem VEGF-A und TIMP-3 über längere Zeit. GelMA/sHA Hydrogele haben das Potential zukünftig als bioaktive „off-the-shelf“-Wundauflagen angewandt zu werden

    Titel: Machbarkeit und Wirksamkeit der ICG-gesteuerten laparoskopischen Hepatektomie zur Reduzierung perioperativer Komplikationen und Verbesserung der onkologischen Ergebnisse Propensity Score-Matched Analyse aus Single-Center-Erfahrung Einleitung: Die laparoskopische Leberresektion (LLR) hat die Leberchirurgie grundlegend verändert, da sie gegenüber konventionellen offenen Verfahren zahlreiche Vorteile bietet. Allerdings weist die konventionelle LLR (K-LLR) bestimmte Einschränkungen auf, wie die fehlende Möglichkeit, Tumoren manuell zu ertasten, sowie die Notwendigkeit einer kontinuierlichen Ultraschallführung während der Operation, was mit einem erhöhten Risiko für Komplikationen einhergehen kann. Echtzeit-Tumor-Tracking-Technologien, wie die Indocyaningrün-(ICG)-Fluoreszenzbildgebung, haben sich als vielversprechend erwiesen, um die Tumorerkennung zu verbessern und die perioperativen Ergebnisse zu optimieren. Ziel der Studie: Diese Studie, bezeichnet als The Liver Light Study, untersucht die Machbarkeit und die perioperativen Ergebnisse der Kombination aus ICG-Fluoreszenzbildgebung und einem 4K-Overlay im Vergleich zur K-LLR. Dabei wird die Wirksamkeit der ICG4K-gesteuerten laparoskopischen Hepatektomie in Bezug auf die Reduzierung perioperativer Komplikationen, die Verbesserung onkologischer Ergebnisse sowie die Erreichung eines tumorfreien Resektionsrandes bei gleichzeitiger Schonung gesunden Lebergewebes bewertet. Methode: Diese Studie (The Liver Light Study) ist eine prospektive, monozentrische Studie, in die 42 Patienten eingeschlossen wurden, die sich einer Kombination von ICG-Fluoreszenzbildgebung mit einem 4K-Overlay unterzogen. Diese Patienten wurden im Verhältnis 1:1 mittels Propensity-Score-Matching (PSM) mit Patienten aus retrospektiven Daten verglichen, die eine K-LLR durchlaufen hatten. Potenzielle Störfaktoren wie Geschlecht, Alter, Body-Mass-Index, Tumortyp und die Komplexität der LLR wurden für das Matching ausgewählt. Anschließend wurden die postoperativen chirurgischen und onkologischen Ergebnisse zwischen den beiden Gruppen verglichen. Ergebnisse: Nach der PSM-Analyse zeigte die Gruppe, die mit einer Kombination aus ICG-Fluoreszenzbildgebung und einem 4K-Overlay operiert wurde, mehrere signifikante Vorteile gegenüber der Gruppe mit K-LLR. Die Operationszeiten waren in der ICG4K-LLR-Gruppe kürzer (155,2 Minuten vs. 196,6 Minuten, p=0,002), der Blutverlust war geringer (300,0 ml vs. 501,2 ml, p=0,006) und es gab weniger Bluttransfusionen (0 % vs. 14,3 %, p=0,011). Kein Patient in der ICG4K-LLR-Gruppe wies Tumor-positive Resektionsränder auf, während dies bei 14,3 % der Patienten in der K-LLR Gruppe der Fall war (p=0,011). Die Inzidenz von klinisch relevantem Posthepatektomie-Leberversagen (p=0,040) und schwerwiegenden Komplikationen (p=0,035) war in der ICG4K-LLR-Gruppe ebenfalls signifikant niedriger. Darüber hinaus war die einjährige krankheitsfreie Überlebensrate in der ICG4K-LLR-Gruppe deutlich höher (92,3 % vs. 64,3 %, p=0,004). Schlussfolgerung: Die Kombination aus ICG-Fluoreszenzbildgebung und einem 4K-Overlay in der laparoskopischen Leberresektion, ist eine sichere und effektive Navigationsmethode, die die perioperativen chirurgischen und onkologischen Ergebnisse im Vergleich zur K-LLR verbessert.2.2. Summary Title: Feasibility and Effectiveness of ICG-Guided Laparoscopic Hepatectomy in Reducing Perioperative Complications and Improving Oncological Outcomes: A Propensity Score-Matched Analysis from a Single-Center Experience. Introduction: Laparoscopic liver resection (LLR) has revolutionized liver surgery by offering several advantages over traditional open procedures. However, conventional LLR (K-LLR) has certain limitations, such as the inability to manually palpate tumors and the necessity of continuous ultrasound guidance during surgery, which can increase the risk of complications. Real-time tumor-tracking technologies, such as indocyanine green (ICG) fluorescence imaging, have proven to be promising in enhancing tumor detection and optimizing perioperative outcomes. Study Aim: This study, referred to as The Liver Light Study, investigates the feasibility and perioperative outcomes of combining ICG fluorescence imaging with a 4K overlay (ICG4K) compared to K-LLR. The effectiveness of ICG4K-LLR is evaluated in terms of reducing perioperative complications, improving oncological outcomes, and achieving tumor-free resection margins while preserving healthy liver tissue. Methods: This study (The Liver Light Study) is a prospective, single-center investigation that included 42 patients who underwent a combination of ICG fluorescence imaging with a 4K overlay. These patients were matched in a 1:1 ratio using propensity scores with patients from retrospective data who underwent K-LLR. Potential confounding factors, such as sex, age, body mass index, tumor type, and the complexity of laparoscopic liver resection, were selected for matching. Postoperative surgical and oncological outcomes were then compared between the two groups. Results: After Propensity score-matched analysis, the group operated with a combination of ICG fluorescence imaging with a 4K overlay demonstrated several significant advantages compared to the group undergoing K-LLR. Operation times were shorter in the ICG4K-LLR group (155.2 minutes vs. 196.6 minutes, p=0.002), we found reduced blood loss (300.0 ml vs. 501.2 ml, p=0.006) and fewer blood transfusions (0% vs. 14.3%, p=0.011). Furthermore, no patients in the ICG4K-LLR group had positive resection margins, while 14.3% of patients in the K-LLR group did (p=0.011). The incidence of clinically relevant post-hepatectomy liver failure (p=0.040) and major complications (p=0.035) was also significantly lower in the ICG4K-LLR group. Additionally, the one-year disease-free survival rate was significantly higher in the ICG4K-LLR group (92.3% vs. 64.3%, p=0.004). Conclusion: The combination of ICG fluorescence imaging with a 4K overlay in laparoscopic liver resection is a safe and effective navigation system that improves perioperative surgical and oncological outcomes compared to K-LLR

    Cardiovascular Remodeling and Potential Controversies in Master Endurance Athletes—A Narrative Review

    While the interest and participation in general endurance training and recreational sports competitions have continuously increased in recent decades, the number of recreational master-level endurance athletes has additionally multiplied. Athletes, active men and women older than 40 years of age, who participate in competitive athletics are usually referred to by the term master athletes (MAs). Previous research revealed the significant benefits of regular moderate physical activity, i.e., its positive influence on cardiovascular risk factors and cardiovascular health; however, recent data have raised concerns that long-term endurance exercise participation is associated with cardiac remodeling and potential adverse cardiovascular outcomes. Previous research also indicated potential structural, functional, and electrical remodeling in MAs due to prolonged and repeated exposure to high-intensity endurance exercise—a condition known as athlete’s heart. In this review, we focus on the association between extreme levels of endurance exercise and potential cardiovascular controversies, such as arrhythmogenesis due to new-onset atrial fibrillation, accelerated coronary artery atherosclerosis, and exercise-induced cardiac remodeling. Additionally, the exercise-dependent modulation of immunological response, such as proteomic response and cytokine alterations, is discussed. Furthermore, we discuss the impact of nutritional supplements in MAs and their potential benefits and harmful interactions. We aim to provide sports medicine practitioners with knowledge of these contemporary longevity controversies in sports cardiology and to highlight the importance of shared decision making in situations of clinical uncertainty

    Zusammenfassung Katheterassoziierte Infektionen in Abhängigkeit von der Liegedauer: eine retrospektive Registerdatenanalyse Hintergrund: Infektionen gehören zu den schwerwiegenden Komplikationen eines Schmerzkatheters. Dass ein enger Zusammenhang zwischen Liegedauer und steigendem Risiko einer Infektion existiert, ist bereits bekannt. Bislang gab es jedoch keine validen Aussagen über genaue Zeiträume und Inzidenzen. Ziel dieser Arbeit ist es, eine wissenschaftlich fundierte Aussage zu treffen, basierend auf einer großen Studienpopulation von 44.555. Methode: Eine finale Studienpopulation von 44.555 Fällen konnte analysiert werden. Diese wurde in einem Zeitraum zwischen September 2007 und Dezember 2017 im Netzwerk zur Sicherheit in der Regionalanästhesie in Deutschland (NRA) gesammelt. Die Studienpopulation wurde in Patienten aufgeteilt, die einen peripheren bzw. einen epiduralen Katheter erhielten. Eine maximale Katheterliegedauer von 15 Tagen stellte den Beobachtungszeitraum dar. Zur Berechnung wurde die Infektionsstufe herangezogen, die zuerst bei der Visite aufgefallen war. Alle Ergebnisse wurden mit dem zugehörigen 95%-Konfidenzintervall dargestellt. Für die Sensitivitätsanalyse konnte eine größere Population von 65.291 Patienten benutzt werden. Der Einfluss der Variablen wurde mit Hilfe einer Adjustierung durch die Cox-Regressionsanalyse überprüft. Außerdem stellte sich die Frage, welche Veränderungen bei Kathetern auftraten, die trotz nachweislicher Infektion in situ belassen wurden. Ergebnisse: Die Wahrscheinlichkeit für katheterassoziierte Infektionen bei peripheren Kathetern lag bei 2,9%, davon waren 2,5% leichte, 0,3% mittlere und 0,07% schwere Infektionen. Die Wahrscheinlichkeit, dass ein peripherer Katheter in den ersten 4 Tagen infektionsfrei war, lag bei 98%, nach 7 Tagen bei 91% und nach 15 Tagen bei 65%, nach Adjustierung durch die Cox-Regressionsanalyse waren es nach Tag 4 99%, nach Tag 7 96% und nach Tag 15 73%. Bei den epiduralen Kathetern betrug die Wahrscheinlichkeit für eine Infektion 3,9%, davon waren 3,3% leichte, 0,6% mittlere und 0,07% schwere Infektionen. Dass innerhalb der ersten 4 Tage keine Infektion auftrat, traf bei 99% zu, nach 4 Tagen 93% und nach 15 Tagen 65%. Nach Adjustierung waren es an Tag 4 99%, nach Tag 7 95% und nach Tag 15 73%. Bei allen in situ belassenen infizierten Kathetern verschlechterte sich die Situation, was im Extremfall eine chirurgische Intervention nach sich zog. Schlussfolgerung: In den ersten Tagen nach einer Katheteranlage ist die Gefahr einer Infektion bei sorgfältigem Umgang mit der Einstichwunde gering. Das Risiko steigt etwa ab dem 4. Liegetag sowohl bei den peripheren als auch bei den epiduralen Kathetern an. Infizierte Katheter sollten unverzüglich entfernt und je nach Schweregrad der Infektion behandelt werden. Um eine Katheterinfektion frühzeitig zu erkennen, bedarf es einer täglichen Visite durch geschultes Personal.Summary Catheter associated infections in relation to length of time in regional anesthesia: a retrospective registry analysis Background: Catheter associated infections are among the most serious complication. It is already known that there is a close correlation between the duration of use and the increasing risk of infection. So far, however, there are no valid statements about exact time periods and incidences. The aim was to make a scientifically wellfounded statement based on a large population of 44.555 cases. Method: A final study population of 44.555 cases could be analysed. The data was collected over a period between september 2007 and december 2017 from the Network for Safety in Regional Anaesthesia in Germany (NRA). The population was divided into the patients who received a peripheral catheter and those who received an epidural catheter and examined a maximum time span of 15 days with the catheter in situ. The infection level that was first noticed during clinical rounds was used for the calculation. All results were presented with the corresponding 95% confidence interval. A larger population of 65.291 patients was used for the sensitivity analysis. The influence oft the variables was checked by adjusting with the Cox regression analysis. It was also concerned with the question of what changes occur in catheters that were left in situ despite evidence of infection. Results: The incidence of catheter-associated infections in peripheral catheters was 2,9%, of which 2,5% were mild, 0,3% moderate and 0,07% severe. The probability that a peripheral catheter was infection-free in the first 4 days was 98%, after 7 days 91% and after 15 days 65%. After adjustment using Cox regression analysis, the probability was 99% after day 4, 96% after day 7 and 73% after day 15. The incidence of epidural catheter infection was 3,9%, of which 3,3% were mild, 0,6% moderate and 0,07% severe infections. The probability of an infection-free epidural catheter days was 99% in the first 4, on day 7 93% and on day 15 65%. After adjustment using Cox regression analysis, it was 99% on day 4, 95% after day 7 and 73% after day 15. All infected catheters left in situ deteriorated, requiring in the extreme cases surgical intervention. Conclusion: In the first days after catheter placement, the risk of infection is low if the puncture wound is handled carefully. The risk increases from about the 4th day onwards for both peripheral and epidural catheters. Infected catheters should be removed immediately and treated according to the severity oft the infection. In order to detect a catheter infection at an early stage, daily rounds by trained staff are necessary
